Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]

Objective To compare a newly developed minimally-invasive method for percutaneous transforaminal epidural injection (INJ group) with the existing method for lumbar epidural catheterization (CATH group).Method Through anatomical review of experimental rats, the cephalic one fourth of the neural foramen was selected as the target point for drug delivery. After the rats had undergone lumbar epidural catheterization, lidocaine, and 1% methylene blue were injected through the unilateral or bilateral L5/6 neural foramen in the INJ group, and through an epidural catheter in the CATH group. Measurement of body weight and the mechanical allodynia test before and after injection of lidocaine, and fine dissection after injection were performed.Results Results of the mechanical allodynia test of 1.0% lidocaine 50 ml injection in the CATH group were statistically similar to those of 0.5% lidocaine 100 ml injection in the INJ group. The results of 2.0% lidocaine 50 ml injection in the CATH group were statistically similar to those of 1.0% lidocaine 100 ml injection in the INJ group. After dissection, only one distal partial spinal nerve was stained by methylene blue 50 ml through the transforaminal pathway. However, the dorsal root ganglion, nerve root, and adjacent hemi-partial spinal cord were stained by methylene blue 100 ml through the transforaminal pathway.Conclusion The percutaneous transforaminal epidural injection is practical, easy, and safe, and, in particular, does not cause significant pain compared to the existing lumbar epidural catheterization. We expect this method to be effective in an animal study showing that drug delivery to the spinal epidural space is necessary.

Ulnar neuropathy at the wrist is rarely reported as complications of carpal tunnel release. Since it can sometimes be confused with recurrent median neuropathy at the wrist or ulnar neuropathy at the elbow, an electrodiagnostic study is useful for detecting the lesion in detail. We present a case of a 51-year-old woman with a two-week history of right ulnar palm and 5th digit tingling sensation that began 3 months after open carpal tunnel release surgery of the right hand. Electrodiagnostic tests such as segmental nerve conduction studies of the ulnar nerve at the wrist were useful for localization of the lesion, and ultrasonography helped to confi rm the presence of the lesion. After conservative management, patient symptoms were progressively relieved. Combined electrodiagnostic studies and ultrasonography may be helpful for diagnosing and detecting ulnar neuropathies of the wrist following carpal tunnel release surgery.

Erythema elevatum diutinurum is a relatively rare disease of unknciwr etiology. It is charaterized by persistent erythematous plaque and nodule that are usually affected symmetrically over extensor surface of extremities. Our patient, a 16-year-old female was referred for evaluation of asy nptomatic papules and nodules on extensor aspects of the hands, legs and buttocks. She complained of arthralgia on the knees. The laboratory values were all within normal limits with the exception of slight elevation of white blood cell counts and erythrocyte sedimenation rate. A skin biopsy specimen we obtained from the nodular lesion of the leg that showed preclominantly perivascular neutrophilic influration with occasional lymphocytes and unclear dusts. She was treated with 75mg/day of dapsone. Shin lesions were much improved after ten days. Eight weeks later, the lesion had almost resolved. After the discontinuation of dapsone, new lesions developed on the legs. Retreatment with dapsone resulted it a dramatic response of her lesions after three weeks. The nucleotide sequence of the β-glucosidase gene, coding for the β-glucosidase of Pseudomonas sp. LBC505 was determined. An open reading frame identified could encode a protein of Mr 38,5000, which is close to the size(about 40,000) of the polypeptide determined by SDS-gel electrophoresis of crude enzyme. Analysis of the amino-terminal residues of the protein produced in Escherichia coli suggested that it had the signal peptide processed by a signal peptidase. A sequences within the upstream of the β-glucosidase gene were found act as a promoter and a Shine-Dalgarno(SD) sequence for expression vectors, pKK223-3 and pASI, which were regulated by IPTG and temperature respectively. Under the maximum conditions the recombinant β-glucosidase genes, pKKB10 and pASB20, were expressed 8 and 6.2-fold higher levels of β-glucosidase activity than pGL 1, respectively.
SiC 휘스커 보강 Al 복합재료의 미세조직과 기계적 특성에 관한 연구

This study aims at establishing the correlation of microstructure and mechanical properties of 2124-T6 Al composites reinforced with 7, 15, 20 v/o SiC whiskers. Tensile tests along with Charpy impact tests were conducted over the test temperature range from -196℃ to 100℃. Notched round tensile tests and subsequent metallographic observations as well as in-situ SEM observation of crack opening processes were performed for the purpose of identifying the micro-mechanical processes involved in void initiation. The experimental results indicated that the impact energy was found to increase monotonically with decreasing the amount of SiC whiskers and with inceasing the test temperature. Microvoids were observed to nucleate at the interface between the aluminum matrix and SiC whikskers, confirming that the formation of voids at the interfaces is the main metallurgical factor which contributes to the reduction of fracture toughness. In addition, the Mn-containing intermetallic particles strongly affect the fracture processes in these composites.
주조 A356.0 Al-SiC 복합재료의 기계적 성질과 파괴과정에 대한 공정 Si 입자의 영향

In this study an attempt has been made to improve mechanical properties of a cast aluminum matrix composite. Detailed microstructural analyses of a cast A356.0 Al-SiC composite indicated that SiC particulates and eutectic Si particles were segregated to the intercellular regions. The presence of these eutectic Si particles resulted in cleavage fracture along the intercellular regions, which deteriorated mechanical properties of the cast composite. In-situ SEM fracture observations clearly showed that eutectic Si particles were cracked first before the matrix-SiC decohesion or the SiC breakage occurred. In order to modify the distribution of eutectic Si particles the cast composite was re-cast with adding small amount of Sr. Tensile properties of the re-cast composite with 0.03wt.% Sr were significantly improved. The improvement of mechanical properties is attributed to a more homogeneous distribution of SiC and eutectic Si particles due to the addition of Sr and recasting.
급냉응고 Al-Li-X (Zr, Cu, Mg) 합금에서 첨가 합금원소가 미세조직 및 기계적 성질에 미치는 영향

A study was made of the effects of alloying elements on the structure and mechanical properties of high Li($gt;3wt%) containing RSP Al-Li-X(X=Zr, Cu, Mg) alloys. Particular emphasis was placed on the role of Mg and Cu, which are the major alloying elements in ingot metallurgy (IM) Al-Li alloys. RSP Al-Li-X alloys with several alloy compositions were subjected to various thermal treatments without any pre-aging cold working such as stretching. It has been found that, in the case of plain Al-Li-Zr alloy, the deformation behavior and the resultant mechanical properties are mainly controlled by the presence of uniformly distributed composite Al₃Li/Al₃Zr precipitates. Composite precipitates are also the major strengthening precipitates which control the deformation behavior of the alloys containing Cu and Mg. Addition of Cu and Mg to Al-Li-Zr alloys, when subjected to thermal treatment without any mechanical working, do not promote the precipitation of other strengthening phases such as T₁ and S' as expected ; however, they have a very important effect on increasing the ultimate tensile strength of Al-Li-Zr alloys due to their solid solution strengthening effects. Comparison of the present results with those of IM Al-Li alloys indicates that optimum mechanical properties can be achieved in high Li and Zr containing Al-Li alloys by simple thermal treatment without stretching, and this is due to the presence of uniformly distributed composite precipitates.
