감각이상을 주소로 내원한 당뇨병 환자 8례에 대한 임상보고

Objective : The purpose of this study was to report the effect of Bogijetong-tang on diabetic peripheral neuropathy. Methods : Nine patients who were diagnosed or suspected with diabetic peripheral neuropathy enrolled in this study. They took Bogijetong-tang 2 or 3 times a day and were given acupuncture therapy. To evaluate the therapeutic effect, Visual Analog Scale (VAS) or Total Symptom Score (TSS) were examined at intervals of about 7 days. Result : TSS score decreased an average of 2.74 points and VAS scores decreased an average of 3.67 points. As the pain decreased, accompanied gait disturbance also improved in some patients. Conclusion : Bogijetong-tang is effective in treating diabetic peripheral neuropathy, but study of the concrete mechanism and efficacy is needed.
대장균내에서 P22 tailspike 유전자의 클론닝 및 발현

A plasmid-expression system was established in Escherichia coli for the tailspike gene (gene 9) of Salmonella phage P22 in which temperature sensitive folding mutations had been thoroughly characterized previously. Expression of the recombinant tailspike gene was under control of tac promoter and the yield of expression was 2% of total cellular protein when the the synthesis of recombinant protein was fully induced. Three mutations (tsH304, tsU11, and tsU24) of gene 9 behaved off the E. coli plasmid exactly the same way as they did in P22-infected Salmonella; at 25℃ the mutant polypeptides fold and mature properly into native structure, while at 39℃ they could not fold but aggregate. The construction will allow a systematic approach to examine effect of amino acid substitution on protein folding and stability.

Human αl-antitrypsin cDNA was expressed in yeast Saccharomyces cerevisiae under the regulation of GAL10-CYC1 hybrid promoter. A suitable Gal^+ host strain was constructed by tetrad analysis of a diploid from a cross of two Gal^- strains, each affected in a different galactose utilizing gene. The resulting strain KY8 was transformed with the expression plasmid pGAT7 and the synthesis of αl-antitrypsin was induced in galactose medium. Production of αl-antitrypsin in recombinant yeast cells was confirmed by an elastase inhibitory assay and an immunoblotting method. The molecular weight of recombinant αl-antitrypsin synthesized in yeast was 44 kDa which was expected from the full length cDNA sequence of the αl-antitrypsin structural gene.

Grapefruit seed extract has been reported to have antimicrobial effect. The purpose of this study was to evaluate the antimicrobial and anti-gingivitis effect of chewing gum containing grapefruit seed extract and xylitol. 40 healthy subjects with gingivitis or early periodontitis were divided into two groups. Subjects in the experimental group chewed gum containing grapefruit seed extract and xylitol while subjects in the control group chewed gum containing only xylitol. All subjects received scaling and tooth brushing instruction. 1 week after scaling was set as baseline. Gingival index and plaque index were scored at baseline, 1 week, 2 week, 3 week and 4 week. Bleeding index, probing pocket depth and clinical attachment level were scored at baseline, 2 week and 4 week. The number of total bacteria and Streptococcus mutans in unstimulated saliva of experimental group were counted at 1 week, 2 week, 3 week and 4 week. Gingival indices of experimental group and control group at baseline, 1 week, 2 week, 3 week and 4 week were 0.850${\pm}$0.298, 0.575${\pm}$0.345, 0.533${\pm}$0.332, 0.459${\pm}$0.311, 0.408${\pm}$0.224 and 0.758${\pm}$0.379, 0.592${\pm}$0.276, 0.563${\pm}$0.281, 0.454${\pm}$0.194, 0.413${\pm}$0.199 (mean${\pm}$SD), respectively. Plaque indices of experimental group and control group at baseline, 1 week. 2 week, 3 week and 4 week were 0.497${\pm}$0.500, 0.375${\pm}$0.484, 0.332${\pm}$0.471, 0,286${\pm}$0.452, 0.210${\pm}$0.407 and 0.411${\pm}$0.492, 0.375${\pm}$0.484, 0.354${\pm}$0.479, 0.313${\pm}$0.463, 0.193${\pm}$0.395, respectively. Bleeding indices of experimental group and control group at baseline, 2 week and 4 week were 0.377${\pm}$0.177, 0.298${\pm}$0.152, 0.192${\pm}$0.108 and 0.383${\pm}$0.124, 0.318${\pm}$0.153, 0.225${\pm}$0.126, respectively. Probing pocket depth of experimental group and control group at baseline, 2 week and 4 week were 2.56${\pm}$1.00, 2.40${\pm}$0.65, 2.23${\pm}$0.64 and 2.45${\pm}$0.682.37${\pm}$0.57, 2.19${\pm}$0.57, respectively. Clinical attachment level of experimental group and control group at baseline, 2 week and 4 week were 2.58${\pm}$1.01, 2.43${\pm}$0.67, 2.26${\pm}$0.65 and 2.49${\pm}$0.70, 2.40${\pm}$0.59, 2.22${\pm}$0.62, respectively. The % of reduction of total bacteria in saliva of experimental group at 2 week, 3 week and 4 week were 46 ${\pm}$ 53%, 53 ${\pm}$ 5% and 69 ${\pm}$ 33%. The % of reduction of Streptococcus mutans count in saliva of experimental group at 2 week, 3 week and 4 week were 52${\pm}$69%, 88${\pm}$30% and 89${\pm}$17%. From these findings, it can be concluded that regular use of grapefruit seed extract /xylitol chewing gum may be effective to control and prevent gingivitis and may have caries-preventive effect.
병원서비스지역 내 병원자원과 의료서비스 이용 간의 관련성 분석

Background: This study explored the relationship between hospital resources and services uses in outpatient/inpatient-based hospital service area (HSA) in Korea. Methods: Study hospitals included all acute care hospitals except tertiary hospitals. Inpatient and outpatient hospital claims from the Korean National Health Insurance (NHI) program in 2010 were used to identify the service uses. Hospital resources and the degree of insurance premium in study areas were identified with the NHI corporation data. Study variables were computed by summing the service uses or hospital resources of study hospitals in each HSA. Service uses were represented by the total medical charges and number of visits/inpatient days. Hospital resources were measured by number of beds, number of doctors, and number of computed tomography (CT). The economic status of NHI enrollees in each HSA was controlled by the average monthly premium of NHI program per household in each HSA. The degree of using local hospitals was controlled with the localization index. Results: Analysis results showed that hospital resources such as beds, CT were statistically related to the service uses. And also localization index was found to have positive significant relationships with service uses. Conclusion: Hospital resources such as beds, CT had not only positive impacts on inpatient service uses, but also influences on the outpatient setting. Health policy makers will require monitoring and assessing the hospital resources in Korea.
Localization of a KEM1::lacZ Fusion Protein in Yeast Cells

KEM1 is known to control the spindle pole body or microtubule function, probably in response to the cellular nutritional conditions in Saccharomyces cerevisiae. Transposon insertions were performed in the cloned KEM1 gene using mini-Tn10-LUK element carrying E. coli ${\beta}$-galactosidase structural gene. A collection of ranfom Tn10-LUK insertions defined an approximately 3.5 kb region required for the KEM1 function. From this collection functional KEM1::lacZ protein fusions were identified. Indirect immunofluorescence using anti-${\beta}$-galacatosidase antibodies localized the KEM1::lacZ fusion protein to the periphery of the nucleus. Saccharomyces cerevisiae의 KEMI 유전자는 세포의 영양 상태에 따라 spindle pole body나 microtubules의 기능을 조절하는 것으로 알려져 있다. 이 유전자 산물의 세포내 분포 및 기능을 규명하기 위하여, KEM1::lacZ 융합 유전자를 제조하였다. 즉, 클론된 KEM1 유전자에 대장균의 ${\beta}$-galactosidase 구조유전자를 갖는 mini-Tn10-LUK element를 무작위 삽입한 pool을 제조하고, 이를 분석하여 KEM1의 기능 부위가 약 3.5kb에 해당함을 확인하였고, KEM1의 기능이 살아있는 KEM1::lacZ 융합 유전자의 클론을 선별하였다. 이 클론을 ${\beta}$-galactosidase 항체를 이용한 indirect immunofluorescence 방법으로 분석하여 KEM1::lacZ 융합 단백질이 핵주변에 위치함을 확인하였다.
