Purpose: Grafting with autograft skin remains the most effective method for treating skin defects. When insufficient donor sites are present or patients are afraid of the operation, a skin graft is impossible. Cultured allogenic keratinocytes speed wound healing by providing cover and by producing growth factors and extracellular matrix protein. We report an application of cultured allogenic keratinocytes ($Kaloderm^{(R)}$, Tegoscience, Seoul, Korea) in the treatment of an acute partial thickness skin defect. Methods: From March 2005 to January 2006, 20 patients with a partial thickness skin defect were treated with cultured allogenic keratinocytes. The wound was covered with a sheet of cultured allogenic keratinocytes and ointment with $Bactigras^{(R)}$ gauze. The wound was inspected every two or three days. We regarded completion of epithelialization as wound healing. Results: The mean period between time of injury and time of $Kaloderm^{(R)}$ application was 7.5 days. The time taken from application of $Kaloderm^{(R)}$ to complete closure of the wounds was 7.2 days. Conclusion: In view of the favorable outcome, cultured allogenic keratinocytes are safe and effective biologic dressing materials for use in the treatment of open wounds.

Purpose: Since Rheinwald and Green laid the foundation of epidermal cell culture technology in 1975, many clinicians and scientists have attempted to prove the effectiveness of cultured epidermal autologous(CEA) or homogenetic(CEH) grafts in the wound healing process. In contrast to CEA which cultured from a patient's skin on demand, Cultured Epidermal Homograft (CEH) can be readily available to use on cleaned wounds. In this study, we conducted a controlled clinical trial in order to confirm the effectiveness of CEH in treating partial-thickness 2nd degree burn wounds.Methods: From July 2003 to January 2004 at Hangang Sacred Heart Hospital, we performed a clinical trial in which 35 patients who suffered from 2nd degree burns were enrolled. Wounds were randomly divided into two parts, control and test sites. Test sites were treated with allogeneic keratinocyte sheets (Kaloderm, Tegoscience Inc.), a CEH commercialized in Korea.Results: All wounds healed completely without any major complication. The complete healing took 8.3± 2.8(mean±S.D.) days in the test sites as opposed to 11.7±3.3 days in the control sites.Conclusion: Based on these results, we concluded that CEH accelerates re-epithelialization of partial thickness burn wounds and CEH can be an safe alternative to skin grafts for 2nd degree burns.
비후성 반흔 각질세포와 정상 각질세포의 유전자 비교분석

Purpose: There is no clear evidence of the original cause of hypertrophic scar, and the effective method of treatment is not yet established. Recently the steps of searching in gene and molecular level are proceeding. we are trying to recognize the difference between keratinocytes of hypertrophic scar and normal skin. Then we do support the comprehension of the scar formation mechanism and scar management. Methods: Total RNAs were extracted from cultured keratinocytes from 4 hypertrophic scars and normal skins. The cDNA chips were prepared. A total of 3063 cDNAs from human cDNA library were arrayed. And the scanning data were analyzed. Results: On microarray, heat shock protein, pyruvate kinase, tumor rejection antigen were more than 2 fold intensity genes. Among them, heat shock 70 kd protein showed the strongest intensity difference. Conclusion: In this study, it can be concluded that heat shock proteins play an important role in the process of wound healing and scar formation. This study provides basic biologic information for scar research. The new way of the prevention and treatment of scar formation would be introduced with further investigations.
Moisture Wound Healing Characteristics of Alginate Sponge and Hydrogel

건조/가교법으로 제조한 알지네이트 스폰지 및 하이드로젤의 물리적, 생물학적 특성 및 창상치유 특성을 고찰하였다. 하이드로젤은 스폰지 대비 높은 평형 함수율을 보유하였고, 자체적으로 수분을 함유하고 있기 때문에 상대적으로 우수한 습윤 창상치유 환경을 제공할 수 있었다. 알지네이트 스폰지 및 하이드로젤의 사이토카인 결속효과에 기인하여 대식세포로부터 분비되는 전염증성 사이토카인의 함량이 감소됨을 확인하였으며, 특히 하이드로젤의 사이토카인 억제효과가 더욱 두드러지게 나타났는데, 이는 보다 팽윤된 상태에서 알지네이트 분자의 사이토카인에 대한 결속력이 증가함을 의미한다. 창상형성 초기 하이드로젤에 의한 창상치유 및 수축 효과가 스폰지에 비해 우수한 것으로 나타났으나, 상피화는 스폰지를 적용했을 때 보다 우세하게 진행되었다. 조직학적 평가와 RNA 발현 분석으로부터 알지네이트 스폰지 및 하이드로젤은 혈관 및 콜라겐 섬유의 형성, 상피조직의 재생 및 단백질의 생성 등을 촉진함을 확인하였다. Alginate sponge and hydrogel were prepared by a drying/crosslinking method and their wound healing characteristics were investigated comparatively. The alginate hydrogel had a higher equilibrium water content than the sponge, providing a moist wound healing condition without absorbing exudate from a wound. The amounts of proinflammatory cytokines released by macrophages were lowered due to the cytokine-binding effects of the alginate sponge and hydrogel. The hydrogel lowered the cytokine level more dominantly than the sponge, suggesting that the affinity of alginate molecules to cytokines increases at a more swollen state. The hydrogel allowed superior wound healing and contraction at the early stage of application. However, epithelialization was conspicuous when the sponge was applied. It was confirmed through histological examination and RNA expression analysis that angiogenesis, formation of collagen fibers, regeneration of epithelium, and production of protein were promoted using the alginate sponge and hydrogel as wound dressing materials.
