        『捷解新語』의 이유표현

        Such difference in the conjunctive particles also show the clear regularities in terms of historical change. That is, those conjuntive particles which don’t differentiate each other in their usage and meaning, cannot coexist resulting in the difference of those not in use while the conjunctive particles that differentiate each other in the usage and the meaning, can coexist thus, each assuming the role of expressing the condition of the different parts. Such difference in the meaning and the usage depends on the modality. In comparison with contemporary Japanese language, we can conclude that the above-mentioned grammatical rules are common phenomena in the history of Japanese language.

        실험견의 급성 저산소성 폐고혈압증에서 K+ Channel 의 역할

        N/A Objectives: Numerous studies about mediators in acute hypoxic process have been done. These studies revealed that endothelium derived relaxing factor(EDRF, known as nitric oxide) contributed greatly to the pathogenesis of hypoxic pulmonary hypertension and endothelium derived hyperpolarizing factor(EDHF) regulated the tone of vascular smooth muscle. EDHE is known to open K+ channel, hyperpolarizes vascular smooth muscle cells, closes voltage-dependent Ca++ channel and finally relaxes vascular smooth muscle. Some studies revealed that acute hypoxic pulmonary hypertension was developed by inhibition of EDRF &EDHF. To investigate the role of K+ channel in pulmonary hypertension during acute hypoxia, we measured changes of hemodynamic parameters in experimental dogs after adding glibenclamide(K+ channel blocker), methylene blue(S-guanylate cyclase inhibitor), L-NNA(NO synthase inhibitor) and nicorandil(K+ channel opener and S-guanylate cyclase activator). Methods: Six dogs were anesthetized with thiopental sodium and mechanically ventilated with Harvard volume-cycled animal ventilator. Venous and arterial catheters were placed in the limb vein for infusion and femora} artery to measure systemic arterial pressure. Swan-Ganz catheter was inserted via right internal jugular vein for measuring pulmonary arterial pressure, pulmonary capillary wedge pressure asnd cardiac output. We measured the influence of glibenclamide (3mg/kg). methylene blue(lmg/kg), L-NNA (30mg/kg) and nicorandil(300μg/kg) on the changes of hemodynamic parameters during normoxia and hypoxia. Results: The infusion of nicorandil did not affect mean pulmonary arterial pressure during normoxia and significantly inhibited the increase of mean pulmonary arterial pressure during hypoxia. Glibenclamide did not change mean pulmonary arterial pressure during normoxia, but significantly augmented the increase of mean pulmonary arterial pressure during hypoxia. Methylene blue infusion did not affect mean pulmonary arterial pressure during normoxia, but moderately augmented the increase of mean pulmonary arterial pressure during hypoxia. The infusion of L-NNA did not affect mean pulmonary arterial pressure during normoxia and augmented the increase of mean pulmonary arterial pressure singnificantly during hypoxia, The increase of pulmonary arterial pressure induced by glibenclamide, methylene blue, and L-NNA, under hypoxic state was inhibited by nicorandil. Conclusion: Glibenclamide (K channel blocker) did not affect the mean pulmonary arterial pressure in normoxic state, but augmented the pulmonary hypertension during hypoxia and this effect was inhibited by nicorandil. It is suggested that K+ channel contributes to pulmonary hypertension developed during hypoxia, and the depressive effect of nicorandil seems to be attributable to its dual actions as a K' channel opener and an activator of S-guanylate cyclase.

        Biphasic 자동형 제세동기 시스템 개발

        본 연구에서는 심장 돌연사(sudden cardiac death, SCD)의 주된 원인인 심실세동(ventricular fibrillation)을 기존의 monophasic 제세동기와는 달리 낮은 에너지에서 효율적으로 제거할 수 있는 biphasic 자동형 제세동기를 개발하였다. 개발한 제세동기는 고전압 충 $.$ 방전부와 신호 처리부의 하드웨어와 세동검출 알고리즘과 시스템 제어 알고리즘의 소프트웨어로 구성하였고, 160번의 연속적인 충 $.$ 방전 테스트를 통하여 안정성을 확인하였으며, ECC simulator에서 발생되는 6종의 총 30가지 세동신호에 실시간 세동점출 알고리즘을 적용하여 검출능력을 평가하였다 또한 그 시스템의 임상적 효용성과 안전성을 검증하기 위하여 5마리의 돼지를 대상으로 시스템의 적절한 세동 검출 및 세동 제거 능력을 실험하였고 그 후 시스템의 효용성을 향상시키기 위한 연구로 동일한 에너지를 다른 전압 레벨에서 방전시켜 이에 따른 제세동 효율을 조사하였다. In this paper, an automatic external biphasic defibrillator that removes ventricular fibrillation efficiently with a low discharging energy has been developed. The system is composed of software including a fibrillation detection algorithm and a system control algorithm, and hardware including a high voltage charging/discharging part and a signal processing part. The stability of the developed system has been confirmed through continuous charging/discharging test of 160 times and the detection capability of the real-time fibrillation detection algorithm has been estimated by applying a total of 30 various fibrillation signals. In order to verify the clinical efficiency and safety, the system has been applied to five pigs before and after fibrillation inductions. Also, we have investigated the system efficiency in removing fibrillation by applying two different discharging waveforms, which have the same energy but different voltage levels.

        RAM of weapons in the defense is an important performance factor and essential to improve the RAM from design to test and operational stage. The previous researches have been studied to establish the RAM goal. The characteristic of harbor underwater surveillance system (HUSS), which is operated in underwater and monitor a wide range, has problems that make it difficult to repair. However, the previous researches commonly do not provide the design criteria of weapon system for enhancing the RAM. This limitation is fatal problem because HUSS requires high reliability. This paper proposes a method of establishing RAM goal for HUSS using allocation and verification in a line replacement unit (LRU) level. The proposed method provides the optimal design criteria and can reduce the development period and cost, improving its RAM through the redesign and part change.

        당뇨병에 합병된 비안와형(Rhinoorbital) 모균증 1 예

        Mucormycosis is a rare, rapidly progressive and often fatal opportunistic fungal infection, which occurs most commonly in patients with uncontrolled diabetes. We reported a case of rhinoorbital mucormycosis confirmed by nasal mucosal biopsy, which showed characteristic fungal hyphae and vascular invasion. The patient was a 41-year-old woman, who had diabetes mellitus for 5 years, presented with a 2-day history of redness and swelling in the left eyelid and maxillary area. On the admission, blood glucose was 412 mg/dl. Brain CT revealed pansinusitis, air-fluid level in the maxillary sinus, preorbital swelling and soft tissue density in the nasal cavity. But there was no demonstrable bony destruction. After aggressive surgical resection, systemic intravenous amphotericin B (1.0 mg/kg/day) was given for 7 weeks to a total dose of 2.0 g. On the 72th hospital day, she developed sudden, unexpected alteration of consciousness followed by coma. Two days later, the patient died and an autopsy was not performed.

        정상 성인의 심박변화율과 운동량 척도 및 인체계측적 요소와의 관계

        To know the relations between the heart rate variability and physical activity score, the heart rate variabilities were measured through the R-R interval variation(R-RIV) at rest, deep breathing and Valsalva maneuvering state. The subjects included in this study were 60 normal non-sedentary persons who had no my systemic diseases. And the values of heart rate variabilities were correlatively analysed with age, weight, height, body mass index(BMI), physical activity score(PAS) and amplitude of R-wave. And the results were as follows; The mean heart rate variabilities of subjects were 0.176±0.085, 0.314±0.146, and 0.322±0.174 at rest, deep breathing and Valsalva maneuver, respectively. There are significant gender difference in heart rate variabilities at deep breathing and Valsalva maneuvering. The heart rate variabilities were negatively correlated with age(p<0.05) and BMI(p>0.05), but positively correlated with the height(p<0.05), weight, physical activity score and amplitude of R-wave. The physical activity score of the subjects was positively correlated with heart rate variability during deep breathing and Valsalva maneuvering state with statistical significance.(p<0.05) As a conclusion this study that the physical activity score had significant correlativity with the heart rate variability and the heart rate variablity could be used as a useful tool to measure the training effects of exercise on the heart.
