Aims: The prognosis of hepatocellular carcinoma (HCC) with portal vein tumor thrombus (PVTT) is very poor. According to the BCLC treatment recommendations, sorafenib or other palliative treatment (PT) is recommended as the first-line therapy when it happens. In real world, however, a significant number of selected patients with HCC and PVTT suffered from surgical resection (SR). Methods: PubMed, Embase, Medline and Cochrane library were searched for studies comparing SR with PT (including TACE, sorafenib, etc.) for HCC with PVTT, which were published before September 2017. Results: 4,810 patients from 7 studies were enrolled in this meta-analysis, which divided into the SR group (n = 2,344) and the PT group (n = 2476). When compared with the PT group, the pooled hazard ratio (HR) for the 1, 3 and 5-year OS rates of the SR group were 0.56 (95% CI 0.52-0.60, P=0.03), 0.56 (95% CI 0.53-0.59, P<0.001) and 0.55 (95% CI 0.54-0.57, P<0.001). For subgroup analysis, when compared with the mere TACE group, the pooled HR for the 1, 3 and 5-year OS rates of the SR group were 0.54 (95% CI 0.43-0.67, P=0.81), 0.75 (95% CI 0.65-0.87, P=0.25) and 0.76 (95% CI 0.67-0.88, P=0.25). Conclusions: This meta-analysis demonstrated SR had better OS than TACE or other palliative therapy for HCC with PVTT. SR may be suitable as the first-line treatment for selected patients with resectable HCC and removable PVTT.

Lei Shi,Xue-Liang Dang,Xin-You Liu,Hua-Mei Wei,Meng-Meng Yang,Yan Zhang 대한약학회 2014 Archives of Pharmacal Research Vol.37 No.12
Carbamazepine (CBZ), an antiepilepticwith narrow therapeutic window, is a substrate ofCYP 3A4 which metabolizes CBZ to carbamazepine-10,11-epoxide (CBZE). CBZE is an active and toxicitymetabolite, and it is a substrate of MRP-2. UsingCBZ for a long time can cause hepatic injury. Sophora flavescens (SF) is a medicinal herb used forthe protected hepatic injury. This study investigatedthe acute and chronic effects of SF on the pharmacokineticsof CBZ in rats. The concentrations of CBZand CBZE in plasma and tissues were determined byHPLC method. The results showed that SF whichsignificantly decreased the AUC0-t of CBZ, increasedCBZE conversely. Tissue analysis showed that theconcentrations of CBZ and CBZE in brain and liverwere decreased by SF. In addition, the distribution ofCBZE in kidney was reduced significantly, whichinfluenced the CBZE excretion and increased the drugtoxic potentially. Results in the current study suggestthat patients using CBZ might be cautioned in the useof SF extract or Sophora-derived products. Meanwhile, patients receiving drugs which are substrates ofCYP 3A4 and/or MRP-2 should be advised of thepotential herb–drug interaction to reduce the risk oftherapeutic failure or increased toxicity of conventionaldrug therapy.
Probing the nature of high‐<i>z</i> short GRB 090426 with its early optical and X‐ray afterglows

<P><B>ABSTRACT</B></P><P>GRB 090426 is a short‐duration burst detected by <I>Swift</I> (<IMG src='/wiley-blackwell_img/equation/MNR_17419_mu1.gif' alt ='inline image'/> s in the observer frame and <IMG src='/wiley-blackwell_img/equation/MNR_17419_mu2.gif' alt ='inline image'/> s in the burst frame at <I>z</I>= 2.609). Its host galaxy properties and some gamma‐ray‐related correlations are analogous to those seen in long‐duration gamma‐ray bursts (GRBs), which are believed to be of a massive star origin (so‐called Type II GRBs). We present the results of its early optical observations with the 0.8‐m Tsinghua University–National Astronomical Observatory of China Telescope (TNT) at Xinglong Observatory and the 1‐m LOAO telescope at Mt Lemmon Optical Astronomy Observatory in Arizona. Our well‐sampled optical afterglow light curve covers from <IMG src='/wiley-blackwell_img/equation/MNR_17419_mu3.gif' alt ='inline image'/> to 104 s after the GRB trigger. It shows two shallow decay episodes that are likely due to energy injection, which end at <IMG src='/wiley-blackwell_img/equation/MNR_17419_mu4.gif' alt ='inline image'/> and 7100 s, respectively. The decay slopes after the injection phases are consistent with each other (<IMG src='/wiley-blackwell_img/equation/MNR_17419_mu5.gif' alt ='inline image'/>). The X‐ray afterglow light curve appears to trace the optical, although the second energy‐injection phase was missed due to visibility constraints introduced by the <I>Swift</I> orbit. The X‐ray spectral index is <IMG src='/wiley-blackwell_img/equation/MNR_17419_mu6.gif' alt ='inline image'/> without temporal evolution. Its decay slope is consistent with the prediction of the forward shock model. Both X‐ray and optical emission are consistent with being in the same spectral regime above the cooling frequency (<IMG src='/wiley-blackwell_img/equation/MNR_17419_mu7.gif' alt ='inline image'/>). The fact that <IMG src='/wiley-blackwell_img/equation/MNR_17419_mu8.gif' alt ='inline image'/> is below the optical band from the very early epoch of the observation provides a constraint on the burst environment, which is similar to that seen in classical long‐duration GRBs. We therefore suggest that death of a massive star is the possible progenitor of this short burst.</P>

Purpose Chronic lymphocytic leukemia (CLL) is one of the most frequent type of B-cell chronic lymphoproliferative disorders and chronic inflammation takes part in the development of CLL. However, there has been no valid immune biomarker to predict the prognosis of untreated CLL patients. Materials and Methods In this retrospective study, we analyzed the clinical correlations and prognostic value of albumin-to-fibrinogen ratio (AFR) detected at diagnosis in 191 CLL patients. Results The cut-off value of AFR was 9.7 calculated by X-tile. Patients who were more than 65 years old were often accompanied by low level of AFR (p < 0.001). Survival analysis showed that patients with low level of AFR had shorter overall survival (OS) than patients with high level of AFR (p < 0.001). Multivariate analysis illustrated that AFR had a negative impact on OS (p=0.003) and was independent of parameters involved in CLL international prognostic index and other prognostic markers such as CD38 and ZAP-70. Conclusion These data provide a comprehensive view of AFR and shows that AFR at diagnosis is an adverse prognostic factor in untreated CLL patients.

Background: This study was performed to analyze the prognostic implications of pretreatment or preoperative thrombocytosis in women with gynecologic malignancies. Material and Methods: We surveyed 2 medical databases, PubMed and EMBASE, to identified all relevant studies. A total of 14 (n=3,490) that evaluated the link between thrombocytosis and 5-year survival were included. REVMAN version 5.1 was used for our analysis and publication bias was evaluated using the Begg's funnel plot and tested by STATA 11.0. Risk ratios (RRs) with 95% confidence intervals (CIs) generated by the random effect model were used to assess the strength of any association. Results: 709(20.3%) of the 3,490 patients exhibited thrombocytosis (platelet counts > $400{\times}10^9/L$) at primary diagnosis, and their mortality was 1.62-fold higher compared with the others (RR=1.62, 95%CI=[1.28-2.05], p<0.0001). Thrombocytosis failed to have a stronger effect on the survival of advanced patients of stages III to IV in our study (n=478, RR=1.29, 95% CI=[1.13-1.48], p=0.0003), nor in women with cervical cancer in stage IB (n=1371, RR=1.73, 95% CI=[1.71-2.58], p=0.007). In addition, when adjusted for different carcinoma, it was associated with worse prognosis for all except the ones with vulvar cancer (n=201, RR=0.43, 95% CI=[0.14-1.29], p=0.13). Conclusions: This meta-analysis indicated that thrombocytosis might be associated with a worse prognosis for patients with gynecologic malignancies but without specificity or sensitivity for the ones in advanced stage. When adjusted for different gynecologic malignancies, it showed a significant effect on survival of all except vulvar cancers.

Background: Among the various ginseng strains, Shizhu ginseng is endemic to China, mainly distributed in Kuandian Manchu Autonomous County (Liaoning Province, China); however, not much is known about the compounds (especially saponins) in Shizhu ginseng. Methods: A rapid, sensitive, and reliable ultra-high performance liquid chromatography coupled with MS/MS (UHPLCeMS/MS) method was developed to separate and identify saponins in Shizhu ginseng. Results: The separation was carried out on a Waters ACQUITY UPLC BEH C18 column (100 mm × 2.1 mm, 1.7 ㎛) with acetonitrile and 0.1% formic acid aqueous solution as the mobile phase under a gradient elution at 40℃. The detection was performed on a Micromass Quattro Micro API mass spectrometer equipped with electrospray ionization source in both positive and negative modes. Under the optimized conditions, a total of 31 saponins were identified or tentatively characterized by comparing retention time and MS data with related literatures and reference substances. Conclusion: The developed UHPLCeMS/MS method was suitable for identifying and characterizing the chemical constituents in Shizhu ginseng, which provided a helpful chemical basis for further research on Shizhu ginseng.

The main pathological mechanism of intervertebral disc degeneration (IVDD) is the programmed apoptosis of nucleus pulposus (NP) cells. Oxidative stress is a significant cause of IVDD. Whether mitophagy is induced by strong oxidative stress in IVDD remains to be determined. This study aimed to investigate the relationship between oxidative stress and mitophagy and to better understand the mechanism of IVDD in vivo and in vitro. To this end, we obtained primary NP cells from the human NP and subsequently exposed them to TBHP. We observed that oxidative stress induced mitophagy to cause apoptosis in NP cells, and we suppressed mitophagy and found that NP cells were protected against apoptosis. Interestingly, TBHP resulted in mitophagy through the inhibition of the HIF-1α/NDUFA4L2 pathway. Therefore, the upregulation of mitochondrial NDUFA4L2 restricted mitophagy induced by oxidative stress. Furthermore, the expression levels of HIF-1α and NDUFA4L2 were decreased in human IVDD. In conclusion, these results demonstrated that the upregulation of NDUFA4L2 ameliorated the apoptosis of NP cells by repressing excessive mitophagy, which ultimately alleviated IVDD. These findings show for the first time that NDUFA4L2 and mitophagy may be potential therapeutic targets for IVDD.
Optical Characters Measurement and Simulation of 2D Cross Grating Microstructures of Butterfly Wings
Wang Zhang,Di Zhang,Wei-Xin Chen,Jia-Jun Gu,Qin-Lei Liu,Shen-Ming Zhu,Hui-Lan Su,Chuan-Liang Feng,문원진 한국정밀공학회 2012 International Journal of Precision Engineering and Vol. No.
In this paper, the reflectance of the butterfly wings are measured by using the microspectrophotometer as the optical character with the reflectance on several scales on the wings. Reflectance spectra from different wing scale zones with the same color are dramatically different. And it suggests that the tiny structural difference has the great influence on the reflectance property. In addition, the microstructures of the butterfly wings can be simplified to a 2D crossing grating. By simulating a two-dimensional model using rigorous coupled-wave analysis technique, the optical properties of the butterfly wings were investigated. The simulation results depend strongly on the structural parameters and refractive index. The research will reveal the mechanisms of the structural color of 2D crossing grating butterfly wings.
