        상아질접착제에 대한 광조사가 접착에 미치는 영향

        The purpose of this study was to investigate the influence of light irradiation over self-priming adhesive on dentin bonding. After acid etching the exposed dentin, a self-priming adhesive (Prime&Bond□NT dental adhesive system, Dentsply DeTrey, GmbH, Konstanz, Germany) was applied and light irradiation was done for 20 sec with regular intensity (600 mW/㎠) in group I and for 3 sec with ultra-high intensity (1930 mW/㎠) in group III. No light irradiation was done over self-priming adhesive in groups II and IV. Composite resin was added on the self-priming adhesive and irradiated for 40 sec with regular intensity (600 mW/㎠) in groups I and II and for 3 sec with ultra-high intensity (1930 mW/㎠) in groups III and IV. To see the effect of light curing time on dentin bonding, another 3 group specimens were prepared. Without light-irradiation over self-priming adhesive, added composite resin was irradiated for 3, 6, or 12 sec with ultra-high intensity light. After bonded specimens were stored in 37℃ distilled water for 24 hours, shear bond strength were measured using a universal testing machine (4202, Instron, Instron Co., U.S.A.) and fractured surfaces were examined under a stereomicroscope (SZ-PT Olympus, Japan) . Statistical analysis were done with one-way, two-way ANOVA and chi-square test. The results were as follows: 1.The shear bond strengths from the groups irradiated over self-priming adhesive were significantly higher than those from the groups without irradiation (p<0.05). 2.There was no significant shear bond strength difference between regular intensity light irradiation groups and ultra-high intensity ones (p>0.05). 3. There was no significant shear bond strength difference among various irradiation time groups with ultra-high intensity ones (p>0.05). 4.In stereomicroscopic examination of fractured surfaces, adhesive-cohesive mixed failure mode was mostly seen in all groups, and there was no significant difference in failure mode among groups (p>0.05) .         하악골 결손부의 골형성에 미치는 Bioplant HTRⓡ의 영향

        The present study was performed to investigate the effect of HTR□ (Hard Tissue Replacement) on osteogenesis in the mandibular bone defects. Eight adult male white rabbits weighing 2.5 to 3.0kg were used. Four bone defects (8mm in diameter and 4mm in depth) were made at the both mandibular body. In the control group, the right mesial bone defect was filled with blood clot and spontaneously healed, In the DFDB group, the right distal bone defect was filled with xenogenic demineralized freeze-dried bone. In the HTR□ group, the left mesial bone defect was filled with HTR□. In the HTR□-membrane group, the left dis- tal bone defect was filled with HTR□ and covered with BioMesh membrane. The rabbits were sacrified at 2,4,6 and 9 weeks after the operation and microscopic examination was performed. Results obtained were as follows: In the control and DFDB groups, inflammatory cells and the fibrous connective tissue existed and the bone growth was slower than HTR□ group by 6 week, and there was intervention of the soft tissue at 9 week. In the HTR□ group, bone trabeculi extended between the HTR□ particles without intervention of inflammatory cells and the connective tissue at 4 and 6 weeks. In addition, extensive osseous ingrowth into the HTR□ particles was observed at 9 week. Bone formation was more active in the HTR□ group than the control and DFDB groups. There was not obvious difference in the bone healing rate between the HTR□ and the HTR□-membrane group. These results suggest that the HTR□ promotes osteogenesis in the bone defects and the HTR□ group has no difference in comparison with the HTR□-BioMesh□ membrane group in bone healing.

        The incidence of double primary cancer is about 10% in all kinds of cancers. Double primary cancer is usually accompanied by gastrointestinal cancer. Esophageal cancer is supposed to be frequently accompanied by squamous cell carcinoma of the head and neck or the upper respiratory tract, because of such common carcinogens as smoking and alcohol ingestion. Esophageal cancer and hypopharyngeal cancer are usually diagnosed by esophagogastric endoscopy with biopsy. We performed endoscopic ultrasound-guided Trucut biopsy(EUS-TCB) to diagnose a mediastinal mass in a 49 year-old patient, who visited our hospital because of voice change. During procedure, we accidently found enlarged celiac lymph node, and obtained tissue from the mediastinal lymph node and the celiac lymph node. We confirmed double primary cancer of squamous cell carcinoma in the esophagus and the hypopharynx via esophagogastric endoscopy.

      • 방사선 치료를 받는 암 환자들의 사회적 지지와 삶의 질과의 관계

        Objective : This study was performed to investigate the relationship between social support and quality of life among cancer patients receiving radiation therapy. Matehals and Methods : The data were collected from 98 patients, who were receiving radiation therapy at two university hospitals located in GwangUJu, used by structured questionnaire. For statistical analyses of the association between quality of life and various characteristics, data were analyzed using t-test, ANOVA, Pearson 's correlation, and multiple regression analysis. Results: 1. There were 56(57.1%) males and 42(42.9%) females. Age ranged from 21 to 82 years. The primary sites of cancer were gastrointestinal tract (24.5%), lung (23.5%), breast (21.4%), and head and neck (11.2%) in order. 2. The mean scores of social, family, and medical support were 4.30 0.58, 4.49 0.78, 4.11 0.65, respectively. The score of quality of life was 5.83 1.63 (range: 1.95 ~ 9.05). 3. An analysis of the association between several factors of patients and quality of life showed that the statistically significant factors were age, the presence of distant metastasis, family support, medical support and social support. 4. As a result of the multiple regression analysis, only social support was significant (β=0.932, P=0.02) with quality of life, but age and presence of distant metastasis were not significant. Conclusion : This suggests that quality of life in cancer patients could be improved by strengthening the social support which consists of family and medical support. Further study would be necessary to evaluate separately several aspects of quality of life among cancer patients.

        정중운동신경과 척골운동신경의 전기생리학적 연구

        The determination of peripheral nerve conduction velocity is and important part to electrodiagnosis. Its value as neurophysiologic investigative procedure has been known for many years but normal value of median and ulnar motor nerve was poorly reported in Korea. To evaluate of median and ulnar motor nerve terminal latency, amplitude of CMAP(compound muscle action potential), conduction velocity and F-wave latency for obtain clinically useful reference value. 71 normal volunteers(age,19-65 years;142 hands) examined who has no history of peripheral neuropathy, diabetic mellitus, chronic renal failure, endocrine disorders, anti-cancer medicine, anti-tubercle medicine, alcoholism, trauma, radiculopathy. Nicolet Viking Ⅱ was use for detected terminal latency, amplitude of CMAP, conduction velocity and F-wave latency of median and ulnar motor nerve. Data analysis was performed using SPSS. Descriptive analysis was used for obtain mean and standard deviation, independent t-test was used to compare between Rt and Lt side also compare between different in genders. The results are summarized as follows: 1. Median motor nerve terminal latency was right 3.00ms, left 2.99ms and there was no significantly difference between right and left side and genders. 2. Median motor nerve amplitude of CMAP was right 17.26mV, left 17.50mV and there was no significantly differences between right and left side and genders. 3. Median motor nerve conduction velocity was right 57.89m/sec, left 58.03m/sec and there was no significantly difference between right and left side and genders. 4. Median motor nerve F-wave latency was right 25.74ms, left 25.59ms and there was significantly differences between genders. 5. Ulnar motor nerve terminal latency was right 2.38ms, left 2.45ms and there was significantly differences between right and left side. 6. Ulnar motor nerve amplitude of CMAP was right 15.99mV, left 16.02mV and there was no significantly differences between right and left side and genders. 7. Ulnar motor nerve conduction velocity was right 60.35m/sec, left 59.73/sec and there was no significantly differences between right and left side and genders. 8. Ulnar motor nerve F-wave latency was right 25.53ms, left 25.57ms and there was significantly differences between genders.

        척골 지단 신경의 전기생리학적 연구

        The ulnar nerve extends down the arm, across the elbow, and into the hand. It provides sensation to the little and ring fingers and activates many of the small muscles in the hand. The determination of peripheral nerve conduction velocity is an important part of ulnar nerve evaluation. The electrodiagnostic value as neurophysiologic investigative procedure has been known for many years but normal value of digital nerve was not reported in korea. The purpose of this investigation was to measure the digital nerve conduction velocity of ulnar nerve for obtain clinically useful reference value and compare difference in each fingers and then compare with the other countries. 71 normal Korean volunteers (age, 19-65 years; 142 hands) examined who has no history of peripheral neuropathy, diabetic mellitus, chronic renal failure, endocrinedisorders, anti-cancer medicine, anti-tubercle medicine, alcoholism, trauma, radiculopathy. Nicolet Viking Ⅱ(EMG machine) was use for detected conduction velocity and amplitude of digital nerve in ulnar nerve. Data analysis was performed using SPSS. Descriptive analysis was used for obtain mean and standard deviation and independent t-test was used to compare with ring and little finger. Conduction velocity of the right ring finger was 57.44m/sec and little finger was 55.32msec. The left ring finger was 55.55msec and little finger was 54.11msec. Amplitude of the right ring finger was 30.28µV and little finger was 48.36µV. The left ring finger was 30.67µV and little finger was 57.76µV. There were significantly difference between ring and little in amplitude (p<.05) but there were no statistically difference between conduction velocity of ring and little finger (p>.05). The amplitude of little finger are greater than ring finger. The present results revealed that electodiagnosis can easily perform in little finger for digital nerve of ulnar nerve study.

        감염 조절용 차단막의 두께가 광중합기의 중합광에 미치는 영향

        연구목적: 본 연구는 감염 조절용 차단막을 여러 겹으로 사용했을 때 광중합기의 광강도와 파장, light diffusion 등에 미치는 영향에 대해 조사하였다. 연구 재료 및 방법: 감염 조절용 차단막은 투명 랩 (크린랩)을 사용하였고 광중합기는 할로겐 광중합기 (Optilux 360)와 LED 광중합기 (Elipar FreeLight 2)를 사용하였다. 차단막을 1겹, 2겹, 4겹, 8겹으로 광중합기의 광섬유말단을 감싸고 휴대용 광강도 측정기 (Cure Rite)로 광중합기의 광강도를 측정하였다. 광중합기를 주문제작한 optical breadboard에 고정시킨 후 휴대용 spectroradiometer (CS-1000)를 이용하여 광중합기의 파장을 측정하였고, DSLR (Nikon D70s)을 이용하여 광중합기의 light diffusion을 사진 촬영하였다. 결과: 광강도 측정 결과는 차단막의 두께가 증가할수록 광강도가 유의하게 감소하였으나 할로겐 광중합기에서 1겹과 2겹 사이에는 유의차가 없었으며, 4겹 이상의 차단막을 투과할 때 광강도가 더 많이 감소하였다. 여러 겹의 차단막을 투과한 광중합기의 전체적인 파장 형태와 peak wavelength의 변화는 관찰되지 않았다. Light diffusion 사진 촬영 시, LED 광중합기에서는 차단막의 두께가 미치는 영향이 없었으나 할로겐 광중합기에서는 차단막을 4겹 사용했을 때부터 중합광이 조사되는 각도가 감소하기 시작하여 8겹 사용했을 때 통계적으로 유의하게 감소하는 것을 볼 수 있었다 (p < 0.05). 결론: 광중합형 복합레진을 광중합할 경우 감염 조절용 차단막이 찢어지는 경우를 대비하여 1겹으로 사용하기 보다는 2겹으로 사용하는 것이 환자간의 교차감염을 예방하는데 유리할 것으로 사료된다. Objectives: This study investigated the effect of infection control barrier thickness on power density, wavelength, and light diffusion of light curing units. Materials and Methods: Infection control barrier (Cleanwrap) in one-fold, two-fold, four-fold, and eightfold, and a halogen light curing unit (Optilux 360) and a light emitting diode (LED) light curing unit (Elipar FreeLight 2) were used in this study. Power density of light curing units with infection control barriers covering the fiberoptic bundle was measured with a hand held dental radiometer (Cure Rite). Wavelength of light curing units fixed on a custom made optical breadboard was measured with a portable spectroradiometer (CS-1000). Light diffusion of light curing units was photographed with DSLR (Nikon D70s) as above. Results: Power density decreased significantly as the layer thickness of the infection control barrier increased, except the one-fold and two-fold in halogen light curing unit. Especially, when the barrier was four-fold and more in the halogen light curing unit, the decrease of power density was more prominent. The wavelength of light curing units was not affected by the barriers and almost no change was detected in the peak wavelength. Light diffusion of LED light curing unit was not affected by barriers, however, halogen light curing unit showed decrease in light diffusion angle when the barrier was four-fold and statistically different decrease when the barrier was eight-fold (p < 0.05). Conclusions: It could be assumed that the infection control barriers should be used as two-fold rather than one-fold to prevent tearing of the barriers and subsequent cross contamination between the patients.

        미술치료가 부-자가정 자녀의 문제행동과 가족관계에 미치는 효과에 관한 단일사례연구

        본 연구는 미술치료 프로그램이 부-자가정 자녀의 문제행동과 가족관계에 어떠한 효과를 미치는 지 밝히고자 하였다. 부-자가정의 아버지, 그리고 초등학교 4학년인 아들, 초등학교 2학년 딸을 대상으로 상담을 실시하였으며 한 개인의 문제행동은 모든 가족구성원에게 직,간접적인 영향을 미치며, 가족관계의 개선이야말로 아동의 문제행동 소거에 근본적인 문제해결책이 될 수 있다는 관점에 초점을 맞추었다. 또한 언어표현이 부족한 아동을 주 대상으로 삼았기 때문에 방어가 적고 무의식의 감정을 표현하기에 용이한 미술치료프로그램으로 상담을 구조화하였다. 본 연구는 2003년 12월 말부터 2004년 5월 초까지 초기면접과 사후검사, 회기 외 상담을 제외하고 총13회기를 실시하였다. 또한 상담 결과를 살펴보기 위해 평가도구에 의한 분석과 회기별 질적 분석을 병행하였으며 그 결과는 다음과 같다. 첫째, 미술치료가 대상아동의 문제행동 개선에 긍정적인 영향을 미쳤다. 둘째, 미술치료를 실시한 결과 대상아동의 가족관계 및 대인관계에 긍정적인 영향을 미쳤다. 한 개인의 행동 하나하나는 가족 구성원과의 상호작용에 직접적으로 혹은 간접적으로 영향을 미치게 되므로, 한 개인의 문제는 그 사람만의 문제가 아니라 가족 전체의 문제가 된다. 이 연구에서 미술치료를 통해 우선 자신의 감정을 표현하는 계기를 마련해주고, 가족 구성원간의 심리적 갈등이나 불쾌한 감정을 표현함으로써 억압된 정서가 안정되고 가족간의 긍정적인 상호작용을 회복함으로써 가족상호 간의 문제들이 감소했다고 볼 수 있다. This study is to investigate the effects of art therapy on a father-children family problem behaviors and family relationship. The subjects of this investigation are the father, his son (4th grade in a elementary school) and his daughter (2nd grade in a elementary school) of a father-children family. This research focuses on that personal problem behaviors influence one`s family members directly or indirectly and the improvement of family relationships can be a basic solution for the extinction of problem behaviors. As the client is a child whose speech development is incomplete, an art therapy program is used, which is useful to reduce self-defence and represent unconscions emotions. The program was composed of 13 sessions except a information interview, before and after tests and nonsession interviews. And it was implemented from late December 2003 to early May 2004. Analysis by evaluation method and quality analysis by each session were carried out. the results of analysis are: 1. Art therapy had a positive effect on the improvement of the client`s problem behaviors. 2. The family relationships and personal relationships have improved as the result of art therapy. A problem which a family member has doesn`t belong just to oneself and becomes to a problem of all of one`s family as one`s behaviors influence interaction between family members directly or indirectly. This study shows that art therapy provides a client with opportunities to express negative emotions and it stabilizes psychological conflicts and repressed emotions. It also reduces problems in family interaction through the recovery of positive interchange.
