Purpose: Transient tachypnea of the newborn (TTN) is a respiratory disorder resulting from delayed clearance of fetal lung fluid. Initiation of empiric antibiotic therapy for possible early-onset sepsis is usually recommended until negative blood cultures for 48 hours. The aim of this study was to compare outcomes of infants with TTN managed with a risk-factor-based restrictive antibiotic use policy. Methods: A single institution retrospective study was conducted on full-term infants admitted with TTN from January, 2008 to December, 2013. Infants were stratified into two groups based on whether they received or did not receive antibiotics. The decision to administer antibiotics depended upon the covering physician at admission. The clinical and laboratory outcomes were evaluated between two groups. Result: A total of 98 full-term infants diagnosed with TTN were admitted to the neonatal intensive care unit; of them 39 (39.8%) received and 59 (60.2%) did not receive antibiotics. Physicians had tendency to start antibiotics in patient with mild-to-moderate chest retraction, need oxygen therapy, high white blood cell count, segmented neutrophil and positive C-reactive protein. All of them had negative blood cultures, no readmission, and no late-onset sepsis. The duration of hospital stay was longer in patients received antibiotics group (10.7±3.0 days vs. 9.0±4.4 days, P=0.04). Conclusion: This study suggests that empiric antibiotic therapy may not be recommended for full-term infants with classic TTN without perinatal risk factors. With the application of strict criteria for classic TTN and the close observation, the empiric use of antibiotics may be avoidable.

Intussusception in premature infants is very rare. Here, we report a case of multiple intussusceptions in an extremely preterm infant, born at 23+1 week’s gestation, who underwent an explolaparotomy, for bowel perforation and misdiagnosed necrotizing enterocolitis, at 20 days of life. To our knowledge, this is the most premature case that has survived with multiple intussusceptions.
단량체 조성 변화에 의한 수성 아크릴계 공중합체 제조 및 점착특성

박리강도와 내수성이 향상된 친환경적인 수성 점착제 제조를 위해 Tg가 상온에 가깝도록 아크릴 모노머인 methyl methacrylate(MMA), butyl acrylate(BA), methacrylic acid(MAA)의 비율을 조절하고 ammonium persulfate를 개시제로 이용하여 아크릴 공중합체(Acrylic copolymer)를 유화 중합하여 아크릴 에멀젼 점착제를 제조하고 평가하였다. MMA:BA:MAA를 55.2:43.5:1.3의 mole 비율로 합성한 결과, GPC와 입도분석으로 아크릴 공중합체 에멀젼이 합성된 것을 확인하였으며 휘발성 유기물(VOCs)도 검출되지 않았다. 이론적 Tg는 18.1℃이며, 피착재로써 폴리우레탄 폼, 폴리프로필렌 폼, 폴리프로필렌을 적용하여 초기, 최종, 침수 후 접착력을 평가한 결과 박리강도가 기존 제품에 비해 향상되었음을 확인하였다. In order to manufacture an eco-friendly aqueous adhesive with improved peel strength and water resistance, we adjusted the ratio of acrylic monomers methyl methacrylate (MMA), butyl acrylate (BA), and methacrylic acid (MAA) so that Tg is close to room temperature, and used ammonium persulfate as an initiator. Then, an acrylic copolymer was emulsion-polymerized to prepare and evaluate an acrylic emulsion adhesive. As a result of synthesizing MMA:BA:MAA in a mole ratio of 55.2:43.5:1.3, it was confirmed that an acrylic copolymer emulsion was synthesized by GPC and particle size analysis, and volatile organic substances(VOCs) were not detected. Theoretical Tg is 18.1°C, close to room temperature, and as a result of evaluating the initial, final, and post-immersion adhesion by applying polyurethane foam, polyethylene foam, and polypropylene as adherends, it was confirmed that the peel strength was improved compared to existing products.

The subchronic toxicity study of rHuEPO, a newly developed recombinant erythropoietin, was investigated for 13 weeks in Beagle dogs intravenously treated with doses of 100, 500 and 2,500 IU/㎏/day. There were no significant changes in body weight, food intake, physical and opthalmic examination, urine analysis, etc. Any toxic response was not observed except for enlarged spleen and extramedullary hematopoiesis. These results indicate that the no-observed adverse effect level (NOAEL) of rHuEPO is 100 IU/㎏ in Beagle dogs.
