        일렬 자가조혈모세포이식을 받은 다발성 골수종 환자에서 발생한 뇌 톡소포자충증 1례

        Toxoplasmosis is a rare but fatal complication in hematopoietic stem cell transplant recipients, usually associated with allogeneic hematopoietic stem cell transplantation (HSCT). We report a case of cerebral toxoplasmosis in a patient with multiple myeloma, following tandem autologous stem cell transplantation. A 55-year-old Korean male presented with weakness in both legs that had progressed to both arms. A magnetic resonance imaging scan of the brain revealed multiple, variablesized ring-enhancing lesions with surrounding edema in the cerebral hemispheres and brain stem. Stereotactic biopsy revealed bradyzoites of Toxoplasma gondii in the brain tissue. The patient received trimethoprim-sulfamethoxazole, followed by pyrimethamine and sulfadiazine, accompanying treatment for progressive multiple myeloma. Cerebral toxoplasmosis should be considered as one of the differential diagnoses in patients with neurologic signs following autologous HSCT.

        치아 수복용 불소함유 glass filler의 제조 및 특성

        The purpose of this study was the reaching test of restorative glass according to composition. Mixtures of SrF₂, CaF₂and SiO₂were fused in platinum crucible. The glasses were ground and fraction which passed through a 400 mesh (38㎛) sieve were collected. The powder specimens were analyzed by a DTA and XRD. Then powder specimen were inserted to D. I. water with various of dissolution time and composition at 37℃. Fluoride ion, cation release, solubility, and pH change were estimated. It was clear that the glasses were formed except S0.65. In S0.40, Tg, Ts, and Tc were 630, 672, and 813℃ respectively. In initial 0.5 hrs., amounts of released fluoride ion and cation were about 90% of total released amounts and released rate was decreased with dissolution time. Amount of released fluoride ion and cation were increased with decreasing SiO₂ in glass filler. And released rate was decreased rapidly by adding CaF₂. This was caused by structural variation in glass filler. Solubility was similar tendency with ion released. The pH of D.I. water was 4.5. But in intial 0.5 hrs., pH was 8.2∼9.8., and decreased according to dissolution time. This is caused by ionic changing with cation and H* ion.

        반복적인 부비동염으로 내원한 Good 증후군 1예

        Good 증후군은 흉선종과 면역결핍이 동반되는 질환으로, B세포의 감소로 인한 범저감마글로불린혈증과 CD4^(+)T세포의 감소로 인한 세포성 면역저하가 동시에 나타나는 것을 특징으로 한다. 본 증례는 18개월 전부터 발생한 반복적인 부비동염을 주소로 내원한 43세 남자 환자로, 내원 1년 전 흉선종절제수술을 시행받았으며, 면역학적 검사상 범저감마글로불린혈증과 T세포의 감소 등의 이상 소견이 발견되었다. 정주 면역글로불린과 경험적 항생제(amoxicillin/clavulanic acid) 사용 후 부비동염이 호전되었으며, 예방접종과 정기적인 면역글로불린 투여 후 특이 합병증없이 경과 관찰 중이다. Good's syndrome is the association of thymoma with immunodeficiency, characterized by hypogammaglobulinemia, B-cell lymphopenia and variably defects in cellular immunity with CD4^(+) T-cell lymphopenia and an inverted CD4^(+):CD8^(+) T-cell ratio. We report a 43-year-old male patient who presented with a 18-month history of productive cough and postnasal drip. One year ago, he underwent the operation for resection of a thymoma. Despite of appropriate management, sinusitis relapsed multiple times. He was found to have hypogammaglobulinemia with nearly absent B cells(4/μL). The CD4^(+) T-cell count was 554/μL with an inverted CD4^(+):CD8^(+) T-cell ratio of 0.6. His symptoms and signs improved with antibiotic treatment and monthly administration of intravenous immunoglobulin (IVIG, 400 mg/kg).

        The vacuum interrupter operated by permanent magnet actuator(PMA) gives outlook on improved characteristic, higher reliability and cost price reduction as well as the feature of simple structure and few component. For the anlysis and design of permanent magnetic actuator, not only electric, magnetic and mechanical phenomena but also material properties should be taken into account. Aim of the present paper is modeling of permanent magnet actuator(PMA) used in medium voltage distribution systems and remodeling by permanent magnetic. Coupled finite element method is used to analysis the modeling and we compared with previous PMA model and improved PMA model.

        폐좌상시 폐단락률과 PEEP치료의 효과

        To evaluate the physiologic extent of pulmonary contusion and effect of PEEP therapy for pulmonary contusion, we studied 16 patients received PEEP therapy with pulmonary contusion from nonpenetrating chest trauma. Hemodynamic parameters including pulmonary vascular resistance index and intrapulmonary shunt fraction were calculated from standard measurement, and arterial oxygen tension was measured. Pulmonary vascular resistance index and intrapulmonary shunt fraction were increased in patient group. Arterial oxygen tension was decreased with increase of the intrapulmonary shunt fraction(R=0.75). Arter minimal PEEP therap(5-10 ㎝H₂O), pulmonary vascular resistance index was remained unchanged and intrapulmonary shunt fraction was decreased without significant changes of pulmonary hemodynamics. Increment of arterial tension was increased with decrease of intrapulmonary shunt fraction(R=0.43). Decreased stroke volume index suggested of cardiac injury such as cardiac contusion in patient group. These results of our study suggested that increased intrapulmonary shunt fraction caused arterial hypoxia in pulmonary contusion and arterial oxygen tension was increased as a result of reduced intrapulmonary shunt by PEEP therapy.

        This paper presents a new magneto-thermal finite element analysis for predicting the temperature rise of the EHV GIS bus bar. The power losses of a bus bar calculated by the magnetic field analysis are used as the input data to predict the temperature rise for the thermal analysis. The heat-transfer coefficients on the boundaries are analytically calculated by applying the Nusselt number considering material constant and model geometry for the natural convection. The temperature distribution in a bus bar by coupled magneto-thermal finite element analysis shows good agreement with the experimental data.

      • 생분해성 다공질 Calcium Phosphate 세라믹스의 제조

        When porous calcium phosphate ceramics are used as a bone graft substitute, it is known that new tissue or blood vessel is grown into the porous implant due to their excellent biocompatibility. In this study, the most chemically stable β-crystalline form of calcium metaphosphate, Ca(PO₃)₂is prepared by the controlled thermolysis of monocalcium phosphate, Ca(H₂PO₄)₂. The diameter of cylindrical pores formed during cooling was controlled by a holding time at the melting point of a monocalcium phosphate and by the change of a recrystallization temperature, to obtain the most appropriate size(about 200㎛) of pores. It was observed that the increasing holding time at the melting point of monocalcium phosphate results in the decreases of pore sizes.

      • 트랜잭션 처리를 기반으로 하는 ARS 자동 생성기

        이 논문에서는 서비스의 흐름의 표현을 보다 쉽게 하기 위해 ARS 서비스 내용을 사용자와 ARS 서비스간의 동작 단위인 트랜잭션으로 나누고, 모든 트랜잭션을 표현하기 위해 트랜잭션을 분석하여 일반화한 처리노드를 제시하였다. 이 처리노드를 통한 서비스의 기술 및 실행을 할 수 있는 개발 환경인 ARS 자동생성기 모델을 제시하고 구현하였다. ARS 자동 생성기는 서비스의 기술을 GUI(Graphic User Interface)를 통해 할 수 있는 그래픽 에디터와 서비스 기술 정보를 이용하여 실제 동작을 수행하며 정보처리 및 데이터관리를 위한 응용프로그램의 인터페이스를 지원하는 ARS 엔진으로 구분된다. 이 논문에서 제시한 처리노드를 이용함으로 서비스의 흐름을 보다 쉽게 기술할 수 있다. ARS services could be presented by a series of transactions which are the units representing the interaction between ARS and user. In this paper, we propose a model for a processing node which represents the operation of a generalized transaction. We also implement an ARS generation tool that performs an ARS service with the information of processing nodes. The ARS generation tool consists of three components: the Operation Database, the Engine, and the Application Interface. The information for the processing nodes is stored in the Operation Database. ARS Engine is responsible for the middleware operations. It proceeds the processing nodes. The concept of processing node makes it convenient to describe the operation of an application ARS.

      • 계층적 유한 상태 기계에 의한 ARS 구현 모델

        ARS(Audio Response System)를 구현하기 위하여, ARS를 지원하는 하드웨어의 기본 지식을 알아야 하고, 하드웨어 생산 업체에서 제공하는 원시 라이브러리와 C언어를 사용하여 어렵게 프로그램을 해야했다. 따라서 개발의 용이성을 위하여 ARS 소프트웨어의 개발을 단순화할 필요가 있다. 본 논문에서는 ARS의 처리 과정을 분석하여 외부동작과 내부동작으로 분리하여 두 단계의 유한 상태 기계로 표현하는 ARS 구현 모델을 제시하였다. 외부동작은 응용분야에 의존하는 부분으로 대기상태, 처리상태, 종료상태로 표현하였다. 내부동작은 응용분야와 무관한 부분으로, 하드웨어 제어와 외부동작을 지원한다. 내부동작을 처리하는 부분을 외부동작과 분리하여 캡슐화하고, 외부동작에 필요한 정보를 파일의 형태로 제공함으로써 ARS 시스템을 구현할 수 있다. 본 논문에서 제시하는 ARS 구현 모델에 의하여 ARS 구현이 단순해진다. The ARS program has been written in C language using the supported by the hardware vendor. Since this process is not so simple, we need to develop an easy method that could make the ARS implementation simple. In this paper, we propose an ARS implementation model which consists of two-level FSM (Finite State Machine). From the analysis of common ARS operations, we represent it as external and internal operations. The external operation is the dependent part of ARS to the application area. On the other hands, the internal operation in independent from the application area. The internal operation corresponds to the ARS engine that includes the hardware control and supports the external operation. The function of external operation is decided by a sequence of description that contains the voice filename, input type, variable name, and the next-state names according to the input values. In the proposed system, a new application of ARS can be generated by simply writing a new description file for the external operation.
