      • 조혈모세포이식 후 골성장인자의 변화 및 골대사에 미치는 영향 : Impact on Bone Mineral Metabolism

        연구배경: 각종 장기이식의 시행이 많아지고 이식 후 생존율이 증가함에 따라 이식 후 합병증에 대한 관심 또한 높아지고 있다. 조혈모세포이식 후에도 다양한 내분비적 합병증이 발생할 수 있으며 골격에 대한 합병증도 문제점으로 대두되고 있다. 조혈모세포 이식 후 발생하는 골소실에는 이식 후 초기의 골형성 저하와 골흡수 증가가 중요한 역할을 담당하리라고 추측되는데 이러한 골재형성불일치(biochemical uncoupling)에 골 성장인자들이 미치는 영향에 대해서는 알려진 바가 없다. 본 연구에서는 조혈모세포이식 전, 후로 말초 혈액에서 IGF-I, FGF-2, M-CSF같은 성장인자의 변화를 알아보고, 이들 성장인자의 변화가 조혈모세포이식 후의 골형과 골흡수에 미치는 영향 및 이식 후 발생되는 골량 소실과의 연관성을 확인해 보고자 하였다. 방법: 여러 가지 혈액질환으로 인해 동종 골수이식을 시행 받은 환자들을 전향적으로 관찰하였으며 이식 전 및 이식 후 1주, 2주, 3주, 4주 및 3개월, 6개월 1년에 말초 혈액에서 골교체표지자를 측정하였다. 이식 전 및 이식1년 후에 요추골 및 대퇴골 골밀도를 측정할 수 있었던 36명의 환자들을 대상으로 냉동 보관되어 있던 혈청을 이용하여 IGF-I, FGF-2 및 M-CSF를 시기별로 측정하였으며 이들 성장인자와 골교체표지자의 변화 및 골밀도 변화 사이의 상관관계를 확인하였다. 결과: 골흡수 표지자인 혈청 ICTP는 이식 전에 비해 이식 후 4주까지 점차 의의 있게 증가하다가 이후 6개월까지 더욱 증가한 후 감소하였다. 골형성 표지자인 osteocalcin은 이식 후 3주까지는 점차 감소하다가 이후 증가하여 이식 후 3개월 및 6개월에 기저치보다 통계적으로 유의하게 증가한 후 감소하였다. 혈청 IGF-I과 FGF-2는 각각 이식 후 3주 및 1주까지 의미있게 감소하다가 이후 증가하였으며 혈청 M-CSF는 이식 후 1주째에 기저치에 비해 의미 있게 증가하였다가 이후 기저치로 회복되었다. 이식 1년 후 평균 요추부 골밀도는 5.2% 감소하였고 평균 근위대퇴골 골밀도는 11.6% 감소하였다. 이식 전 및 이식 후 3주에 측정한 IGF-I과 같은 시기에 측정한 오스테오칼신 사이에 유의한 상관관계가 관찰되었으며 이식 후 3주째의 M-CSF와 골흡수표지자인 M-CSF 사이에서 의미 있는 양의 상관관계를 관찰할 수 있었다. 이식 후 3주 및 3개월에 IGF-I이 낮은 환자일수록 이식 1년 후 근위대퇴골에서의 골소실이 많은 것으로 분석되었다. 결론: 조혈모세포이식 후 발생하는 골소실에는 기존에 알려진 기저질환의 영향, 성호르몬의 감소, 면역억제의 투여, 골수기질세포와 조골세포의 손상 및 이식초기 사이토카인의 변화이외에도 골성장인자가 관련이 있음을 확인하였고, 이는 이식 후 발생되는 골량소실에 중요한 역할을 할 것이라고 사료된다. Background: A loss of bone mass is usually detected after a bone marrow transplantation (BMT), especially during the early post-transplant period. We recently reported that enhanced bone resorption following a BMT was related to both the steroid dose and the increase in IL-6. We also suggested damage to the marrow stromal microenvironment, by myoablation, partly explains the impaired bone formation following a BMT. It is well known that some growth factor play important role in bone growth and osteogenesis. However, the pathogenetic role of bone growth factors in post-BMT bone loss is unknown and data on the changes in the growth factors, in accordance with bone turnover markers and bone mineral density (BMD) changes are scarce. We investigated changes in bone growth factors such as IGF-I (Insulin-like growth factor-I), fibroblast growth factor-2 (FGF-2) and Macrophage colony stimulating factor (M-CSF), during the post-BMT period, and assessed whether the growth factor changes influenced the bone turnover and post-BMT bone loss. The present study is the first prospective study to describe the changes in bone growth factors following a BMT. Methods: We prospectively investigated 110 patients undergoing a BMT, and analyzed 36 patients (32.4±1.3 years, 17 men and 19 women) whose BMDs were measured before, and 1 year after, the BMT. The serum biochemical markers of bone turnover were measured before, 1, 2, 3 and 4 weeks, 3 and 6 months, and 1 year, after the BMT. The serum, FGF-2, IGF-I and M-CSF levels were measured before and 1 and 3 weeks, and 3 months after the BMT. The correlation between the changes of growth factors and various bone parameters was analyzed. Results: The mean bone losses in the lumbar spine and total proximal femur, calculated as the percentage change from the baseline to the level at 1 year, were 5.2(p<0.05) and 11.6%(p<0.01), respectively. the serum type I carboxyterminal telopeptide(ICTP), a bone resorption marker, increased progressively until 6 months after the BMT, but thereafter decreased, to the base value after 1 year. Serum osteocalcin, a bone formation marker, decreased progressively, until 3 weeks after the BMT but then increased transiently, and finally returned to the base level at 1 year. The serum IGF-I and FGF-2 also decreased progressively until 3 weeks 1 week after the BMT, respectively, then increased to the base values at 3 months. The serum M-CSF increased briskly at 1 week post-BMT, then decreased to the base level. There were positive correlations between the percentage changes from the baseline proximal femur BMD and the IGF-I levels 3 weeks and 3 months (r=0.52, p<0.01, r=0.41, p<0.05) post BMT. A significant correlation was found between the IGF-I and osteocalcin levels pre-BMT, and 3 weeks after the BMT. Another positive correlation was found between the M-CSF and the ICTP levels at 3 weeks post BMT (r=0.54, p<0.05). Conclusion: In conclusion, there were significant changes in the serum IGF-I, FGF-2 and M-CSF levels in the immediate post-BMT period, which were related to a decrease in bone formation and loss in the proximal femoral BMD during the year following the BMT (J Kor Soc Endocrinol 17:664∼674, 2002).

        피로와 의료이용, 업무상 사고 및 질병결근

        Objectives: This study examined the relationship of fatigue to medical utilization, occupational accident and sickness absence. Subjects and Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project, 2002~2004). A total of 30,146 employees categorized within the Korean Standard Industrial Classification and the Korean Standard Occupational Classification were recruited. A structured-questionnaire was used to assess the participants' sociodemo-graphics, job-related factors, health-related behaviors, personality trait, self=perceived fatigue (MFS) and the frequency of medical utilization, occupational accident and sickness absence. Data on medical utilization and occupational accident were confirmed from company health records. Chi-square test and logistic regression analysis were used to elucidate the relationship of self-perceived fatigue to medical utilization, occupational accident and sickness absence. Results: In logistic regression analyses, self-perceived fatigue was associated with medical utilization (outpatient), occupational accident and sickness absence although the strength of the associations was higher in the blue collar workers except for medical utilization. For medical utilization, the white collar worker's adjusted odds ratios of outpatient utilization for workers scoring in the third quartile and the highest quartile, compared to workers scoring in the lowest quartile were 1.50 (95% CI: 1.20-1.87) and 2.16 (95% CI: 1.69-2.75), respectively. The blue collar workers' adjusted odds ratios of outpatient utilization for workers scoring in the second (OR: 1.23, 95% CI: 1.06-1.43), third (OR: 1.42, 95% CI: 1.22-1.66) and highest quartile (OR:1.83, 95% CI: 1.55-2.17), respectively, compared to workers scoring in the lowest quartile were statistically significant. However, no associations were found between fatigue and inpatient utilization for either the white or blue collar workers. The blue collar workers' adjusted odds ratios of being injured for workers scoring in the second, third and highest quartile were 1.41 (95% CI: 1.00-2.00), 1.57 (95% CI: 1.10-2.22) and 2.41 (95% CI: 1.69-3.44), respectively, compared to those scoring in the lowest quartile, but, those of the white collar workers were not significant. There was more likely to an increased risk of sickness absence in workers scoring in the second, third and highest quartile compared to those scoring in the lowest quartile in both white and blue collar workers, but the magnitudes of risk were higher in the blue collar workers than the white collars workers. Conclusion: This result suggests that fatigue is a determinant predictor of medical utilization, occupational accident and sickness absence, and that the pattern of risks for the three outcomes differs according to the occupational type. Some limitations of this study and considerations for future study were also discussed.

        사무직 근로자들의 직무 스트레스와 피로

        목적: 이 연구는 전국 규모의 조사 연구에서 수집된 자료 중 사무직 근로자들을 대상으로,사회인구학적 특성,직업 특성,건강행태,직무 스트레스와 피로수준 간의 관련성을 분석하는 데 있다. 방법:‘한국인 직무스트레스 측정도구의 표준화 전국 조사연구National Study for Development and Standardization of Occupational Stress (NSDSOS Project: 2002∼2004)’를 위해 2002년 5월 1일부터 2003년 5윌 30일 까지 수행되었으며,전국의 사업장에 근무하는 근로자 30,146명(남자: 84%,여자: 16%) 중 사무직 근로자 4,457명(남자: 68.9%,여자 31.1%)을 연구 대상으로 하였다. 자료수집방법은 사업장을 방문하여 보건 및 안전관리자 등에게 연구의 취지를 알린 후 설문조사에 대한 협조를 구하고 응답자 직접 기입법을 이용 하였으며,사회인구학적 특성,직업적 특성,건강관련 요인,KOSS-SF를 이용한 7개의 직무 스트레스 요인,그리고 MFS를 이용하여 피로 수준에 영향을 주는 요인에 대해 분석하였다. 결과: 일반적 특성,작업관련 특성,건강행태관련 특성 및 직무 스트레스 요인이 근로자들의 피로와 강한 관련성이 있음이 입증되었으며,특히 직무 스트레스의 하부 요인 중에서도 피로와 관련성을 보이는 요인이 남녀별로 다소 상이 하게 관련된다는 것을 발견할 수 있었다. 남자의 경우는 직무 스트레스의 하부 영역 중 직무 요구도,직무 불안정성,보상부적절 등이 영향력이 높은 주요 직무 스트레스 요인이었던 반면,여자는 직무 요구도,보상부적절,직장문화가 주요한 직무 스트레스 요인이었다. 결론: 직장인 피로가 갖는 보건학적 의학적 경제학적 중요성을 감안할 때,조직의 생산성 향상과 근로자 개개인의 삶의 질 향상 및 건강증진을 위해선 직무 스트레스 관리 프로그램이 절실히 요청된다고 볼 수 있다. Background & Objectives: A growing body of research has documented that occupational stress is closely associated with increased risk of fatigue. This study was conducted in an effort to assess the relationship of occupational stress to self-perceived fatigue among Korean white collar employees. Methods: Data were obtained from the National Study for Development and Standardization of Occupational Stress (NSDSOS Project, 2002-2004). Among them, a total of 4,502 white collar employees were recruited. A structured questionnaire was employed to evaluate the participants' sociodemographics, job-related factors, health-related behaviors, occupational stress, and self-perceived fatigue. Occupational stress and self-perceived fatigue were assessed using the Korean Occupational Stress ScaleShort form (KOSS-SF) and the Multidimensional Fatigue Scale (MFS), respectively. Results: In logistic regression analyses, occupational stress was associated with an increased risk of fatigue, and some domains of occupational stress had different effects on fatigue by gender (job demand, job insecurity, and lack of reward for men, job demand, lack of reward, and discomfort in occupational climate for women), which indicates that occupational stress may perform a slightly different role in increasing the risk of fatigue by gender. Conclusion: The results of this study suggest that occupational stress is a determinant predictor of self-perceived fatigue. Thus, a stress management program for the reduction of occupational stress, and the promotion of white collar worker's health and quality of life is strongly recommended. Some limitations of this study and considerations for future studies were also discussed.

        청소년 물질남용 입원환자의 치료효과

        Objective:The aim of this study was to evaluate the effect of an adolescent substance abuse treatment model with various measurements. Method:Twenty adolescent substance abusers referred by various agencies participated in structured treatment program during 8 weeks. Measurements used to evaluate the combined psychopathology were the Children’s Depression Inventory, State-Trait Anxiety Inventory for Children and Conner’s Scale. Measurements used to evaluate the treatment effect was Symptom Checklists (in Comprehensive Substance Treatment and Rehabilitation Questionnaire), Readiness to Change-drug, Herth Hope Index, Impulsivity Scale, and Satisfaction questionnaire (in CSTAR). Result:1) There were significant differences in CDI, SAIC, Conner’s Scale between patients and control group (thirty high school students). 2) There were significant changes in Symptom checklists, Readiness to Change-drug, Impulsivity scale. 3) There were positive responses in satisfaction questionnaire of treatment program except question about staff quality. Conclusion:These results suggest that multi-disciplinary structured treatment program is effective for adolescent substance abusers and multi-dimensional assessment of patients is inevitable to evaluate the change of patients.

      • Identification of Potential Serum Biomarkers in Mercury-Treated Mice Using a Glycoproteomic Approach

        <P>Mercury is a well-recognized health hazard and a deleterious environmental contaminant. Exposure to mercury can cause neurotoxic manifestations, nephrotoxicity, and immune function alterations; however, the mechanisms and related proteins responsible for these effects are still unclear. Our goal is to understand the relationship between the toxicity of mercury and the proteins affected by this toxic heavy metal and to define biomarkers for mercury intoxication. Two different forms of mercury, organic methylmercury or inorganic mercury sulfide, were orally administered to the mice for 4 weeks. To reduce complexity of the serum proteome, we enriched glycoproteins from mice serum with lectin concanavalin A resin, and the tryptic peptides of the purified glycoproteins were subjected to nanoultra performance liquid chromatography-Quadrupole time-of-flight for identification and label-free quantification. In this study, we characterized approximately 209 proteins from mice serum, and, among them, 21 proteins were differentially expressed in organic methylmercury-treated mice serum compared with the control group. Two proteins, serum amyloid P component (SAP) and inter α-trypsin inhibitor heavy chain 4 (ITI-H4), were upregulated in organic methylmercury-treated mice and confirmed with different doses of both types of mercury by Western blot analysis. Results of immunohistochemistry also confirmed the validity of SAP and ITI-H4 as biomarker candidates for organic methylmercury exposure. Findings of this study may assist in understanding the relationship between toxicity of mercury and upregulated proteins in mouse serum. Furthermore, the proteins identified here might be used as biomarker candidates in mercury intoxication.</P>

        조선시대 진주검무와 현행 진주검무의 춤사위 비교연구

        본 연구는 조선시대의 진주검무와 현재 전승되고 있는 국가무형문화제 제12호 진주검무를 살펴봄으로써 춤의 변화과정에 대한 연구를 하는데 그 목적이 있으며, 연구방법으로는 문헌분석 기법 및 영상자료 분석을 사용하였다. 결론은 첫째, 조선시대 진주검무의 복식은 전립, 남색쾌자, 그리고 군복의 일종인 융복을 착용하였으며, 이는 현행 진주검무의 복식도 동일하나 술띠와 한삼은 조선시대 이후에 사용되었다. 둘째, 조선시대 진주검무는 삼현육각으로 연주하였음을 추정할 수 있으며, 현행 진주검무는 대풍류 삼현육각으로 연주된다. 셋째, 조선시대 진주검무 대형은 2열 종대, 2열 종대 무진무태, 원형 대형으로 나타났으며, 현행 진주검무 대형은 2열 종대, 2열 종대 무진무태, 2열 종대 뒤보기 대형, 원형 대형, 1열 횡대로 연행되는데 1열 횡대는 조선시대에는 사용하지 않았던 대형이다. 넷째, 조선시대와 현행 진주검무의 전체적인 춤사위는 동일하지만 마무리 동작과 한삼춤은 조선시대에는 없는 현행 진주검무의 변화된 춤사위라고 할 수 있다. The purpose of this study is to investigate the process of changes of dance by examining the Jinju sword dance of Joseon era and the Jinju sword dance, the no.12 National Intangible Cultural Asset which has been handed down, and this study used literature analysis and image data analysis. In conclusion, first, costumes of Jinju sword dance of Joseon era included Jeonrip, Namsaekqweja, and Yungbok, a kind of military uniform, and the costumes for the current Jinju sword dance are the same, but Sulddi and Hansam were used after the Joseon era. Second, it can be assumed that Jinju sword dance of Joseon era was played in Samhyeon Yukgak, and the current Jinju sword dance is performed in Daepungrye Samhyeon Yukgak. Third, Jinju sword dance formation of Joseon era was shown as in two columns, two column Mujinmutae, and circular formation, and the current Jinju sword dance formation is performed in two rows column, two rows column Mujinmutae, two rows column back view formation, two column, two column Mujinmutae, two rows column backside formation, circular formation, and one row line, and one row line is a formation that was not used in the Joseon era. Fourth, the overall dance movement of Jinju sword dance of Joseon era and the current Jinju sword dance is the same, but the closing motion and Hansamchum are the changed dance movements of the current Jinju sword dance that was not in the Joseon era.
