        등속성 운동이 어깨 충돌증후군 환자의 회전근개 근력과 어깨 기능에 미치는 영향

        The purpose of this study is to investigate effects of isokinetic exercise on shoulder external and internal rotator strength and shoulder function in patients with shoulder impingement syndrome. Twelve patients who were diagnosed shoulder impingement syndrome were randomly divided into two groups; a isokinetic exercise (IE) group (55.3 ±18.16yr) and a Home exercise (HE) group (52.83±26.27 yr). IE group participated in 5 weeks of isokinetic exercise, consisted of strengthening program on shoulder external/internal rotator with maximal isokinetic contraction. Both groups were educated and conducted home based exercise for 5 weeks. HE group participated in only home based exercise. The shoulder ortator cuff strength was measured by isokinetic strength evaluation machine and shoulder function was evaluated by the University of California at Los Angeles scale(UCLA scale), Shoulder Pain and Disability Index(SPADI) score at pre- and post- intervention. After 5 weeks, IE group showed significant improvement on external rotation peak torque(p<.05), external/internal rotation total work(p<.05). But HE group showed no significant improvement on rotator cuff strength (p>.05). Also IE group showed significant improvement on Shoulder function(p<.05). IE group showed significant improvement on UCLA score (p<.05), SPADI score (p<.05) but HE group only showed significant improvement on UCLA score (p<.05). No significant differences were observed in shoulder rotator cuff strength and shoulder function at before and after intervention in between two groups (p>. 05). We found that isokinetic exercise improved shoulder rotator cuff strength for relatively short period. Also home based exercise has positive effects on shoulder functions. On basis of these results, we concluded that isokinetic exercise is effective on shoulder rotator cuff strength, and shoulder functions in patients with shoulder impingement syndrome. Finally, Home based exercise included rotator cuff strength exercise and scapular stabilization exercise, shoulder stretching exercise would be necessary for treatment of shoulder impingement syndrome.

        일회성 등속성 운동이 심장 자율신경 조절 및 근속성에 미치는 영향

        Purpose : The aim of this study was to identify the influence of transient isokinetic exercise on cardiac autonomic modulation and muscle properties in healthy male subjects. Methods : Twenty-eight healthy males underwent isokinetic exercise of both knee joints using a Biodex systems 3 isokinetic dynamometer with an angular velocity of 60 °/sec. The changes in activity of the autonomic nervous system, as determined by heart rate variability (HRV), and in muscle properties were evaluated at three times: pre-exercise, immediately post-exercise, and 10 min post-exercise. Results : The time domain analysis of HRV revealed significant changes in the beat count and mean and minimal heart rate (HR) measured at pre-exercise, immediately post-exercise, and 10 min post-exercise (p<.001). The beat count and mean HR were markedly increased immediately post-exercise compared to pre-exercise, but then significantly decreased at 10 min post-exercise (p<.001). All parameters of the frequency domain were significantly altered by isokinetic exercise (p<.01). The low frequency/high frequency (LF/HF) ratio, as an index for the sympathovagal balance, was elevated by exercise and remained at a similarly high level at 10 min post-exercise (p<.01). The muscle properties of rectus femoris were changed as follows: Muscle tone and stiffness were significantly increased between pre-exercise and immediately post-exercise (p<.001), and between pre-exercise and at 10 min post-exercise (p<.001). Whereas, the elasticity showed no significant change. Conclusion : These results demonstrated that transient isokinetic exercise could induce changes in cardiac autonomic control and muscle properties. In particular, up-regulation of LF/HF ratio after exercise signifies thus enhanced sympathetic modulation by isokinetic exercise. Therefore, it is needed to understand the cardiovascular risks that may arise during isokinetic exercise for providing the basic evidence to establish appropriate isokinetic exercise protocols as effective rehabilitation exercises.

        발목테이핑 후 트레드밀 운동시간에 따른 등속성 발목근력 변화

        This study was to investigate changes of muscle strength as exercise time duration after ankle taping in baseball players. For this study, highschool baseball players whose had no ankle injuries were volunteered, and all subjects were randomly divided into two groups (TG - taping group, n=15 and CG - control group, n=15). Dominant side ankles of all subjects in TG were taped by closed basketweave technique before exercise. For exercise stimulation, 10, 20, 30 and 40 minutes of running exercise was carried out on the treadmill (10 km/h, 0%) with 48 hours intervals. Cybex 770 Model (Lumex, U.S.A) was used to measure isokinetic muscle strengths at 30°/sec and 120°/sec angular velocity To analyze the datas, repeated measured two way ANCOVA were used and significance was set at p<.05 for all tests. The results are as follows Isokinetic muscle strengths of dorsi flexor at 10, 20, 30 and 40 min after exercise were significantly increased compared to before exercise in both groups. Isokinetic muscle strengths of eversor at 10 and 30 min after exercise were significantly lower in TG compared to CG, and isokinetic muscle strengths of eversor were significantly increased after exercise compared to before exercise in both groups. Isokinetic muscle strengths of inversor at 40 min after exercise were significantly lower in TG compared to CG, and isokinetic muscle strengths of inversor were significantly increased after exercise compared to before exercise in both groups. Powers of plantar flexor at 10 min after exercise were significantly lower in TG compared to CG. Powers of eversor at 10, 20 and 30 min after exercise were significantly lower in TG compared to CG. Powers of eversor were significantly decreased until 20 min after exercise and then significantly increased from 30 min after exercise in TG, however, powers of eversor were significantly increased at 10, 20 and 30 min after exercise compared to before exercise in CG. Powers of inversor at 20 and 30 min after exercise were significantly lower in TG compared to CG. Powers of inversor at 10 min after exercise were significantly increased compared to 20 and 30 min after exercise in TG, and Powers of inversor at 40 min after exercise were significantly increased compared to before exercise, 10 and 20 min after exercise in. In conclusion, taping on ankle can have an negative effect on ankle muscle strength and when baseball players need taping, exercise duration should be considered. but It is possible to decrease effects in ankle joint muscular power, which should not be overlooked. Taping care should be ankle condition of the players. then, that will affect an injury prevention and light performance improvement.

        승마운동이 초보기승자의 혈중지질성분, 등속성 하지근력과 근지구력에 미치는 영향

        The aimed to investigate the preliminary effects of horseback-riding exercise on serum lipids, isokinetic muscles strength and muscles endurance in novices rider. Twenty-nine men (aged 46-50 years) with horseback novices rider participated, completing a horseback-riding exercise program for 70 min a day, three weekly, for 8 weeks. Serum lipids(TC, TG, LDL-C, HDL-C, Glucose), isokinetic strength and isokinetic endurance were measured using blood sampling and isokinetic machine before and after horseback-riding exercise. Statistical analyses were performed using wilcoxon singed rank test of non-parametric statistics analysis. Participants with horseback novies rider showed significantly improved glucose(Z=-4.705, p<0.01), TC(Z=-4.703, p<0.01), TG(Z=-4.703, p<0.01), LDL-C(Z=-4.704, p<0.01), HDL-C(Z=-4.706, p<0.01), isokinetic muscles strength(60°/sec, Rt. knee extension/ flexion, Z=-3.256, p<0.01/ Z=-2.391, p<0.05; Lt. knee extension/ flexion, Z=-4.121, p<0.01; Z=-2.110, p<0.05) and isokinetic muscles endurance(180°/sec, Rt. knee extension/ flexion, Z=-2.759, p<0.01/ Z=-3.126, p<0.01; Lt. knee extension/ flexion, Z=-4.383, p<0.01/ Z=-2.154, p<0.05) after completing the horseback-riding exercise. We recorded significant effects of the horseback-riding exercise on serum lipids, isokinetic strength and isokinetic endurance, suggesting the feasible inclusion of horseback-riding exercise in novices rider. In particular, the results of this trial will likely provide new information on the effects of health promotion, through change in serum lipids, isokinetic strength and isokinetic endurance of individuals with novices rider after horseback-riding exercise.

        닫힌 사슬운동과 슬링운동프로그램 병행이 뇌졸중 편마비환자의 균형과 보행 및 등속성근기능에 미치는 영향

        This study conducted closed kinetic chain and sling exercise with 16 patients with hemiplegia after stroke for 12 weeks to examine the effects of closed kinetic chain and sling exercise on balance, walking, and isokinetic muscle function. The subjects were male patients who were diagnosed as hemiplegia caused by stroke in J University Hospital of G Metropolitan City and had chronic hemiplegia over 12 months after a stroke attack. They should participate in exercise based on physiatrist’s opinion, walk independently using walking assistance devices, and not have subcutaneous atrophy and orthopedic-diseases of lower extremity. Their consents to the whole process of experiment were obtained and they were willing to participate in the study. The subjects were randomly assigned to the exercise group(n=10) and the control group(n=8). 2 participants of the exercise group dropped out. The purpose of this study was to examine how closed kinetic chain and sling exercise for 12 weeks made a difference to balance, walking, and isokinetic muscle function in patients with hemiplegia after stroke. The results of analysis and comparison were as follows. First, for the change of balance ability, patients with hemiplegia after stroke who participated in closed kinetic chain and sling exercise showed improved balance. It suggests that closed kinetic chain and sling exercise had a positive effect in patients with hemiplegia after stroke. Second, for the change of walking ability, patients with hemiplegia after stroke who participated in closed kinetic chain and sling exercise showed improved walking in both TUG and 10MWT. It suggests that closed kinetic chain and sling exercise had a positive effect in patients with hemiplegia after stroke. Third, for the change of isokinetic muscle function, patients with hemiplegia after stroke who participated in closed kinetic chain and sling exercise showed the improvement of affected extension, unaffected extension, affected flexion, and unaffected flexion. It suggests that closed kinetic chain and sling exercise had a positive effect in patients with hemiplegia after stroke. Based on the findings, closed kinetic chain and sling exercise had a positive effect on the improvement of balance, walking, and isokinetic muscle function in patients with hemiplegia after stroke. However, it is difficult to generalize the findings for all patients with hemiplegia after stroke because the number of the subjects of this study was small. It is considered that more various exercise programs should be applied to patients with hemiplegia after stroke who are usually limited in exercise.

        탄력저항성 운동과 평형성 운동 트레이닝이 여성 노인의 등속성 근기능, 균형 능력 및 균형 자신감에 미치는 영향

        본 연구의 목적은 12주간의 탄력저항성 운동과 평형성 운동 트레이닝이 여성 노인의 등속성 근기능, 균형 능력 및 균형 자신감에 미치는 영향을 규명하는 것이었다. 탄력저항성 운동 집단(17명)의 피검자는 한 회에 60분씩, 일주일에 3회 밴드를 이용하여 저항성 운동을 실시하였고, 평형성 운동 집단(17명)의 피검자는 정적, 동적, 그리고 시각을 자극하는 평형성 운동을 탄력저항성 운동 집단과 동일한 시간과 빈도에 따라 실시하였으며, 통제 집단(18명)의 피검자는 같은 기간 동안 평소의 생활을 그대로 유지하였다. 노인의 건강을 위협하는 낙상의 주원인으로 알려진 하지 근력과 평형성을 평가하기 위하여 선정한 하지의 등속성 근기능, 균형 능력 및 균형 자신감을 12주간의 처치 전, 후에 모든 피검자를 대상으로 측정하였으며, 반복이 있는 이원변량분석을 이용하여 분석하였다. 12주간의 처치에 따라 발목의 배측굴곡(180°/sec)에서 검사의 주효과와 집단과 검사의 상호작용이 유의하게 나타났다. 12주간의 처치에 따라 무릎의 등속성 근기능에서 검사의 주효과가 유의하게 나타났다. 탄력저항성 운동 집단의 경우 모든 등속성 근기능 관련 변인에서 유의한 증가가 나타났다. 평형성 운동 집단의 경우 굴근과 신근의 최대 힘효율(90°/sec), 굴근의 체중당 최대 힘효율(90°/sec)에서 통계적으로 유의하게 증가된 것으로 나타났다. 균형 능력과 관련된 변인은 탄력저항성 운동 집단에서 증가된 경향을 보였으나, 통계적으로 유의한 차이는 나타나지 않았다. 반면에 평형성 운동 집단에서는 균형 능력과 관련된 모든 변인들이 통계적으로 유의하게 증가된 것으로 나타났다. 균형 자신감 점수는 두 집단 모두에서 통계적으로 유의하게 증가된 것으로 나타났으나, 평형성 운동 집단의 증가 폭이 더 큰 것으로 나타났다. 이상의 연구 결과를 토대로 하여, 탄력저항성 운동과 평형성 운동은 모두 낙상의 주원인으로 알려진 하지의 근기능과 균형 능력의 향상에 도움이 되는 것으로 밝혀졌다. 특히, 평형성 운동 트레이닝 프로그램은 탄력저항성 운동에 비하여 균형 능력을 더욱 크게 향상시킨 것은 물론 등속성 근기능과 균형자신감의 향상에도 매우 효과적인 것으로 나타나, 향후 낙상의 예방을 위한 운동 프로그램을 작성할 때는 평형성 운동을 적극적으로 포함시켜야 할 것으로 판단된다. The study was designed to examine the effects of 12 weeks of resistance and balance exercise training on isokinetic function of lower extremities, postural stability, and balance efficacy in elderly women. Fifty-two healthy elderly women were volunteered to participated in the study as subjects and were assigned to one of three groups, i.e., resistance exercise training group(RETG: n=17), balance exercise training group(BETG: n=17), and control group(CG: n=18). The subjects in RETG participated in resistance exercise training program which was consisted of thera-band exercise, 60 minutes per session, and three times per week. The subjects in BETG went through the balance exercise training program for same duration and frequency as RETG. The variables regarding to isokinetic function, postural stability, and balance efficacy were measured before and after each intervention in three group. The data were analyzed utilizing two-way ANOVA with repeated measures. The results were as follows: 1) There was a significant main effect of test and interaction between group and test in dorsi flexion(180˚/sec) of ankle after 12 weeks of interventions. There was a significant main effect of test in isokinetic function of knee after 12 weeks of interventions. All variables regarding to isokinetic function in RETG increased significantly after 12 weeks of resistance exercise training. Peak torque of flexion and extension(90˚/sec), peak torque per body weight of flexion(90˚/sec) in BETG increased significantly after 12 weeks of balance exercise training. 2) The variables related to postural stability in RETG showed a tendency to be improved, but the changes failed to reach statistical significant level. These variables in BETG increased significantly after 12 weeks of balance exercise training. 3) The scores in balance efficacy increased significantly in both training groups due to the exercise training(P<.01). It was concluded that both resistance and balance exercise training programs were effective in enhancing the falling injury-related variables such as isokinetic strength, postural stability, and balance efficacy. Especially, it was shown that balance exercise training was more effective in enhancing the variables compared to resistance exercise training, indicating that the exercise training program for preventing the falling injury in the elderly should be composed mainly of balance exercises.

        12주간 필라테스 운동이 비만 여대생의 하지 등속성 근력, 혈중 Cortisol 및 건강관련 삶의 질에 미치는 영향

        This study was investigated the effects of 12 weeks Pilates exercise on limb isokinetic strength, serum cortisol, and health-related quality of life (HRQL) in obese female college students. Twenty three subjects, aged 21.97±0.98 yrs, were randomly assigned into two groups: pilates exercise group (Pilates, n=12) and non-exercise control group (Control, n=12). The subjects in the pilates group participated in the pilates exercise program for 55-60 minutes a day, 3 days per week for 12 weeks. Body mass index (BMI) was measured by impedance method before and after experience period. The serum cortosol level was also collected before and after exercise. HRQL was measured by using the Medical Outcome Survey Short Form-36 version 1.0 before and after experience period. BMI was significantly decreased (p<0.05) in pilates exercise group after 12 weeks, and then isokinetic peak torque was significantly increased (p<0.05) between knee extension and flexion in pilates exercise group. Serum cortisol level was significantly decreased (p<0.05) in pilates exercise group. Among the 8 domains of HRQL, 4 domains which involve physical functioning, vitality, role limitation physical, and role limitation emotional were significantly improved (p<0.05) in pilates exercise group. These results suggest that prolonged pilates exercise improves BMI, muscle strength, cotisol level, HRQL of obese female college students.

        테이핑 처치 전·후 등속성 근력과 운동시간 경과에 따른 고유수용감각의 변화 연구

        The purpose of this study was to investigate changes of isokinetic muscle strength proprioceptive function as exercise time duration after ankle taping in baseball players. To measure proprioceptive function, hop test and Romberg test were conducted. To analyze the datas, repeated measured two way ANCOVA and T-test were used and significance was set at p<.05 for all tests. The results are as follows; 1) Isokinetic muscle strengths of plantar flexor were significantly decreased by taping(p<.01). 2) Power of dorsi flexor, plantar flexor and eversor were significantly decreased by taping(p<.05). 3) The distance of hop test after exercise were significantly increased compared to before exercise in TG, and the distance of hop test at 20 min after exercise were significantly increased compared to before exercise in CG. 4) The duration of Romberg test of all measurements were significantly decreased in TG compared to CG. The duration of Romberg test at 10 min after exercise were significantly increased compared to before exercise and then significantly decreased at 20 min after exercise and these at 10 min after exercise were significantly increased compared to 20 and 30 min after exercise in TG. In conclusion, taping on ankle can have an negative effect on ankle muscle strength, power, proprioceptive function and when baseball players need taping, exercise duration should be considered.

        전동식 운동기기의 부하방식에 따른 근력증진 효과

        This study is to compare muscle strength between isotonic exercise and isotonic & isokinetic exercise. Participants are 12-man and 10-woman whom they are healthy without medical history in shoulder, elbow and lumbar joint. We performed experiment total 4-weeks that exercise 3-days a week each exercise pattern. We measured shoulder, elbow and lumbar joint torque with BIODEX and circumference of muscle in upper arms once a week. The result showed that isotonic & isokinetic exercise pattern significantly more improved joint torque in shoulder, elbow, lumbar than isotonic exercise pattern. Because that isotonic & isokinetic exercise pattern supplied muscle strengthen and caused muscle contraction. This exercise pattern can be used new exercise training method for major athlete and normal people. Also this pattern can be used rehabilitation treatment.
