        Lewis Lung Carcinoma(LLC) 이식 생쥐에 있어서 천연운지 단백다당체(Copolang)의 면역조절활성

        Immune functions of mice bearing Lewis Lung Carcinoma (LLC) were significantly suppressed when evaluated with mitogen responsiveness, IL-2 production and non-specific suppressor activity. Based on these immunosuppressive characteristics of LLC bearing mice, immunomodulating activates of Copolang were investigated in this model. After 15 days of LLC inoculation, Copolang was intraperitoneally administered for 7 consecutive days with doses of 20 or 200 mg/kg. Immune functions were evaluated 3 days after the final administration of Copolang. The results showed that the growth of LLC solid tumor was not inhibited by Copolang. But, mitogens-induced proliferation, IL-2 production and responsiveness to recombinant IL-2 of splenocytes were significantly augmented by the treatment of Copolang. However suppressor cell activity was not affected by Copolang. These results indicate that Copolang expresses potent immunomodulating activates through the augmentations of IL-2 production and responsiveness to recombinant IL-2, which have been generally known to be suppressed in tumor bearing mice, without affecting the growth of tumor.

        아킬레스 프로젝트 : 계절에 따른 발 질환 발병율과 환자의 복용 순응도 평가를 위한 역학조사 An Epidemiology Survey to Assess the Seasonal Incidence of Foot Diseases and Patient Compliance

        Background: Dermatological diseases affecting the feet, e.g. fungal infections and nail infections, are often not fully perceived as real medical problems even when they experience great discomfort, including pain. These misperception often prevents from seeking timely treatment, which can result in very severe conditions. This survey, so called "ACILLES PROJECT" was a "foot screening” project that would be focused on all parts of the body below the Achilles heel: foot, toe and toenails. Objective: This study was undertaken for both medical doctors and patients to raise the awareness of foot diseases and additional insights into disease that could affect foot. And also, in seasonal variation, we tried to compare the epidemiological data and to look over the relationship of dermatophytosis in other parts with fungal infections in foot and to analyze the compliance and efficacy of antifungal agents in the treatment of patients who were diagnosed as dermatophytosis in their feet in the treatment of antifungal agents. Methods: Achilles survey was conducted for one week from 7^th July to 13^th July, 1997 and 16^th March to 17^th April, 1998 co-worked by the Korean Society for Medical Mycology and Janssen Korea. In 1997, 19,298 patients, who visited dermatology and urology, and 353 investigators participated in this survey. And Achilles 98, in 1998, was composed of two parts, Unit I and Unit ll. Unit I survey was a foot disease consult program and Unit Ⅱ was a patient compliance survey. 29,922 patients, 391 investigators and 16,383 patients, 350 investigators were involved in Unit I and Unit Ⅱ respectively. Results: Among total 29,922 patients who responded to the survey, 57.7% of them had more than one foot disease. There was no big difference comparing with the number (67%) in the survey of last summer. And it was observed that fungal infection was the most dominant disease (79.8%) regardless of seasons (83.7% in July, 1997). Furthermore, there were no significant differences in the prevalence of other foot diseases for each season (eczema 11.1%, corns 5.1%, warts 2.7%, 2.1%). Oral antifungal agents were prescribed to the patients who were diagnosed as having fungal infections in their feet in order to survey drug compliance by filling out the patients' diary. In compliance, only 24% of patients in itraconazole pulse therapy for 7 days and 16% of patients in itraconazole continuous therapy for 28 days compiled with their dosage guideline exactly. It is observed that the longer the duration of drug therapy is, the worse the patients' compliance is. Conclusion: According to the epidemiological survey in 1997 and 1998, the prevalence of foot disease was not significantly different seasonally, and it was reported that the awareness of and the intention for the treatment of foot diseases were not high enough based on the compliance data of patients.

        피부 진균 질환의 가족 내 유병율에 대한 역학 조사

        Background: Superficial fungal disease is one of the most common human infectious diseases. A recent clinical survey reported that more than half of patients with superficial fungal diseases might be infected at home. Objective: The aim of this study is to investigate the correlation of the fungal infection between the patients and their family members. Methods: Dermatologists of 71 general hospitals were involved and evaluated 7,323 patients and 1,037 their family members who were confirmed microscopically to have superficial fungal diseases. Results: According to the survey results, 46.1% of the patients answered they might have been infected at home and 33.6% of the patients remembered there were other fungus-infected patients among their family members at the same time. The isolated organisms were Trichophyton rubrum(1686 cases, 80.9%), Trichophyton mentagrophytes (269 cases,12.9%), Candida albicans (112 cases, 5.4%), Microsporum canis (12 cases, 0.6%), Trichophyton tonsurans (3 cases, 0.1%), Epidermophyton floccosum (2 cases, 01%). On the basis of the fungus culture, it was proven that 85.4% of the patients and their family members had the same kind of microorganism. Conclusion: As a result, we could conclude that there is a high possibility of transmitting the fungal infection from one family member to another. [Kor J Med Mycol 6(2): 57-63]

        전풍에서 Itraconazole 1주 요법의 효과

        Background: Pityriasis versicolor is often found following pathologic transformation of Malassezia fungi from yeast to mycelial forms by a poorly understood mechanism and is characterized by squamous skin lesions manifesting hyper- or hypo-pigmentation. Itraconazole is a synthetic oral antifungal agent of triazole family, which acts as a selective inhibitor of the cytochrome P-450-dependent synthesis of ergosterol and is reported to demonstrate a potent antifungal efficacy against Malassezia yeasts. Object: The present clinical study was designed to evaluate the therapeutic efficacy of one-week regimen of itraconazole by studying 20 patients with pityriasis versicolor, who were diagnosed both clinically and mycologically. In addition, we wished to identify Malassezia species from skin lesions of pityriasis versicolor and examine the possible correlation of a certain Malassezia(M.) species and pityriasis versicolor. Methods: Twenty patients (ages, 18 to 49) were included in the study, following the diagnosis of pityriasis versicolor, which was based on clinical examinations and mycological studies (microscopy and culture). Patients received a single 200 mg dose of itraconazole (Sporanox^�) capsules daily with a full meal for one week. For all patients, clinical examinations and culture and microscopic studies were done before treatment, at the end of treatment, and 4 weeks later. Scales obtained from skin lesions were studied by Parker-KOH smear examination and culture studies to identify Malassezia yeasts. Cultures were done using agar media of Leeming & Notman. Results: By comparing skin lesions before and immediately after treatment, marked improvement was observed in 7 patients (35%) and moderate improvement in 12 patients (60%) among a total of 20 patients, and 1 patient (5%) showed no response. On the other hand, by comparing skin lesions examined before treatment and 4 weeks after completing medication, complete cure was observed in 13 cases (65%), marked improvement in 5 (25%), and moderate improvement in 2(10%). With culture studies before treatment, M. globosa, M. furfur, and M. restricta were isolated as a major fungal species in 18, 1, and 1 patients, respectively. Conclusion: The result suggested that administration of a single dose of 200 mg itraconazole(Sporanox^�) capsules daily for a week is a very effective treatment for pityriasis versicolor and also indicated that pityriasis versicolor is very closely related with the infection of M. globosa.         이 트라코나졸은 약물역동학적으로 피부 각질에 친화성이 높고 투약 종료 후에도 4주간 지속되며 고용량 단기간 투여가 효과적이라는 것이 알려지면서 각화형의 수장부/족부 백선 치료에도 고용량단기간 투여법이 시도되고 있다. 본 연구는 각화형의 수장부/족부 백선 환자 97명을 대상으로 이트라코나졸 1일 400 mg을 7일간 투여하여 효과 및 안전성을 평가하였다. 투약 전, 투약 후, 투약 종료 4주후에 임상적, 진균학적으로 효과를 평가하여 임상적으로 73.8%에서 중등도 이상의 개선, 진균학적으로는 84.5%의 완치율을 얻었다. 97.6%의 환자에서 양호한 내약성을 나타내었고, 2.4%에서 위장관 증상, 피부 발진 등이 발생하였으나 특별한 치료 없이 호전되었다. 이상과 같은 결과로 각화형의 수장부/족부 백선 치료에 이트라코나졸 400mg 7일 투여는 효과적이면서 동시에 안전한 치료법임을 확인하였다.         한국의 발톱진균증 환자의 삶의 질에 관한 연구

        Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]
