      • 광배근 유리 피판술을 이용한 사지 재건술

        Latissimus dorsi(LD) muscle is the largest transplantable block of vascularized tissue. Since LD free flap was introduced in 1970's, this flap has been widely used for the reconstruction of large soft tissue defect of the limb. From 1981 to 1996, we had experienced 37 cases of LD free flap. Serratus anterior muscle was combined with LD in three of them whose defects were very large. The average age of the patients was 31 years(range : 4-74 years), and thirty one patients were male. Trauma was cause of the defect in every case. For the recipient sites, the foot and ankle was the most common(22 cases); and the knee and lower leg(11 cases), the elbow and forearm(2 cases), the hand(2 cases) were the next. The duration of follow-up was averaged as 16 months(range: 6 months-12 years). Thirty one cases(84%) out of 37 were successful transplantations. In one case the failure of the flap was due to heart attack and subsequent death of the patient. One failure was caused by sudden violent seizure of the patient who had organic brain damage. Immediate reexploration of the flap was performed in 4 patients, and the flap survived in three of them. There was one necrosis of the grafted split-thickness skin on the survived LD flap. LD free flap was considered as one of the good methods, for the reconstruction of the large soft tissue defect of the limb.

        만성폐쇄성폐질환환자 1례에 대한 임상적 고찰

        We report the treatment case of patient for chronic obstructive pulmonary disease. The patient was 65 year-old woman with the history of smoking and asthma. She had complained of dyspnea, hyperpnea, cough and wheezing sound. We treated her with general management, bronchodilator and five kinds of herbal medicine(Samjahwadamjeon etc). Among them, the major herbal medicine is Samjahwadamjeon. Having been treated for 35 days, the patient improved in clinical symptoms and pulmonary function in PFT(Pulmonary function test).

      • 유리 동맥화 정맥 피판을 이용한 수부 결손의 재건

        Since Nakayama's first report about venous flap, many experimental and clinical studies were done about this new type of flap. And due to its various benefits, its applications as arterialized venous free flap type have increased recently. In this study we have attempted to reconstruct composite of defects of the hand with new modification of arterialized venous free flap and simultaneous reconstruction of skin, nerve, tendon were performed successfully. From 1994 to 1999, the defects of the hands in 35 patients were reconstructed with various modifications of arterialized venous free flaps. The range of age was from 19 to 55 years and size of flap ranged from $1{\times}2cm\;to\;14{\times}9cm$. Among them, 12 cases of flap over 20cm in size were included. Indications of flaps were as follows: resurfacing of the defects of the skin (9 cases), simultaneous reconstruction of extensor, skin and digital nerve(2 cases), reconstruction of the skin with extensor(5 cases), as a flap-through type vascular reconstruction(6 cases), for digital nerve reconstruction(2 cases), contracture release(3 cases), and finger tip reconstruction(9 cases). All of the cases except one survived with marginal skin necrosis less than 10%. And relatively large flaps over 20cm in size successfully survived without any delay procedures. Composite reconstructions including tendon and nerve were successful with new modifications of this flap. Arterialized venous free flap is one of the useful procedure in reconstruction of the hand because it has many advantages such as non-bulky and good quality of flap, variable length of pedicle, preservation of major vascular pedicle, less operation time, single operative field and in addition possibility of various modifications.

        유전체장벽방전에 의한 질소함유 활성종의 개발 및 저온 GaN 박막 성장

        TMGa와 유전체 장벽방전에 기초한 질소함유 활성종을 이용하여 (0001) 사파이어 기판위에 GaN 박막을 저온에서 성장시켰다. III-V 질소화합물 반도체의 에피막 성장에 있어서 암모니아는 유기금속 화학증착법에서 지금까지 알려진 가장 보편적인 질소 공급원이며 충분한 질소공급을 위해 $1000^{\circ}C$ 이상의 고온 성장이 필수적이다. GaN 박막을 비교적 저온에서 성장시키기 위하여 질소 공급원으로 암모니아 대신 유전체 장벽방전을 이용하였다. 유전체 장벽방전은 전극사이에 유전체 장벽을 설치하여 arc를 조절하는 방전이며 수 기압의 높은 공정압력보다 훨씬 높으므로 기판표면까지 전달하는데도 이점이 있다. GaN 박막의 결정성과 표면형상은 성장온도, 완충층에 따라 변화하였으며, $700^{\circ}C$의 저온에서도 우수한 (0001) 배향성을 갖는 GaN 박막을 성장할 수 있었다. GaN films were deposited on sapphire [$Al_2O_3(0001)$] substrates at relatively low temperature by MOCVD using N-atom source based on a Dielectric Barrier Discharged method. Ammonia gas($NH_3$is commonly used as an N-source to grow GaN films in conventional MOCVD process, and heating to high temperature is required to provide sufficient dissociation of $NH_3$. We used a dielectric barrier discharge method instead of $NH_3$ to grow GaN film relatively low temperature. DBD is a type of discharge, which have at least one dielectric material as a barrier between electrode. DBD is a type of controlled microarc that can be operated at relatively high gas pressure. Crystallinity and surface morphology depend on growth temperature and buffer layer growth. With the DBD-MOCVD method, wurtzite GaN which is dominated by the (0001) reflection was successfully grown on sapphire substrate even at $700^{\circ}C$.

      • 중수골 부위 절단상(transmetacarpal amputation)에 시행한 재접합술

        From march 1993 to march 1998, twenty consecutive transmetacarpal replantations and revascularizations were reviewed retrospectively. Nine patients sustained severe and diffuse crush injuries, four patients had local crush injuries, and seven suffered guillotine type amputation. Six replantations and fourteen revascularizations were performed. 76 of 81 replantable digits(93%) were salvaged. 15 patients required secondary surgery, 10 patients for tendon and joint scarring and 5 for nonunions or malunions. Intrinsic muscle function and pinch and grip strengths were weak or absent. According to Chen's grading system of functional return, 2(10%) were grade I, 6(30%) were grade II, 10(50%) were grade III, and 2(10%) grade IV. The follow-up period ranged from 6months to 46 months. Only 3 patients resumed his prior occupation(one as a supervisor); 2 were permanently disabled, 4 pursued new occupations as a manual worker, 1 were still in therapy. Only two of the manual laborers were able to return to their preinjury occupation. Despite these unacceptable functional results, all patients were satisfied with the surgery.

        천년초 발효물의 라디칼 소거능

        The aim of this work was to select suitable fermentation treatments for the efficient bioconversion of cactus (Opuntia humifusa Raf.) bioactive components with an improved radical scavenging activity for use as a nutraceutical. To obtain microorganisms for the microbial conversion of cactus, Leuconostoc mesenteroides ATCC8294, Lactobacillus plantarum KCTC 3099, Lactobacillus plantarum KERI 236 and Monascus pilosus KCCM 60029 (ATCC 22080) were used for fermentation. Fermentation by Lac. plantarum KCTC 3099 was the most effective at scavenging 1,1-diphenyl-2-picrylhydrazyl hydrate (DPPH) and 2,2-azino-bis-(3-ethylbenzothiazoline-6-sulfonic acid) (ABTS) radicals and reducing iron (III). In particular, uronic acid levels showed a remarkable increase in fermentation. The polyphenol and quercetin content of the fermented cactus showed large increases from $108.65{\mu}g/mL$ and $2.71{\mu}g/mL$ to $227.83{\mu}g/mL$ and $9.73{\mu}g/mL$, respectively, showing a maximum level at 36 h of fermentation with Lac. plantarum KCTC 3099. Thus, cactus fermentation with Lac. plantarum is an useful process for the enhancement of antioxidant contents and activity of fresh cactus.

        Purpose: The purpose of this study was to present the clinical analysis of the results and our experience of the replantation surgery of the great toe. Materials and Methods: Between March 1995 and December 2002, twelve great toes, amputated from the distal phalanx to proximal phalanx were replanted. The complete types were 5 cases and incomplete types were 7 cases. The guillotine injuries were 4 cases and the crushing injuries were 8 cases. Results: Replantation in eleven out of the twelve amputations survived. The cases of revision were 3 cases due to venous thrombosis. Patients were followed up for a mean period of 2.2 years. The mean total active motion of the first MTP joint was $80^{\circ}$. The fusion of IP joint was carried in 7 cases due to the amputation of the IP joint level. The shortening of the replanted great toes was present, with average 0.9cm. The two point discrimination was 7-8mm except 2 cases with loss of nerve. Conclusion: Although replantation of the great toe remains to be controversial, replantation of the great toe should be considered in well-motivated patients because successful replantation regains an important component of the foot and good functional, cosmetic results.

        세그먼트 차원압축을 이용한 HMM의 음절인식

        본 논문은 단음절 전구간에 대해 4프레임폭과 7프레임폭을 결합하여 만든 40차원의 세그먼트를 K-L전개와 신경망으로 각각 10, 14, 20차원으로 압축하여 연속분포 HMM의 음성인식 특징파라미터로 사용하였다. 그리고 이산지속시간, 희귀계수 그리고 혼합분포를 특징 파라미터로 추가한 경우와 비교검토하였다. 단음절 100개에 대한 인식실험결과 연속분포 HMM의 인식률 85.19%에 비해 희귀계수를 부가한 경우 1.4%, 혼합분포를 이용한 경우 2.36%, 이산 지속시간제어를 한 경우 2.78%의 인식률이 향상되었다. 그리고 K-L전개에 의한 압축파라미터만 이용한 경우는 멜켑스트럼 + 희귀계수의 경우보다 인식률이 낮았으나, K-L전개에 의한 압축파라미터에 멜켑스트럼과 희귀계수를 부가한 경우는 동등한 결과를 얻을 수 있었다. 신경망에 의한 압축파라미터를 이용한 경우에는 비선형 변환인 시그모이드 함수를 사용하므로 음성의 동적변화가 잘 반영되어 K-L전개 및 다른 방법에 비해 향상된 인식결과를 얻을 수 있었다. In this paper, a 40 dimensional segment vector with 4 frame and 7 frame width in every monosyllable interval was compressed into a 10, 14, 20 dimensional vector using K-L expansion and neural networks, and these was used to speech recognition feature parameter for CHMM. And we also compared them with CHMM added as feature parameter to the discrete duration time, the regression coefficients and the mixture distribution. In recognition test at 100 monosyllable, recognition rates of CHMM +${\bigtriangleup}$MCEP, CHMM +MIX and CHMM +DD respectively improve 1.4%, 2.36% and 2.78% over 85.19% of CHMM. And those using vector compressed by K-L expansion are less than MCEP + ${\bigtriangleup}$MCEP but those using K-L + MCEP, K-L + ${\bigtriangleup}$MCEP are almost same. Neural networks reflect more the speech dynamic variety than K-L expansion because they use the sigmoid function for the non-linear transform. Recognition rates using vector compressed by neural networks are higher than those using of K-L expansion and other methods.

        현대정치와 실존양식

        정치행위를 인간의 실존을 구현하는 집합적 활동이라고 본다면, 정치제도는 바로 인간의 실존양식을 대표하는 것이다. 실존을 존재의 형태로 보다는 자유의 형태로 본다면, 정치제도에 내재된 인간의 자유는 바로 인간실존의 내용과 한계를 구성할 것이다. 이러한 전제를 가지고 자유주의, 공화주의 및 민주주의가 융합된 현대의 정치제도 속에 구현된 현대인의 실존양식을 살펴보려면, 세 가지 정치이념이 지지하는 자유의 개념을 분석해야 한다. 소극적 자유, 적극적 자유 및 공화적 자유의 개념을 분석해보면, 현대인의 실존양식은 자유주의의 실존양식과 공화주의의 실존양식 및 민주주의적 실존양식이 삼중적으로 융합되어 있다는 것이 드러난다. 삼중적인 융합관계는 세가지의 실존양식이 한꺼번에 융합되어 있다기보다는 두 가지 실존양식끼리 융합된 세 가지의 실존양식이 복합된 모양새를 띠고 있다. 그것은 바로 입헌자유주의적인 실존양식과 자유민주주의적인 실존양식 및 민주공화적인 실존양식으로 복합되어 있는 것이다. 이렇게 될 수밖에 없었던 까닭은 자유주의적 실존양식과 민주주의적 실존양식이 서로 대척적인 관계에 있기 때문이다. 이러한 대척적인 관계는 공화주의적인 실존양식이 매개역할을 함으로써 상호보완적인 관계로 탈바꿈되었다. 다시 말해서 입헌자유주의적인 실존양식과 민주공화적인 실존양식이 구성됨으로써 자유민주주의적인 실존양식이 현실성을 얻게 되었다는 것이다. 따라서 현대인의 실존을 확보해야하는 현대정치의 과제는 삼중적인 실존양식의 적절한 균형점을 찾아내는 것이다. If politics is a kind of collective activity for realizing human existence, then mode of human existence shall be represented by political system. If human existence is a form of liberty, then liberties supported by political system will constitute contents of human existence. With these premises, the contemporary mode of human existence embedded in the contemporary political system shall be analysed. The contemporary political system is a complex system of three political ideas such as liberalism, republicanism and democracy. Thus, in order to examine the contemporary mode of human existence, it is necessary to analyze the concepts of freedom supported by these political ideas. The result is that the contemporary mode of human existence is a threefold complex of liberal, republican and democratic modes of human existence. The task of contemporary politics is to find a proper balance of these three modes of human existence.

      • VTS 관제사 근무여건과 개선방안

        : 해상교통관제는 관제사의 업무 특성과 작업 환경에 따라 제공되는 서비스의 질이 달라질 수 있다. 본 논문에서는 해상교통관제사의 근무 여건을 분석하여 추후 대한민국 해상교통관제 작업 환경 개선과 이를 통한 양질의 서비스 제공을 실현할 수 있는 방안을 제시하고자 하였다. 전국 20개소의 해상교통관제 센터에서 근무하고 있는 관제사를 대상으로 근무 여건 문제점 및 개선방안에 대한 인식을 설문하였다. 해상 교통관제사의 작업 환경과 작업 스트레스, 이직의도 등을 분석하여 향후 해상교통관제 센터의 작업 환경과 조직 개선을 위한 기초 자료로 사용할 수 있을 것으로 기대한다. The quality of service provided by Vessel Traffic Services depends on the tasks characteristics and the work environment of Vessel Traffic Service operators. In this paper, the working conditions of Vessel Traffic Service operators are analyzed in order to propose methods to improve the working environment of Vessel Traffic Services and to provide high quality service through them. The questionnaire survey was conducted for Vessel Traffic Service operators working at 20 Vessel Traffic Service centers. It is expected that it might be used as the basic data for improvement of work environment and organization of Vessel Traffic Service center in the future.
