        통합 의학 치료로 5년 생존 및 완전 관해에 도달한 췌장암 증례 보고 1례

        Objectives: This is a five-year survival and complete response (CR) report on pancreatic cancer treated with western medicine and Korean traditional medicine. Method: A 59-year-old woman diagnosed with pancreatic cancer visited ○○ Korean traditional medicine hospital after neoadjuvant chemotherapy and pylorus-preserving pancreaticoduodenectomy. She was treated with Korean traditional medicine, including acupuncture, abdominal moxibustion, wild ginseng pharmacopuncture, and herbal medicine, which was based on integrated medicine therapy (IMT), from March 2018 to September 2022. The tumor size was measured by scanning with computed tomography (CT), magnetic resonance imaging, and positron-emission tomography/CT. Adverse events were evaluated using laboratory conclusion and National Cancer Institute Common Terminology Criteria for Adverse Events version 5.0. Result: During four years and three months of treatment, IMT maintained safety. The patient finally reached five-year survival without any recurrence or complication (CR) on October 26, 2022. Conclusion: We suggest that an integrative approach including Korean traditional medicine can be a meaningful treatment option for pancreatic cancer. Further studies should be performed to establish the proper treatment protocol of integrative medicine for pancreatic cancer.

        식용색소에 의한 CEROMER 수복물의 색안정성

        Statement of problem : Ceramic and composite resin have been used to fulfill the demand for esthetic prosthesis. However, ceramic is easy to break and wears off the opposite natural teeth. Conventional composite resin also has low abrasive resistance and color stability. Ceramic Optimized Polymer (ceromer) was developed in mid-1990s to overcome the shortfalls of ceramic and composite resin. Ceromer has similar abrasiveness with the natural tooth and has relatively high strength. Color stability affects esthetics and long-term prognosis of the prosthesis. Purpose The purpose of this study was to compare color stability of ceromers(2 types : $Artglass^{(R)}$. $Targis^{(R)}$) with ceramics ($Vintage^{(R)}$-polishing, $Vintage^{(R)}$-glazing). Material and Method : The color difference(${\Delta}E^*$) was measured by spectrophotometer with different immersion time. Twenty disks, 3mm in thickness and 10mm in diameter, were fabricated for each specimen in shade A2(Vita Lumin shade guide), Specimens (5 samples in each group) were immersed in the food colorants (Red no.3. Yellow no.4, Blue no.1, Distilled water) for 24 hours, 48 hours and 72 hours respectively. $L^*,\;a^*$ and $b^*$ value were measured with spectrophotometer (CM 503i : Minolta Co., Japan) and mean ${\Delta}E^*$ value was calculated for statistical analysis Results : The results of this study were obtained as follows. 1. The ${\Delta}E^*$ values of all test samples increased with the time of immersion. 2. The ${\Delta}E^*$ values of all materials increased in order of Distilled Water, Yellow no.4, Blue no.1 and fed no.3. There was significant difference between Red no.3 and the other food colorants(p<0.05). 3. The ${\Delta}E^*$ values increased in order of $Vintage^{(R)}$-glazing, $Vintage^{(R)}$-polishing. $Artglass^{(R)}$ and $Targis^{(R)}$. There was significant difference between $Vintage^{(R)}$-glazing and the other materials (p<0.05). Conclusion : By means of the above results, immersion time was found to be a critical factor for color stability of ceromer. For the long-term color stability of prosthesis it is recommended patients having ceromer prosthesis ($Artglass^{(R)},\;Targis^{(R)}$) to reduce the habitual intake of Red no.3 colorants con taming foods.

        여대생의 자기효능감과 대인관계능력이 진로성숙도에 미치는 영향 관계에서 자아탄력성의 매개효과

        본 연구는 자기효능감, 대인관계능력과 자아탄력성을 결합시켜 여대생의 진로성숙도 증진 프로그램 개발을 위한 예비조사로 수행하였다. 이를 위해 2017년 3월 6일 부터 3월 20일까지 진로지원센터를 운영하는 00도 00대학에 재학 중인 여대생 345명에게 설문을 실시하여, 333부를 최종 분석에 사용하였다. SPSS v.22를 활용하여 분석한 결과는 다음과 같다. 첫째, 가정의 수입과 학년은 진로성숙도에서 집단 간 차이를 나타내지 않았다. 둘째, 자기효능감은 진로성숙도에 영향을 미치는 것으로 나타났다. 셋째, 대인관계능력은 진로성숙도에 영향을 미치는 것으로 나타났다. 넷째, 자아탄력성은 자기효능감과 진로성숙도, 대인관계능력과 진로성숙도 사이에서 모두 매개효과를 미치는 것으로 나타났다. 이러한 연구 결과는 의도된 변인들을 결합시킨 진로성숙도 증진 프로그램이 여대생의 진로를 지원하는데 유용한 자료로 활용될 것으로 기대한다. This study was carried out as a preliminary study to help prepare a career support program that combines self-efficacy, interpersonal competence, and self-resilience for female college students. To that end, a survey was executed on 345 female students of two universities, one in the Southern area and one in Norther area of Kyunggi-do from March 6 through 20, 2017, and 333 questionnaires were used for final analysis using SPSS v.22. Major study findings are as follows: First, family income and school years showed no statistically significant differences among groups. Second, self-efficacy was found to influence career maturity. Third, interpersonal competence was found to influence career maturity. Fourth, self-resilience was shown to have a mediating effect in the relationship between both self-efficacy and career maturity, and interpersonal competence and career maturity. These findings have significance in that a career-maturity enhancing program combing all the factors mentioned herein can be served as useful information for universities and colleges which intend to offer such program to female students.

        초기 청소년의 사회부과 완벽주의와 자기구실 만들기와의 관계에서 정서인식 명확성의 매개효과

        본 연구는 초기 청소년의 사회부과 완벽주의와 자기구실 만들기와의 관계에서 정서인식 명확성의 매개효과를 확인하고자 하였다. 이를 위하여 경기도 소재 초등학교 5, 6학년에 재학 중인 초기 청소년 604명을 대상으로 2018년 3월 20일~3월 28일까지 사회부과 완벽주의, 자기구실 만들기, 정서인식 명확성 척도를 사용하여 설문을 실시하였다. 자료처리는 SPSS 23.0과 AMOS 22.0으로 통계처리 하였다. 본 연구의 주요결과를 요약하면, 첫째, 사회부과 완벽주의는 자기구실 만들기와 정적 상관관계, 정서인식 명확성과는 부적 상관관계, 정서인식 명확성과 자기구실 만들기는 유의미한 부적 상관관계를 나타냈다. 둘째, 사회부과 완벽주의와 자기구실 만들기와의 관계에서 정서인식 명확성의 매개효과를 알아보기 위해 연구모형과 경쟁모형을 설정 하고 이를 검증한 결과, 정서인식 명확성이 부분 매개하는 연구 모형이 더 적합한 것으로 확인되었다. 이러한 결과는 자기구실 만들기에 영향을 미치는 주요변인들을 확인하여 초기 청소년의 자기구실 만들기 감소 및 예방을 위한 상담 개입방안의 기초 자료를 제공하였다. This study attempted to identify the mediating effect of emotional clarity in the relationship between early adolescents’ socially‐prescribed perfectionism and self‐ handicapping. For this purpose, for604 fifth and sixth grade early adolescent students attending elementary schools in Gyeonggi‐do, this study conducted a questionnaire survey using a scale of socially‐prescribed perfectionism, self‐handicapping and emotional clarity from March 20 to March 28, 2018. Data process was performed with SPSS 23.0 and AMOS 22.0. To summarize the main results of this study, first, socially ‐prescribed perfectionism showed a positive correlation with self‐handicapping, a negative correlation with emotional perceptual clarity, and a significantly negative correlation with emotional clarity and self‐handicapping. Second, in the results of setting up and verifying a research model and a competition model to examine the mediating effect of emotional clarity in the relationship between socially‐prescribed perfectionism and self‐handicapping, the model partially mediated by the emotional perceptual clarity was found to be more appropriate. These results of this study provided the basic data for the counseling intervention strategies for reducing and preventing early adolescents’ self‐handicapping by identifying the main variables affecting self‐handicapping.

        축열식 촉매 산화(RCO) 반응의 성능 최적화를 위한 전산 해석

        본 논문에서는 RCO 반응기의 최적 조업 조건을 도출하고자 현재 조업 중인 RTO 반응기 촉매층의 압력분 포, 유속분포, 유선장, 체류시간, 온도분포 등에 대하여 분석하였고 최적의 조업 조건을 도출하기 위한 전산유체해석 을 수행하였다. 전산 해석 결과, 현재 조업중인 반응기 촉매층의 축열재로 인한 압력손실은 크지 않은 것으로 나타 났으며, 세라믹 필터 이후 유속은 1.8∼2.7 m/s로 특별한 편류없이 비교적 안정적 유속분포를 보이고 있었다. 기류분 포를 좀 더 개선하려고 한다면 설비 내부에 플래넘과 촉매층 접속 구간 연장 등의 방법을 고려할 수 있을 것으로 판단된다. 그러나 기류 안내판이나 다공판 부착 등의 방법은 큰 효과는 없을 것으로 나타났다. The computational fluid dynamics was analyzed for the pressure distribution, stream velocity distribution, stream line field, retention time and temperature distribution which are applied to the catalyst layer in the RCO reactor to derive the optimum operating condition of the heat condensing type catalytic oxidation (RCO) reactor. The results from the computational analysis revealed that the pressure loss due to the ceramic honeycomb in the catalytic bed of the reactor which is operating currently is not significant and the stream velocity (1.8∼2.7 m/s) after the ceramic filter is working in stability without big channeling. To improve the stream velocity distribution of the air stream, it is necessary to extension of the connecting range between the plenum and catalytic bed inside the facility. However, the method of attaching the air stream guide vane or the perforated plate inside the reactor was not so effective.

        Wii Balance Board를 이용한 Sit-to-Stand와 보행시 지면반발력의 타당도 분석

        PURPOSE: A good, valid, and feasible tool for evaluating sit to stand (STS) is needed to help clinicians quantify the STS ability of stroke patients and people with balance disorders. The purpose of this study was to evaluate the concurrent validity of the Nintendo Wii Balance Board (WBB) and a force plate during STS and gait. METHODS: Seventeen healthy adults performed five trials of STS and gait on the WBB placed on the force plate. The force plate and the WBB were compared in regard to center of pressure (COP) and ground reaction force (GRF) data that were collected simultaneously. The variables used for analysis were time (s), integral summation (%), COP path length (㎜), COP x range, and COP y range, all of which were measured for both tasks. Counter (%), peak (%), and rebound (%) were analyzed for STS, and 1<sup>st</sup> peak (%), min peak (%), and 2nd peak (%) were analyzed for gait. The concurrent validity was analyzed using an intraclass correlation coefficient (ICC) and a standard error of measurement (SEM) with a 95% confidence interval. RESULTS: The concurrent validity of the WBB for STS ranged from fair to good (ICC=.701∼.994, SEM=.029∼3.815). The concurrent validity for gait was good (ICC=.869 ∼.989, SEM=.007∼2.052) aside from path length and x and y ranges of COP (ICC=-.150∼.371, SEM=3.635∼4.142). CONCLUSION: The GRF of the WBB has a good validity for STS and gait analysis. The WBB is remarkably portable, easy to use, and convenient for clinically assessing STS and gait.

        HMIPv6 환경에서 새로운 동적 페이징 기법

        최근 무선 접속망의 급속한 발전으로 이동 통신 환경에서 인터넷 서비스 제공에 대한 요구가 늘어남에 따라, IP기반의 이동성 지원과 마이크로 이동성에 따른 위치관리 트래픽을 줄일 수 있도록 해주는 HMIPv6 프로토콜이 제안되었다. HMIPv6에 페이징을 적용함으로써 MN의 위치등록에 필요한 시그널링 및 위치정보 데이타베이스의 갱신을 위한 처리를 줄일 수 있다. 본 논문에서는 각기 다른 특성을 갖는 이동 노드들에 하나의 동일한 페이징 영역이 정적으로 적용되는 것이 아닌, 각 이동 노드의 특성를 고려하여 각각의 이동 노드에 적합한 최적의 페이징 영역이 설정되는 HMIPv6 환경에서의 새로운 동적 페이징 기법(DP-HMIPv6)을 제안한다. 수학적 성능 분석을 통해 정적 페이징 기법보다 제안 기법인 동적 페이징 기법을 이용하였을 때 비용이 절감되었음을 확인 할 수 있었다. 또한 네트워크 상에 이동 노드의 수가 많아질수록 동적 페이징 기법을 통한 비용 절감은 더욱더 효과적임을 알 수 있다. With increasing demand of using mobile Internet, HMIPv6 is proposed by IETF to reduce location management traffics due to micro mobility and to provide IP-based movement. A paging mechanism combined with HMIPv6 can reduce the signaling traffics of a mobile node. We propose a dynamic paging mechanism in HMIPv6 which decides an optimal paging area for each mobile node by considering its characteristics. Total costs of a static paging mechanism and a dynamic paging mechanism are analyzed by mathematical formulas. As a result, we proved that the dynamic paging mechanism reduced total signal traffic costs more than a static paging mechanism.
