        Elderly kidney transplant recipients have favorable outcomes but increased infection-related mortality

        Background: The number of elderly patients with end-stage kidney disease has been increasing, but the outcomes of kidney transplants (KT) remain poorly understood in elderly patients. Therefore, we evaluated the clinical outcomes of elderly KT recipients and analyzed the impact of elderly donors. Methods: This retrospective cohort study included patients who underwent KT between 2000 and 2019. KT recipients were divided into four groups according to a combination of recipient and donor age (≥60 or <60 years); elderly recipients: old-to-old (n = 46) and young-to-old (n = 83); young recipients: old-to-young (n = 98) and young-to-young (n = 796). We compared the risks of mortality, graft failure, and acute rejection between groups using Cox regression analysis. Results: The incidence of delayed graft function, graft failure, and acute rejection was not different among groups. Annual mean tacrolimus trough level was not lower in elderly recipients than young recipients during 10-year follow-up. Mortality was significantly higher in elderly recipients (p = 0.001), particularly infection-related mortality (p < 0.001). In multivariable Cox regression analysis, old-toold and young-to-old groups had increased risk of mortality (adjusted hazard ratio [aHR], 2.89; 95% confidence interval [CI], 1.14– 7.32; p = 0.03; aHR, 3.06; 95% CI, 1.51–6.20; p = 0.002). However, graft failure and acute rejection risks were not increased in elderly recipients. Conclusion: In elderly recipients, graft survival and acute rejection-free survival were not inferior to those of young recipients. However, mortality, especially risk of infection-related death, was increased in elderly recipients. Thus, low immunosuppression intensity might help decrease mortality in elderly recipients.

      • 기독교청소년과 비기독교청소년 간 성의식 차이 분석 및 복음주의 상담학적 대안

        본 연구는 기독교청소년과 비기독교청소년 간 성의식의 차이를 분석하고 복 음주의 상담학적인 대안을 제시하는 것이다. 연구목적은 첫째, 기독교청소년과 비기독교청소년 간 성의식의 차이가 있는가? 둘째, 기독교청소년의 신앙배경에 따른 성의식의 차이가 있는가? 셋째, 청소년 가족의 신앙배경에 따라 혼전 성 허용성의 차이가 있는가? 등을 설정하였다. 본 연구의 대상은 서울, 경기도 지 역의 중, 고등학생(314명)이었으며, 본 연구에 사용된 측정도구는 성의식 검사 지로써 공주대학교대학원 이경숙이 2006년에 사용한 것을 본 연구자들이 신뢰 도를 측정한 뒤 수정, 보완하여 사용하였다. 수집된 자료는 SPSS(Satistical Package for the Social Science) 12.0을 이용하여 빈도분석, t-test, one way ANOVA를 적용하여 통계 처리 하였다. 본 연구의 결과를 요약하면 다음과 같다. 신앙과 청소년의 성의식은 유의미 한 차이가 없는 것으로 나타났다. 그러나 신앙생활과 가족의 신앙배경 그리고 자신의 구원의 확신에 따른 혼전 성허용성은 유의미한 차이가 있는 것으로 나 타났다. 가족 모두가 기독교신자이거나 신앙생활을 시작한 기간이 오래된 경 우, 구원의 확신이 있는 경우에 혼전 성허용성에 대한 기독교청소년들의 생각 은 부정적인 것으로 나타났다. 본 연구는 기독교신앙이 청소년의 성의식에 영 향이 있다는 것을 밝힘으로 청소년의 바람직한 성문화와 성태도를 가르치는 일에 필요한 자료가 될 것이다. This paper is to clarify the impact of Christian faith on adolescents` sexuality perception. When children grow up and enter adolescence, they not only experience physical maturation including voice change but also develop secondary sex characteristics, getting away from a sexual latency period. Adolescence is a time for growth spurts both physically and psychologically. Sexual hormonal changes speed up sexual development in boys and girls and increase their interests in and concerns about sex. Against this backdrop, this study explores desirable attitude and behavior toward sex among adolescents, taking distinctive aspects of the young into account. The study is based on the following three assumptions: first, there would be difference in sexuality perception between Christian and non-Christian youth. Second, there would be difference in sexuality perception among Christian adolescents in accordance with religious background of their family. Third, adolescents would demonstrate different views on premarital sex in accordance with family`s religious background. A survey was conducted on 314 junior high and high school students living in Seoul and Gyeonggi province. Two types of tests questioning about Christian beliefs and sexuality perception respectively were utilized to prove the assumptions. The first test on Christian beliefs was first applied to Christian adolescents by Joseph Jeon in 1988 and has been altered for this study. The second test on sexuality perception was used by Kyungsook Lee at Gongju Graduate School of Educational Psychology in 2006 and has been adjusted for the study with careful consideration on reliability of survey results. To analyze 314 answer sheets collected, frequency analysis, t-test, and one way ANOVA were applied, using Statistical Package for the Social Science (SPSS) 12.0. The conclusion of the study is as follows: Christian faith does not have significant impact on sexuality perception among the young. In contrast, the influence of the faith of adolescents themselves and their family as well as assur- ance of salvation on their views about premarital sex appears to be statistically significant. Boys and girls who have long been Christians or have assurance of salvation, or whose entire family believes in Christianity disclose negative views toward premarital sex. The study sheds light on the correlation between Christian belief and sexuality perception of adolescents and will lay the groundwork for teaching the youth sound culture and attitude toward sex.

        자연산 점몰개, Squalidus multimaculatus Hosoya et Jeon의 새인두흡충, Clinostomum complanatum (Rudolphi, 1814) 감염

        경상북도 영덕군의 하천에 서식하는 점몰개 spotted barbel gudgeon (Squalidus multimaculatus Hosoya et Jeon)의 근육에서 새인두흡충 Clinostomum complanatum (Rudolphi, 1814)의 피낭유충이 발견되었다. 감염된 점몰개는 "yellow grub"인 기생충의 피낭유충으로 인해 표면이 울퉁불퉁하게 보였으며, 수면위에 힘없이 유영하였고 일부 폐사한 개체도 발견되었다. 분리된 탈낭유충의 형태는 혓바닥 모양이었으며, 크기는 2.85~5.89 mm×0.8~1.99 mm였다. 어체당 1~98 개의 피낭유충을 분리하였으며 평균 감염수는 25개로 나타났다. 특히, 성어크기인 평균 7 cm 정도의 점몰개는 100% 감염되어 있었다. 본 연구는 한국의 자연수계에 서식하는 어류에서 C. complanatum 의해 질병이 발생하고 폐사가 일어난 첫 보고이다. The metacercariae of Clinostomum complanatum (Rudolphi, 1814) were isolated in the whole body muscle of wild freshwater fish, spotted barbel gudgeon (Squalidus multimaculatus Hosoya et Jeon) from the local river in Yeongdeok, Gyeongsangbuk-do, Korea. The infected fish with bumpy body surface by "yellow grub" showed erratic swimming behavior and some fish were died. The isolated excysted metacercariae were tongue shaped and 2.85~5.89 mm × 0.8~1.99 mm in size. 1~98 metacercariae were isolated in individual fish and mean infection intensity was 25. All examined adult spotted barbel gudgeon sizing 7 cm in body length were infected. This is the first report of disease outbreak and mortality caused by C. complanatum infection in wild freshwater fish in Korea.

      • Helicobacter pylori와 대장균의 Shuttle Vector 개발

        In this study, a vehicle vector using cryptic plasmids was constructed for gene transfer in Helicobacter pylori. pHP51(3.9 kb) and pHP489(1.2 kb) were selected for constructing vectors from cryptic plasmid of H. pylori isolates in Korea. The HindⅢ-digestedDNA fragment(1.2kb) of pHP489 and 1.6kb DNA fragment of pHP51 were ligated with a kanamycin resistance gene(aph3'-Ⅲ) from C. jejuni to produce the recombinant plasmids pHP489K and pHP51K, respectively. Transformation frequency of pHP51K by electroporation was low. But pHP489K could be effectively transformed into various H. pylori strains. In order to design an intermdiate vehicle vector for gene transfer into H. pylori, pBlueHP489K was prepared by recloning pHP489K DNA into pBluescript and pTZ19R vector. This vector permitted the DNA fragment containing pHP489 sequence, aph3'-Ⅲ, and cloned DNA to be cut and self-ligated in the SacⅠ site after cloning. ureA and ureB gene were inserted into pBlueHP489K, resulting in pBlueHP489K/AB. The DNA fragment containing pHP489, kanamycin resistance gene(aph3'-Ⅲ), and urease structural gene was cut away from pBlueHP489K/AB and self-ligated to generate pBlueHP489K/AB. pBlueHP489K/AB made urease-negative H. pylori strains restore their urease activity. By this experiment, pBlueHP489K was confirmed to be the vehicle system for transferring H. pylori genes.

        A industrial development and the high level of industrial technology since the Industrial Revolution in 18th century result in the improvement of qualities of human's lives, while they bring about the shortage of natural resources and the destruction of environment. Today as qualities of human's lives are improved, a lot of consumers are concerned about the environment of the earth and these thoughts are expanded rapidly. Therefore this study is planed to search the desirable direction which Green Marketing should achieve, through the direct and practical cases of Green Marketing, which becomes the factors of threat and chance to enterprises, in the view of approaching of the environmental problems which let mankind realize the serious crisis. The meaning Green Marketing has as the survival strategy of enterprise is prior to all management strategy. The thing required in the situation like this is that the solution and means which can cope with the environmental pressure effectively should be developed and generalized. And then it should be able to realize the pursuit of the enterprise's profit with the result of environmental improvement. However, the basic solution of this environmental problem isn't solved only in endeavoring of enterprise. As the cooperating of each related subject and the social responsibility are required, Green Marketing is more and more important. As Green Marketing combines with consumers and government as well as enterprise unlike the existing marketing, if it has the strong will of environmental harmony, it can be the driving force enabled the environmental management of enterprise effectively. To enable the induction and development of Green Marketing, the construction of environmental management system and the environmental mind are stretched like a blood vessel to the system of enterprise and are managed synthetically. When this system is realized, the beginning of the environmental difference can be acquired.

      • Retinol 국소도포에 의한 눈가주름 제거효과

        In 3 months randomized, double-blind, vehicle-controlled study of retinol (vitamin A 0.1%-3000IU/g) in the treatment of photoaging skin, especially eye wrinkles. 24 patients applied retinol contained cream to the both eye wrinkles and vehicle cream(placebo) to the same site. Half of the patients received retinol contained cream and half received vehicle cream(placebo). All 24 patients who completed the study showed statistically significant improvement in photoaging eye wrinkles on the retinol contained cream treated group, but not on the vehicle treated group. Eight of the 12 patients who received retinol contained cream to eye wrinkles had improvement in photoaging, whereas only three of the 12 patients who received vehicle cream(placebo) improved. In retinol contained cream group, there is a some relationship between water contents and effects. In other words, as water contents were the higher, effects were the better. The side effects were limited to mild irritation of retinol contained cream treated skin. Retinol contained cream appeared to be safe and effective in the treatment of eye wrinkles.

        치주인대 세포의 교원질 생성에 대한 Substance P의 효과

        Substance P는 교정력이 가해진 치아의 치주인대 중 인장력을 받는 부위에 많이 분포하는 neuropeptide 중의 하나이며, 또한 여러 조직에서 neurogenic inflammation을 야기하는 neuropeptide 중의 하나로도 알려져 있다. 그러나 중요한 세포의 단백기질인 교원질의 생성에 대한 Substance P의 효과는 잘 알려져 있지 않다. 따라서 이 연구의 목적은 배양 치주인대 세포에서 교원질 생성에 대한 Substance P의 효과를 평가하는 것이었다. collagenase-digestion method로 교원질 생성을 평가하였고 mRNA 수준에서 작용효과를 평가하기 위하여 Northern blot hybridization을 시행하였다. 이 연구는 또한 교원질 생성에 대한 prostaglandin과 gelatinase 생성도 포함하였으며 변성된 교원질의 분해를 평가하기 위하여 Zymography를 이용하였다. 비교원성 단백질, 교원성 단백질, 상대교원질에 대한 dose-dependent effect를 보면 Substance P는 비교원성 단백질 합성을 증가시켰으나 교원성 단백질 합성은 감소시켰다. 그리하여 총 단백합성에 대한 상대적인 교원질 생성을 나타내는 상대교원질은 7%에서 3.6%로 감소시켰다. 세포를 indomethacin과 동시에 처리할 때 substance P의 교원질 합성 억제효과는 나타나지 않았다. 이것은 Substance P의 교원질 합성 억제효과가 prostaglandin의 생성 때문이라는 것을 의미한다. Substance P의 교원질 합성 억제효과가 procollagen mRNA의 정상(steady-state)수준에 부합하는가를 평가하기 위하여 northern blot hybridization을 시행한 결과 Substance P는 α1(1) procollagen mRNA의 양적 변동을 일으키지 않았다. Substance P의 교원질 생성 억제효과는 전사이후의 어떤 단계에서 이루어지는 현상임을 나타낸다. 치주인대세포에서 gelatinase 생성에 대한 Substance P의 효과를 알아보기 위하여 zymography를 이용하였다. zymogram을 보면 Substance P는 치주인대세포에서 gelatinase 생성에는 아무 효과도 나타내지 않음을 알 수 있다. Substance P의 교원질 생성 억제효과가 치주인대세포에 대해 선택적인가 아닌가를 알아보기 위하여 MC3T3-E1세포를 이용하였는데 Substance P는 MC3T3-E1세포의 교원질 합성에는 영향을 미치지 않았다. 이상에서 Substance P는 인간의 치주인대세포에서 교원질 합성을 억제하였다. 이 효과는 procollagen mRNA와 gelatinase 생성의 정상(steady-state) 수준의 변화 때문이 아니라 prostaglandin 생성과 연관이 있음을 알았다. Substance P is one of the neuropeptide which presents highly in tension site of periodontal ligament during the orthodontic tooth movement. It has been also known as one of the neuropeptides which cause neurogenic inflammation in various tissues and organs. However, there is no report about the effect of substance P on major extracellualar matrix protein, collagen production. The purpose of this study was to evaluate the collagen production by substance P in human periodontal ligament cell, The collagenase-digestion method was used to evaluate collagen production and also used Northern blot hybridization for the evaluation of collagen mRNA level. This study also included in terms of prostanglandins and gelatinase production with respect to collagen production. For the collagen degradation, zymography was used to estimate denatured collagen degradation. Dose-dependent effect of substance P on noncollagen protein, collagen, and percent collagen was that substance P increased noncollagen protein synthesis, but decreased collagen systnisis. So the percent collagen, which determined by relative collagen production against total protein production, was decreased from 7% to 3.6%. This inhibitory effect of substance P on collagen production was disappeared when cells were treated concomitantly with indomethacin. It means that substance P-induced inhibitory effect on collagen production was due at least in part to the production of prostaglandins. To evaluate whether substance P-induced inhibitory effect on collagen production is correspond to the steady-state levels of procollagen mRNA, Northern blot hybridizartion was performed and it showed that substance P has no effect on the steady-state level of α1(1) procollagen mRNA. It means that the inhibitory effect of substance P on collagen production was due to the change of a certain mechanism after posttranscription. In this context, gelatinase production by substance P in periodontal ligament cells was evaluated by zymography. Zymogram showed that substance P has no effect on gelatinase production in periodontal ligament cells. To explore wheter substance P-induced inhibitory effect on collagen production is selevtive in periodontal ligament cells or not, MC3T3-31 cells which originated from mouse calvaria was used. It showed that substance P has no effect on collagen production in MCDTD-E1 cells. Taken together, substance P inhibits collagen production in human periodontal ligament cells. This effect was not due to the change of the steady-state level of procollagen mRNA and gelatinase production, but due at least in part to the change of prostaglandins production.

      • 계측 장비를 위한 무선 RF모듈의 개발

        The existing equipment with wire communication can expect the stability of data transmission. However, according to increasing a number of Measuring Instruments, wiring work becomes a heavy burden. Restriction to application about portable Measuring Instruments and the lack of flexibility are another drawback of wire communication. This paper presents the scheme of the design and implementation of a RF(Radio Frequency) based module. The RF based module is designed to the multi communication between Measuring Instruments. The RF based module consists of RF circuit and microprocessor. The main properties of RF circuit are maximum 5[Kbps] transmission rate, maximum 90m transmission distance and 433[MHz] frequency band. The microprocessor rearrange the data into AHDLC(Advanced High level Data Link Control) format and then instructs RF circuits to transmit/receive the data. The RF module is expacted to have a wide application field such as fire/security alarm, remote control/measurement etc..

      • 저강도 초음파 및 레이저를 이용한 장관골 불유합의 치료

        목 적: 저강도 초음파는 인체 및 동물 실험에서 골조직에 낮은 미세한 물리적 압력을 가하여 골절의 치유에 유리한 영향을 주고, 레이저 (LASER)는 심부 조직에 혈류를 증가시키고, 세포 내 잠재된 에너지를 증가시키는 것으로 알려져 있다. 이에 저자들은 임상적으로 확진된 장관골 붕류합의 치료에 있어 펄스형 저강도 초음파와 레이저를 적용하고 그 효용성을 알아보고자 하였다. 대상 및 방법: 초음파/레이저의 치료 효과를 비교하기 위하여 장관골 불유합으로 확진된 16예 중 8예는 연구군으로 골이식술 후 저강도 초음파와 레이저 치료를 추가하였고 대조군 8예에 대해선 일반적인 골이식술만 시행하여 조사하였다. 남자가 11예, 여자가 5예, 평균 연령은 41.7세 (19~62세)였고, 불유합 부위는 대퇴골이 10예, 경골이 6예였다. 첫 수상으로 인한 골절 후 초음파/레이저 치료까지 시간(골절 연력)은 평균 502일이었다. 결 과: 8예의 연구군에서는 완전 골유합이 6예에서 이루어졌으며 유합까지의 시간은 평균 141일 (101~202일)로 나타난 반면, 8례ㅢ 대조군에서는 5예에서 골유합이 이루어졌고, 유합까지의 시간은 평균 240일 (183~283일)이었다. 결 론: 저강도 초음파/레이져 골절 치료기가 반복된 수술로 인한 난치성 장관골 불유합의 치료에 있어 골이식술과 함께 사용될 경우 긍정적인 효과가 있었고, 특히 최초 가골 형성에 있어 대조군에 비해 그 시간을 줄일 수 있었따. Purpose: Low-intensity, pulsed ultrasound (LIPUS) has demonstrated a stimulation and acceleration of the normal fracture-repair process in cellular bases as well in animal and human models. In this study, the adjunctive effect of LIPUS and LASER was investigated in established nonunion of the long bones. Materials and Methods: Study group consisted of eight patients to whom conventional bone graft and adjunctive LIPUS and LASER was applied. On the other hands, eight patients in the control group underwent bone graft only. Eleven men and five women were included in this study and the average age of the patients was 41.7 years (range, 19~62 years). Six of the tibias and ten of the femurs met the criteria for established nonunion. The average fracture age, the post-fracture period before the start of LIPUS/LASER treatment, was 502 days. Results: Seven of the ten nonunions who were treated by LIPUS and LASER healed completely in an average treatment time of 141 days (range, 101~202 days) and otherwise, in control group, five of the eight nonunions healed completely, in 240 days (range 183~283 days). Conclusion: This clinical study showed a positive effect of LIPUS and LASER on the rate of osseous repair, especially accelerated time to initial callus cortical bridging compared to that of conventional treatment.

      • CT flouroscopy를 이용한 중재적 시술의 유용성

        Conventional CT-guided needle localization has been reported many times, but a few reports of interventional procedures by using multidetector CT scan with CT-fluoroscopy have been described. We presented 36 cases of CT-fluoroscopy guided interventional procedures focusing on the puncture technique. Two cases of small pneumothorax and one case of pulmonary hemorrhage out of 2lcases after lung biopsy, were not needed any treatments We can easily and accurately access to the lesion by using CT-fluoroscopy and unique our marker system.
