진행성 폐 종양 환자에서 종양 내 OK-432 주입술의 효과

Background: Intratumoral injection of OK-432 produced significant antitumor effects. There are reports that OK-432 was injected directly into tumor tissue on gastrointestinal tracts and HCC tissue, producing decrease in serum alpha-fetoprotein level. We adopted this technique for treating locally advanced lung cancer. The object of this study is to evaluate the local therapeutic efficacy, side effects of injection for treating symptomatic mass. Methods: Twenty lung cancer patients (13 males and 7 females) underwent sonography guided OK-432 injection. We analyzed relationships between response of OK-432 and response of skin test, histology and age. Pain that patients feel was evaluated by VAS (Visual Analog scale). Results: A significant decrease of tumor size was observed. The mean pre-injection diameter of the lesions was 54.7±11.3mm, PR rate were 15.0% (3/20) and minimal change (50%<tumor size decreased.) patients were 35.0% (7/20). The VAS score before injecting OK-432 into the lesion was 5±3, this was decreased to 2.5±1.5 (p=0.04). Object response was significantly correlated with skin response, adenocarcinoma, and age. Conclusion: A significant decrease of tumor size and pain was observed. So OK-432 is a valuable treatment option for patients with symptomatic lesion from advanced lung cancer who are not amenable to further therapy.

We report a 70 year-old man with Binswanger's disease associated with mood symptoms. He was apparently well until 15 years ago when the treatment-resistant atypical mood symptoms had been appeared. He showed a fluctuating course in mental disturbance, such as irritable and labile affect, emotional incontinence, and irritable behavior. Two years ago, he was diagnose as hypertension, renal failure, and heart failure. He was admitted to psychiatric ward due to irritable mood, dysarthria, lack of bladder control, gait disturbance, and impulsive behaviors, which had been aggravated during the last 2 months. In physical examination, he had hypertension and both pretibial pitting edema was revealed, and also in neurological examination, exaggeration of deep tendon reflexes and weakness of extremities were found. On mental status examination, he showed irritable and labile mood with mild cognitive disturbances. Brain MRI demonstrated multifocal high signal intensity in periventricular white matter, both basal ganglia, and thalami on the T2 imaging. These findings were compatible with Binswanger's disease with mood symptoms.

The opportunistic human fungal pathogen Cryptococcus neoformans is the main causative agent of fungal meningoencephalitis in immunocompromised patients. The mannoprotein MP84 of C. neoformans, reported as an adhesion protein and as an immunoreactive antigen to host CD4(+) T cell, is highly mannosylated cell wall protein. In this study, we investigated the structure of N-glycans attached on MP84 and the effect of altered N-glycan structures on its interaction with host cells. We expressed the His tagged MP84 as a secretory form in WT and alg3Δ mutant, a N-glycosylation defective strain, to generate MP84 with the truncated core N-glycans. In in-vitro adhesion and uptake assay, the purified MP84 proteins carrying the truncated N-glycans did not show significant difference in host cell interactions compared to MP84 carrying the WT N-glycans. Moreover, we expressed a secretory MP84 lacking N-glycosylation site, constructed by protein engineering, to investigate the effect of full-deglycosylation on the interaction with MP84 with host cells. Aglycosylated MP84 revealed much higher adhesion affinity to host cells than MP84 carrying the WT and truncated N-glycans. On contrast, aglycosylated MP84 still retains the cytokine induction activity comparable to WT, although MP84 with truncated N-glycans showed a decreased immunostimulatory activity. In conclusion, the mixed effects of N-glycan structures of MP84 on the interaction with host cells strongly suggest the modulatory role of N-glycans to avoid non-specific interaction with host cells.
Two Cases of Russell Body Gastritis Treated by Helicobacter pylori Eradication

Russell body gastritis was first defined in 1998, but not many cases have been reported since then. The exact causes and process of this condition are unknown yet; however, considering the reported cases, it has been highly suggested to have correlation with Helicobacter pylori infection. Russell body gastritis has a non-specific clinical presentation of gastritis such as gastric mucosal edema in the macroscopic view. It can be mistaken as xanthoma, signet ring cell carcinoma, or a malignant lymphoma including mucosa-associated lymphoid tissue lymphoma and plasmocytoma. Russell body gastritis features polyclonal immunoglobulin and is differentiated from Mott cancer, of which immune globulin has monoclonal aspect. Authors report here two cases of Russell body gastritis with examined endoscopic findings as well as a review of related literature on the association of all reported cases of Russell body gastritis with H. pylori infection.
저장탱크 화재⋅폭발사고 사례연구 및 안전 확보방안에 대한 고찰

최근 화학물질 취급 사업장 등에서 화재⋅폭발사고가 빈번하게 발생하고 있다. 다량의 위험물을 취급하고 여러 공정⋅설비 등이 밀집되어있는 사업장에서 화재⋅폭발사고가 발생할 경우, 수많은 인명피해를 초래할 뿐만 아니라 인근 설비 등의 손상 또는 2차 사고의 위험이 있으므로 대형 사고로 이어질 가능성이 매우 크다. 그러므로 체계적인 안전조치를 통해 사고를 예방할 필요가 있으며, 이를 위해서는 과거 사례로부터 위험요인을 파악하여 이를 기반으로 안전대책을 수립하는 것이 필요하다. 이에 2019년부터 2022년 5월까지 사업장에서 사상자가 발생한 화재⋅폭발사고 30여 건을 분석한 결과, 주로 정비⋅보수작업과 같은 비일상 작업에서 사고가 잦았으며, 사고 설비로는 저장탱크에서 가장 많은 사고가 발생하였다. 특히, 인화성 물질 잔존 상태 또는 퍼지 미실시로 유증기 등이 잔류된 상태에서 화기작업 실시 등으로 화재⋅폭발사고가 빈번하게 발생되고 있는 것으로 나타났다. 이처럼 동일한 유형의 사고가 지속적으로 발생하고 있기 때문에, 재발방지를 위해 사고의 근본원인을 파악하는 것이 매우 중요하다. 이에 본 연구에서는 주 사고유형인 비일상작업 중 저장탱크 화재⋅폭발 사고사례를 분석하여 사고의 근본원인과 위험요인을 도출하였으며, 이를 기반으로 동종⋅유사사고 재발방지를 위한 안전 확보방안에 대해 고찰하였다.
