만성폐쇄성폐질환환자 1례에 대한 임상적 고찰

We report the treatment case of patient for chronic obstructive pulmonary disease. The patient was 65 year-old woman with the history of smoking and asthma. She had complained of dyspnea, hyperpnea, cough and wheezing sound. We treated her with general management, bronchodilator and five kinds of herbal medicine(Samjahwadamjeon etc). Among them, the major herbal medicine is Samjahwadamjeon. Having been treated for 35 days, the patient improved in clinical symptoms and pulmonary function in PFT(Pulmonary function test).

초등교과서는 발행되자마자 역사교육연대회의에서 오류를 검토할 정도로 뜨거운 관심을 받았다. 사실을 정확하게 기술한다는 것은 역사의 기본인 만큼 꼭 수정되어야 할 것이지만, 그에 못지않게 중요한 것은 교과서가 전체적으로 정합성을 갖는 체제를 갖추는 일이다. 이런 관점에서 초등교과서의 내용을 목차를 중심으로 살펴보았다. 고려편의 ‘세계와 활발하게 교류한 고려’는 고려가 아랍과 빈번한 교류를 했다는 점을 염두에 둔제목으로 사실과 달라 ‘문벌귀족사회인 고려’로 수정해 보았다. 이어서 대단원과 중단원의 정합성을 고려하면서 목차를 수정해보았다. 그리고 중단원과 소단원의 정합성과 소단원과 서술의 정합성에 대해서 살펴보았다. 이러한 작업을 통하여 아동들의 흥미와 인지발달 정도에 맞춘 서술로 생활사와 문화사 중심의 서술이 아직 학계에서 구체적인 연구성과를 얻지 못한 점이 있어 때로는 꿰어 맞추기 식으로 되어 버린 경우도 찾을 수 있었다. 아동들에게 역사에 대한 흥미를 부여해줄 수 있는 서술체계를 갖추기 위해서 더욱 정진된 연구가 필요하다는 점도 부각되어야 할 것이다. However, lack of study of research groups don`t reveal the contemporary characteristics. This statement sometimes mislead the children. Further research will be able to give an interest in history to children.

미국흑인 민간속요의 스토리텔링 콘텐츠

미국흑인 민간속요는 아프리카계 미국인들이 고향의 구전전통을 새로운 땅에 이식하여 토착화한 버내큘라 예술이다. 노예시절 이전부터 구전되어온 이 민간속요는 운율이 있는 서사적 가요로서, 당대의 전반적인 사회상과 흑인사회의 집단적 심리상태를 잘 반영해 준다. 외형에 있어서 주로 영국 포크발라드의 전통 형식을 차용한 흑인 민간속요는 특히 19세기 후반부터 20세기 초반까지 전환기 격동의 역사를 중심으로 흥미로운 서사와 캐릭터를 담고 있다. 여러 모습의 소(小)영웅들을 둘러싸고 벌어지는 일련의 사건들이 ‘시그니파잉’이라는 흑인 특유의 유희적 언어관습과 이를 토대로 한 ‘토스트’라는 구연(口演) 방식을 통해 민간속요의 서사를 구성한다. 이 민간속요는 오늘날 대중문화에서 중요한 위치를 차지하고 있는 미국흑인예술에 공헌한 바가 크므로, 그 내용뿐 아니라 지금까지 이를 활용한 과정과 방식도 면밀히 살펴볼 만하다. 민간속요의 스토리텔링 콘텐츠들은 그동안 여러 차례에 걸쳐 소설, 영화, 연극, 뮤지컬, 만화, 대중음악 등에서 재현되어 왔다. 특히 전형적인 ‘나쁜 남자’ 이야기는 힙합의 상업적 성공에 힘입어 우리나라를 비롯해 세계 각지에서 신드롬을 일으키는 데 커다란 역할을 하였다. 이러한 성과들을 본보기로 삼아, 흑인 민간속요 못지않게 양적ㆍ질적 기반을 갖추고 있는 우리나라의 민간속요가 훌륭한 스토리텔링 콘텐츠로 활용될 수 있도록 우리도 보다 활발한 연구를 전개할 필요가 있다. Folk ballads were first introduced to the United States by African-Americans who brought their oral tradition from their home countries. This vernacular art form gradually took root in the country. These narrative and rhythmical popular songs, which began to be orally transmitted before the days of slavery in the U.S., reflect American society of that time and African-Americans’ mentality. These songs, whose form was derived from that of traditional British folk ballads, contain interesting narrations and characters particularly focused on the turbulent era which lasted from the end of the 19th century to the beginning of the 20th century. To be more specific, these ballads describe what happen to little heros with different characters by means of “signifying,” African-Americans’ own witty language, and “toast,” a storytelling technique based on this language. These songs have greatly contributed to African-American art which takes a large part in today’s popular culture. It is thus worth studying in detail not only their content but also the process and method of using it. The storytelling content of folk ballads has frequently been used for novels, movies, plays, musicals, animations and popular songs. In particular, typical “badman” stories have gained popularity in many countries including Korea, thanks to the commercial success of hip hop music. In this context, it is necessary to learn from these results and to conduct research more actively so that Korean folk ballads, which are as great as African-American ones in terms of both quantity and quality, can also be used as excellent storytelling content.

증 례 : 급성췌장염으로 발현한 췌관내유두상점액종양 연관 침습암 1예

급성 췌장염 중에 1차 검사에서 원인이 불명확한 경우 급성 특발성 췌장염이라고 하며 그 중 종양에 의한 것은 비교적 드물지만 진단이 늦어질 경우 예후가 좋지 않아 먼저 의심하는 것이 매우 중요하다. 본 증례는 급성 특발성 췌장염 환자에서 악성종양을 의심하고 적극적으로 추적관찰하던 과정에서 초음파내시경상 이상소견이 확인되었고, 적절한 시기에 수술을 결정하여 췌관내유두상점액종양 관련 침습암을 진단한 경우이다. 따라서 원인이 불분명한 췌장염에 대해 악성종양을 의심하는 것이 필요함을 강조함과 동시에 초음파내시경의 추적관찰이 진단에 도움이 된 실제 사례를 보고함으로써 앞으로의 진료 및 의사결정에 유익할 것으로 보여 문헌과 함께 보고하는 바이다. Idiopathic acute pancreatitis (IAP) is defined as acute pancreatitis for which the standard initial evaluation has failed to reveal the etiology. Although a tumor is a rare cause of IAP, it is important to consider it in advance, because prognosis is poor if diagnosis is delayed. We describe a case of presumed IAP at the index presentation that was finally diagnosed as intraductal papillary mucinous neoplasm with an associated invasive carcinoma in a 70-year-old woman after surgery to remove a mass identified during follow-up endoscopic ultrasonography. Therefore, this is a case in which suspicion of malignancy and follow-up endoscopic ultrasonography helped to appropriately diagnose and treat the tumor.

지역사회 만성 조현병 환자에서 원예 활동과 웰빙(Well-Being)과의 관계

연구목적 : 본 연구는 만성 조현병 환자를 대상으로 자아존중감 증진, 행복감 향상, 자살사고와 충동성과 같은 증상의 호전, 일상생활기능 증진, 재활과 같은 환자의 웰빙(well-being)과 원예활동의 관계를 평가하기 위해 시행하였다. 방 법 : 대전시 대덕구의 정신건강증진센터에 등록된 만성 조현병 환자들 중 주간재활프로그램에 참여하면서 옥상 텃밭에서 시행하는 원예 활동에 자원한 25명을 대상으로 하였다. 참여자들은 2013년 6월부터 11월까지 총 30번의 옥상 텃밭에서 원예 활동을 하였고, 원예 활동을 시작하기 전과 후 로젠버그 자기존중감 척도(Rosenberg Self-Esteem Scale), 단축형 행복척도(Concise Measure of Subjective Well-Being), 역기능적 충동성 척도(Dysfunctional Impulsivity Scale ), 자살 생각 척도(Scale for Suicidal IdeationBeck), GAF 척도(Global Assessment of Functioning scale)를 평가하였다. 통계 분석은 원예 활동 전과 후에 평가한 각 척도들에 대해 대응표본 t-검정을 시행하였다. 결 과 : 옥상 텃밭의 원예활동 참여 전과 후를 비교한 결과 자아존중감(p<0.01)과 행복감(p=0,02)은 유의미하게 증가하였고, 충동성(p= 0.02)과 자살 사고(p<0.01)는 유의미하게 감소 하였다. GAF 척도는 원예활동 참여 전후에 유의미한 변화는 없었다. 결 론 : 옥상 텃밭의 원예 활동은 만성 조현병 환자의 자아존중감과 행복감을 증가시키고, 자살 사고와 충동성과 같은 증상을 감소시킴 으로써 웰빙을 높이는 것을 나타났다. Objectives : The purpose of the present study was to determine whether gardening activities would have a positive effect on the psychological well-being of patients with chronic schizophrenia living in a community setting. Methods : This study was a retrospective analysis of the records of a community mental health center and included 25 patients with chronic schizophrenia who participated in a gardening activities program. Each subject engaged in gardening activities 30 times over the course of 6 months, from June 2013 to November 2013. The Rosenberg Self-Esteem Scale (RSES), Concise Measure of Subjective Well-being (COMOSWB), Dysfunctional Impulsivity Scale (DIS), Scale for Suicidal Ideation-Beck (SSI), and Global Assessment of Functioning (GAF) scales were conducted at the beginning and end of the gardening activities program. Results : Paired t-tests revealed that 6 months of gardening activities resulted in significant improvements in self-esteem (p<0.01) and happiness (p=0.02), and significantly reduced impulsivity (p=0.02) and suicidal thinking (p<0.01). There was not a significant change in the GAF score. Conclusion : In the present study, gardening activity improved self-esteem and happiness and reduced impulsivity and suicidal thinking, having a positive effect on well-being in patients with chronic schizophrenia living in a community setting.

고속 DRAM을 위한 Duty Cycle 보정 기능을 가진 Analog Synchronous Mirror Delay 회로의 설계

본 논문에서는 duty cycle-corrected analog synchronous mirror delay(DCC-ASMD)라고 불리는 새로운 구조의 내부 클럭 생성기를 제안한다. 제안된 회로는 임의의 duty ratio를 가진 외부 클럭에 대하여 duty ratio가 $50\%$로 보정된 내부 클럭을 2클럭 주기 만에 생성할 수 있다. 그러므로, 본 내부 클럭 생성기는 double data-rate (DDR) synchronous DRAM (SDRAM)과 같은 듀얼 에지 동기형 시스템(dual edge-triggered system)에 효율적으로 이용될 수 있다. 제안된 기술의 타당성을 평가하기 위하여, $0.35\mu$m CMOS 공정기술을 이용하여 제안된 내부 클럭 생성기를 구현하여 모사실험을 실행하였다. 실험 결과, 제안된 내부 클럭 생성기는, $40\~60$의 duty ratio를 갖는 외부 클럭 신호에 대하여, 50$\%$ duty ratio를 갖는 내부 클럭 신호를 2 클럭 주기 만에 발생시킬 수 있음을 확인하였다. This paper describes a novel internal clock generator, called duty cycle-corrected analog synchronous mirror delay (DCC-ASMD). The proposed circuit is well suited for dual edge-triggered systems such as double data-rate synchronous DRAM since it can achieve clock synchronization within two clock cycles with accurate duty cycle correction. To evaluate the performance of the proposed circuit, DCC-ASMD was designed using a $0.35\mu$m CMOS process technology. Simulation results show that the proposed circuit generates an internal clock having $50\%$ duty ratio within two clock cycles from the external clock having duty ratio range of $40\;\~\;60$.
