항암화학요법을 받는 암 환자에 대한 Nucare^R의 영양지지효과

Purpose : The purpose of this study was to evaluate the effect of nutritional support with enteral liquid supplement in cancer patients receving chemotherapy for possible benefit in nutritional, immunologic and golbal function of patients. Patients and Methods : From October 1995 to February 1997, 30 advanced cancer patients receving chemotherapy were divided two roups. The Nycare group, in addition to normal diet, Nucare^R enteral supplement was given for 1week right after chemotherapy for the duration of 2 chemotherapy cycles. Control group received only normal diet without parenteral fluid supplement for 2 chemotherapy cycles. Results : Median ages were 53 end 56 years for Nucare group and Contrl group respectively. Performance scores was less than 2 by ECOG scale. All patients were stage Ⅲ or Ⅳ. The physical parameters, such as weight, arm muscle circumference(AMC) and triceps skin fold(TSF) were decreased in both groups after 2 cycles of anticancer therapy. but it was less severe in Nucare group(p<0.05). Serum transferrin was maintained in mild deficit state in Nucare group, whereas it aggravated form mild to moderate deficit in Control group(p<0.05). Serum albumin level increased in Nucare group without statistical significance. but it decreased from normal to mild deficit in Control group. Serum total protein did not change significantly in Nucare group. but in Control group, serum total protein was decreased from 7.24±0.9 to 6.52±0.5(P<0.05). Total lymphocyte count did not change significantly in both groups. Conclusion : This study shows that the nutritional support with Nucare^R was effective in the prevention of nutritional deficit status in patients receving a nticancer chemotherapy.

브랜드이미지와 자아이미지가 제품 선택시 미치는 영향에 관한 연구

브랜드 이미지와 자아이미지가 제품구매 선호에 어떠한 영향을 미치는가를 살펴 본 결과 브랜드 이미지는 제품의 물적 속성과 상징적 의미를 공유하였으며 이러한 상징은 구매하는 사람의 의미를 다른 사람에게 커뮤니케이션 하는 역할이 될 수 있는 것으로 나타났다. 또한 개성을 갖기도 하는데 특히 소비자들의 기억 속에 자리 잡은 브랜드이미지는 구매의사결정 과정 중 대안 평가 단계에 영향을 미침으로써 지속적·반복적 구매 여부의 결정 요인이 되고 있다. 브랜드는 제품의 물적 속성과 아울러 상징적 의미에 의해 그 이미지가 형성되며 제품이 상징적 의미로 사용될 경우에는 그 물적 특성보다는 소비자의 가치 체계가 더 많이 작용한다. 제품 자체가 상징적 의미로 사용될 때 브랜드이미지의 형성은 소비자의 가치체계에 초점을 맞춤으로써 효과적으로 소비자에게 소구 할 수 있을 것으로 판단된다. 소비자의 가치체계·준거의 틀로서의 자아이미지는 스스로에 대한 주관적이고 객관적인 지각이며 소비자와 관련하여 의식적 혹은 무의식적으로 또는 적극적·소극적으로 자아이미지를 유지 고양하기 위해 노력하는데 이는 소비자의 구매의사 결정 과정에서 자아이미지가 영향을 미친다는 것을 의미한다. In modern society, there exists a growing tendency that the product image, rather than the substance of the product, is given more and more emphasis. In other words, consumers tend to purchase a product whose brand image coincides with their self image. Today, it is one of the essential factors for marketing strategy of a modern enterprise to have brand image perceived by consumer self-image. The image, by affecting the perception of consumers, explains the reason why some consumers choose one brand out of many brands that are available to them. Furthermore, brand-image perceived by consumers becomes more important factor in improving the company's market share. For this research, we investigated the theoretical background based on previous studies and collected statistical data through a market survey. The results of the actual research indicate that the attributes of the images differ notably depending upon demographic attributes; that the brand image judged by a customer is positively correlated to the customer's self image; and that the regression analysis of the purchase intention in accordance with coincidence of independent variables proved to be insignificant. But the correlation analysis of the variables shows that the sensuous image of a perceived brand image product notable differences in the purchase intention. Most of Consumers tend to prefer certain brand of causal wears in accordance with their cognized brand-image and self-image: social self-image and actual self-image. Corporate must not spare any effort to establish differential advantage through better image or changing image. Also marketers have to identify the strengths and weakness of their brand in relation to the competition in setting up their brand management strategy. In conclusion, the outcomes of the study suggest that the consumers purchase the images of the brands, rather than the products themselves. Accordingly, in the clothes markets with similar quality levels, the emphasis should be put on differentiation of the brand images in order to further increase sales and maximize profits.

하올리브 손상이 순막 조건반응의 파지 및 전이훈련에 미치는 효과

순막조건화에 관련된 신경회로에서 소뇌가 결정적으로 중요한 구조물이라는 실험적 증거가 축적되었으며 소뇌로 들어가는 주된 입력중 하나인 등상섬유가 무조건자극정보를 소뇌로 전달할 것이라는 이론이 제안되었다. 등상섬유는 하올리브에서 기시하며 특히 눈가의 체감각정보를 받는 영역이 하올리브의 문내측 부위이다. 본연구는 순막조건화에서 하올리브의 역할을 규명하기 위하여 소리를 조건자극, 안와주변전기쇼크를 무조건자극으로 사용하여 토끼의 오른쪽눈을 순막조건화시킨 후 문내측 하올리브를 손상시켰다. 2주일간의 회복기간후 재훈련시켰다. 실험결과, 문내측 하올리브가 손상된 피험동물들은 오른쪽눈의 조건반응의 파지 및 재학습이 불가능하였으며 왼쪽눈 역시 학습되지 않았다. 이는 하올리브가 순막조건화에 관련된 신경회로에서 중요한 구조물임을 나타낸다. It has been demonstrated that the cerebellum is a critically important structure for the conditioning of nictitating membrane response(NMR). The aim of the experiment was to determine whether inferior olive is part of a neural circuit which provides unconditioned stimlus(US) information to the cerebellum during NMR conditioning. Rabbits were lesioned on the inferior olive after they were conditioned using tone as conditioned stimulus(CS) and periorbital shock as US. And then 2-week of recovery period was followed by retraining period. Lesions of the rostro-medial portions of the inferior olive(rmIO) abolished the conditioning and prevented subsequent acquisition on either side. The lesion had no effect on the performance of the unconditioned response. Lesions of the lateral portions to and those of dorsal portions above the rmIO did not impair NMR conditioning. The results suggest rmIO is an essential part for the NMR conditioning.

공구흔 동일성 여부 감정기법 개발

Genestone 190 체외충격파쇄석기를 이용한 요로결석 치료의 초기 경험

Shock wave lithotripsy monotherapy was performed in 180 patients with urinary calculi using Genestone 190 lithotriptor between January 1996 and April 1998. Age range of the patients was 17 to 71 years (mean of 44.7 years)and male to female ratio was 2.3:1. There were 54 cases(30%) with renal calculi and 126 cases (70%) with ureteral calculi. NO regional or general anesthesia was required, but parenteral analgesics were required in some patients. The spark voltage was from 16 to 22Kv and average numbers were 2.04 sessions and the mean number of shock per session was 2230±350. The success rates of urinary calculi below 5㎜, 6-10㎜, 11-15㎜, 16-20㎜ and over 21㎜ were 95%, 94%, 91%,75% and 50%, respectively. The success rates of renal, midureter and lower ureter calculi were 85%, 98% and 73%, respectively. There were no significant complications. We think that SWL monotherapy using Genestone 190 lithotriptor is effective and safe procedure for calculi of kidney and upper ureter and calculi below 15㎜.

조혈모세포이식 후 발생한 주폐포자층 폐렴에 대한 고찰

Background : Pneumocytitis cainii pneumonia (PCP) can occur in immunocompromised hosts especially such as AIDS or cancer patients. Although recent research had focused on PCP in AIDS patients, few studies have described the clinical presentations of PCP in recipients of stem cell transplantation (SCT). We evaluated the clinical manifestations of PCP in SCT patients admitted at St. Mary's hospital, Seoul, Korea. Methods : The medical records of 17 PCP patients undergoing SCT between Feb. 1998 and Feb. 2000 were reviewed. The diagnosis of PCP was confirmed through the demonstration of Pneumocytitis cainii via either cytology of brochoalveolar lavage (BAL) or histological technique of lung biopsy. CMV disease and CMV infection were confirmed by BAL culture and antigenemia respectively . Results : Seventeen patients were all recipients of allogeneic SCT and 7 of 17 patients were performed non-sibling SCT. Patients presented with symptoms including brief period (4 ∼23 days) of fever (76%), dyspnea (70%), cough (64%), and signs such as rare(58.8%), Sixteen patients (94%) had been receiving immunosuppressive agent such as cyclosporine A (64%) or Fk506 (35%) without PCP prophylaxis. Eleven patients (64%) were treated with corticosteroid with mean dose of 16 mg/day prednisolone and mean duration of 4.6 months after post-SCT period. Twelve patients were co-infected with CMV. Another co-infected miCroorganisms were Pseudomonas aeruginosa, Mycobacterium tuberculosis, herpes simplex virus, parainfluenza virus, Average duration of treatment with trimethoprim-sulfamethoxazole (TMP/SMX) was 21 ±9 days. Four patients died, and three of them were related with PCP. Conclusion : PCP developed frequently in patients who were taking immunosuppressive drug due to graft versus host disease or were not taking TMP/SMX prophylaxis. High risk patients showing fever, cough, or dyspnea should be considered to take early bronchoscopic intervention for detection of PCP. When treat for PCP, it also be considered to the possibility of coinfection such as CMV. (Korean J Infect Dis 33:273∼279, 2001)

무증상의 현미경적 혈뇨의 비뇨기과적 고찰

We intended to know whether all of the patients with asymptomatic microscopic hematuria should be urologically investigated routinely. We reviewed the medical records with asmptomatic microscopic hematuria from January 1995 to December 1997. Urological studies included intravenous urography or ultrasonography, urine cytology, cystoscopy and biopsy, urine AFB smear and culture and urine PCR for AFB. The age with patients ranged from 9 to 74 years(mean age: 39.7 years). Benign diseases were found in elderly patients and patients with high degree of hematuria, but there was no patients who had urological malignant disease. According to the results, we suggest that follow-up with urine cytology is sufficient to younger patients or low degree of hematuria with asymptomatic microscopic hematuria.

발기부전 검사에서 천연색 복합초음파촬영술과 야간음경발기검사의 일치도

We intended to know that the result of color duplex ultrasonography corresponded to that nocturnal penile tumescence rigidity in the diagnosis of erctile dysfunction. We performed both color duplex ultrasonography and nocturnal penile tumescence rigidity in 15 erectile dysfunction men. Colr duplex ultrasonography was done with a 7 MHz. color Doppler unit after intracorporeal pharmacological injection and measured maximal arterail diameter, peak systolic velocity, end-diastolic velocity and the results were compared with rigidity and tumescence in nocturnal penile tumescence rigidity using RigiScan. The positive predictive value, negative predictive value, sensitivity, specificity, overall accuracy of color duplex ultrasonography was reasonable when the result of color duplex ultrasonography was compared with the that of nocturnal penile tumescence rigidity. According to this results, color duplex ultrasonography could not only predict but also correspond to nocturnal penile tumescence. Therefore, we suggest that color duplex ultrasonography can relpace nocturnal penile tumescence rigidity if psychogenic and neurogenic erectile dysfunction are distinguished by history and neurologic examination.
