        Effect of obesity and metabolic health on urolithiasis: A nationwide population-based study

        Purpose: To investigate the risk of symptomatic urolithiasis requiring surgical treatment according to obesity and metabolic health status using a nationwide dataset of the Korean population. Materials and Methods: Of the 5,300,646 persons who underwent health examinations between the year 2009 and 2016, within one year after the health examination, 35,137 patients who underwent surgical treatment for urolithiasis were enrolled. Participants were classified as “obese” or “non-obese” using a body mass index (BMI) cutoff of 25 kg/m2. People who developed ≥1 metabolic disease component in the index year were considered “metabolically unhealthy”, while those with none were considered “metabolically healthy”. Results: Out of 34,330 participants excluding 843 missing, 16,509 (48.1%), 4,320 (12.6%), 6,456 (18.8%), and 7,045 (20.5%) subjects were classified into the metabolically healthy non-obese (MHNO), metabolically unhealthy non-obese (MUNO), metabolically healthy obese (MHO), and metabolically unhealthy obese (MUO) group, respectively. Mean BMI was 22.1±1.9 kg/m2, 22.9±1.6 kg/m2, 26.9±1.8 kg/m2, and 27.9±2.4 kg/m2 respectively. After adjusting the age and sex, the subjects in the MUNO group had an HR (95% CI) of 1.192 (1.120–1.268), those in the MHO group, 1.242 (1.183–1.305), and those in the MUO group, 1.341 (1.278–1.407) for either extracorporeal shockwave lithotripsy or surgery, compared to those in the MHNO group. Conclusions: Metabolically healthy, obese individuals have a higher risk of developing symptomatic urolithiasis than non-obese, unhealthy, but have a lower risk than obese, unhealthy. It suggests that metabolic health and obesity have collaborative effects, independently affecting the development of symptomatic urinary stone diseases.

        Comparison between Proton Radiography Images Using a High-power Femtosecond Laser and a Tandem Van de Graff Accelerator

        We performed two experiments of proton radiography by using protons generated by a femtosecond laser and by a Tandem Van de Graff accelerator to compare their image characteristics. In the laser experiment, the maximum energy of the protons was about 1.8 MeV, and they had a broad energy distribution. The protons in the accelerator experiment were monoenergetic with an energy of 1.8 MeV. In order to evaluate the radiography images, we used a step wedge phantom and a character-type phantom. A CR-39 solid state nuclear track detector (SSNTD) was used as a radiography screen. The radiography images were obtained by using an optical microscope. As a result of the comparison and evaluation of the two experiments’ radiography images, protons from the femtosecond laser were found to be more useful than protons from the accelerator. If good proton radiography images are to be obtained using CR-39, utilizing a certain proton energy distribution rather than monoenergetic protons is efficient.

        2023 Korean sexually transmitted infections guidelines by the Korean Association of Urogenital Tract Infection and Inflammation: Human papillomavirus vaccination

        The Korean Association of Urogenital Tract Infection and Inflammation (KAUTII) and the Korea Disease Control and Prevention Agency updated the guidelines for human papillomavirus (HPV) vaccine against sexually transmitted HPV infections in Korea to respond to changing epidemiologic trends, evolving scientific evidence, and advances in laboratory diagnostics and research. Main purpose and recommendation of vaccination against HPV are as follows: (1) the purpose of HPV vaccine is to reduce the risk of genital warts and HPV-related cancers including cervical and vulvar cancer, head and neck cancer, anal cancer, and penile cancer; (2) in Korea, bivalent (16, 18) vaccines, quadrivalent vaccines (6, 11, 16, 18), and 9-valent vaccines (6, 11, 16, 18, 31, 33, 45, 52, 58) are used depending on the type of HPV; (3) bivalent and quadrivalent vaccines are national immunizations targeting girls aged 11–12 years and low-income young females aged 18–26 years (age and range of inoculation: routinely administered at 11 or 12 years of age, 2 doses at 0 and 6 months for 12–14 years of age; for females aged 15–26 years, 3 doses depending on the type of vaccine; vaccination can be given to those aged up to 45 years through consultation with a clinician); (4) in the case of administering 2 doses, at least 5 months apart; in the case of administering 3 doses, it is recommended to keep 4 weeks between the 1st and 2nd doses, 12 weeks between the 2nd and 3rd doses, and 5 months between the 1st and 3rd doses; (5) immunocompromised patients such as those with HIV, malignant neoplasms, and autoimmune diseases, and those undergoing transplantation or immunosuppressive therapy should receive 3 doses. HPV vaccine is not recommended during pregnancy.

        Preventive effects of tamsulosin for postoperative urinary retention after lower limb arthroplasty: A randomized controlled study

        Purpose: This prospective, randomized, controlled study investigated the use of tamsulosin, a selective alpha-blocker, as a prophylactic medication to prevent postoperative urinary retention (POUR) following lower limb arthroplasty. Materials and Methods: The criterion for diagnosing POUR was used a postoperative bladder volume over 400 mL with incomplete emptying. Patients who underwent primary total hip or knee arthroplasty were randomly assigned at a 1:1 ratio to tamsulosin treatment and non-treatment groups at a single center from September 2018 to November 2018. The treatment group received 0.2 mg of tamsulosin orally once at night for 3 days starting on postoperative day 1. During this 3-day period, an indwelling Foley catheter was maintained. The incidence of POUR according to tamsulosin treatment following lower limb arthroplasty was the primary outcome. Results: In total, 100 patients were enrolled, of whom 5 discontinued participation. POUR was diagnosed in 20 of the remaining 95 patients (21.1%). The treatment group contained 48 patients, of whom 6 (12.5%) developed POUR, whereas POUR occurred in the 14 of the 47 patients (29.8%) in the non-treatment group. Tamsulosin treatment reduced the risk of POUR by two-thirds (odds ratio [OR], 0.337; 95% confidence interval [CI], 0.117–0.971; p=0.044). The risk reduction associated with tamsulosin treatment remained robust post-adjustment for potential covariates (OR, 0.250; 95% CI, 0.069–0.905; p=0.038). Conclusions: Tamsulosin administration immediately after lower limb arthroplasty reduced the incidence of urinary retention and diminished the need for long-term catheterization.

        Ureteral Stent Insertion in the Management of Renal Colic during Pregnancy

        To determine an optimal invasive intervention for renal colic patients during pregnancyafter conservative treatments have been found to be unhelpful. Among the availableinvasive interventions, we investigated the reliability of a ureteral stent insertion,which is considered the least invasive intervention during pregnancy. Between June2006 and February 2015, a total of 826 pregnant patients came to the emergency roomor urology outpatient department, and 39 of these patients had renal colic. The meanpatient age was 30.49 years. In this retrospective cohort study, the charts of the patientswere reviewed to collect data that included age, symptoms, the lateralities and locationsof urolithiasis, trimester, pain following treatment and pregnancy complications. Based on ultrasonography diagnoses, 13 patients had urolithiasis, and 13 patients hadhydronephrosis without definite echogenicity of the ureteral calculi. Conservativetreatments were successful in 25 patients. Among these treatments, antibiotics wereused in 15 patients, and the remaining patients received only hydration and analgesicswithout antibiotics. Several urological interventions were required in 14 patients. Themost common intervention was ureteral stent insertion, which was performed in 13patients to treat hydronephrosis or urolithiasis. The patients’ pain was relieved followingthese interventions. Only one patient received percutaneous nephrostomy due topyonephrosis. No pregnancy complications were noted. Ureteral stent insertion is regardedas a reliable and stable first-line urological intervention for pregnant patientswith renal colic following conservative treatments. Ureteral stent insertion has beenfound to be equally effective and safe as percutaneous nephrostomy, which is associatedwith complications that include bleeding and dislocation, and the inconvenience of usingexternal drainage system.

In 21th century driven by economic competitiveness, Owing to the globalization and the endless change in the construction industry. A construction market was required new, complex, complicated and competitive paradigm by a buyer. However, Up to now the definition of CM doesn’t clearly set up and an organization for CM project of Construction Company also isn’t established. So this paper was to study a role and organization of CM for the high Quality in the construction Industry.
