Intussusception in premature infants is very rare. Here, we report a case of multiple intussusceptions in an extremely preterm infant, born at 23+1 week’s gestation, who underwent an explolaparotomy, for bowel perforation and misdiagnosed necrotizing enterocolitis, at 20 days of life. To our knowledge, this is the most premature case that has survived with multiple intussusceptions.

<P>In this study, we report on the development of a stretchable, transparent, and skin-attachable strain sensor integrated with a flexible electrochromic device as a human skin-inspired interactive color-changing system. The strain sensor consists of a spin-coated conductive nanocomposite film of poly(vinyl alcohol)/multi-walled carbon nanotube/poly(3,4-ethylenedioxythiophene):poly(styrenesulfonate) on a polydimethylsiloxane substrate. The sensor exhibits excellent performance of high sensitivity, high durability, fast response, and high transparency. An electrochromic device (ECD) made of electrochemically synthesized polyaniline nanofibers and V2O5 on an indium tin-oxide-coated polyethylene terephthalate film experiences a change in color from yellow to dark blue on application of voltage. The strain sensor and ECD are integrated on skin via an Arduino circuit for an interactive color change with the variation of the applied strain, which enables a real-time visual display of body motion. This integrated system demonstrates high potential for use in interactive wearable devices, military applications, and smart robots.</P>

<P>The aim of the present study was to describe a method of gastric lymphatic basin dissection for sentinel node biopsy using natural orifice transluminal endoscopic surgery with laparoscopic assistance (hybrid NOTES) in a porcine model. Lymph node dissection was performed in three healthy female domestic farm pigs (each around 40 kg) between October, 2007, and December, 2007. The pigs were administered a general anesthetic and laparoscopy-guided transvaginal colpotomy was performed. A two-channel endoscope was then inserted through the incision into the peritoneal cavity via the transvaginal route. An endoscope was inserted simultaneously into the mouth and indocyanine green solution was injected into the submucosal layer of the gastric wall at four sites. Dyed omentum and lymphatics were dissected using a laparoscopic dissector and the grasping forceps of a transvaginal endoscope. Lymphatics and omentum (mean 13.3 cm, range 8-20 cm) were removed transvaginally. The mean number of detected and resected sentinel nodes was 2.6 (range 1-4, diameter 2∼12 mm). Sentinel lymphatic basin dissection was performed successfully and without intraoperative complications in all three cases. Hybrid NOTES is technically feasible, and this procedure may represent an alternative to laparoscopic sentinel lymph node dissection of the stomach.</P>
만성 신부전 환자의 영양평가 지표로서의 DEXA의 유용성

It has been considered that the nutrition affects the mortality of chronic renal failure patients. Thus, several studies reported the method of examination about nutritional status in chronic renal failure patients. The purpose of the present study was to evaluate the nutritional status of 32 chronic renal failure patients, and recommend DEXA as a objective method. Thirty two chronic renal failure patients and 24 disease-free persons on kidney(control group) were included in this syudy. There were no difference in mean age, sex, weight, height, body mass index between two groups. We measured % total body fat, fat-free mass, limb fat, trunl fat, limb fat/trunk fat ratio with Hologic QDR 4500. The results were as follows. 1)% Total body fat of chronic renal failure patients was 20.7±7.9%. That was lower than 26.3±7.9% of control group(P$lt;0.05). 2)Fat-free mass of chronic renal failure patients revealed lower result compared with control group. Each result was 40.2±12.2kg, 46.2±9.1kg(P$lt;0.05). 3)Despite ofsignificant difference between two groups in ratio of limb fat and trunk fat, limb fat and trunk fat were statistically different(Limb fat:CRF-5.6±2.4kg, Control-7.1±1.9kg, Trunk fat:CRF- 5.2±3.0kg, Control-7.7±2.7kg)(P$lt;0.05). 4)Triceps skinfold thockness was positively correlated with % total body fat, limb fat, trunk fat, and negatively correlated with fat-free mass. Mid- arm muscle area circumference had inverse relationship above(P$lt;0.05). 6)Severe malnurished patients(SGA-C group) had lower result compared to mild-moderate malnurished patients(SGA-B group) about %TBF, LF,TF, total lymphocyte count. In addition to DEXA, we examined triceps skinfold thickness and mid-arm muscle area circumference. The result showed significant comparison between two groups, too. But albumin, cholesterol, potassium level was no statistical significant difference, except lymphcyte count. In conclusion, as suggested by above results, chronic renal failure patients has protein-calorie malnutrition. This nutritional staaffects long-term survival of chronic renal failure patients. DEXA can give objective data accompanied with simple anthropometric measurement in nutriti onal status. Thus, we recommend DEXA as objective method of nutritional examination. In future, more precise method will be discovered, and that contribute more long-term survival of chronic renal failure patients.
