Interferon-alpha is an established therapy for the patients with myeloproliferative disease. Unusual immune- mediated side effects have been reported during treatment. Among them, renal and cardiac toxicities are rare complications. Recently we experienced a case of hemolytic uremic syndome found in a 40-year old male undergoing interferon-alpha therapy for chronic myelogenous leukemia, chronic phase. On admission, his chief complaints were generalized edema and azotemia. Bone marrow biopsy revealed transformation into accelerated phase. bcl/abl gene was positive. Peripheral blood smear showed hemolytic anemia. Despite prompt diagnosis, institution of plasmapheresis, and steroid, his renal function did not recover, as expected. Hemolytic uremic syndrome was a chronic form of renal toxicity due to interferon- alpha. In this case, renal function did not recovered to normal level. However, early detection of renal toxicity such as proteinuria or acute tubular necrosis, and the discontinuation of interferon-alpha could achieve the recovery of normal renal function. Therefore, periodic examination of renal function should be performed in the patients with the use of long-term interferon-alpha. (Korean J Nephrol 2002; 21(2):341-347)
Immune regulation through tryptophan metabolism

Amino acids are fundamental units of molecular components that are essential for sustaining life; however, their metabolism is closely interconnected to the control systems of cell function. Tryptophan (Trp) is an essential amino acid catabolized by complex metabolic pathways. Several of the resulting Trp metabolites are bioactive and play central roles in physiology and pathophysiology. Additionally, various physiological functions of Trp metabolites are mutually regulated by the gut microbiota and intestine to coordinately maintain intestinal homeostasis and symbiosis under steady state conditions and during the immune response to pathogens and xenotoxins. Cancer and inflammatory diseases are associated with dysbiosis- and host-related aberrant Trp metabolism and inactivation of the aryl hydrocarbon receptor (AHR), which is a receptor of several Trp metabolites. In this review, we focus on the mechanisms through which Trp metabolism converges to AHR activation for the modulation of immune function and restoration of tissue homeostasis and how these processes can be targeted using therapeutic approaches for cancer and inflammatory and autoimmune diseases.
트립토판 대사체 3-hydroxyanthranilic acid 의 TRAIL-유도 활성 T 세포 사멸 효과

Indoleamine 2,3-dioxygenase (IDO)에 의한 트립토판 대사체의 생성은 T 세포에 강력한 억제효과를 미치지만 여전히 그 작용기전에 대한 연구보고는 미비한 실정이다. 본 연구자는 트립토판 대사체 3-HAA가 선택적으로 활성 T 세포의 사멸을 촉진시키는 효과가 있음을 확인하였고, 이는 세포주기 억제와는 관련이 없었다. 3-HAA처리 시 활성 T 세포에서 TRAIL과 그의 수용체의 발현이 현저히 증가하고, 이들 상호작용을 차단하였을 때 3-HAA-매개 활성 T 세포사멸 효과가 유의하게 낮아졌다. 본 연구를 통해 트립토판 대사체 3-HAA의 선택적 T세포 억제 효과가 TRAIL-유도 세포사멸과 관련됨을 알 수 있다. Generation of tryptophan-derived metabolites by indoleamine 2,3-dioxygenase (IDO) is a potent immunoregulatory mechanism in T cell responses. However, the mechanism remains unclear. We showed that 3-hydroxyanthranilic acid (3-HAA), the most potent metabolite, selectively induced apoptosis in activated T cells, but not in resting T cells. This was not associated with cell cycle arrest. We found that TRAIL expression was selectively induced in activated T cells by treatment of 3-HAA. Blockade of the TRAIL: DR4/DR5 pathway significantly inhibited 3-HAA-mediated T cell death. Our data suggest that TRAIL-induced apoptosis is involved in the mechanism of 3-HAA-mediated T cell death.

The aim of the current study was to analyze the active ingredients and to screen the pharmacological properties of freshwater laver, Prasiola japonica, the only species grown in Korea. According to results of gas chromatography-mass spectrometry assay, components from P. japonica were more diverse than those from sea laver. Of particular interest, our results indicated that ethanol extract of P. japonica (PJE) contained loliolide, sorbitol, mannitol, and alverine, which were known to have an anti-oxidant, anti-oral microbial, osmotic diuresis, and smooth muscle relaxant, respectively. In addition, five solvent fractions of PJE (water, butanol, chloroform, ethyl acetate, and hexane) significantly inhibited the production of lipopolysaccharide-induced nitric oxide and a higher amount (>100 μg/mL) of chloroform, ethyl acetate, and hexane fraction were considered to play a specific role in cancer cell death. PJE and its solvent fractions found to be effective scavengers of free radicals, particularly, hydroxyl radicals. Glucose uptake in L6 myoblast cell line that stably expresses the glucose transporter type 4 (GLUT4) proteins was also remarkably enhanced upon treatment with solvent fractions, remarkably chloroform fraction. Taken together, we concluded that P. japonica may have potent pharmacological properties and thus contribute to development of novel natural candidates for various disease targets.

The origin and clinical behavior of gastrointestinal stromal tumors (GISTs) have been a source of recent speculation and controversy. GIST has been applied to mesenchymal tumors that represent neither their typical leiomyomas nor schwannomas. GISTs are spindle cell tumors occurring mainly in adults with 5-year survivals in 50-56% and 10-year survivals in 35-43%. Because of its indolent and slow-growing nature, it is difficult to define it as malignancy. We report a case of benign stromal tumor of the proximal jejunum presenting melena in a 32-year-old male. Small bowel series and computed tomography of the abdomen revealed submucosal tumor in the proximal jejunum. Histological studies on the surgical resection specimen revealed a gastrointestinal stromal tumor of a benign type with no evidence of metastasis. The patient was treated with local excision of the tumor and is now in a favorable state. (Korean J Gastroenterol 2001;38:371-375)
