        부산·울산·경남지역 직업병 감시체계

        Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.

        The ring expansion chemistry is one of the most important methods to prepare five-and six-membered heterocylic compounds. We have studied about γ-lactam ring expansion with stereoselectivity from β-lactam derivative. To do this chemistry, proton of N-benzyl group in β-lactam derivatives is eliminated by LDA and make carbanion.This anion make unstable 4-membered lactam intermediate, and then the ring is opened by homo clevage or hetero cleavage for releasing ring strain, followed by recyclization to one carbon to make γ-lactam ring with stereoselectivity in β-, γ- positions.

        INVERTED MATTRESS SUTURE를 이용한 선천성 이개연골 변형의 교정

        There are many kinds of congenital auricular deformities but some congenital deformities have no lack or excess of cartilage, which are mild to a moderately constricted ear, cryptotia and Stahl's ear etc. Even if some kinds of surgical treatments, such as cartilage graft, cartilage turn over and cartilage flap were reported for the correction of them but they were comlex and sometimes unsatisfactory. The method of inverted mattress suture to correct the cartilage deformities is not a new method and some authors reported it previously to correct ear cartilage deformities. Authors have tried to correct several kinds of congenital ear deformities, 2 constricted ears, 5 cryptotias, 2 stahl's ears. 1 satyr ear by using of simple inverted cartilage mattress suture and added Z-plasty or V-Y advancement to corrcet the auricular skin in each cases, respectively. The result was promising without recurrences during average 3 months folloe up periods. This method is immediative, simple, easy to perform and a very useful procedure for the correcton of congenital ear auricular cartilage deformities.

        실내사무환경의 환경성담배연기(ETS)의 지표물질에 관한 연구

        The purpose of this research is to evaluate indoor office environment using the concentrations of nicotine, 3-enthenyl-pyridine(3-EP), and respirable suspended particulate(RSP), which are indicators for environmental tobacco smoke(ETS) and the correlations between indicators and environmental conditions(smoking density, smoking index). The mean air charge per hour (ACH) in smoking rooms was 10.4 and most of the smoking rooms showed non-compliance withe ASHRAE standard value except only one smoking rooms. The concentrations of RSP, 3-EP, nicotine showed log=normal distributions, and became different statistically depending on smoking condition(p〈0.01). The geometric mean concentration of RSP in smoking room was 441.7 ug/㎡ that is far exceeded environmental standard(150 ug/㎡). This implies that fine particulate in smoking room should be carefully controlled considering smoking density and ventilation fate. The mean concentrations of nicotine and 3-EP were 93.4 ug/㎥, respectively. The correlation coefficients between RSP and SI, 3-EP and SI, and Nicotine and SI were 0.67, 0.84 and 0.74, respectively. The correlation coefficient between nicotine and 3-EP, nicotine and RSP, and RSP and 3-EP were 0.76,0.78 and 0.57 respectively.

        사용자 수술명과 수술분류 code(ICD-9-CM) 일치율 향상에 관한 연구 : 수술실 OCS program 사용 활성화를 통하여

        Background : The necessity of unify and standardize codes used at hospital has been emphasized since OCS (Order Communicating System) was adopted. Therefore, the purpose of this study were to standardize operation code by continuous training of the ICD-9-CM code that is used as standard code in OCS program at operating room. Method : In 400 operation data, operation code entered in OCS program at operating room was compared to operation name recorded in medical record. In addition, a matching rate between input data of operation code by medical record department and computing input data of operation code in 3,710 cases was compared for each department. User operation name and operation code were matched and major diagnosis by operation department and operation name were also matched. Results : User operation name was reflected in operation classification code in detail, and operation code entered on user was registered. Input rate and matching rate of operation code were gradually improved after improvement activity. In particular, a matching rate was high at ophthalmology where operation name is segmented. Plastic surgery and orthopedics with a lot of emergency operation and comprehensive operation name show low input rates. Conclusions : As the medical field makes progress in computerlization, awareness of information exchange and sharing becomes higher. Among codes to classified medical institution, codes related to surgical operation are all different by user of hospital and department. Computerlization and standardization is essential. And when efforts of standardization continue in alliance with individual hospital and institution, initiative of preparing medical policy data at a national level will be accelerated.

        중학교 수학 교과서에서 일차함수와 상관관계의 내적 외적 연결성 증대를 위한 제언

        통계학은 응용수학으로서, 중등 교과에서 이를 소개할 때에, 적어도 해당 교과 내용의 실용적 목적을 알 수 있게 해 주어야 한다. 현재 중학교 수학 9-나 단계에서 소개되는 상관관계 단원은 상관관계의 유무와 종류, 강약만을 상관도를 이용하여 소개하고 있다. 이런 시각적 방법에만 의존하여 상관관계를 지도하는 것의 위험성은 이미 너무 잘 알려진 것이지만 국내의 교과서에는 반영되어 있지 않다는 문제점에도 불구하고, 본 연구에서는 그 문제보다는 상관관계란 무엇이고, 그것을 아는 것이 왜 필요한지, 상관관계를 알아서 어떤 실용적 이득이 있는지도 함께 지도되어야만 상관관계 지도의 의미가 분명해지고 나아가 이 과정에서 8-가 단계의 일차함수 단원과 상관관계 단원의 외적 연결성을 강화시켜 주어 일차함수 단원의 실용적 목표도 더욱 분명해 질 수 있다고 보고, 이를 외국의 교과서와 국내 수학 지도의 현장 전문가인 교사들의 의견을 중심으로 살펴보았다. 우선 미국의 경우 8학년 교재인 Glencoe Algebra1 과 Algebra2에서는 상관도(scatter plot)와 최적합 직선(best-fit line)에 대해 두 점을 지나는 직선의 식 또는 계산기를 이용한 최소제곱법에 의한 직선의 식 등을 관측값과 예측값 사이의 편차라는 관점에서 관찰 의논하도록 소개하고 있다. 또 영국의 경우도 Key Stage3에서 유사한 내용을 소개하고 있다. 국내외 교과서의 이런 차이에 대한 교사들의 의견은 다음과 같았다. 우선 목표 타당성 측면에서는 현재보다 실용성 측면에서 목표가 더 구체적이어야(약 93%)하고, 그래서 현재의 상관도, 상관표보다 더 실용적 내용이 필요하다(58%), 상관도는 일차함수와 연관되어 있으므로(86%), 상관도에서 일차함수 학습의 실용성을 알려주기 위해(59%) 직선의 식을 얻어 x값을 알 때 y값을 예측해 보는 생각이 필요하다(93%)고 답하였으며, 이 때 상관도에 적절한 직선의 식을 구하는 방법으로는 그냥 눈으로 보고(49%) 구하거나 미국이나 영국처럼 계산기, 컴퓨터 등을 이용하여 구하자(40%)는 의견 등이 많았다. 이상의 연구 결과를 토대로 일차함수와 상관도의 수학적 연결성을 고려한 지도 방안에 대하여 제언을 한다면 다음과 같다. 첫째, 상관도의 상관관계 지도는 일차함수의 식과 연결하여 가르칠 필요가 있으며 학생들에게 자료를 분석함에 있어서‘상관관계가 있다 없다’에만 국한하지 말고 자료를 예측할 수 있도록 일차함수를 활용할 수 있어야 한다. 둘째, 상관도에서 일차함수 식을 얻는 과정은 손으로도 구할 수 있지만 가능하면 컴퓨터나 계산기를 이용할 수 있게 한다. 셋째, 상관관계 단원의 학습 목표를 ‘상관도가 무엇인지 안다’와 같은 지식 중심 목표보다는 ‘상관관계의 유무를 조사하는 목적을 알게 하고 두 변량 사이의 관계를 식으로 나타낼 수 있게 하며, 이 식을 이용하여 문제를 해결 할 수 있다’와 같은 실용적 목표, 문제 해결 중심의 목표로 전환할 것을 제안한다. 넷째, 이상의 제안은 외국 교과서와 현장 전문가의 의견에 근거하였으나, 이 제안의 적절성은 결국 교수학습적 측면에서 실험을 통해 검증되어야 한다. 이를 위한 추가의 연구가 필요하다. Topics in correlation are introduced and terminated at the 9th level in the Korean school math curriculum. The concept of it is introduced via scatter gram and existence of it, positive or negative type of it and its strength have been discussed in sequence. However the aim of the correlation analysis and its practical usefulness are not found in those textbooks. It is well known that the objective of correlation analysis is to use the partial informations to predict or to make decisions. As a result, once the existence of correlation being detected, it is natural to discuss how to make use of the insufficient informations to make decisions. As compared to the United Kingdoms or to the United States, we found that only the Korean curriculum does not contain the subject, say fitting equation to data. We asked these to Korean highschool math teachers. And they supported to teach this subject to school kids. They believe that this innovation may result in stronger connections between the correlation chapter and the linear function chapter and also believe that this would make clearer the purpose of correlation chapter. We would like to suggest some further developmental studies on these to see if this kind of innovations may fit to Korean students' cognitive level.

        The purpose of this study is to examine the condition of the transferred site in Daegu. The conditions of the case study areas were investigated with the change of use, the density and the infrastructure in the survey area. The results of this study are as follows ; First, the transferred site is developed rapidly into high-density without urban planning control. So, it causes the lack of infrastructure capacity. Second, park and green space is required because most of residual space is used as a parking lot. Third, as high-rise apartment buildings were constructed in transferred site, the conditions around that areas get worse. So, the development of transferred site has to be harmonized with its surroundings.

      • 자연냉방시스템(Passive Cooling System)의 원리와 활용 방법

        The purpose of this study is to review the principle and application device in the passive cooling system. The result is as follows ; 1. The Passive cooling system is defended by cooling system to produce thermal comfort zone for occupant without mechanical aid. The methods of there are composed of 5 elements comfort ventilation, radiant cooling, evaporation cooling, earth cooling, removal of latent heat. 2. The historical meaning of the passive cooling system appeared through the vernacular architecture in the world. 3. The Passive cooling system is influenced by the principle of the flow. The primary factor of the system is the natural convective phenomenon, which is varied according to the pressure and Bernouii Effect. Therefore, the building site and the form of the building, that is, The position of the openings like window(door), the solid overhang, the type of partition, and the space planning etc.-are deeply related with cooling system. 4. The Passive cooling system is prominent in air flow diagram device to form the comfort ventilation.
