      • 근위축성측삭경화증 환자를 돌보는 가족간호자의 삶의 질과 영향 요인

        Purpose: The purpose of this study was to investigate QOL and the factors related to family caregivers who care for Amyotrophic Lateral Sclerosis (ALS) patients. Methods: The subjects were 83 family caregivers caring for ALS patients who visited the neurology outpatient section of a tertiary hospital located in Seoul between January and July of 2008. Their demographic characteristics were assessed and data were collected using the amyotrophic lateral sclerosis functional rating scale (ALSFRS), the caregiver burden inventory (CBI), HADS, and SF-36. Results: The mean score of the physical health component (PHC) of the family caregiver`s QOL was 62.6±24.4, and the mean score of the mental health component (MHC) of their QOL was 57.7±22.4. The mean score of caregiver burden was 76.5±30.7. Anxious family caregivers amounted to 55.4% and depressed family caregivers accounted for 63.9% overall. In a multivariate analysis, the PHC of QOL was explained by caregiver burden (41.1%), depression (9.4%), caregiver gender (3.8%), and caregiver age (3.1%). Anxiety (39.8%), caregiver burden (10.9%), patient gender (5.4%) and depression (1.7%) were predictive factors of the MHC of QOL. Conclusion: Caregiver burden and depression affected both the PHC and the MHC. Caregiver burden affected the PHC more than it did the MHC of QOL, but anxiety affected the MHC more than it did the PHC of QOL.

      • 경피적 관상동맥중재술 후 반좌위 체위변경이 환자의 요통과 불편감에 미치는 효과

        Purpose: The purpose of this study was to examine the effects of position change from supine to Fowler`s on back pain and discomfort in patients who remained on bed rest after undergoing PCIs with a vascular closure device. Methods: Data was collected from 35 inpatients who were hospitalized in coronary-care unit to perform PCIs with a vascular closure device at S hospital in Seoul from December, 2006 to May, 2007. Back pain, discomfort, presence and grade of bleeding and hematoma from femoral arterial puncture site and blood pressure/pulse rate were measured prior to, 10 minutes, 1 hour and 2 hours after position change. Data was analyzed with descriptive statistics, x2 tests and t-tests using SPSS/WIN 12.0 for Windows program. The level of significance(α) was set at 0.05 for this study. Results: We found that there was no significant difference in back pain and discomfort in 10 minutes after position change between the two groups. However, the experimental group reported significantly less back pain and discomfort than the control group in 1 hour (p<.01, respectively) and 2 hour (p<.01, respectively) after position change. There was no significant difference in the presence and grade of bleeding and hematoma at the puncture site, blood pressure, and pulse rate in 10 minutes, 1hour and 2hours after position change between the two groups. Conclusion: Fowler`s position change after PCIs in, therefore, safe and effective method of reducing back pain and physical discomfort without causing additional bleeding and changes in blood pressure and pulse rate.

        석·박사 학위논문에서 생리적 변수를 다룬 연구에 대한 분석 : 1962년부터 1996년까지 발표된 국내 학위논문을 중심으로 with reference to the thesis published in Korea from 1962 to 1996

        Objectives : The studies in biological, behavioral and psychosocial perspectives in nursing research are fundamental in the balanced development of nursing knowledge. Many nursing researchers have placed a strong emphasis on developing psychological and behavioral aspects of nursing knowledge as is evidenced by the large body of research in these areas. However, the paucity of nursing research using the principles of biological science for measurements and techniques have often invited open criticism by many nursing researchers. This study attempts to characterize the researches performed in master and doctoral thesis which used physiological variables. Methods : We analyzed masters' thesis from 1962 to 1996 and doctoral thesis from 1982 to 1996 listed in CD from Korean Nurses Association. Results : Out of 3060 masters' thesis from 1962 to 1996, 342 (11.2%) thesis used physiological variables whereas 43 (10.1%) doctoral thesis used physiological ones. Number of thesis with physiological variables was the highest in Seoul National University for masters' thesis whereas for the doctoral thesis the number was the highest in Yonsei University. The research subjects in these thesis with physiological variables were mostly patients (53.5% in masters' and 69.8% in doctoral). Master's thesis often conducted either nonexperimental or survey research (69.9%) whereas experimental research design prevailed vital signs(l81 thesis), blood tests(133), body composition(124), microorganisms(74), gastrointestinal functions(36), lab test(33), drug(2l), urinary functions(20), movement(l7), Apgar Score(l5), reproductive functions(9), skin related functions(6), body fluid and electrolytes(4), parasite(4), metabolism(3), and sensory(1). Among the 338 master' thesis which studied the common nursing concepts such as anxiety, pain, stress, and depression, 48(14.2%) thesis utilized physiological variables while 15 out of 35 doctoral thesis addressed these concepts using physiological variables. Conclusion : In summary, our results indicate that despite the large amount of nursing research performed over the last decades, there are a few nursing researches done with physiological variables either in masters' or doctoral thesis. To enrich nursing knowledge with different perspectives of nursing including especially biological ones, the efforts have to be made to conduct bionursing science research.

        신규간호사를 위한 맞춤형 멘토 프로그램(새싹프로그램)의 개발 및 적용 : 일개 대학병원 중환자실을 중심으로

        Purpose: The purpose of this study was to develop specialized mentor program to improve adaptation for new nurses of intensive care unit and to identify the effect of the program on early resignation rate. Methods: The study adopted nonequivalent control group, non synchronized design. Based on the identification of problems and the needs of new nurses, a 6 month specialized mentor program was developed. The program was consisted of three parts; developing knowledge, improving interpersonal relationship and increasing coping competency for emergency situation. Data were collected between July 2006 and 2008 from 37 nurses and the early resignation rate was compared before and after the implementation of the mentoring program. Results: Early resignation rate of the nurses who received the mentor program was significantly lower than that of the nurses who didn't. The resignation rate within 1 year dropped from 44.5% to 8.3%(p=.034). Conclusions: The specialized six month mentoring program was effective in reducing early resignation rate.

        중환자실 간호사의 기초간호과학 지식의 필요성 분석

        The purpose of this study was to identify the knowledge contents of basic nursing sciences needed by nurses in the practices of the intensive care unit(ICU). To attain the goal of this study, the nurses working at 10 hospitals in the areas of Seoul and Kangwon Province were randomly selected. They were primarily interviewed, and the open question was secondarily put to them through the questionnaire. In the process of the 1st interview, the interviewees were asked of the question, “What is the knowledge of basic sciences such as anatomy, pathology, physiology. microbiology, pharmacology and the like thought to be lacking when you communicate with doctors in the ICU and when you carry out your nursing practices in it?” The contents of the interview were tape-recorded. The period of data collection ranged from May 1, 2001 to Sept 30. The interviews were conducted with total of 20 nurses. The open-end questionnaire was secondarily mailed to nurses. 113 questionnaires were returned. 100 questionnaires except 13 ones thought to be poorly completed in content were used for data analysis. Three coders classified data obtained from the interview and the questionnaire research into 5 detailed items relating to such as anatomical physiology, pathology, pharmacology. microbiology and basics of nursing. The three coders had experiences in nursing education of 18 years, 8 years and 6 years, respectively, and of them one coder was professor in basic nursing science. Data were statistically treated using frequency analysis and percentage by the SAS program. As a result, the following findings were obtained: It was found that the contents that ICU nurses responded were most needed in the field of Human structure and function were water and electrolytic balance(38%), blood and circulatory system(20%), changer in the patient’s skin(12%), the arrangement of the human body(10%) and the endocrine system(10%), nervous system(6%), and assessment of the state of the patient's consciousness(4%). It was found that the contents that ICU nurses responded were most needed in the field of pathology were found to be the process of the progress of the disease(32%), symptoms of the disease(27%), prognosis of the disease(22%), followed by the injury-healing process, clinical pathological examination, and examination by radiation. It was found that the contents that nurses responded were most needed in the field of pharmacology were the effect of drug(25%), the side effect of drug(22%), the relationship between diseases and drug(20%), the relationship between disease-causing bacteria and drug(20%) and chemotherapy(2%). It was found that the contents that ICU nurses responded were most needed in the field of microbiology were the relationship between diseases and disease-causing bacteria(45%), Kinds and characteristics of disease-causing bacteria(18%), infection control(16%), application of the aseptic technique(12%), isolation(9%) and the like. It was found that the basic knowledge that ICU nurses responded were needed were the identification of the patient’s current state(36%), understanding of the therapeutic process(22%), the operating principle of medical equipment and instrument(20%), medical terminology(9%), equipment and instrument management(7%), calculation of the dose of injection(2%) and the like.

        기초간호자연과학의 병태생리학, 병원미생물, 약물의 기전과 효과 내용별 필요도에 대한 연구

        The purpose of this study was to define the content of the requisite knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for clinical knowledge for nursing practice. Contents of knowledge on pathological physiology, clinical microbiology, and mechanisms and effects of drugs were constructed from syllabus of basic nursing subjects in 4 colleges of nursing, and textbooks. The degree of need of 72 items was measured with a 4 point scale. The subjects of this study were college-graduated 136 nurses from seven university hospitals in Seoul and three in Chonnam Province, Kyungbook Province, and Inchon. They have been working at internal medicine ward, surgical ward, intensive care unit, obstetrics and gynecology ward, pediatrics ward, opthalmology ward, ear, nose, and throat ward, emergency room, rehabilitation ward, cancer ward, and hospice ward. The results were as follows : 1. The highest scored items of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice were side effects of drugs, anticoagulants, mechanisms of drugs, antihypertensive drugs, tolerance and addiction of drugs, interactions among drugs, hospital infection in the order of importance. The lowest scored item was structure of microrganisms. 2. The highest order of need according to unit was repair in tissue injury unit, definition·etiology·classification of inflammation in inflammation unit, trasplantation and immunologic response in alterations in immunity unit, thrombus and thrombosis in disorders of cardiovascular function unit, gene disorders in genetic disorders unit, hospital infection in infection unit, virus in microrganisms unit, side reactions of drugs in introduction unit, anticonvulsants in drugs for central nervous system unit, local anesthesia in anesthesia unit, anticoagulants in drugs for cardiovascular system unit, anti-inflammatory drugs in antibiotics unit, anti-ulcer drugs in drugs for digestive system unit, and bronchodilators in drugs for respiratory system unit. 3. The common content of the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs needed for all clinical areas in nursing were side effects of drugs, anticoagulants, interactions among drugs, and hospital infection. However, the degree of need of each pathological physiology, clinical microbiology, and mechanisms and effects of drugs was different depending on clinical areas. 4. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as tissue changes due to injurious stimuli, degenerative changes of tissue, alterations in metabolism of carbohydrates, ischemia, hyperemia and congestion, hospital infection, structure of microorganism, classification of microorganism, bacteria, virus, antidepressants, antipsychotic drugs, antiemetic drugs, antiparkinsonism drugs, antianxiety drugs, antibiotics, tuberculostatics, antiviral drugs, antifungal drugs, parasiticides, antiulcer drugs, antidiarrheais, and anti constipation drugs were shown according to the work area. 5. Significant differences in the knowledge of pathophysiology, clinical microbiology, and mechanisms and effects of drugs necessary for nursing practice such as transplantation and immunologic response, alterations in the metabolism of uric acid, structure of microorganism, classification of microorganism, immunosuppresants, drugs for congestive heart failure were demonstrated according to the duration of work. Based on these findings, all the 72 items constructed by Korean Academic Society of Basic Nursing Science should be included as contents of the knowledge of pathophysiology, clinical microbiology. and mechanisms and effects of drugs
