        The Relationship Between Muscle Fatigue and Balance in the Elderly

        Objective To investigate the effect of gastrocnemius muscle fatigue on postural control ability in elderly people.Methods Twenty-four healthy elderly people participated in this study. The postural control ability of single leg standing was evaluated with Health Improvement & Management System (HIMS) posturography before and after fatiguing exercises. After evaluating initial postural control ability, the maximal voluntary contraction (MVC) of ankle plantarflexion was assessed using a surface electromyogram from the medial belly of the gastrocnemius muscle. After a 5-minute resting period, subjects began submaximal isometric ankle plantarflexion (40% MVC) until 40% of MVC was dropped below 95% for 5 seconds, or subject couldn’t continue working out due to muscle fatigue. And postural control ability was assessed after fatiguing exercise. The mean deviation of center of pressure (COP), length of COP movement, occupied area of COP were measured, and analyzed by paired t-test. Results Mediolateral deviation, length of COP movement, and area of COP occupied were increased after fatiguing exercise of the gastrocnemius muscle. Anteroposterior deviation and length of COP movement were also increased, but had low statistical significance. Conclusion These findings suggest that the gastrocnemius muscle fatigue affects mediolateral stability and accuracy during single leg standing in elderly people. Therefore muscle endurance training is necessary to prevent falls in elderly people.

        주관증후군에서 임상양상에 따른 초음파와 전기진단학적 비교

        Objective: To evaluate the relationship among the clinical characteristics, electrophysiological severity and sonographic findings of cubital tunnel syndrome (CuTS). Method: We enrolled 19 patients with CuTS and 19 healthy adults. We evaluated body mass index (BMI), symptom duration, self-administered questionnaire of the ulnar neuropathy at the elbow (SQUNE), McGowan score and electrophysiological severity score by Mondelli. We measured the crosssectional area of the ulnar nerve and cubital tunnel, and the ratio of the ulnar nerve to cubital tunnel. The long and short axis of the ulnar nerve were measured in diameters with the elbow flexed and extended. Results: The ulnar nerve, with the elbow flexed and extended, was larger in the CuTS group than in the healthy control group (p< 0.05). However, there is no significant difference between BMI and the cubital tunnel area. The electrophysiological severity score was correlated with the SQUNE (r=0.738, p=0.010) and McGowan score (r=0.810, p=0.002), but not with the symptom duration. Sonographic findings did not correlate with the symptom duration, SQUNE, McGowan score, and electrophysiological severity score. Conclusion: Sonography can detect the morphological changes in the ulnar nerve in patients with the CuTS. However, the best predictors of clinical characteristics in the CuTS seem to be the electrophysiological severity rather than the sonographic findings.

        Effects of Transforaminal Injection for Degenerative Lumbar Scoliosis Combined with Spinal Stenosis

        Objective The objectives of this study were to clarify the short-term effects of transforaminal epidural steroid injection (TFESI) for degenerative lumbar scoliosis combined with spinal stenosis (DLSS), and to extrapolate factors relating to the prognosis of treatment. Method Thirty-six patients with lumbar radicular pain from DLSS were enrolled. Subjects were randomly assigned to one of two groups (steroid or lidocaine group). We compared the effect of pain suppression at 2, 4 and 12 weeks after the procedure between the two groups. Radiographic analysis included measurement of the Cobb’s angle, the upper endplate obliquities of L3 and L4, and maximal lateral olisthy between two adjacent lumbar vertebrae. Sagittal plane measurement included lumbar lordosis, and thoracolumbar kyphosis. Statistical analysis of both radiographic and clinical parameters along with treatment outcome was performed to determine any significant correlations between the two. Results There were no significant differences in the demographic data, initial visual analogue scale (VAS) or Oswestry disability index (ODI) between the steroid group (n=17) and the lidocaine group (n=19). Two, 4, and 12 weeks after injection VAS, ODI showed a significantly greater improvement in the steroid group compared to the lidocaine group (p<0.05). The radiographic and clinical parameters were not significantly correlated with treatment outcome. Conclusion Our findings suggest that fluoroscopic transforaminal epidural steroid injections appear to be an effective non-surgical treatment option for patients with degenerative lumbar scoliosis combined with spinal stenosis (DLSS) and radicular pain.

        디펩타이드의 B16 악성흑색종세포에서 멜라닌 생성억제작용

        본 연구에서는 B16 악성흑색종 세포에서 디펩타이드(dipeptide)의 멜라닌생성 저해 효과를 연구하였다. 실험결과 WV (트립토판-발린), WM (트립토판-메치오닌), CQ (시스테인-글루타민)는 멜라닌 생성을 농도 의존적으로 감소시켰다. 그러나 디펩타이드는 멜라닌 생합성과정의 속도 조절 단계 효소인 타이로시네이즈(tyrosinase)의 활성을 직접 감소시키지는 않았다. 따라서 타이로시네이즈의 발현양상을 조사하였고, 실험 결과 α-MSH가 유도한 타이로시네이즈 발현이 WV, WM, 그리고 CQ에 의해 억제되었다. 그러므로 WV, WM, 그리고 CQ가 타이로시네이즈의 억제성 조절(down-regulation)을 통해 멜라닌 생성을 감소시킨다고 제안될 수 있다. In the present study, we investigated the effects of dipeptides on melanogenesis in B16 melanoma cells. It was found that WV (Trp+Val), WM (Trp+Met), and CQ (Cys+Gln) decreased melanin production dosedependently. However, dipeptides did not directly inhibit tyrosinase activity, the rate-limiting melanogenic enzyme. Therefore, we further investigated the expression of tyrosinase. Our results showed that α-MSH-induced tyrosinase expression was down-regulated by WV, WM, and CQ. Thus, we propose that WV, WM, and CQ show hypopigmentary activity through tyrosinase down-regulation.

        투석 환자에서 투석 중 운동프로그램이 투석의 효율도 및 자율신경기능에 미치는 영향 -예비연구-

        The study investigated the effects of an intradialytic exercise program on autonomic function and dialysis efficacy in maintenance hemodialysis in 18 patients. The patients undertook a 6-month intradialysis exercise program. Each session consisted of a 5-minute warm-up, 40-minute main exercise program, and 5-minute cool-down period. Autonomic symptom questionnaire and autonomic function test by heart rate variability on supine and standing position, and dialysis efficacy were assessed at the beginning and end of the study by comparison using the Wilcoxson signed rank test. Heart rate variability in the supine position was not improved. In the standing position the ratio between the low frequency and high frequency (LF/HF) increased significantly after 6 months of exercise (p<0.05), with the LF component being normally dominant. Statistical differences were evident at 6 months in autonomic symptoms questionnaire and autonomic function (p<0.05), but not in dialysis efficacy. The results suggest that intradialytic exercise is an effective therapy to improve autonomic symptoms and a heart rate variability parameter.

        중증 수근관증후군에서 제2충양근과 제2골간근의 잠시 차이의 유용성

        Objective: The purpose of this study was to compare the diagnostic usefulness of various nerve conduction studies when the routine median motor and sensory responses are absent. Method: Between January 2002 and March 2008, 30 hands of 25 patients were studied, with the median motor and sensory responses shown to be absent in the routine nerve conduction studies. We performed three nerve conduction studies, median-radial latency difference to the thumb, median-ulnar latency difference to the ring finger and the second lumbrical-interosseous latency comparison method. Results: The subjects’ (3 men and 27 women) ages ranged from 32 to 84 years. All hands had no median sensory response to the thumb and the ring finger. 5 hands had no median motor response to the second lumbrical muscle. 25 hands showed a significant prolongation of the lumbrical-interosseous latency difference (>0.4). In all the subjects, the needle EMG revealed abnormal spontaneous activity in the abductor pollicis brevis. Conclusion: The lumbrical-interosseous latency comparison method is helpful in patients with severe CTS and with no median motor and sensory responses.

        편측성 수근관증후군의 진단에서 적외선체열촬영의 유용성

        Objective: To evaluate the usefulness of infrared thermography in the diagnosis of unilateral carpal tunnel syndrome (CTS). Method: Thirty six patients confirmed electrodiagnostically as CTS and fifteen adults with normal electrodiagnostic study were included in this study. For the thermographic diagnosis, we measured the skin temperature of the palmar area of all 5 fingers, thenar area, hypothenar area and volar area of forearm bilaterally. We compared the interside peak temperature difference between median nerve innervated area and noninnervated area in three different groups (normal control, mild CTS and moderate to severe CTS groups). Results: Thermography of the CTS patients showed significant temperature differences in more than one area of the median vasomotor nerve territory, and no significant temperature difference in all the areas that were not innervated by the median vasomotor nerve. When we considered interside temperature differences of more than 0.3°C as abnormal findings, the sensitivity of thermography was 2.7% and the specificity was 86.6%. With more than 0.6°C as abnormal, the sensitivity was 25% and the specificity was 66.6%. With more than 1.0°C, the sensitivity was 8.3% and the specificity was 73.3%. There was no significant interside peak temperature difference in all groups. Conclusion: This study suggests that infrared thermography has low sensitivity and specificity, which implies that it is not useful for the diagnosis of CTS.

        경증의 수근관증후군에서 여러 전기진단학적 지표들의 유용성

        Objectives: The purpose of this study is to evaluate the sensitivities and specificities of various electrodiagnostic parameters and to investigate the useful methods to increase the sensitivity in the electrodiagnosis of mild carpal tunnel syndrome (CTS). Methods: The 178 hands of 142 patients with mild CTS based on the Steven's electrodiagnostic criteria were studied. We performed seven electrodiagnostic tests: distoproximal ratio in the second or third finger (M2DP, M3DP); median-radial latency difference to the thumb (MRLD); median-ulnar latency difference to the ring finger (MULD); second lumbrical-interosseous motor latency difference (LILD); median sensory-ulnar motor latency difference to the second or third finger (M2SUM, M3SUM). The sensitivity and specificity of each test were calculated. Results: The sensitivities of electrodiagnostic parameters were noted in the M2DP(98.9%), M3DP(98.9%), MRLD(87.6%), MULD(86.5%), M3SUM(74.2%), M2SUM(55.6%), LILD(54.5%). Among electrodiagnostic parameters, M2DP(49.0%) and M3DP(68.0%) revealed relatively low specificities than others. Percentage of mild CTS hands which was abnormal at two differently combined electrodiagnostic test: in M2DP and M3DP(98.3%), M3DP and MULD(86.0%), M3DP and MRLD(81.5%), MRLD and MULD(79.2%), MULD and M3SUM(70.2%); in M3DP or LILD(100%), M3DP or M2SUM(100%), M3DP or MRLD(99.4%), M3DP or M2DP(99.4%), M3DP or MULD(99.4%), M3DP or M3SUM(99.4%). Conclusion: The most sensitive parameter was distoproximal ratio (M2DP, M3DP), as useful method in the electrodiagnosis of mild CTS but it may cause greater reporting of false positive. Additional parameters including MULD, MRLD, LILD, M2SUM, M3SUM may be of value in the diagnosis of mild CTS patients.
