      • 적출한 고양이 위(胃)의 전기 활동 및 근수축에 미치는 Morphine의 영향

        위(胃)의 전기 활동성과 위 근수축 운동에 대한 morphine의 영향을 알아보기 위하여, 12마리의 고양이 위를 적출하여 95% O( )와 5% CO( )가 계속 공급되는 Krebs용액(36℃)내에 두고 가느다란 은선(AgCl wire)이 들어 있는 모세관 전극을 사용하여 단극성으로 위의 체부 및 유문부에서 위전도(서파)를 기록하였다. 안정 상태에서 위전도를 그린 다음 acetylcholine(5×10( )M) 또는 acetylcholine과 naloxone(10( )M)의 전처치 다음, 이어서 morphine(10( )M)을 각각 투여한 후 위전도의 서파 발생 빈도 및 유문부 근수축의 변화를 관찰하여 다음과 같은 결과를 얻었다. 1. acetylcholine의 투여후 서파발생 빈도가 증가하였으며, 이와 더불어 유문부 근수축이 발생하였으며, morphine의 투여로 이 유문부 근수축이 더욱 증가하였다. 2. acetylcholine과 naloxone을 함께 투여한 후의 서파 발생 빈도와 유문부 근수축의 변화는 acetylcholine의 단독 투여시와 마찬가지였으며, 이어서 morphine의 투여로 인한 더 이상의 유문부 근수축의 증가가 없었다. 이상의 결과로 미루어 morphine은 acetylcholine의 영향하에서 고양이 위(胃) 유문부의 근수축에 촉진적 영향을 미치는 것으로 사료된다. Twelve cats of either sex, ranging in weight from 2.0 to 2.5kg, were used to investigate the effect of morphine on the electcical and the mechanical activities of the isolated cat stomach. After peeling off mucous membrane from the ventral portion of the isolated stomach, the strip of muscle was pinned down on the paraffin covering the bottom of the recording bath which contained the Krebs solution at 36±0.5℃. The electrical activity was recorded by three capillary electrodes, of which two were placed on the corpus and one on the antrum. Each capillary electrode was a glass capillary containing a silver chloride wire(Ag-AgCl electrode) in it. The antral adge was anchored to a steel hook connected to a force transducer by a thread in order to record the isometric contraction of the antral muscle. After recording of the electrical and the mechanical activities in normal Krebs solution, acetylcholine(??) or acetylcholine and naloxone(??) was administered and then morphine(??) added to each solution. The mean frequency of the slow wave from each electro-gastrogram and the antral contractile activity were measured. 1. After acetylcholine administration, the frequency of the gastric slow wave increased significantly and the increase of slow wave frequency was followed by the appearance of isometric antral contraction. 2. By morphine administration following acetylcholine treatment, the isometric contraction of antral muscle increased markedly compared with that under the influence of acetylcholine alone. 3. The addition of naloxone abolished the stimulatory effect of morphine on the antral muscle contraction under the influence of acetylcholine. 4. Naloxone did not affect the acetylcholine-inducing change in the frequency of gastric slow wave as well as the isometric contraction of antral muscle. It is inferred from the above results that morphine has a stimulatory effect on the contraction mechanism of the cat antral muscle under the influence of acetylcholine.

        예방적 lamivudine 투여로 성공적인 동종골수이식술을 시행한 HBV 양성 중증 재생불량빈혈 환자 1예

        저자들은 만성 B형 감염을 동반한 중증 재생불량빈혈 환자에게 lamivudine을 예방적으로 투여하여 HBV의 재활성 없이 성공적으로 동종골수이식술을 시행하였기에 문헌고찰과 함께 보고하는 바이다. Reactivation of hepatitis B virus (HBV) infection has been known to be a serious complication of immunosuppressive or cytotoxic chemotherapy in HBV carriers or chronic hepatitis B patients. We report here a 25-year-old woman who has severe aplastic anemia and chronic hepatitis B underwent successful allogeneic bone marrow transplantation (BMT) with prophylactic lamivudine treatment and showed no evidence of reactivation of hepatitis B, HBV DNA elevation, or liver dysfunction. This result suggests that prophylactic administration of lamivudine to a BMT recipient of chronic hepatitis B might be a safe and promising measure to prevent fatal liver dysfunction.

      • 자가 말초혈액 조혈모세포 이식술의 임상적용

        Responses to chemotherapy correlate with the increased dose of chemotherapeutic agents in some cancers, e.g. breast cancer, Ewing's sarcoma, Hodgkin's disease(HD) and non-Hodgkin's lymphoma(NHL), then for the improvement of the chemotherapy responsiveness, high-dose chemotherapy(HDCT) was proposed. But, it's application was limited due to complications, especially bone marrow suppression. HDCT and succeeding autologous peripheral blood stem cell transplantation(APBSCT) was introduced to overcome this problem. This study was designed to determine the clinical pictures including clinical parameters and the responsiveness of HDCT followed by APBSCT in Hanyang University Hospital. Ten patients were enrolled. They were 8 adults (3 breast cancers, 3 multiple myelomas, 1 HD, 1 NHL) and 2 children (1 acute myelogenous leukemia (AML), 1 neuroblastoma). Mobilization chemocherapy(MCT) followed by Granulocyte Colony Stimulating Factor(G-CSF) was administered to mobilize stem cells from bone marrow to peripheral blood. The stem cells were collected by using CS-3000 and cryopreserved at -196℃ with programmed controlled rate freezer as the mixture with 10% dimethyl sulfoxide(DMSO). HDCT was administered and cryopreserved peripheral stem cells were infused on day 0 as planned. Patients' mean age was 41 years old. After induction chemotherapy three patients achieved complete response and 5 partial response. The median time for bone marrow recovery after MCT was 11.5 days for neutrophil(〉500/㎣) and 4.8 days for platelet(〉20,000/㎣). There were three cases of neutropenic fever and one case of gingivitis. After HDCT with APBSCT, one patient was converted partial response to complete response, and one patient achieved complete response after HDCT without induction chemotherapy. The median time for bone marrow recovery was 12.7 days for neutrophil(〉500/㎣) and 14.7 days for platelet(〉20,000/㎣). Eight patients developed neutropenic fever and required systemic antibiotics. Transfusion was required 1.6 pints for packed red cell and 23.4 pints for platelet concentrates. Toxicity consisted mainly of vomiting, diarrhea, and mucositis which were mild. HDCT with APBSCT can be performed safely with minimal complications. For the evaluation of the exact role of HDCT with APBSCT in malignant diseases, further studies are required as a large scale of patients and lung-term follow up.

        대뇌피질 유발 저작 운동과 연수 망상체 신경세포 활성도에 관한 연구

        This experiment was performed to clarify the role of brainstem for rhythm generation of mastication induced by stimulation of cerebral cortex. Rabbits were anesthetized with urethane and a pair of wire electrode was inserted into anterior digastric and masseter muscles, respectively. Experimental animal was placed to stereotaxic apparatus and occipital craniotomy and cervical laminectomy was performed to expose caudal brainstem. Cortically-induced mastication was evoked by electrical stimulation of cerebral masticatory area with concentric bipolar electrode and neural activity of brainstem and EMGs of masseter and anterior digastric muscle were recorded. The results obtained were as follows: 1. The neural activity of bulbar reticular formation was recorded at rostral 2-4㎜, lateral 0.5-1.5㎜ to obex and 1.5-2.5㎜ below the bulbar surfafce. 2. The site that neural activity corresponding with EMGs of masticatory muscles was recorded was medial bulbar reticular formation(MBRF) 3. The neural activity of medial bulbar reticular formation(MBRF) was corresponded with anterior digastric or masseteric EMG, respectively. 4. The neural activity of medial bulbar reticular formation(MBRF) and anterior digastric EMG was simultaneously recorded by twin pulse stimulation of contralateral cerebral masticatory area, and stimulation of ipsilateral medial bulbar reticular formation(MBRF) evoked anterior digastric EMG.

        아드레날린 동작성 약물이 구강점막의 리도케인 Clearance에 미치는 영향

        Some adrenergic drugs have been used as vasoconstrictors in local anesthetics for prolongation of acting duration and minimization of side effects. But, the exact role of vasoconstrictors on the clearance of local anesthetics in oral mucosa was not investgated. This experiment was carried out to clarify the influences of some adrenergic drugs on the clearance of lidocaine in oral mucosa. Sprague-Dawley rats were devided into control(no vasoconstrictor) group, epinephrine(1:100,000)-contained group, norepinephrine(1:30,000)-contained group and phenylephrine(1:2,500)-contained group. In urethane(1.2g/㎏) anesthetized rats, 300μl of 2% lidocaine with or without vasoconstrictor injected in the palatal mucosa. According to time schedule, they were sacrificed by exsanguination immediately, 30 and 60 minutes later after injection, respectively. The palatal mucosa was removed and homogenized by mechanical force and ultrasonicated in 0.1N HCl(500μl). The samples were centrifuged after akalization. Aliquat of the supernatant was injected into HPLC. Also, systemic effects of vasoconstrictors were evaluated with recording arterial blood pressures in common carotid artery. In control animals, lidocaine clearances reached 85.4% and 89.3% at 30 and 60 minutes after injection, respectively. At 30 minutes after injection, lidocaine clearances of vasoconstrictor contained group were different from control group significantly(p<0.01), and lidocaine clearance of phenylephrine contained group appeared higher than those of the other vasoconstrictor contained groups(p<0.05). But, at 60 minutes after injection, only epinephrine contained group had lower lidocaine clearance than control group(p<0.01). Intraperitoneal injection of the same dosage of palatal mucosa injecton of these vasoconstrictors did not elicit the change of blood pressure and heart rate.

      • 악성 질환에서의 고용량 화학요법과 자가말초혈액 조혈모세포이식술 : 단일기관 치료 경험

        연구배경: 고용량 화학요법은 항암제의 투여 용량에 따라 항암효과가 비례하는 용량반응 관계가 있는 악성종양에서 효과를 기대할 수 있으며 현재 급성백혈병, 악성림프종, 다발성골수종 등의 혈액 종양과 유방암, 고환암, 난소암, 신경아세포종 등의 고형암 환자에서 시행되고 있다. 저자들은 고용량화학요법에 이어 자가조혈모세포이식을 시행 받은 악성 종양 환자를 대상으로 치료효과 및 부작용 등의 임상상을 알아보고자 하였다. 방법: 1997년부터 2003년까지 한양대학교 병원에서 자가조혈모세포이식을 시행 받은 악성 종양 환자 31예(남서 : 여성 15 : 16, 중앙연령: 50세, 다발성골수종 14예, 비호지킨림프종 9예, 유방암 4예, 호지킨림프종 3예, 급성골수성백혈병 1예)를 대상으로 후향적 분석을 시행하였다. 결과: 채집된 단핵구의 중앙값은 5.1×10^(8)/kg (범위 1.6~189.3)이었고 CD34양성세포수의 중앙값은 12.0×10^(6)/kg (범위 1.3~75.1)이었다. 말초혈액 과립구가 500/μL 에 도달하는 중앙값은 11일(범위 8~27)이었고 혈소판이 20,000/μL에 도달하는 중앙값은 11일(범위 0~32)이었다. 고용량 항암화학요법 후 21예가 완전관해, 5예가 부분관해에 도달하였다. 생존한 환자의 중앙추적기간은 29.5개월이었고 전체 환자의 2년 무병생존기간은 55.7%이었고 중앙값은 48.2개월이었다. 2년 생존기간은 62.9%이었으며 중앙값에는 도달하지 않았다. 10예의 사망환자 중 7예가 질병의 진행으로 사망하였고, 조기사망 3예(심부전과 신부전 1예, 장파열 1예, 패혈증 1예)가 있었다. 결론: 이 연구는 포함된 환자의 수가 적고 추적기간이 짧은 단점이 있으나, 다발성골수종, 악성림프종 등의 악성 질환에서 고용량 항암화학요법과 자가조혈모세포이식은 안전하고 효과적인 치료임을 추정할 수 있었다. Background: We performed the current study to evaluate the safety and efficacy of high-dose chemotherapy (HDC) with autologous peripheral blood stem cell transplantation (ASCT) in malignant diseases. Methods: Between January 1997 and March 2003, 31 patients (14 multiple myeloma, 9 Non-Hodgkin's lymphoma, 3 Hodgkin's disease, 4 breast cancer, and 1 acute myelogenous leukemia) underwent HDC with ASCT. Stem cells were collected by CS-3000 after mobilization treatment with chemotherapy and granulocyte colony-stimulating factor. The conditioning regimens included high-dose melphalan, BEAM (BCNU, etoposide, cytosine arabinoside, melphalan), BEAC (BCNU, etoposide, cytosine arabinoside, cyclophosphamide), CPB (cyclophosphamide, cisplatin, BCNU) and others. Results: There were 15 male and 16 female patients and median age was 50 years (range: 29~77). The median number of mononuclear and CD34+ cells was 5.1×10^(8)/kg (1.6~189.3) and 12.0×10^(6)/kg (1.3~75.1), respectively. The median time of neutrophil (>500/μL) and platelet (>20,000/μL) engraftment was 11 (8~27) and 11 (0~32) days after ASCT, respectively. Twenty-two patients experienced neutropenic fever. Twenty-one patients achieved complete remission and 5 patients achieved partial remission after HDC with ASCT. The median follow-up duration for surviving patients was 29.5 months. Causes of death included disease progression (7), sepsis (1), bowel perforation (1), and renal failure (1). Median progression free survival (PFS) was 48.2 months and median overall survival (OS) was not reached. Two-year PFS and OS was 55.7% and 62.9%, respectively. Conclusion: Our results suggest that HDC with ASCT was safe and effective modality in patients with various malignant diseases.

        Ultraprecision machining and MEMS technology have been taken more and more important position in machining of microparts. Micro endmilling is one of the prominent technology that has wide spectrum of application field ranging from macro parts to micro products. Also, the method of micro-grooving using micro endmilling is used widely owing to many merit, but has problems of precision and quality of products due to tool wear and tool fracture. This investigation deals with state monitoring using acoustic emission(AE) signal in the micro-grooving. Characteristic evaluation of AE raw signal, AE hit and frequency analysis for state monitoring is also presented in the paper.

        Discrimination of Floral Scents in Chrysanthemum morifolium Cultivars using Electronic Nose

        The aim of the present study was to investigate the differences in volatile profiles among different Korean chrysanthemum cultivars grown as cut flowers. To optimize a method for sampling and comparing scents, the scents were evaluated by electronic nose (E-nose) at different flowering stages and in different organs and cultivars. The values of maximum resistance changes of metal oxide semiconductor sensors and relative aroma intensity were highest at flowering stage III (full flower stage of ray florets and initial opening disc florets) among different stages and in disc florets among different floral organs of cut chrysanthemums. Among the 12 chrysanthemum cultivars, the highest values for response change and relative aroma intensity were observed in the flowers of the ‘Ilweol’ cultivar. To compare scent patterns among the cultivars, E-nose sensor data was subjected to multivariate statistical analysis. Principal component analysis and discriminant function analysis of the volatile metabolic profile data indicated that chrysanthemum samples were clearly discriminated in a cultivar-dependent manner. These results show that different cultivars are characterized by distinct volatile profiles. Hierarchical cluster analysis showed that the 12 chrysanthemum cultivars were separated into 3 main groups according to the relationship of volatile profiles; however, chrysanthemum cultivars were not clustered according to the flower shape. We propose that these results could be used as the basis on which to improve the breeding of cut chrysanthemums based on volatile characteristics.
