        A Pediatric Case of a D-Penicillamine Induced ANCA-associated Vasculitis Manifesting a Pulmonary-Renal Syndrome

        D-penicillamine has been reported to cause antineutrophil cytoplasmic antibody (ANCA)- associated vasculitis presenting as rapidly progressive glomerulonephritis or pulmonary- renal syndrome mostly in adults. We report a pediatric case of D-penicillamine induced ANCA-associated vasculitis that manifests as a pulmonary-renal syndrome with a mild renal manifestation. A 13-year-old girl who has been taking D-penicillamine for five years under the diagnosis of Wilson disease visited the emergency room because of hemoptysis and dyspnea. She had diffuse pulmonary hemorrhage, microscopic hematuria, and proteinuria. Myeloperoxidase ANCA was positive, and a renal biopsy revealed pauci-immune crescentic glomerulonephritis. Under the diagnosis of D-penicillamine-induced ANCA-associated vasculitis, D-penicillamine was switched to trientine, and the patient was treated with plasmapheresis, glucocorticoid, cyclophosphamide, and mycophenolate mofetil. Pulmonary hemorrhage improved rapidly followed by the disappearance of the hematuria and proteinuria five months later.

        Response Prediction of Altered Thyroglobulin Levels After Radioactive Iodine Therapy Aided by Recombinant Human Thyrotropin in Patients with Differentiated Thyroid Cancer

        Purpose Thyroglobulin (Tg) may be released from damaged residual thyroid tissues after radioactive iodine (RAI) therapy inpatients with differentiated thyroid carcinoma (DTC). We investigated whether altered levels of serum Tg after recombinanthuman thyrotropin (rhTSH)-aided RAI therapy could be a prognostic marker in patients with DTC. Methods We evaluated 68 patients who underwent RAI therapy after total thyroidectomy. Serum Tg levels were measured justbefore RAI administration (D0Tg) and 7 days after RAI therapy (D7Tg). Patients with a D0Tg level greater than 2.0 ng/mL wereexcluded to more precisely evaluate the injury effect of RAI in small remnant tissues. The ratioTg was defined as the D7Tg leveldivided by that on D0Tg. The therapeutic responses were classified as acceptable or non-acceptable. Finally, we investigatedwhich clinicopathologic parameters were associated with therapeutic response. Results At the follow-up examination, an acceptable response was observed in 50 patients (73.5%). Univariate analysis revealedsignificant differences in N stage (P = 0.003) and ratioTg (acceptable vs. non-acceptable responses, 21.9 ± 33.6 vs. 3.8 ± 6.5; P =0.006). In multivariate analysis, only ratioTg significantly predicted an acceptable response (odds ratio 1.104; 95% confidenceinterval 1.005–1.213; P = 0.040). A ratioTg above 3.5 predicted an acceptable response with a sensitivity of 66.0%, specificity of83.3%, and accuracy of 70.6% (area under the curve = 0.718; P = 0.006). Conclusions Altered levels of serum Tg after RAI therapy, calculated as the ratioTg (D7Tg/D0Tg), significantly predicted anacceptable response in patients with DTC.

        연체동물 대량염기서열 분석을 위한 PANM 데이터베이스 버전 3 업데이트

        The PANM Database (Protostome DB) was constructed for the first in the year 2015. The aim of the construction was to provide a public platform for researchers to efficiently annotate the next-generation sequencing (NGS) data arising out of genome sequencing projects of species belonging to Arthropoda, Mollusca, and Nematoda (Protostome group). The web-based interface has paramount importance in improving the process of sequencing data with a greater number of noteworthy hits. In the subsequent year, we released an update to the database (PANM-DB v1.0) that contained twice the number of protein sequences (7,571,246) as compared to the original database and the inclusion of Cephalopod sequences. We now release the third database update (PANM-DB v3) with the incorporation of 11,615,243 protein sequences. The size of PANM-DB v3 is smaller, only 6% of NCBI nr database, and significantly reduces the NGS data annotation time. Like the previous versions of the database, PANM-DB v3 is freely downloadable from http://panm.sch.ac.kr/ for local BLAST analysis.

        Metallothionein 유전자를 활용한 제주배꼽달팽이(Aegista chejuensis)의 분자계통학적 연구

        Metallothioneins (MT) are cysteine-rich small proteins known for their high affinity binding to metal ions. MT confers cytoprotection against oxidative stress and acts as an efficient modulator of immunity and infection. There have been ever-increasing reports on the function of MT in environmental monitoring programmes. Lately, MT has been studied as a biomarker towards the taxonomic clustering of various species, including the molluscans. The Korean endemic land snail, Aegista chejuensis is native to Jeju Island and South coastal regions of Korea. Functional genomics information for the species is limited, although the Illumina-based DNAseq and transcriptome information are registered with the Sequence Read Archive (SRA) of National Center for Biotechnology Information (NCBI). While annotating the ORF-sensitive unigenes from the transcriptome, a MT sequence comprising of 288 bp coding region and 96 amino acid residues translatable protein was deciphered. In silico analysis using ClustalX and the MEGA7 program grouped A. chejuensis under the Stylommatophora clade of eupulmonate mollusk families. The study was able to confirm the evolutionary position of the species and established the MT sequence as a candidate for taxonomic classification of the species.

        도로터널 화재발생시 연기유동에 관한 축소모형실험 연구

        In this study, smoke movement in tunnel fire with natural and longitudinal ventilation systems has been investigated. Reduced-scale experiments were carried out under the Froude scaling using 14.55kW fire source with a wick and experimental data is obtained with 1/18 model tunnel test. Temperature profiles were measured under the ceiling and vertical direction along the center of the tunnel and poisonous gas was measured at emergency exit point. The results show that refuge time for 225m intervals of emergency exit in case of natural ventilation systems is 256 seconds and critical velocity for sufficient back-layer prevention is 2.8m/s for fire strength of 20MW. 도로터널내 화재가 발생한 경우 자연환기와 종류식 환기시스템에서의 연기유동에 대한 연구가 수행되었다. Froude Scaling에 기초한 1/18 축적의 축소모형실험장치가 제작되었으며, 화원으로는 연료심지를 이용한 14.55kW 규모의 연소기가 사용되었다. 화원으로부터 등비간격으로 터널천정부근과 수직방향의 온도분포가 측정되었으며, 연소가스의 농도가 피난대피구 입구에서 측정되었다. 연구결과로서 자연환기시스템에서 비상대피구가 225m 간격으로 설치되어 있을 경우 비상대피구 구간의 연기확산시간은 256초가 되며, 20MW의 화재강도에 대해 화염역류를 완전히 방지하기 위해서는 터널풍속을 2.8m/s로 형성해 주어야 한다.

        연 재배지를 활용한 자유수면형 인공습지의 수질정화효율

        In order to investigate the treatment efficiency of pollutants in free water surface constructed wetlands (FWS CWs) with lotus (Nelumbo nucifera) cultivation pond, the experiment was consisted of two sites (site Ⅰ and Ⅱ) in Lake Juam, Korea. The sites were configured a lotus cultivation pond (with fertilizer application) - a dropwort bed - a reed bed for site I, and a lotus cultivation pond (without fertilizer application) - a dropwort bed - a reed bed for site Ⅱ. Removal rate of COD in site Ⅰ and Ⅱ were 13.3% and 26.0%, respectively. Removal rate of total nitrogen (TN) was 29.7% for site Ⅰ, and 36.3% for site Ⅱ. Removal rate of total phosphorus (TP) in site Ⅰ and Ⅱ were 36.0% and 36.5%, respectively. COD, TN and TP in effluent from site Ⅰ (with fertilizer) was higher than that in site Ⅱ (without fertilizer), showing that COD, TN and TP in effluent were strongly influenced by fertilizer addition. Therefore, in order to satisfy established water-quality standards, the amount of fertilizer used in lotus cultivation showed be evaluated. 연재배지를 활용한 자유수면형 습지에서 오염물질별 처리특성과 처리효율을 조사하기 위하여 site Ⅰ은 연 재배지조(비료처리)-미나리조-갈대조로 구성하였고, site Ⅱ 는 연 재배지조(무비료)-미나리조-갈대조로 구성하였다. 연재배지를 활용한 자유수면형 습지에서 최종방류수 중의COD 처리효율은 site I(비료시비) 및 Ⅱ (무비료)가 각각 13.3% 및 26.0%이었다. 자유수면형습지에서 최종방류수 중의 TN 처리효율은 site Ⅰ 및 Ⅱ 가 각각29.7% 및 36.3%이었으며, TP 처리효율은 site Ⅰ 및 Ⅱ 가 각각 36.0% 및 36.5%이었다. 비료를 시비한 site I에서 COD, TN및 TP 함량은 전반적으로 비료를 시비하지 않은 site II에 비해 높아 비료시비가 방류수 수질에 영향을 미치는 것으로 판단된다. 따라서 연재배지를 활용한 자유수면형 습지에서 방류수 수질기준을 만족하기 위해서는 연재배지에 사용된 퇴비의 시비량을 우선적으로 고려해야 할 것이다.

        3가지 유형의 Primer 를 이용한 4개국 (중국, 일본, 러시아, 미얀마) 산 수입 이매패류의 분자계통분류학적 연구

        Imports of aquatic products continue increasing every year. When imported into Korea, the common name of a similar aquatic product is used instead of a scientific name, causing many confusions. In this study, in order to cope up with this issue, four countries with the highest number of imported bivalves were selected. The collected bivalves were totally classified into 10 orders, 17 families, 39 species. Also, the collected bivalves in the current study were identified by morphological classification. Moreover, sequence analysis of COI gene was carried out for all the collected species. The COI gene sequence was obtained with 3 type of primers. All COI sequences analyzed were subjected to molecular phylogenetic analysis. As a result, it was possible to draw conclusions very similar to the morphological classification results. Taken together, it is preferable to perform the universal primers and the lobo primer simultaneously for analyzing the COI sequence of bivalves. On the other hands, to obtain the results for scallops, the Matsumoto primers were available to conduct the analysis.
