      • 대장수술 후에 발생한 급성신부전의 위험인자

        연구 배경 : 급성 신부전은 술 후 발생할 수 있는 합병증 중에서 사망률은 큰 영향을 줄 수 있으므로 이 연구를 통해 위험인자를 알아보려고 한다. 재료 및 방법 : 2004년 1월부터 2006년 12월까지 3년간 영남대학교 의과대학 부속병원에서 대장 수술을 받은 570명의 환자들을 대상으로 하였다. 환자들의 성별과 연령, 미국 마취과학회 신체등급, 동반질환, 수술의 종류, 응급수술의 여부, 수술 시간, 재수술의 여부, 술 중의 투약 상황, 술 후에 자가 통증 조절기에 사용한 약제, 술 중에 저혈압의 유무, 수혈 여부, 술 후 기계적 환기를 시행한 경우를 비교하였다. 결과 : 비교 결과에서 성별과 수술의 종류, 응급 수술의 여부, 개복여부 등에는 유의한 차이를 보이지 않았다. 반면 환자의 나이와 수술 시간, 재수술의 여부, 술중에 이뇨제 등을 사용한 경우, 술 중에 저혈압의 유무, 술 후에 기계적 환기를 시행한 경우 등에는 유의한 차이를 보였다. 결론 : 이상의 결과로 급성신부전의 원인은 어느 한 가지라고 하기 보다는 위험인자에 얼마나 노출되어 있는가가 결정하는 것 같고, 그 원인들이 모여서 상승효과를 내는 것 같다. 그러므로 수술 전부터 많은 위험에 노출된 환자는 술 중 더 적극적인 감시를 시행하여 수술 후의 합병증 발생을 줄이는 노력을 기울여야 하겠다. Background : Acute renal failure is one of the leading causes of postoperative morbidity and mortality. The purpose of this study was to determine the risk factors that are associated with acute renal failure after colorectal surgery. Materials and Methods : Five hundred seventy patients who operated colorectal surgery at the Yeungnam University Medical Center over three years from 2004 to 2006 were enrolled in this study. The effects of gender, age, ASA classification, concomitant disease, surgery type and duration, reoperation, urogenital manipulation, medication, hypotension, hypovolemia, transfusion, and postoperative ventilatory care on the occurrence of acute renal failure after colorectal surgery were studied. Results : The major risk factors of acute renal failure after colorectal surgery were age of patients (P=0.003), ASA classification (P<0.001), concomitant disease (P<0.001), duration of the time surgery (P=0.034), reoperation (P=0.001), use of intraoperative diuretics (P=0.005), use of postoperative diuretics (P<0.001), intraoperative hypotension (P=0.018), intraoperative transfusion (P<0.001), postoperative transfusion (P<0.001), and postoperative ventilatory care (P=0.001). Conclusion : Multiple factors cause synergistic effects on the development of acute renal failure after colorectal surgery. Therefore, efforts to reduce the risk factors associated with acute renal failure are needed. In addition, intensive postoperative care should be provided to all patients.

        A 11-year-old Yorkshire terrier anesthetized for computed tomography. The dog showed cyanosis and syncope during postanesthetic recovery, therefore drug administrated parenterally. Goose-honking was noticed after full recovery. As a result of survey radiography and fluoroscopy, grade 4 tracheal collapse was diagnosed. Prescription for the dog was oral administration of hydrocodone 0.22 mg/kg. theophylline 20 mg/kg, prednisone 0.5 mg/kg, amoxicillin 22 mg/kg and chlorpheniramine 2 mg/kg twice a day for 3 days. A 10-year-old Pomeranian was diagnosed as grade 2 traceal collapse at the local animal hospital 2 years before being referred to the veterinary teaching hospital of Chungnam National University. On physical examination, goose-honking, coughing and intermittent open-mouth breathing. Tracheal narrowing was observed on survey einradiography and fluoroscopy. Oral administration of theophylline 13 mg/kg, enrofloxacin 5 mg/kg, chlropheniramine 0.5 mg/kg twice a day was prescribed for 2 weeks.

        The thermal decomposition process of Mo(CO)_6 on chemical vapor deposition was investigated by analyzing the effects of deposition temperature, pressure, and reaction gas on the phase change of deposited films. Mo_2C was deposited at or below 350℃ due to the incompleteness of thermal decomposition of Mo(CO)_6, but Mo films was successfully deposited at 400℃ or higher temperatures. The variation of deposition pressure did not change the dependence of decomposition process on deposition temperature. The activation energy of surface reaction was 5.8 ㎉/mole.

      • Robotic single-site surgery versus laparo-endoscopic single-site surgery in ovarian cystectomy: A retrospective analysis in single institution

        Objective: Robotic single-site (RSS) surgery is being utilized in gynecologic surgery. However, its cost-effectiveness and limitations of the system make the application of the technique to benign surgery controversial. The aims of this study were to compare surgical outcomes of LESS and RSS surgeries for adnexal tumors and to investigate if RSS surgery is feasible for a patient with a benign adnexal mass. Methods: Patients who underwent ovarian cystectomy from January 2013 to December 2018 at Korea University Anam Hostital were divided into two groups according to the surgery: RSS or LESS. After exclusion of ineligible patients, 29 patients who underwent RSS surgery and 80 patients who underwent LESS surgery were included in this study. Surgical outcome data regarding bleeding, surgical time, length of stay, and the learning curves of surgical time were obtained using the cumulative sum method. Results: Patient demographics showed no differences between the groups except for tumor histology. There were no differences in bleeding, postoperative length of stay, surgical wound complication, or surgical preparation time between the groups. RSS surgery had a longer surgical time than LESS surgery (130.41 vs. 96.96 minutes; P<0.001). In a learning curve analysis, seven cases were required to be proficient in docking the RSS system. Comparing the learning curves of surgical time, RSS surgery required fewer cases to attain proficiency than LESS surgery (19 vs. 22 cases). Conclusion: In an analysis of the learning curve, the docking in RSS surgery did not require many cases to attain sufficient skills. In conclusion, RSS ovarian cystectomy is a feasible and safe option with the benefit of a quick learning curve. The longer surgical time could be overcome with further improvement of surgical techniques and development of the robotic instruments.

        Development and Validation of Liquid Chromatography-Tandem Mass Spectrometry Method for Quantification of Plasma Metanephrines for Differential Diagnosis of Adrenal Incidentaloma

        <P>High-resolution imaging techniques have increased the detection rate of adrenal incidentaloma. We developed a method of liquid chromatography-tandem mass spectrometry (LC-MS/MS) for detection of plasma free metanephrine (MN) and normetanephrine (NMN) and evaluated its analytical performance and clinical efficacy in differential diagnosis of adrenal incidentaloma. After solid-phase extraction, chromatographic isolation of the analytes and internal standard was achieved by column elution in the LC-MS/MS system. The analytes were detected in multiple-reaction monitoring mode by using positive electrospray ionization: MN, transition m/z 180.1→165.1; NMN, m/z 166.1→134.1. This method was validated for linearity, precision, accuracy, lower limits of quantification and detection, extraction recovery, and the matrix effect. Plasma concentrations of MN and NMN of 14 patients with pheochromocytoma were compared with those of 17 healthy volunteers, 10 patients with essential hypertension, and 60 patients with adrenal adenoma. The assay's linear range was 0.04-50.0 and 0.08-100.0 nmol/L for MN and NMN, respectively. Assay imprecision was 1.86-7.50%. The accuracy ranged from -7.50% to 2.00%, and the mean recovery of MN and NMN was within the range 71.5-95.2%. Our LC-MS/MS method is rapid, accurate, and reliable and useful for differential diagnosis of adrenal incidentaloma.</P>

        Late and Insufficient Phosphorus Sup-plementation is Associated with Early Severe Hypophosphatemia in Extremely Low Birth Weight Infants with Early Amino Acid Administration

        Objective: To investigate the incidence of early severe hypophosphatemia, and examine the associated clinical factors and outcomes, in extremely low birth weight infants (ELBWI) who received early amino acid administration. Methods: Medical records of 82 ELBWI were retrospectively reviewed. Severe hypophosphatemia was defined as a serum phosphate level <2 mg/dL during the first week after birth. Results: Nineteen ELBWI (23.2%) experienced severe hypophosphatemia. The supplementation of phosphorus was started significantly later in the hypophosphatemia group compared to that in the control group (P=0.036). Small for gestational age infants (SGAI) (P=0.006) and bronchopulmonary dysplasia (P=0.016) were more prevalent in the hypophosphatemia group compared to that in the control group. Conclusion: Early severe hypophosphatemia is common in ELBWI. Late and insufficient supplementation of phosphorus and SGAI were associated with severe hypophosphatemia.

        In vitro anti-skin-aging effects of dried pomegranate concentrated powder

        Purpose : In this study, we intended to observe the anti-wrinkle and moisturizing effects of dried pomegranate juice concentration powder (PCP) using in vitro test. Materials and methods : Antioxidant effects of PCP were determined by free radical scavenging capacity (DPPH assay) and the cytotoxicity of PCP was examined in human keratinocyte (HaCaT) and human primary dermal fibroblast-neonatal (HDF) cells. To investigate the moisturizing effect of PCP, hyaluronan synthesis was examined in HaCaT cells. Activity of procollagen production were assessed in HDF cells and elastase inhibition properties of PCP were evaluated in cell free condition, to determine their anti-wrinkle effects. Metalloproteinase 1 (MMP-1) activity was also assessed following UVB irradiation, in the current in vitro experiment. Results : No PCP treatment related significant cytotoxic effects were demonstrated against to the both HDF and HaCaT cells. PCP showed favorable free radical scavenging activities in dose-dependent manner. In PCP-treated HaCaT cells, hyaluronan synthesis was non-significantly but markedly increased, and pro-collagen productions were significantly increased in HDF cells, at all three different concentrations (0.25, 0.75 and 1 mg/ml), and elastase inhibitory activities were observed by PCP treatment. A significant decrease in UVB-induced MMP-1 activity was also observed in 1 mg/ml PCP-treated HDF cells as compared to those of UVB-exposed cells. Conclusions : Taken together, these results suggest that PCP has favorable antioxidant, anti-wrinkle and moisturizing effects without meaningful cytotoxicity on HDF and HaCaT cell lines.

      • Tunable Sub-nanopores of Graphene Flake Interlayers with Conductive Molecular Linkers for Supercapacitors

        <P>Although there are numerous reports of high performance supercapacitors with porous graphene, there are few reports to control the interlayer gap between graphene sheets with conductive molecular linkers (or molecular pillars) through a pi-conjugated chemical carbon-carbon bond that can maintain high conductivity, which can explain the enhanced capacitive effect of supercapacitor mechanism about accessibility of electrolyte ions. For this, we designed molecularly gap-controlled reduced graphene oxides (rGOs) via diazotization of three different phenyl, biphenyl, and para-terphenyl bis-diazonium salts (BD1-3). The graphene interlayer sub-nanopores of rGO-BD1-3 are 0.49, 0.7, and 0.96 nm, respectively. Surprisingly, the rGO-BD2 0.7 nm gap shows the highest capacitance in 1 M TEABF(4) having 0.68 nm size of cation and 6 M KOH having 0.6 nm size of hydrated cation. The maximum energy density and power density of the rGO-BD2 were 129.67 W h kg(-1) and 30.3 kW kg(-1), respectively, demonstrating clearly that the optimized sub-nanopore of the rGO-BDs corresponding to the electrolyte ion size resulted in the best capacitive performance.</P>
