        후십자인대 재건술의 임상적 고찰 - 골슬개건골을 이용한 -

        The posterior cruciate ligament(PCL) is the most important ligament of the knee joint because of its cross section area, tensile strength and location in the central axis of the knee(Hughston et al. 1976, Kennedy & Grainger 1967). Butler et al. (1980) demonstrated that it provides 95 % of the total restraint to posterior displacement of the tibia. We diagnosed and reconstructed sixteen cases with the injury of the PCL by the modified Clancy method. Twelve of sixteen patients with a follow up of eight months to five years(average twenty three months) were evaluated. Seven patients were augmented with Kennedy LAD. The subjective, objective and functional assessment of the knee made according to the criteria of Hughston and Degenhardt(1982). The incidence of the injury af the PCL was 25 % in one hundred seventy one knee ligament injuries. The injuries of medial collateral ligament were combined most frequently. 1. The objective and functional results were good in nine and fair in three. 2. The instability of the knee was measured as difference between the injured side and the opposite one. The difference is 7 to 8 mm in three and less than 5 mm in nine of twelve patients. 3. Seven patients were augmented with Kennedy LAD. Three patients were received arthroscopic adhesiolysis due to limitation of motion of the knee. Six patients were examined by arthroscopy and all of them had synovial overgrowth and well vascularization at intercondylar notch. We suggest that the reconstruction of the PCL is necessary when the posterior displacement is more than 1 or 1.5 cm, ache on descending stairs and some discomfort in usual life of the young patients. Augmentation of Kennedy LAD helps early motion of the knee and takes up part of the load to the grafted patellar tendon. But further follow up is required to evaluate the stress shielding effect of Kennedy LAD and the grafted patellar tendon.

        척수손상환자에서 Alendronate 복용 후 골밀도의 증가

        Objective: The aim of this study was to investigate the changes of bone mineral density (BMD) according to the postinjury duration, walking ability, and to assess the effect of oral alendronate therapy on BMD and biochemical markers in patients with spinal cord injury. Method: Forty-eight subjects with spinal cord injury were enrolled. One tablet of AlendⓇ (10 mg of sodium alendronate) was administered daily for 6 months. After this, all subjects received placebo for 6 months as the same manner. The baseline quantitative assessments of BMD and biochemical bone markers, serum osteocalcin and C-terminal telopeptide of type I collagen (ICTP), were performed before the administration of drug. The follow up assess-ments were performed at 6 and 12 months after drug and placebo administration. Results: The patients treated with oral alendronate showed significantly higher BMD of femur compared with baseline (p<0.05). Also, ICTP showed significant reduction after alendronate therapy. BMD change rate of alendronate therapy was higher in functional ambulation group compared with wheelchair ambulation group. BMD change rate of alendronate therapy was higher than that of placebo administration. Conclusion: Alendronate therapy may be useful in prevention of loss of BMD after spinal cord injury.

        Evaluation of Elbow Stretch Reflex Using a Portable Hand-driven Isokinetic System in Normal Adults

        Objective To evaluate normal healthy persons without spasticity to observe normal findings of the elbow stretch reflex using a newly developed, portable, hand-driven spasticity-measuring system. Method Thirty normal persons without any disease involving the central or peripheral nervous system were enrolled in this study. The portable hand-driven isokinetic system is able to measure the joint angle, angular velocity, electromyographic (EMG) signals, and torque during elbow passive extension-flexion. One set of 10 passive elbow extension and flexion movements was performed for data acquisition at each angular velocity, including 60, 90, 120, 150 and 180 degrees per second (o/sec). Electromyographic data were collected from the biceps brachii and the triceps brachii. Torque data were collected from sensors around the wrist. Results We were able to detect EMG activity and torque in all subjects by using the new portable hand-driven isokinetic system. EMG activity and torque increased with incremental increase of angular velocities. The joint angle of maximal EMG activity according to different angular velocities did not show any signifi cant difference (116o-127o in elbow extension and 37o-66o in elbow flexion). The joint angles of maximal torque according to different angular velocities were not signifi cantly different either. Conclusion Using the portable hand-driven isokinetic system on the elbows of normal subjects, we were able to obtain expected results. By considering our normal findings of the elbow stretch reflex using this system, we propose that the various aspects of spasticity-related data can be measured successfully.

        전방십자인대 손상에 대한 수술적 치료

        Rupture of the anterior cruciate ligament (ACL) of the knee has been called the beginning of the end for the knee. There is a controversy over the indication for nonoperative teatment or operative management. The patient's goals, motivation, associated ligament injuries and severity of the injury are critical factors. The substitute of ACL has to be placed isometrically as mentioned by Ben Garf. Thirty-five patients with clinical and functional instability of the knee due to anterior cruciate ligament insufficiency underwent the repair or reconstruction. The patients were followed for a minimum of ten months, average twenty-nine months. There were thirty-two males and three females. The average age was twenty-nine years. The most common cause of injuries was traffic accident (85 %). Ruputure sites were found most frequently at the femoral attachment (43 %). Meniscal tears were noted in five patients (14 %). The most frequently combined injury was the reputre of medial collateral ligament. The results were as follows by O'Donough's criteria: 1. In regards to the rupture site and duration from injury to srugery, the results were facorable except for those groups with ruptured at the substance and operation performd in one to two weeks after injury. 2. Significant difference in the final results has been noted between the repair and reconstruction groups. The follow up duration was rather short and further long term study might be necesssry. When the general condition of patient was poor, the surgical treatment could be delayed for 2 to 3 weeks without compromising the final result.

        Fine Mapping and Candidate Gene Analysis of Small Round Grain Mutant in Rice

        Grain size and panicle architecture are important traits determining yield. Here we report a new allele of DEP2 (dense and erect panicle 2) which has a pleiotropic effect on grain size/shape and panicle architecture. A small round grain and erect panicle mutant was obtained by treating japonica cultivar Hwacheong waxy with MNU (N-methyl-N-nitrosourea). Through fine mapping sequence analysis, we identified the single nucleotide substitution in the mutant, which caused splicing error of the first intron, on previously reported DEP2 locus in rice. Since the dep2-1 and dep2-2 have been reported in previous studies, we named this new allele as dep2-3. The dep2-3 mutant manifested reduced grain size and plant height, dense and erect panicle and erect plant architecture. When we crossed the mutant with wild type, panicle and grain of F1 plant showed intermediate phenotype, indicating that dep2-3 showed incomplete dominance, which was not reported in the previous study. Scanning electron microscopy (SEM) results showed that increase in width of mutant grain was due to the increased width of glume cells. Paraffin section of peduncle revealed that outgrowth in outer layer of peduncle compared to inner layer caused the erectness of the panicle. Our results collectively indicated that the dep2-3 might play a significant role in the regulation of grain size/shape and formation of vascular bundles.

        의료서비스 품질측정모형의 제시와 지역별 의료품질 차이에 관한 연구 : 대도시 및 지방도시지역의 비교를 중심으로

        There are two types of method in time series analysis ; time domain method and frequency domain method. This paper is concerned with the frequency domain method which is based on the assumption that a signal can be approximated by sinusoidal waves of different frequencies. The periodogram and sample spectrum are appropriate tools for analyzing the time series. The proposed procedure has four steps which are model identification, factor analysis, calculation of periodogram and determination of major period. This four-step procedure shows good capability for cyclical factor analysis of time series.
