Comparison of the fit accuracy of zirconia-based prostheses generated by two CAD/CAM systems

PURPOSE. The purposes of this study are to evaluate the internal and marginal adaptation of two widely used CAD/CAM systems and to study the effect of porcelain press veneering process on the prosthesis adaptation. MATERIALS AND METHODS. Molar of a lower jaw typodont resin model was prepared by adjusting a 1.0 mm circumferential chamfer, an occlusal reduction of 2.0 mm, and a 5° convergence angle and was duplicated as an abrasion-resistant master die. The monolithic crowns and copings were fabricated with two different CAD/CAM system-Ceramil and Zirkonzahn systems. Two kinds of non-destructive analysis methods are used in this study. First, weight technique was used to determine the overall fitting accuracy. And, to evaluate internal and marginal fit of specific part, replica technique procedures were performed. RESULTS. The silicone weight for the cement space of monolithic crowns and copings manufactured with Ceramil system was significantly higher than that from Zirkonzahn system. This gap might cause the differences in the silicone weight because the prostheses were manufactured according to the recommendation of each system. Marginal discrepancies of copings made with Ceramil system were between 106 and 117 μm and those from Zirkonzahn system were between 111 and 115 μm. Marginal discrepancies of copings made with Ceramil system were between 101 and 131 μm and those from Zirkonzahn system were between 116 and 131 μm. CONCLUSION. Marginal discrepancy was relatively lower in Ceramil system and internal gap was smaller in Zirkonzahn system. There were significant differences in the internal gap of monolithic crown and coping among the 2 CAD/CAM systems. Marginal discrepancy produced from the 2 CAD/CAM systems were within a reported clinically acceptable range of marginal discrepancy.
의과적 문제가 있고 소수 잔존치를 가지는 환자에서의 치주보철 임상증례: 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관법

치아가 얼마 남지 않은 부분 무치악 환자들에 있어 이중관(Double crown)을 이용한 치료의 성공적인 결과에 대해서 여러 문헌에서 보고 되고 있다. 이중관 가철성국소의치(Double crown removal partial denture)는 잔존치의 치주적인 상태가 좋지 않은 환자에게 치료의 대안이 될 수 있다. 이중관 가철성 국소의치는 특히 치주상태가 좋지 않고, 임플란트를 식립하기에는 의과적으로 문제가 있는 환자(Medically compromised patient)에게는 수술에 대한 부담 없이 치료할 수 있어 심리적이고 경제적인 이점이 존재한다. 본 증례에서는 상악은 다수의 치아가 잔존하여 고정성 보철물로 수복이 가능한 상태였고, 하악은 소수 잔존치만 남아있고 치주 상태가 매우 불량하여 잔존치를 이용한 클라스프 가철성 국소의치로는 수복하는 것이 거의 불가능하였다. 또한 환자는 1년 전에 하악 전치부 부위의 골수염으로 인한 수술병력으로 임플란트 식립이 어려운 상황이었다. 이러한 악조건에서 하악의 소수 잔존치를 이용한 이중관 의치는 2년이상의 기간동안 기능적, 심미적인 면에서 만족스러운 결과를 얻었기에 이를 보고하고자 한다. Successful results of treatments using double crown prostheses for the partially edentulous patients who have a few remaining teeth have been reported in several journals. A double crown removable partial denture can be an alternative treatment for the patients with a poor periodontal condition of remaining teeth. Since a double crown removable partial denture can be applied without the risk of surgical operation to the medically compromised patients with a poor periodontal condition which is inadequate for dental implants, it has psychological and economical advantages. In this case, there were sufficient remaining teeth to be restored with fixed prostheses in maxilla, while there were a few remaining teeth with a very poor periodontal condition so that it was almost impossible to restore with a clasp removable partial denture using these remaining teeth in mandible. In addition, the patient had the medical history of surgical operation due to osteomyelitis in the mandibular anterior areas a year ago, thus difficult to conduct an implant placement. The main objective of this report is to introduce our case because a double crown partial denture using a few mandibular remaining teeth showed satisfactory results in functional and esthetical aspects during more than two years follow-up period in this unfavorable condition.
