Background and Purpose Hand tremor is one of the most frequent symptoms in movement disorders, and differential diagnoses for hand tremor include Parkinson’s disease (PD) and essential tremor (ET). However, accurately differentiating between PD and ET in clinical practice remains challenging in patients presenting with hand tremor. We investigated whether a questionnaire-based survey could be useful as a screening tool in patients with hand tremor. Methods A questionnaire related to hand tremor consisting of 12 items was prospectively applied to patients with PD or ET in three movement-disorder clinics. Each question was analyzed, and a query-based scoring system was evaluated for differentiating hand tremors between PD and ET. Results This study enrolled 24 patients with PD and 25 patients with ET. Nine of the 12 questions differed significantly between PD and ET: 1 about resting tremor, 4 questions about action tremor, and 4 about asymmetry. A receiver operating characteristics curve analysis revealed that the 9-item questionnaire showed a good discrimination ability, with a sensitivity of 88% and a specificity of 84%. Conclusions The developed Hand Tremor Questionnaire might be a good screening tool for hand tremors in patients with PD and ET.

Hand tremor is one of the most frequent movement disorders and embarrassing symptoms of a wide spectrum of diseases. Hand tremor can be classified by form in which the hand tremor occurs, the main distinction being whether the hand tremor occurs at rest or is produced by voluntary muscle contractions. Essential tremor is the most common pathologic tremor in humans. The clinical features of essential tremor are heterogeneous across patients. Parkinson’s disease is a common disease in the elderly with increasing prevalence over the last decade. Rest tremor is one of the cardinal motor features of Parkinson’s disease. Drug-induced hand tremor is also common and should be diagnosed accurately. Hand tremor in Wilson’s disease is very important to identify because the disease can be treatable. Here, the diagnosis and treatment of hand tremor were reviewed to provide a practical guide for managing patients with hand tremor.
Design of Mechanical Compensation System for Hand Tremor of The Old People

Physiological hand tremor is one of the common problems seen in old people. It is an involuntary action to perform activities precisely. In this paper, a flexure-based tremor compensation device is proposed as a table top that acts as a support for hand to suppress tremor. The flexure is designed based on the tremor dynamics data. FE simulation is conducted to study the performance of the flexure mechanism. Using the in-house developed tremor generation device, the flexure mechanism study was conducted and compared with the rigid base. The study shows that the flexure mechanism compensates for tremor better.

Objectives:The purpose of this study is to report the clinical effectiveness of Korean traditional medicine, especially Gagamguibi-tang gamibang on psychogenic tremor. Methods:The patient in this case had suffered from her hand`s tremor especially when she`s nervous. Based on the tremor`s appearance and related symptoms, we could diagnose her tremor as psychogenic tremor. And we treated her with Korean traditional medicine, Gagamguibi-tang gamibang. And we used Clinical Tremor Rating Scale(CTRS) and State-Trait Anxiety Inventory(STAI) for assessment. Results:After 2 and half months of treatment, the tremor and involving symptoms have improved. Conclusions:According to this study, Gagamguibi-tang gamibang is an effective treatment for the cure of psychogenic tremor.

Symptoms of Parkinson’s disease (PD) caused by loss of dopaminergic neurons are accompanied by movement disorders, including tremors, rigidity, bradykinesia, and akinesia. Non-human primate (NHP) models with PD play an essential role in the analysis of PD pathophysiology and behavior symptoms. As impairments of hand dexterity function can affect activities of daily living in patients with PD, research on hand dexterity function in NHP models with chronic PD is essential. Traditional rating scales previously used in the evaluation of animal spontaneous behavior were insufficient due to factors related to subjectivity and passivity. Thus, experimentally designed applications for an appropriate apparatus are necessary. In this study, we aimed to longitudinally assess hand dexterity function using hand dexterity task (HDT) in NHP-PD models. To validate this assessment, we analyzed the alteration in Parkinsonian tremor signs and the functionality of presynaptic dopaminergic neuron using positron emission tomography imaging of dopamine transporters in these models. In addition, a significant inverse correlation between HDT and DAT level was identified, but no local bias was found. The correlation with intention tremor signs was lower than the resting tremor. In conclusion, the evaluation of HDT may reflect behavioral symptoms of NHP-PD models. Furthermore, HDT was effectively used to experimentally distinguish intention tremors from other tremors.
Research on Direct Model Reference-Based Adaptive Controller to Hand Tremors Reduction

A tremor is an involuntary, somewhat rhythmic, muscle contraction and relaxation involving to and from movements (oscillations or switching) of one or more body parts. It is the most common of all involuntary movements and can affect the hands, arms, eyes, face, head, vocal folds, trunk, and legs. Most tremors occur in the hands. In some people, tremor is a symptom of another neurological disorder. This research focuses on the design and analysis of a model reference-based controller for highly nonlinear dynamic multi degrees of freedom, in presence of hand tremors. In order to provide a high performance nonlinear methodology, the inverse dynamical controller is selected. The inverse dynamic controller is the type of model reference controller. It is nonlinear and stable controller. To reduce the challenges of this controller hybrid fuzzy controller is used. However, this method has many advantages, but it has challenged in presence of uncertainties. To solve this challenge model reference-based direct adaptive methodology is used and applied to hybrid inverse dynamic fuzzy controller.

Object : This clinical study is to report the effects of Chengsimyeonjatang-gamibang on tremors. Methods : Two patients was treated with Chengsimyeonjatang-gamibang and acupuncture that was applied at LU8, LR4, HT3, LR2, GV20, GB20. The effect of treatment was evaluated by Visual Analog Scale(VAS) and Motor examination of Unified Parkinson's Disease Rating Scale(UPDRSIII). Results : After Korean medicine treatment, the patients' VAS and UPDRSIII scores were decreased and conditions were improved. Conclusions : The Korean medicine treatment of Chengsimyeonjatang-gamibang and acupuncture might be effective for the tremors.
Active Tremor Cancellation of Handheld Microsurgical Robots

Physiological hand tremor may cause irrecoverable damage to lesion during microsurgery. Robot-aided microsurgery can address this problem as suppressing hand tremor and stabilizing surgical instruments. Active tremor cancellation of microsurgical robots is to deliver voluntary motion to the surgical instrument while suppressing undesired hand motion, such as hand tremor. In this study, we evaluate the performance of active tremor cancellation in a handheld type of microsurgical robots. For active tremor cancellation, a motion filter for the tool tip is designed as a second-order Butterworth lowpass filter with a cutoff frequency of 2 Hz with minimal latency, which attenuates magnitudes.

Involuntary hand tremor has been a serious challenge in micromanipulation tasks and thus draws asignificant amount of attention from related fields. To minimize the effect of the hand tremor, a varietyof mechanically assistive solutions have been proposed. However, approaches increasing human awarenessof their own hand tremor have not been extensively studied. In this paper, a head mount display basedvirtual reality (VR) system to increase human self-awareness of hand tremor is proposed. It shows a usera virtual image of a handheld device with emphasized hand tremor information. Provided with thisemphasized tremor information, we hypothesize that subjects will control their hand tremor more effectively. Two methods of emphasizing hand tremor information are demonstrated: (1) direct amplification of tremorand (2) magnification of virtual object, in comparison to the controlled condition without emphasized tremorinformation. A human-subject study with twelve trials was conducted, with four healthy participants whoperformed a task of holding a handheld gripper device in a specific direction. The results showed thatthe proposed methods achieved a reduced level of hand tremor compared with the control condition.
Effect of tool weight and hand posture on the postural tremor of the upper extremity

In light-weight hand tools, static posture may result in postural fatigue. Psotural tremor of the upper extremity in a static posture was measured to provide guidelines for hand tool weight. Postural tremor was measured on five levels of tool weitht : no weight, 400g, 800g, 1200g, and 1600g. Three types of camcorder recording postures were selected. For each condition, postural tremor was measured together with ENG of biceps, deltoid and pectoralis major, and Borg's CR-20 ratings of perceved exertion. Results of the experiment are as follows : frequency analysis of tremor revealed increased amplitude of frequency bands of 2-4Hz and 10-14Hz. Postural tremor of the upper extremeity maintained the initial level until fatigue developed. After the development of fatigue, the rate of change of postural tremor significantly increased. Different tool weights and hand postures showed different rates of tremor increase. Time tp fatigue and corresaponding endurance time were positively correlated with Borg's RPE scores.
