        척추마취 수술환자의 간호요구

        The Purpose of this study was to identify the needs which were perceived by patients who were received spinal anesthesia for surgery. The subjects consisted of 50 adult Patients who were admitted to 2 university hospitals and 2 general hospitals in Pusan city and 1 general hospital in Koje City for surgery under spinal anesthesia. Thirty eight percent of subjects received information about anesthesia before the operation. The instrument for this study was developed by the researcher based on literature and a Pretest. Data were collected from December 10, 1999 to February 10. 2000 and were analyzed by content analysis. The results were that there were 533 meaningful statements in the needs of spinal anesthesia patients. The needs of spinal anesthesia Patients had 51 items (preoperation (6), induction of anesthesia(5), intraoperation (27), postoperation(13)) and 6 categories (information. emotional welfare, physical welfare, post anesthetic management, control of physical environment, humane treatment). From the results, it can be concluded that: 1.In the pre-operation period. we have to explain anesthesia procedures. Adequate position of anesthesia. duration before anesthesia wears off and sensation of paralysis. We have to supply emotional support to relieve anxiety because of anesthesia. 2.In induction of anesthesia we have to support patient's position for anesthesia, and relieve anxiety so that patients participate in induction of anesthesia well. 3.In intra-operative period, we have to check the level of anesthesia, and keep up a comfortable position for operation and care for physical discomfort such as thirst. nausea, vomiting, dyspnea and to maintain body temperature of the Patient. Since the patient is conscious, we have to communicate with the patient to relieve anxiety, maintain Privacy, inform the patient of the Process of the operation and encourage the surgeon to explain the outcome of the operation. The operating team needs the careful about what they say and to place the instrument well. We have to ventilate the room air and reduce noise. 4.In the post-operative period, we have to explain the Purpose and duration of bed rest, complications of anesthesia and care for physical discomfort such as pain, dysuria, headache, backache. Also we have to maintain body temperature of the patient and maintain privacy.

        간호대학생의 프로그램학습성과에 대한 교육요구 분석

        본 연구는 간호대학생의 프로그램학습성과에 대한 중요도와 현재수준 인식정도를 조사하여 간호교육요구와 우선순위를 분석한 서술적 조사연구이다. K시에 소재한 일 대학 4년제 간호학과 재학생을 대상으로 2017년 10월 30일부터 12월 22일까지 설문조사를 실시하여 최종 256명의 자료를 분석하였다. 수집된 자료는 SPSS Win 20.0 프로그램으로 t-test, ANOVA, Borich 요구도, The Locus for Focus모델을 활용하여 분석하였다. 본 연구 결과 프로그램학습성과 항목 중 중요도는 핵심기본간호술 적용, 통합적 간호술 적용, 간호실무의 법적, 윤리적 기준 이해와 적용 순이었으며, 현재수준은 치료적 의사소통적용, 핵심기본간호술 적용, 간호전문직 표준 이해 순으로 나타났다. Borich요구도와 The Locus for Focus모델의 중복성 확인을 통한 최우선 순위 항목은 통합적 간호술 적용, 핵심기본간호술 적용, 보건의료팀간 협력관계, 간호과정 적용, 간호실무의 법적, 윤리적 기준 이해와 적용이며, 차 순위는 치료적 의사소통, 간호전문직 표준 이해로 나타났다. 본 연구는 프로그햄 학습성과에 대한 교육요구와 최우선순위 및 차순위를 파악하여 교육과정 개선에 필요한 기초자료와 방법론적 접근을 제공하였다는데 의의가 있다. This study was conducted to identify and analyze the educational needs for program outcomes in nursing students. Participants included 256 nursing students at K college, and a survey was conducted from October 30 to December 22, 2017. Data was analyzed with SPSS WIN 20.0 using descriptive statistics, t-test, ANOVA, Borich`s Needs Analysis, and the Locus for Focus Model. Results found that subjects recognized that the present level was high about `Apply therapeutic communication skills,` `Provide fundamental nursing skills` and `Understanding nursing profession standards`.`Provide fundamental nursing skills`, `Apply a nursing intervention`, "Understanding legal and ethical standards of nursing practice and implementation` were the most important sub-elements of program outcomes. In according with the Borich`s needs value and the Locus for Focus Model, `Apply a nursing intervention`, `Explain the importance of the role and cooperation within the healthcare team`, `Provide fundamental nursing skills`, `Execute the nursing process`, `Understanding the legal and ethical standards of nursing practice and implement` were identified as the top priorities of educational needs. Results of this study are expected to be used as basic data to develop outcome-based curriculum and nursing education programs.

        종합병원 간호사의 통증 관리에 대한 지식 및 태도, 수행, 장애 요인

        본 연구는 종합병원 간호사를 대상으로 통증관리에 대한 지식 및 태도, 수행, 장애 정도와 의료인, 환자, 제도 측면의 장애요인을 파악하여 통증관리에 대한 기초자료를 제공하고자 수행하였다. 연구 대상자는 K시 3개 종합병원 간호사 130명이었으며, 자료수집은 2021년 6월 21일부터 7월 5일까지 이루어졌다. 수집된 자료는SPSS 22.0을 사용하여 t-test, one-way ANOVA, Pearson correlation coefficient로 분석하였다. 간호사들의 통증관리에 대한 지식 및 태도는 41점 만점에 21.05±3.90, 정답률 51.3%이었으며, 수행은 3.21±0.63, 장애는 2.06±0.61이었다. 통증관리 지식 및 태도는 임상경력(F=3.33, p =.013)과 근무부서(F=2.96, p =.022)와 유의한 차이가 있었고 통증관리 장애는 학력(t=2.01, p =.047), 근무부서(F=4.41, p =.002)와 유의한 차이가 있었다. 통증관리 장애요인은 환자와의 불충분한 의사소통, 통증관리에 대한 지식부족, 인력부족으로 나타났다. 통증관리 지식 및 태도와 수행 간에 유의한 관계는 없었다. 연구결과를 통해 효과적인 통증관리를 위해서는 통증 기전, 약물요법에 대한 교육과 환자와의 충분한 의사소통, 적절한 인력 확보와 통증관리 수행을 위한 프로그램 개발 및 지속적인 질 개선이 이루어질 수 있는 다양한 접근 방식이 필요한 것으로 사료된다. This study was conducted to provide basic data on pain management by identifying the knowledge, attitude, performance and barrier level of pain management among general hospital nurses and characterizing barrier factors in terms of medical staff, patients and health care system. The study was conducted from June 21 to July 5, 2021. A total of 130 nurses from 3 general hospitals in K city participated in this study. Data were analyzed using descriptive statistics, t-test, one-way analysis of variance (ANOVA) and Pearson correlation coefficient with SPSS 22.0. The nurses" knowledge and attitude on pain management were 21.05±3.90 out of 41, and the correct answer rate was 51.3%. The performance was 3.21±0.63, and barrier level was 2.06±0.61 out of 4 points. Pain management knowledge and attitude were significantly different from work career (F=3.33, p =.013) and clinical unit (F=2.96, p =.022). Barriers to pain management significantly differed in educational level (t=2.01, p =.047) and clinical unit (F=4.41, p =.002). The most common barrier factors for pain management were insufficient communication with patients, lack of knowledge about pain management and lack of staff. There was no significant relationship between pain management knowledge and attitude and performance. According to our results, for effective pain management, knowledge of pain mechanisms and drug therapy, sufficient communication with patients and securing appropriate personnel are required. Various approaches are also needed to develop a program for pain management and achieve continuous quality improvement.

        동아리활동 비만관리 프로그램이 비만아동의 신체조성, 자기효능, 건강증진행위에 미치는 효과

        Purpose: The purpose of this study was to develop an obesity management program for obese children and examine its effects on body composition, self-efficacy, and health promotion. The obesity management program was based on a KAP & HBM mixed model and consisted of health education and group playing exercises. Method: This study was implemented at one elementary school in K city for a total of 12 weeks. The study subjects were a single group, and their body composition, self-efficacy, and health promotion were assessed at both pre-treatment and post-treatment. Data were analyzed by Wilcoxon Signed Rank Test using SPSS WIN 20.0. Result: After the program, the average height of subjects increased by 0.9cm (p=.001), whereas BMI decreased by 0.64kg/m² (p=.003), obesity index decreased by 3.79% (p=.003), and body fat decreased by 1.34% (p=.002). There were significant effects on diet self-efficacy (p=.027), exercise self-efficacy (p<.001), and health promotion (p=.001). Conclusion: This obesity management program reduced the degree of obesity and improved self-efficacy and health promotion. Therefore, the obesity management program is an effective intervention method for elementary-aged obese children.

        간호대학생의 인성에 영향을 미치는 요인

        본 연구는 생태체계이론을 적용하여 간호대학생들의 인성에 영향을 미치는 요인을 확인하기 위한 서술적 조사연구이다. 본 연구는 전국 간호대학생을 모집단으로 영남지역 4개 간호대학생 전체를 성별 및 학년을 기준으로 할당 표본추출한 296명을 대상으로 하였다. 수집된 데이터 분석을 위해 SPSS WIN 21.0 프로그램을 이용하여 t- 검정, ANOVA 및 다중 회귀 분석을 실시하였다. 연구결과 대상자의 인성은 평균 5점 만점에 평균 3.84점이었으며, 대학진학 동기, 교과외 활동, 수업참여도에 따라 인성에 유의한 차이가 있었다. 생태체계이론에 따른 간호대학생의 인성에 영향을 미치는 요인은 개체 변인에 대한 첫 번째 모델에서 긍정정서, 의사소통능력이 간호대학생의 인성을 설명하는 중요한 요인으로 나타났으며, 미시체계 변인을 추가한 두 번째 모델에서는 가족건강성과 전공만족도가 중요한 요인으로 나타났다. 간호대학생의 인성에 영향을 미치는 유의한 요인은 의사소통능력(β=.431, p<.001), 전공만족도(β=.310, p<.001), 가족건강성(β= .176, p<.001)으로 나타났으며 이러한 요인들은 간호대학생의 인성을 55.6 % 설명하였다. 따라서 간호대학생의 인성을 향상시키기 위해서는 의사소통능력과 전공만족도, 가족건강성을 고려한 교육 프로그램을 개발하는 것이 필요하며, 다양한 예측변수를 파악하여 변수들 간의 관계를 규명하는 반복 연구를 제언한다. The purpose of this study was to identify factors associated with character of nursing students using ecological theory. A descriptive cross-sectional study was conducted with 296 nursing students. Collected data from self report questionnaires were analyzed using descriptive statistics, t-test, ANOVA and multiple regression with SPSS WIN 21.0. The Results of this study were as follows. A total of 2 models were examined according to individual, microsystem in ecological system theory. In the first model including individual factors, positive emotion, communication ability were significant factors explain character of nursing students. In the second model adding micro system factors family strength and major satisfaction found to be significant factors. The prediction factors of nursing student’ character were communication ability (β=.431, p<.001), major satisfaction (β=.310, p<.001) and family strength (β =.176, p<.001). The explanation power was 55.6%. These results showed that factors affecting character of nursing students are communication ability, major satisfaction, and family strength. Therefore, we suggest to develop various character education programs considering these factors.

        Course Embedded Assessment 기반 간호교육 프로그램학습성과 평가체제 개발

        Purpose: The purpose of this study was to develop a program outcomes assessment system based on Course Embedded Assessment for nursing education. Methods: This study was conducted in accordance with the procedures of the developmental research method. Results: The major results are as follows. 1) The program outcomes were measured according to the Analytic Hierarchy Process. 2) The Course Embedded Assessment matrix was made according to program outcomes weight, the curriculum-organizing principle, and achievement levels. 3) The Course Embedded Assessment rubric was developed in logical process, and consisted of a performance criterion, and rating scale. The content validity index of the Course Embedded Assessment rubric was 0.85. 4) An evaluation guideline and 12 documents were developed to facilitate the performance of the assessment system. 5) The average content validity index of the Course Embedded Assessment-based program outcomes assessment system was as high as 0.89. Conclusion: A Course Embedded Assessment-based program outcomes assessment system is more suitable for accreditation of nursing education than previous studies. Because this system evaluates the process of achievement as well as program outcomes, the results can also serve as immediate feedback to improve the educational process. Above all, this system facilitates that students check their achievements and strive to acquire core competencies in nursing.

        간호대학생의 인성교육 요구도 분석

        본 연구는 간호대학생의 인성교육 요구도를 파악하여 간호대학생을 위한 인성교 육의 기초자료를 제공하고자 시도되었다. 연구대상은 전국 간호대학생을 모집단으로 하여 영남지역 4개 대학에서 할당표본 추출한 308명이며, 자료수집은 2018년 11 월22일부터 12월 14일까지 실시되었다. 수집된 자료는 SPSS/WIN22를 사용하여 평균, 표준편차, t-test, one-way ANOVA와 Scheffé test, Borich요구도 및 The Locus for Focus모델을 활용하여 분석하였다. 연구결과 인성교육이 필요한 이유는 배려와 나눔인식 부족이 가장 많았으며, 인성교육 방법은 기존 교양과목 내 인성교육 강화를 가장 선호하였다. 성별, 학년, 임상실습 경험, 직전학기 성적, 인성교육 경험에 따라 인성교육 중요도와 현재 수준에 차이가 있는 것으로 나타났다. 인성항목 모두 중요도와 현재 수준이 통계적으로 유의한 차이가 있어 모든 항목에 대해 인성교육 요구가 있는 것으로 나타났다. 교육요구의 최우선 순위 항목에는 Borich요구도와 The Locus for Focus모델의 HH사분면에 중복으로 포함된 자기존중, 자아통제, 도덕적 자기 지식이 포함되었으며, 차 순위에는 반성적 의사결정, 양심, 의지, 습관 등의 항목이 포함되었다. 본 연구결과를 토대로 간호대학생의 인성교육에 영향을 미치는 요인을 확인하고 효과적인 인성교육과정을 개발 할 것을 제언하였다. The purpose of this study was to analyze the educational needs of nursing students for 13 character contents. The data was collected through questionnaires of 308 cases from November, 22th, to December, 14th, in 4 nursing college in Young Nam province. The data were analyzed with SPSS WIN 22.0 using descriptive statistics, t-test, ANOVA, Borich s Needs Analysis, and the Locus for Focus Model. The major findings of this study were as follows; 1. The most popular reason of character education was the lack of consideration and sharing, and the most preferred method of education was the enhancement of character education within existing curriculum. 2. The importance and current level of character contents were found to be different according to gender, grade, clinical practice experience, previous semester grade, and character education experience. 3. There was an educational needs for all character contents. 4. The top priority educational needs of character contents included self-esteem, self-control, and moral self-knowledge, which were included in the Borich requirement and the HH quadrant of the Locus for Focus model, and the next character contents included reflective decision-making, conscience, will, and habit. Based on the results of this study, it was suggested to identify factors influencing character education of nursing college students and to develop an effective character curriculum.

        간호교육 프로그램학습성과 평가를 위한 Course Embedded Assessment(CEA) 적용방안 기초연구

        본 연구는 간호교육 인증평가의 핵심인 프로그램학습성과 평가에 Course embedded Assessment(CEA)를 적용하기 위한 기초연구로 시행되었다. 선행연구과 미국대학 평가 사무국의 자료를 통해 CEA의 개념, 장?단점, 단계, 적용 시 주의사 항 등을 고찰하였으며, 이를 근거로 간호교육 프로그램학습성과 평가체계에 적합한 CEA모형과 세부지침을 제시하였다. CEA 과정은 프로그램학습성과 설정, 학습성과의 가중치 산정, 교육과정에 연계, 루브릭 개발, 평가, 결과 및 성과달성도 분 석, 교육과정개선을 위한 피드백으로 구성하였으며 선행연구에서 제시된 단점과 주의사항을 보완하여 시행 지침을 작성하였 다. 본 연구를 토대로 간호교육 프로그램학습성과평가 체계를 구축한다면 학생의 성장과 성취에 대한 일관성 있는 평가와 지속적인 교육과정 개선으로 간호교육의 질적 향상에 기여할 것으로 사료된다. The purpose of this study was to suggest application of Course Embedded Assessment (CEA) for program outcome in nursing education through literature & Office of Assessment web site reference review on concept, advantage, disadvantage, process, measure, tool and precaution. The process of CEA is program outcome, calculate weight of program outcome, embed program outcome in curriculum, development a rubric for CEA, assessment, analysis the result and achievement, feedback loop. CEA is vary useful to proof the student personal achievement and program quality improvement.
