        In this paper, a new network protocol, AD²R(ATM Distributed queue Dual Ring), is proposed for high-speed communication. This protocol is a slotted using ATM scheme. With the mathematical analysis and computer simulation, it is shown that proposed protocol is appropriate for the high-speed communication. Also, it is verified that the AD²R network performance is enhanced by using the ring topology, the destination release and the distributed queue scheme.

        척추관 협찬증 환자 진료 프로세스 개발

        연구배경: 수술적 치료가 필요한 척추관협착증 환자들은 주로 60세 이상의 고령환자로장기간의 입원 시 기회 감염의 증대와 불필요한 의료비의 증대를 가져오게 되며, 수술후환χ}들의 재원일수의 증가는병원의 병상가 동률을감소시키고, 전공의에게는불필요한 업무를 증가시킨다. 연구목적: 비용·효과면에서 최적화된 진료지침의 개발은 불필요한 의료비의 감소및 Hospital Induced Complication을 줄여 환자만족도를증진시킬 수 있으며, 각 환자에 대한전공의 업무를 줄일 수있다. 의료기관: 서울특별시에 소재한 2,075병상의 종합전문 요양기관 연구방법: 정형외과및 신경외과의 척추관협착증환자 의 처방을 비교하여 최적의 표준진료지침을 개발하고 최종적으로 CP Master Program (EMR 프로그램)에 입력하여 환자에게 적용하였다. 연구결과': CP 적용전, 후비교를통해 재원일수는 3.8일 이 감소하였으며, 이에 따라 병상가동률 및 진료수익이 증가했으리라고 예상되며 현재 비교검토중이다. 또한,CP 개발및 CP Master Program의 사용을 통한 전공의 업무감소에 대해 검토하고 있다.

      • Egr-1 Mediates Transcriptional Activation of IGF-Ⅱ Gene in Response to Hypoxia

        We have previously reported that the exposure of human HepG2 cells to hypoxic conditions results in the overexpression of human insulin-like growth factor Ⅱ (IGF_Ⅱ) mRNA whose size is 6.0 kb. This particular size of IGF-Ⅱ mRNA is transcribed under the control of the IGF-Ⅱ P3 promoter. In the present study, to delineate the molecular mechanism for the activation of the IGF-Ⅱ gene, we examined the induction of P3 promoter activity in HepG2-cells by hypoxia in the transient expression system. In this system, thpoxia induced a linear increase within 24 h in the expression of luciferase that was driven by the IGF-Ⅱ P3 promoter. To pattern of regulators of the P3 promoter, Egr-1, Sp-1, and WT1, were analyzed by reverse transcription-PCR and Northern blot analysis. We contrast, the level of WT1, a repressor of IGF-Ⅱ expression, was markedly decreased during hypoxia. The mRNA stability assay revealed that the induction of transcription is the mechanism of underlying Egr-1 mRNA elevation. We then investigated the effects of hypoxia on the DNA binding activity of Egr-1. Both electrophoretic mobility shift assay and supershift assay demonstrated that the DNA binding activity of the Egr-1 protein was increased by hypoxia. In addition, the level of Egr-1 protein increased under the hypoxia as determined by Western blot analysis. Cotransfection of HepG2 cells with an Egr-1 expression vector and an IGF-Ⅱ P3 promoter-luciferase reporter plasmid showed that the transcription of IGF-Ⅱ was activated by Egr-1 in a dose-dependent manner. Moreover, the elecation of IGF-Ⅱ P3 promoter activity was induced synergistically by the cotreatment of hypoxia whth Egr-1 overexpression. Deletion of sequences in the IGF-Ⅱ P3 promoter containing Egr-1 binding sites did not respond to hypoxic stress. Taken together, these data strongly indicate that hypoxia-induced IGF-Ⅱ expression in HepG2 cells is due to the enhanced activity of Egr-1 on the IGF-Ⅱ P3 promoter and that the Egr-1 binding site in the IGF-Ⅱ P3 promoter is essential for the transcriptional regulation of IGF-Ⅱ under hypoxic conditions.

        Association between serum alanine amino­transferase level and obesity indices in Korean adolescents

        Purpose: To analyze the correlation between serum alanine aminotransferase (ALT) and obesity indices including body mass index (BMI), body fat percentage (BFP), total fat mass (FM), truncal fat mass (TFM), waist circumference (WC), and waist-to-height ratio (WHtR) in Korean adolescents. Methods: This was a cross-sectional study based on data derived from the 2010-2011 Korean National Health and Nutrition Examination Surveys (KNHANES). Subjects were Korean adolescents aged 10-18 years (871 total; 475 boys and 396 girls) who participated in KNHANES. Results: In both sexes, BMI, FM, TFM, WC, and WHtR were higher when ALT levels were in the 4th quartile. In boys, there was a significant positive correlation between ALT level and BMI, BFP, FM, TFM, WC, and WHtR (r=0.55, P<0.0001 for BMI; r=0.52, P<0.0001 for BFP; r=0.58, P<0.0001 for FM; r=0.61, P<0.0001 for TFM; and r=0.56, P<0.0001 for WC; r=0.62, P<0.0001 for WHtR), and the correlation coefficient was higher than that in girls. Conclusion: Our results suggest a significant positive association between serum ALT level and obesity indices in male adolescents.

        Correlation between Capillary Blood-spotted Filter Paper Thyrotropin Results and Serum Thyroid Function Tests in Premature Neonates

        Objective: The interpretation of thyroid function test results in preterm infants is affected by various conditions. Neither a definite thyrotropin cut-off nor intercurrent conditions have been clearly determined for premature neonates. This study aimed to determine the specific considerations required in the interpretation of capillary blood thyrotropin concentration by comparing it to serum thyrotropin when screening for congenital hypothyroidism in preterm neonates. Methods: A retrospective review was performed on 57 premature newborns who were screened for thyroid function test. The comparison of thyrotropin measured by two different methods, dried capillary blood-spotted filter paper using an immunofluorometric assay and serum radioimmunoassay, was performed. Clinical and biochemical factors affecting the difference between the two methods were investigated. Results: Filter paper thyrotropin concentration showed a positive correlation with the serum thyrotropin concentration. In total, 50.9% of premature neonates had a difference in the concentration ≥3.0 mIU/L between the methods. In these subjects, the serum thyrotropin concentration was higher, whereas other factors, such as maternal factors, neonatal anthropometric parameters, Apgar score, and corrected age, were not different when compared to that in subjects with a difference of <3.0 mIU/L. Conclusion: The filter paper thyrotropin concentration differed from the serum level in a substantial portion of premature neonates, and the degree of deviation may be affected by the serum thyrotropin concentration and gestational age at birth. Further investigations and longitudinal studies are warranted to establish clinical consensus guidelines for hypothyroidism screening in preterm neonates.

        Survey of Korean pediatrician’s perceptions of barriers to and improvements in breastfeeding

        Background: Although the benefits of breastfeeding are broadly acknowledged with the efforts of the government and several medical societies, the rate of exclusive breastfeeding through 6 months is lower in Korea than in developed countries.Purpose: This study aimed to investigate pediatricians’ perceptions of breastfeeding barriers and the current breastfeeding counseling environment and propose government policies to encourage breastfeeding in Korea.Methods: Fourteen survey questions were developed during meetings of Korean Society of Breastfeeding Medicine experts. The Korean Pediatric Society emailed a structured questionnaire to domestic pediatricians registered as official members of the Korean Pediatric Society on May 4, 2021, and June 3, 2021. This study examined the survey responses received from 168 pediatricians.Results: The 168 respondents included 62 professors, 53 paid doctors, and 53 private physicians. Breastfeeding was recommended by 146 Korean pediatricians (86.9%). However, only 99 responders (59%) currently provide breastfeeding counseling in hospitals. Most respondents stated providing less than 15 minutes of breastfeeding counseling time in the clinic. Moreover, 89.88% of the respondents responded that they would participate in breastfeeding counseling education if an appropriate breastfeeding counseling program was newly established.Conclusion: This study showed that, although Korean pediatricians had a positive attitude toward breastfeeding, limited counseling was provided for parents. Along with policy support to improve the medical environment through the establishment of an appropriate breastfeeding counseling program, high-quality counseling and an increased breastfeeding rate are expected.

        Alagille syndrome and a JAG1 mutation: 41 cases of experience at a single center

        Purpose: Alagille syndrome is a complex hereditary disorder that is associated with cardiac, hepatic, skeletal, ocular, and facial abnormalities. Mutations in the Notch signaling pathway, such as in JAG1 and NOTCH2, play a key role in embryonic development. A cardiac or hepatic presentation is a critical factor for determining the prognosis. Methods: We conducted a retrospective study of 41 patients with Alagille syndrome or a JAG1 mutation between 1983 and 2013. Results: The first presentations were jaundice, murmur, cyanosis, and small bowel obstruction at a median age of 1.0 months (range, 0-24 months). The JAG1 mutation was found in 27 of the 28 genetically-tested patients. Cardiovascular anomalies were identified in 36 patients, chronic cholestasis was identified in 34, and liver transplantation was performed in 9. There was no significant correlation between the severity of the liver and cardiac diseases. The most common cardiovascular anomaly was peripheral pulmonary stenosis (83.3%), with 13 patients having significant hemodynamic derangement and 12 undergoing surgical repair. A total bilirubin level of >15 mg/dL with a complex surgical procedure increased the surgical mortality (P=0.022). Eight patients died after a median period of 2.67 years (range, 0.33-15 years). The groups with fetal presentation and with combined severe liver and heart disease had the poorest survival (P<0.001). Conclusion: The group with combined severe liver and heart disease had the poorest survival, and a multidisciplinary approach is necessary to improve the outcome.

        Spectral Reproducibility and Quantification of Peptides in MALDI of Samples Prepared by Micro-Spotting

        <P>Previously, we reported that MALDI spectra of peptides became reproducible when temperature was kept constant. Linear calibration curves derived from such spectral data could be used for quantification. Homogeneity of samples was one of the requirements. Among the three popular matrices used in peptide MALDI [i.e., α-cyano-4-hydroxycinnamic acid (CHCA), 2,5-dihydroxybenzoic acid (DHB), and sinapinic acid (SA)], homogeneous samples could be prepared by conventional means only for CHCA. In this work, we showed that sample preparation by micro-spotting improved the homogeneity for all three cases.</P> [FIG OMISSION]</BR>
