        An Unusual Meningothelial Element in a Hairy Polyp of the Hard Palate

        Hairy polyps are a rare malformations of bigerminal origin that comprise of both ectodermal and mesodermal elements. Meningothelial elements are an extremely rare pathologic finding in hairy polyps. Here we report a case of a hairy polyp with a meningothelial element, which originated from the hard palate. A 1-year-old boy was evaluated for an intraoral mass accompanied by multiple congenital anomalies. A small polypoid mass was noted at the midline of the hard palate. The lesion had central fibroconnective tissue with an unusual stromal component showing reticulated anastomosing pseudovascular patterns. Immunohistochemical staining of the cells lining the pseudovascular spaces and the interstitial cells revealed vimentin and epithelial membrane antigen positivity.         Early wound healing of the hard-palate mucosal harvest site using artificial dermis fixation by a transparent plate

        Background There are currently no guidelines for the postoperative wound management of the hard-palate donor site in cases involving mucosal harvesting. This study describes our experiences with the use of an artificial dermis for early epithelialization and transparent plate fixation in cases involving hard-palate mucosal harvesting. Methods A transparent palatal plate was custom-fabricated using a thermoplastic resin board. After mucosal harvesting, an alginic acid-containing wound dressing (Sorbsan) was applied to the donor site, which was then covered with the plate. After confirming hemostasis, the dressing was changed to artificial dermis a few days later, and the plate was fixed to the artificial dermis. The size of the mucosal defect ranged from 8×25 to 20×40 mm. Results Plate fixation was adequate, with no postoperative slippage or infection of the artificial dermis. There was no pain at the harvest site, but a slight sense of incongruity during eating was reported. Although the fabrication and application of the palatal plate required extra steps before and after harvesting, the combination of the artificial dermis and palatal plate was found to be very useful for protecting the mucosal harvest site, and resulted in decreased pain and earlier epithelialization. Conclusions The combination of artificial dermis and a transparent palatal plate for wound management at the hard-palate mucosal donor site resolved some of the limitations of conventional methods.

        Verruciform xanthoma in the hard palate: a case report and literature review

        Oral verruciform xanthoma (OVX) is an uncommon lesion that appears on the oral mucosa. The aim of this paper was to discuss the probable etiopathogenesis of OVX in the hard palate, reinforcing the importance of including this lesion in the differential diagnosis of verrucous lesions. A 43-yearold male smoker presented with a painless lesion with a verrucous surface and erythematous spots on the hard palate. Excisional biopsy revealed oral mucosa consisting of hyperkeratosis, acanthosis, and elongated rete pegs. Subjacent connective tissue showed numerous foam cells with clear cytoplasm and pyknotic nucleus, negative on periodic acid-Schiff staining. Immunohistochemical analysis revealed foam cells positive for anti-CD68 antibody, while anti-KI-67 antibody was restricted to the basal layer of the oral epithelium. A final diagnosis of OVX was established. The patient showed no signs of recurrence after seven months of follow-up. Physical trauma and smoking habits can be directly related to the etiology of verruciform xanthoma because the lesion is chronic and inflammatory with slow growth, and sites if high trauma are more often affected by such a lesion. The hard palate is the second most commonly affected site, and local trauma caused by smoking can be a cause of this type of lesion.

        Vascular leiomyomas are benign tumors of smooth muscle origin arising from the muscularis layer of blood vessel walls. They can occur anywhere in the body where smooth muscle is found and usually occur in the lower extremity as a slow-growing, firm, occasionally painful mass. However they are rare in the head and neck and very rare in hard palate. Here we report a case of a vascular leiomyoma presenting as a soft mass of the hard palate and review the literatures.

        백서 구개 전층 창상의 치유에 미치는 피브린 접착제의 영향

        The purpose of this study was to examine the effects of the fibrin adhesive (Beriplast$^{(R)}$) on healing of full-thickness wounds in the rat's hard palate. Twenty Spraque-Dawley strain white male rats, each weighing 250~300 gm were used. Creation of full-thickness wounds of $4{\times}4mm$ in size were performed on the hard palate. Beriplast$^{(R)}$, a wound dressing material, was applied immediately in the experimental group, but not applied in the control group. All wounds were protected with palatal resin splints. The animals were sacrificed on the 2nd, 4th, 7th, 14th, and 28th day after the operation for macroscopic and microscopic examinations. Results obtained were as follows ; 1. On the 7th day after the operation, epithelial proliferation was greater in the experimental group than that in the control group. 2. The inflammatory reaction of the experimental group was less than the control group on the 2nd and 4th day after the operation. Beriplast was resorbed on the 7th day after the operation. 3. In the control group, the epithelial proliferation occurred from the 7th to the 14th day after the operation, and in the experimental group, epithelial proliferation occurred from the 4th day after the operation. 4. On the 14th and 28th day after the operation, there was no prominent difference between the two groups in histological findings. These results suggest that the use of fibrin adhesive (Beriplast$^{(R)}$) as a palatal wound dressing results in greater epithelial proliferation and less inflammation in the early stage of wound healing.

        Necrotizing Sialometaplasia of the Hard Palate: Diagnosis and Treatment

        We describe the diagnosis and treatment of necrotizing sialometaplasia (NSM) on the hard palate without surgical excision. A 27-year-old female visited the hospital with complaint of right-side hard palate swelling and pain. We diagnosed this hard palate lesion as NSM based on results of biopsy and treated it with conservative treatment. We now report the experience in diagnosing and treating NSM with a detailed review of histologic results.

        경구개에 발생한 여포성 림프 과증식

        The follicular lymphoid hyperplasia is a very rare nonneoplastic lymphoproliferative disease in the oral mucosa. Clinically, it usually presents as asymptomatic, non-ulcerated, normal-colored, soft, sessile, unilateral and slow growing mass in the posterior hard palate. Therefore, soft-tissue mass of the hard palate can be a difficult diagnostic challenge for the clinician. Herein, we report a case of follicular lymphoid hyperplasia located in the left posterior hard palate. A 40-year-old male presented a soft, non-ulcerated, painless mass of 1.5 X 1.0cm in size. Radiographically, it was no bony involvement. Histology revealed germinal center surrounding mantle zone of small lymphocytes. Immunohistochemistry showed CD3,CD20 were postive but Bcl-2 and cyclin D1 were negative. The patient underwent surgical excision and 3-year follow-up has not shown any evidence of recurrence.

        경구개에 발생한 평활근종 1예

        Leiomyoma is a benign smooth muscle tumor that can be classified into three types:solid, vascular and ephithelioid leiomyoma. This disease occurs most frequently in the uterus, gastrointestinal tract and skin. The oral cavity is an unusual site for a leiomyoma and occurrence in the hard palate is very rare because there is so little smooth muscle tissue present in this region. Clinically, oral leiomyomas usually grow slowly and are asymptomatic firm superficial nodule like lesions, although occasional tumors can be painful. The diagnosis and treatment of oral leiomyoma is possible with complete surgical excision and histological studies by special specific stains. Recently, we experienced a 60-year-old man with 6 month history of soft mass on the hard palate. He underwent surgical resection. Pathological analysis confirmed vascular leiomyoma. Here, we report this case with review of literature.

        경구개에 발생한 혈관성 평활근종 1예

        Vascular leiomyoma is an uncommon benign tumor that originates from the smooth muscle in the blood vessel walls. Vascular leiomyoma can occur in the head and neck region, but very rarely involves the hard palate. Vascular leiomyoma usually presents as small painless mass, and the diagnosis is usually confirmed after histopathologic study with immunohistochemical stain. The treatment of choice is complete surgical excision. Recently, we experienced a case of vascular leiomyoma on the hard palate and we report this case with review of literature.

        Sialadenoma papilliferum:

        Sialadenoma papilliferum (SP) is a rare benign neoplasm that normally arises from the minor salivary glands, particularly in the palate. SP is normally encountered in older men with an exophytic papillary surface growth. In the present study, an SP of the hard palate of a 69-year-old woman was examined immunohistochemically. Myoepithelial cell markers, such as S-100, smooth muscle actin and vimentin, were observed in the basal or luminal layer of tumor cells, indicating that myoepithelial cells participate in the pathogenesis of SP. In addition, cytokeratin 7 was also strongly detected in the tumor cells, suggesting that excretory ductal epithelial cells have a role in its histogenesis. A review of the literature of immunohistochemical studies on SP showed that the expression and co-expression of cytokeratins and myoepithelial cell markers have been reported in tumor cells. These results suggested that excretory duct cells and myoepithelial cells participate in the pathogenesis of SP.
