        척추마취 수술환자의 간호요구

        The Purpose of this study was to identify the needs which were perceived by patients who were received spinal anesthesia for surgery. The subjects consisted of 50 adult Patients who were admitted to 2 university hospitals and 2 general hospitals in Pusan city and 1 general hospital in Koje City for surgery under spinal anesthesia. Thirty eight percent of subjects received information about anesthesia before the operation. The instrument for this study was developed by the researcher based on literature and a Pretest. Data were collected from December 10, 1999 to February 10. 2000 and were analyzed by content analysis. The results were that there were 533 meaningful statements in the needs of spinal anesthesia patients. The needs of spinal anesthesia Patients had 51 items (preoperation (6), induction of anesthesia(5), intraoperation (27), postoperation(13)) and 6 categories (information. emotional welfare, physical welfare, post anesthetic management, control of physical environment, humane treatment). From the results, it can be concluded that: 1.In the pre-operation period. we have to explain anesthesia procedures. Adequate position of anesthesia. duration before anesthesia wears off and sensation of paralysis. We have to supply emotional support to relieve anxiety because of anesthesia. 2.In induction of anesthesia we have to support patient's position for anesthesia, and relieve anxiety so that patients participate in induction of anesthesia well. 3.In intra-operative period, we have to check the level of anesthesia, and keep up a comfortable position for operation and care for physical discomfort such as thirst. nausea, vomiting, dyspnea and to maintain body temperature of the Patient. Since the patient is conscious, we have to communicate with the patient to relieve anxiety, maintain Privacy, inform the patient of the Process of the operation and encourage the surgeon to explain the outcome of the operation. The operating team needs the careful about what they say and to place the instrument well. We have to ventilate the room air and reduce noise. 4.In the post-operative period, we have to explain the Purpose and duration of bed rest, complications of anesthesia and care for physical discomfort such as pain, dysuria, headache, backache. Also we have to maintain body temperature of the patient and maintain privacy.

        백서 척수후근절로부터 슈반세포의 효과적인 체외 배양법

        Schwann cells(SCs), an important component of the peripheral nervous system, intract with nerous to mutually support growth and replication for the peripheral nerve regentation. Recently, ading SCs to the lumen of guidance channel is widely tried to improve regeneration or to make regeneration possible over otherwise irreparable gaps. however, it is not easy to isolate and multiplicate SCs as much as enough to help the axonal regeneration. For the allogeneic SCs source for tubular nerve guidance, we developed a little bit improved technique of harvesting and multiplicating SCs. by culturing dispersed dorsal root ganglia in specially designed medium with growth factors and serial processing, we repeatedlly generate relatively homogenous SC cultures. Our technique was compared with other methods of literature using immunostaining methods such as GFAP, S100, BDNF and the total SC count assessment at different time interval after primary culture.

        연 폭로가 남성호르몬에 미치는 영향

        목 적 : 본 연구는 직업성 연 폭로근로자에서 연폭로가 남성 생식기계 독성중 성호르몬에 미치는 영향과 그 정도를 평가하고자 시도되었다.방 법 1998년 하반기에 실시한 일반, 특수건강진단자중 경기도 지역 직업성 연 폭로 근로자 33명과 과거 직업력에서 연 폭로가 없었던 사무직 근로자 33명을 연구대상으로 선정하였다. 연의 생물학적지표로는 혈색소, ZPP, 혈중 연, 및 요중 연을 이용하였으며, 성호르몬으로는 LH, FSH, testos-terone, SHBG 및 FTI를 이용하였다. 연 폭로 지표와 성호르몬과의 상관관계를 조사하였다. 결 과 : 연의 생물학적 지표중 혈색소는 폭로군에서 대조군보다 유의하게 낮았으며, ZPP, 혈중 연및 요중 연등은 폭로군에서 대조군보다 유의하게 높았다. 성호르몬에서 FSH, testosterone, SHBG및 FTI는 모두 폭로군에서 대조군보다 낮았으나 통계적 유의성은 없었다. 폭로군의 성호르몬중에서 연령은 FSH 및 FTI, ZPP는 FSH, 혈중 연은testosterone과 유의한 상관관계가 있었다. 대조군에서 연령은 FSH, testosterone, SHBG 및 FTI와 흡연량은 FSH와 유의한 상관관계가 있었다. 연령군별 폭로군과 대조군의 성호르몬 비교에서 30-39세군의 FTI에서 폭로군이 대조군보다 유의하게 낮았으며 그 밖의 호르몬에서는 모두 유의한 차이가 없었다. 결 론 : 연 폭로군의 FSH, Testosterone, SHBG 및 FTI가 대조군에 비교하여 낮았으나 통계적 유의성은 없었다. 그러나 폭로군에서 연의 생물학적 지표와 일부 성호르몬간에 유의한 상관관계를보였으며 이는 본 연구 대상 근로자의 연 폭로 수준이 일부 성호르몬에 영향이 있음을 보여주는 것으로 판단된다. 향후 이에 대한 지속적인 연구가 진행되어 야 할 것이다. Objectives : This study was performed to evaluate the relationship between biological markers of lead exposure and level of sex. hormones in men occupationally exposed to lead. Methods : In this study, 33 male workers who employed at smelting and battery factories were compared with 33 male workers who were not exposed to lead. As biological markers of lead exposure, hemoglobin, ZPP, blood lead and urine lead were measured. As sex hormones, FSH, LH, testosterone, SHBG and FTI were determined. Results : Hemoglobin level was significantly lower in exposed than in nonexposed group. Level of ZPP, blood lead, and urine lead were significantly higher in exposed than in nonexposed group. There was a trend that level of FSH, testosterone, SHBG, and FTI was lower in exposed than in nonexposed group respectively, but there were no statistical significance. In exposed group, FSH level was correlated with age, ZPP, and blood lead; testosterone level was correlated with hemoglobin, blood lead, urine lead, and SHBG ; and FTI level was correlated with age. Conclusions : Level of some sex hormones were lower in exposed than in nonexposed group but there were no statistical significance between the exposed and the nonexposed group. But level of sex hormones were correlated with several biological markers of lead in exposed group. It suggests that occupational lead exposure might affect sex hormones.

        사람골수 세포에서 B 혈액형 항원의 출현에 대한 면역 전자현미경적 연구

        Membrane antigens are extensively studied on different mammalian cell lines. Antigens of the erythrocyte series are often chemically well defined heterosaccharide determinants. since Yunis and Yunis (1963) gave strong evidence of the presence of A,B and H alloantigens on human normoblasts by indirect agglutination methods, several workers have detected A antigens by immunoelectron microscopy using peroxidase labelled antibodies (Reyes et al.,1974), staphylococcal protein A technique (Karhi et al., 1981) and fluorescence activated cell sorter (Sieff et al., 1981, 1982). Here we report immunoelectron microscopy observation of blood group B antigen on human normoblasts using peroxidase-labelled antibodies which allow detection of surface antigens on separated cells. The bone marrow specimens were selected from the ones which had been depleted of erythrocytes by a Ficoll-Hypaque technique and stored in liquid nitrogen(-196℃) as described (Seung & Kim, 1985). The selected specimens were from eight blood type B individuals with no disturbances in erythropoiesis. These experiments were carried out with cytocentrifuge smears and cell suspensions for light microscopic and electron microscopic observations respectively. The cell suspensions were fixed prior to incubation with reagents. Fixation was achieved by resuspending the cell pellet in phospate-buffered saline solution of 1.25% glutaraldehyde for 30min: followed by washes with a large quantity of the same buffer. They were resuspended in methanol containing 0.3% H₂O₂ for 30min. to block endogenous peroxidase and washed. And they were labelled by an indirect method using human Anti-B(IgM) and rabbit antihuman IgM antibodies labelled with horseradish peroxidase. The cytochemical detection of horseradish peroxidase was carried out by resuspending the cells for 30 min. in the dark in the usual medium containing diamino-benzidine and H₂O₂. Subsequently specimens were washed, postfixed with 1% OsO₄ for 30 min. and processed for dehydration in alcohol and acetone. For transmission electron microscopy they were embedded in Epon, sectioned and stained and stained with uranyl acetate and lead citrate. The results were as follows: 1. Under a light microscope the smears showed that brown positive cells were mixed with negative cells, but the precise identification of the cell types was difficult. 2. Under anelectron microscope a continuous or incontinuous labelling of blood group B antigens was seen in contact with the membrane of all normoblasts. 3. The myeloid precursor cells are not labelled, but occasionally limited labelling of the membrane of granulocytic and monocytic cells could be seen.

        The Dishevelled-binding protein CXXC5 negatively regulates cutaneous wound healing

        <P>Wnt/β-catenin signaling plays important roles in cutaneous wound healing and dermal fibrosis. However, its regulatory mechanism has not been fully elucidated, and a commercially available wound-healing agent targeting this pathway is desirable but currently unavailable. We found that CXXC-type zinc finger protein 5 (CXXC5) serves as a negative feedback regulator of the Wnt/β-catenin pathway by interacting with the Dishevelled (Dvl) protein. In humans, CXXC5 protein levels were reduced in epidermal keratinocytes and dermal fibroblasts of acute wounds. A differential regulation of β-catenin, α-smooth muscle actin (α-SMA), and collagen I by overexpression and silencing of CXXC5 in vitro indicated a critical role for this factor in myofibroblast differentiation and collagen production. In addition, <I>CXXC5<SUP>−/−</SUP></I> mice exhibited accelerated cutaneous wound healing, as well as enhanced keratin 14 and collagen synthesis. Protein transduction domain (PTD)–Dvl-binding motif (DBM), a competitor peptide blocking CXXC5-Dvl interactions, disrupted this negative feedback loop and activated β-catenin and collagen production in vitro. Co-treatment of skin wounds with PTD-DBM and valproic acid (VPA), a glycogen synthase kinase 3β (GSK3β) inhibitor which activates the Wnt/β-catenin pathway, synergistically accelerated cutaneous wound healing in mice. Together, these data suggest that CXXC5 would represent a potential target for future therapies aimed at improving wound healing.</P>

        신생 백서 척수후근절의 슈반세포 배양을 위한 Ara-C 분열억제제의 최적 효과에 대한 연구

        Schwann cell, one of important components of peripheral nervous system, interact with neurons to mutually support the growth and replication of embryonal nerves and to maintain the different functions of adult nerves. The Ara-C, known as an antimitotic agent, have been used to have high effectiveness in eliminating fibroblasts during Schwann cell culture period. This enrichment effect is also known to be cummulative with each successive pulse of Ara-C applied and is due to a progressive loss of fibroblasts. But the cytotoxicity by Ara-C is also cummulative and noticeable over the period. To determine the most effective application time and interval of Ara-C in the Schwann cell culture, we observed the Schwann cell purity and density with the Ara-C treatment in plain and three-dimensional culture from dorsal root ganglion of new born rat. By culturing dispersed dorsal root ganglia, we can repeatedly generate homogenous Schwann cells, and cellular morphology and cell count with mean percentages were evaluated in the plain culture dishes and in the immunostainings of S-100 and GFAP in the three-dimensional culture. The Ara-C treated cultures showed a higher Schwann cell percentage (31.0%±8.09% in P4 group to 65.5%±24.08% in P2 group), compared with that obtained in the abscence of Ara-C (17.6%±6.03%) in the plain culture after 2 weeks. And in the three-dimensional culture, S-100 positive cells increased to 56.22%±0.67% and GFAP positive cells to 66.46%±1.83% in G2 group (p<0.05), higher yield than other groups with Ara-C application. Therefore, we concluded that the Ara-C treatment is effective for the proliferation of Schwann cells contrast to the fibroblasts in vitro culture, and the first application after 24 hours from cell harvesting and subsequent 2 pulse treatment (P2 group in plain culture and G2 group in three-dimensional culture) was more effective than other application protocols.

        정신건강간호사 수련과정의 경험

        본 연구의 목적은 정신건강간호사 수련과정의 경험의 의미와 본질을 파악하여 향후 정신건강간호사 수련과정의 질적 개선을 위한 기초자료를 제공하고자 수행되었다. 연구 참여자는 12명이며, 정신건강간호사 수련과정을 이수한 자이다. 자료 수집을 위해 포커스 그룹 면담을 실시하였으며, Colaizzi의 현상학적 방법에 따라 자료를 분석하였다. 자료 수집은 2017년 3월 9일부터 4월 13일까지였다. 정신건강강간호사 수련과정의 경험에 대한 분석 결과 ‘수련기관의 지원 부족’, ‘현장특성에 맞는 운영 및 정책 개선 필요’, ‘수련과정 평가에 대한 개선 필요’, ‘수련목적 및 현실을 반영한 교육필요’, ‘업무수행 역량 강화를 위한 교육 요구’, ‘다양한 수련생 상황으로 인한 실습지도의 어려움’, ‘실습교육의 중요성 인식’, ‘교육과정의 질 관리 필요’ 등의 8개 주제묶음과 18개의 주제로 범주화되었다. 이러한 결과는 정신건강간호사 수련과정의 질적인 향상 및 수련과정 표준화를 위한 기초 자료가 될 것이다. The purpose of this study was to understand the meaning and nature of the Psychiatric Mental Health Nurse Practitioner(PMHNP) training courses and to develop of quality in PMHNP’s training courses. A phenomenological methodology was used for the study. Participants were twelve PMHNP’s who were training courses in Korea. Focus Group Interview(FGI) were done for data collection and the data were analyzed using Colaizzi s method. Data were collected from March 9, 2017 to April 13, 2017. The results of the analysis on the experience of PMHNP’s were 8 categories and 18 sub-categories: Lack of support from training institutes , Need to improve the operation of the training course and improve the policy , Need to improve the evaluation of training courses , Need to education of purpose and reality , ‘Educational need for strengthening competency of PMHNP’, Difficulty in conducting practical training due to various trainees , Recognizing the importance of practical training of PMHNP‘s , Need to manage quality of PMHNP‘s training curriculum. Based on these results, it will be a basic data for qualitative improvement of PMHNP’s training courses and standardization of training.

      • 포항 가속기연구소 에너지 3GeV 성능 향상을 위한 펄스 모듈레이터 설계

        선형가속기의 에너지를 3.0 GeV로 증강시키기 위하여 마이크로웨이브 증폭기의 펄스 전원를 설계하였다. 현재 클라이스트론-모듈레이터를 12set에서 2set를 추가로 증설할 예정이며 기존의 마이크로웨이브 모듈은 1개의 모듈이 4개의 가속관에 마이크로웨이브를 공급하고 있다. 현재 운전중인 12개의 가속 모듈중에 1개의 모듈을 각각 2개의 가속관으로 나누어서 2개에 모듈로 마이크로웨이브를 공급하고 나머지 9개의 모듈은 4개의 가속관에 마이크로웨이브를 공급할 예정이다. 선형가속기의 에너지 증강사업에서 200 ㎿의 펄스 모듈레이터와 80 ㎿의 클라이스트론 2set 새로운 방식으로 설치할 계획이며 기존의 모듈레이터 또한 성능향상을 할 예정이다. 본 논문에서는 마이크로웨이브 증강 방안 및 새로운 방식의 모듈레이터에 대한 설계에 대하여 발표하고자 한다.

        고출력 펄스 전원으로 PFN Marx 펄스 전원공급장치를 설계 및 제작하였다. PFN Marx는 커패시터와 인덕터 그리고 스파크 갭과 전원을 공급하는 전원공급장치와 전류 인덕터로 구성하여 제작을 하였다. 여기에 사용하는 스파크 갭 스위치는 개스를 채우는 방식으로 개스의 압력을 조정하여 스위치의 스위칭 전압을 조정하여 준다. PFN Marx는 커패시터와 인덕터가 직병렬로 구성이 되며 펄스폭과 PFN의 임피던스를 결정하는 중요한 요소이다. PFN marx는 33단으로 구성을 하였으며 부하를 100Ω으로 설계 및 시뮬레이션을 통하여 제작하였다. PFN Marx 펄스 전원공급장치를 약 500 ㎸, 수백 ㎱까지 시험을 하였다. 본 논문에서는 PFN Marx 펄스전원장치의 설계 제작 및 시험에 대하여 고찰하고자 한다.

PFN Marx는 커패시터와 인덕터가 직병렬로 구성이 되며 펄스폭과 PFN의 임피던스를 결정하는 중요한 요소이다. PFN Marx 펄스 전원공급장치를 시뮬레이션 및 제작하여 시험을 할 예정이며 요구되는 사양은 전압 수십 ㎸, 펄스 폭 수백 ㎱이다. 본 논문에서는 PFN Marx 펄스전원장치의 설계 및 시험에 대하여 고찰하고자 한다.
