        신생아 집중치료를 받은 극소저출생체중아 치료성적의 시기에 따른 변화

        목 적: 출생 체중 1,500 그램 미만의 극소저출생체중아(very low birth weight infant, VLBWI)의 치료성적은 신생아 집중치료술 향상여부를 모니터 하는 주요한 지표로 이용되고 있다. 이에 본 연구자들은 개원이래 최근 7년간 삼성서울병원 신생아 중환자실의 VLBWI 치료성적의 변화양상을 기간별로 관찰 분석하였다. 방 법: 1994년 10월부터 2000년 12월까지 삼성서울병원 신생아 중환자실로 생후 3일 이내 입원한 VLBWI 374명을 대상으로 기간을 나누어(I기 : 이미 성적이 보고된 1994. 10-1996. 9, II기 : 1996. 10-1998. 12, III기 : 1999. 1-2000. 12) 각 기별, 출생체중별, 재태기간별, VLBWI의 발생빈도와 생존율, 유병률 및 생존기간 등에 관해 의무기록지를 통한 후향적조사를 실시하였다. 생존율은(생존아/총출생 VLBWI)${\times}100$으 로 산출하였고 생존아는 신생아 중환자실에서 생존하여 퇴원한 경우로 정의하였다. 결 과 : VLBWI의 발생빈도는 기간별로 차이가 없었고(I기 : 1.3%, II기 : 1.5%, III기 : 1.4%), 생존율은 III기에 의미 있게 향상되었다. 출생체중별 생존율에서 III기에서 750 gm 미만군(vs II기)과, 1,250-1,499 gm 군(vs I기)의 향상이, 1,000-1,249 gm 군에서는 II기(vs I기)의 향상이 의미 있었고 최저출생 생존아는 I기 624 gm($26^{+5}$주), II기 667 gm($25^{+6}$주), III기 480 gm($26^{+2}$주)였다. 재태기간별 생존율에서 III기의 25-26주군과 29-30군이 I, II기에 비해 의미 있게 향상되었고 최저출생 재태기간의 생존례는 I기 26주(970 gm), II기 $23^{+5}$주(791 gm), III기 $24^{+1}$주(740 gm)였다. VLBWI의 주요 유병률 중 III도 이상의 심한 뇌실 내 출혈의 빈도가 III기 5%로 I기 13%, II기 10.5%에 비해 의미 있게 감소되었고, 전체 사망 중 7일 이전 사망률이 III기(15.4%)에 I기(55.5%)에 비해 의의 있게 감소되었다. 결 론 : 최근 VLBWI의 생존율이 주요 유병률의 증가 없이 현저히 향상되었고, 특히 26주 이하, 750 gm 미만의 초극소미숙아의 생존율 향상이 두드러졌다. Purpose : The outcomes of infants weighing less than 1,500 gm(very low birth weight infant : VLBWI) reflect recent progress in neonatal intensive care. In this study, we analyzed changes over time in survival rate and morbidity of VLBWIs during the past seven years. Methods : A retrospective review of medical records was analyzed for VLBWIs admitted to the neonatal intensive care unit of Samsung Medical Center within three days from birth. We compared the outcomes of previous corresponding data(period I : Oct. 1994 to Sept. 1996), with the outcomes of period II(Oct. 1996 to Dec. 1998) and period III(Jan. 1999 to Dec. 2000). Results : As shown in Tables 1 and 3, the distribution of birth weight, gestational age(GA), gender, and inborn admissions did not change during the 7-year study. The overall survival rate of VLBWI increased significantly over time(period I : 72% vs period III : 88.3%, P<0.05). Between period I and period II, the birth weight-specific survival rate increased by 23.6%(75% vs 92.7%, P<0.05) for infants 1,000 to 1,249 gm. Between period II and period III, the birth weight-specific survival rate increased three times(20% vs 66.7%, P<0.05) for infants <750 gm. The survivors of lowest birth weight included infants at 624 gm(GA : $26^{+5}$ weeks), 667 gm(GA : $25^{+6}$ weeks) and 480 gm(GA : $26^{+2}$ weeks) in each period. The gestational age-specific survival rate in period III increased significantly in GA 25-26 weeks and 29-30 weeks(vs period I & period II, P<0.05). The survivors of lowest gestational age included infants at GA 26 weeks(970 gm), GA $23^{+5}$ weeks(791 gm) and GA $24^{+1}$ weeks(740 gm) in each period. The incidence of severe IVH(grade III, IV) and the early death rate(< seven days) decreased in period III(vs period I; P<0.05), reflecting improvements in neonatal intensive care. Conclusion : The survival rate of VLBWI continues to increase, particularly for BW < 750 gm, GA < 26 weeks. This increase in survival is not associated by any increase in major morbidities.

        Pharmacokinetics of a novel antiangiogenic agent KR-31831 in rats

        <P>This study reports the absorption, dose-linearity and pharmacokinetics of a novel antiangiogenic agent KR-31831 in rats after i.v. and oral administration at doses of 5, 10 and 25 mg/kg on both occasions. Concentrations of KR-31831 were determined by a validated LC/MS/MS assay method. After i.v. injection, plasma concentration-time profiles showed multi-compartmental characteristics, and there were no significant differences in Cl (20.8–27.7 ml/min/kg) and dose-normalized AUC (178.1–231 µg·min/ml based on the 5 mg/kg dose) as a function of dose. However, V<SUB>ss</SUB> was significantly increased at the 25 mg/kg dose (4931 ml/kg) compared with those (2288–2421 ml/kg) at lower doses. Subsequently, t<SUB>1/2</SUB> was increased from 143–159 min at the lower doses to 304 min at the 25 mg/kg dose. The altered V<SUB>SS</SUB> was found to be a result of reduced plasma protein binding at relatively high concentrations. Following oral administration (doses 5–25 mg/kg), the absolute oral bioavailability ranged from 37.8% to 46.3%, and there were no significant alterations in dose-normalized AUC, T<SUB>max</SUB>, C<SUB>max</SUB> and t<SUB>1/2</SUB> as a function of dose. The extent of urinary excretion was low for both i.v. (0.35%–0.54%) and oral (0.13%–0.33%) doses. Further discussions on the chemical and microsomal stability were included. In conclusion, dose-independent absorption kinetics were observed at oral doses from 5 to 25 mg/kg in rats. Orally administered KR-31831 could be eliminated mainly by the liver metabolic pathway.         Acute Exacerbation of Interstitial Pneumonia Other Than Idiopathic Pulmonary Fibrosis

        <P>BACKGROUNDS: Acute exacerbation (AE) in idiopathic pulmonary fibrosis (IPF) is increasingly recognized as a relatively common and highly morbid clinical event. However, clinical data on AE in non-IPF interstitial pneumonia are sparse. This study was performed to find the frequency, clinical features, and outcome of AE in non-IPF interstitial pneumonia. METHODS: Retrospective analysis of 10 patients who satisfied the modified Akira criteria for AE during follow-up of 74 patients with surgical lung biopsy-confirmed idiopathic nonspecific interstitial pneumonia (I-NSIP) and 93 patients with biopsy-confirmed interstitial pneumonia associated with collagen vascular disease (CVD-IP). RESULTS: AE occurred in six patients with I-NSIP (1-year frequency, 4.2%) and in four patients with CVD-IP (rheumatoid arthritis [RA], n = 3; scleroderma, n = 1), with 1-year frequency of 3.3%. Median age was 58 years (range, 47 to 75); six patients were female. AE occurred in two patients immediately after surgical biopsy. Median duration of acute symptom before hospital admission was 10 days (range, 1 to 30). Median ratio of Pao(2) to the fraction of inspired oxygen (Fio(2)) was 172 (range, 107 to 273), and Pao(2)/Fio(2) ratio was < 200 in six patients. Surgical lung biopsy performed at the time of AE in two patients revealed diffuse alveolar damage superimposed on nonspecific interstitial pneumonia pattern. Four patients with I-NSIP survived to discharge and were followed up for 24 months (range, 6 to 121). CONCLUSION: AE occurred in the patients with I-NSIP with apparently better prognosis. In patients with CVD-IP, AE occurred mostly with RA-usual interstitial pneumonia in our small series with poor outcome.</P>

        Design Optimization of a Type-I Heterojunction Tunneling Field-Effect Transistor (I-HTFET) for High Performance Logic Technology

        In this work, a tunneling field-effect transistor (TFET) based on heterojunctions of compound and Group IV semiconductors is introduced and simulated. TFETs based on either silicon or compound semiconductors have been intensively researched due to their merits of robustness against short channel effects (SCEs) and excellent subthreshold swing (SS) characteristics. However, silicon TFETs have the drawback of low on-current and compound ones are difficult to integrate with silicon CMOS circuits. In order to combine the high tunneling efficiency of narrow bandgap material TFETs and the high mobility of rn.v TFETs, a Type-I heterojunction tunneling field-effect transistor (I-HTFET) adopting Ge-AIxGa1-xAsGe system has been optimized by simulation in terms of aluminum (AI) composition. To maximize device performance, we considered a nanowire structure, and it was shown that high performance (UP) logic technology can be achieved by the proposed device. The optimum AI composition turned out to be around 20% (x=0.2).

        An intelligent robot control system is developed, which is based on extensible hardwares and softwares. The system could be used to test advanced and complex real time application programs to avoid constraints on present robot control system in executing a complex or precise algorithms, due to the limitation of hardware and software. In this paper we used the RCCL(Robot Control C Library) on SUN4 as a supervisory system that plays the path planning and man-machine interface. And we used VxWORKS as a real time OS on a VME bus CPU equiped with some interface boards. Two systems were connected through the Ethernet network. We used the 4 axis manipulator, FARA, developed by Samsung Electronics Co.

OBJECTIVES:: We assessed the validity and reliability of a virtual environment technology (VET)-based cognitive assessment program that was developed as a measurement tool of cognitive abilities in patients after a stroke. DESIGN:: Twenty participants diagnosed with stroke caused by unilateral brain lesions were enrolled to assess the VET program’s validity and test-retest reliability. Participants underwent evaluation by paper-based neuropsychological tests including the Korean Mini-Mental Status Examination, the Korean-Wechsler Adult Intelligence Scale, the Motor Free Visual Perception Test, Rey-Kim Memory Test, and Kim’s Frontal-Executive Neuropsychologic Test as well as the VET-based cognitive assessment. Major variables and indices of the VET program were calculated. The VET program validity was evaluated using a simple correlation analysis between variables from the VET program and conventional paper-based neuropsychological measurements, and the reliability was evaluated by investigating the test-retest correlation coefficients. RESULTS:: Major variables and indices of the VET program in patients with stroke correlated significantly with the related scores of paper-based neuropsychological tests. In addition, the test-retest reliability analysis revealed that the correlation coefficients ranged from 0.528 to 0.926. CONCLUSION:: The VET-based cognitive assessment program showed adequate reliability and validity as a method of cognitive assessment in patients after stroke.

        A study on the morbidity status of a rural community ; Su-Dong Myun, Nam Yang Ju Gun, Kyung Gi Do was carried out from January 5, to 25, 1982 through analyzing the medical records of Su-Dong Health Subcenter during the period of September, 1980 to August, 1981 and following results was obtained. 1) The number of new patients during a year was 3,651 composed of 49.8% male and 50.2% female, and the cumulated visiting number of the out patients was 5,481. 2) The order of five major diseases which were classified the International Classitication of Diseases(I. C. D.) was Diseases of the Respiratory System(33.2%), Diseases of the Digestive System (21.3%), Diseases of the Skin and Subcutaneous Tissue (17.0%), AccidentsㆍPoisoningsㆍViolence (9.4%), and Diseases of the Nervous System and Sense Organs (8.4%). 3) Annual morbidity rate was 700.6 per 1,000 population and 699.6 for male and 710.6 for female. 4) The highest age specific morbidity rate per 1,000 population was 2,408.1 in the age group of 0 ∼ 4 and the lowest was 272.9 in the age group of 15 19. 5) The average clinic visiting times per out patient was 1. 6. 6) Medical fee of patients visited to Health Subcenter was covered from 3 different sources as follows ; 81.2% of total patients was paid by patient self, 9.5% by government as medical program, and 9.3% by medical insurance. 7) Average medical fee per first visiting was 1,970 ± 1,320(Won) and average medical fee per case was 2,990 ± 2,590(Won). 8) The annual utility rate of Health Subcenter of the community was 710.0 per 1,000 population, and the highest rate (1,350.4) was shown in the people of Ribsouk 2 Rie and the lowest(381.0) in Cidoon Rie.

        GC-ECD를 이용한 유기염소계 및 Pyrethroid계 농약 동시 분석법

        Pesticides were extracted from samples with 70% acetone and methylene chloride in order, and then cleaned up via open-column chromatography apparatus packed with florisil, and finally analyzed simultaneously the organochlorine and pyrethroid pesticides using GC(ECD). An ultra-2 fused silica capillary column was used to separate and identify the products. The resolution between the last isomeric peak of cypermethrin(59.987min) and the first isomeric peak of flucythrinate(60.043min) was not satisfactory. The last isomeric peak of fenvalerate(62.344min) and the first isomeric peak of fluvalinate(62.397min) were overlapped. Recoveries of soybean sample for the most pesticides were 73.3% to 102.4%. Detection limits were between 0.004 and 0.063 ㎍/㎎ when this method was used.
