        난치성 전두엽 간질의 외과적 치료

        Recent advances of neuroimaging and electrophysiological monitoring technique have contributed in improving the surgical outcome of frontal lobe epilepsy. The authors have analysed 36 consecutive cases of intractable frontal lobe epilepsy that were treated at our institute between September. 1992 and December, 1995 to determine the clinical, electrophysiological, and neuroimaging characteristics as well as to delineate the efficacy of surgical treatment in improving the seizure outcome. The patients consisted of 19 males and 17 females. The mean age of the seizure onset and the mean duration of epilepsy were 14.9 and 11.6 years, respectively. Magnetic resonance imaging(MRI) showed no lesion in 9 patients and pathological lesions in 27 patients. The types of the lesions observed included primary brain tumor, mostly low-grade glioma in 10 patients, cortical dysplasia in 4, posttraumatic scar in 4, neocortical gliosis in 3, cyst in 3, and postencephalitic atrophy in 3. Intracranial recordings were performed in 23 cases to determine the epileptogenic zone. The surgical outcome was graded according to Engle’s Classification as Class A in 20 patients. Class B in 2, Class C in 9, and Class D in 5. The surgical outcome was promising regardless of the presence or the absence of the lesion on MRI, invasive recording, age of seizure onset, and duration of epilepsy.

        선택적 후근절제술의 수술중 근전도 감시방법과 치료효과

        Selective posterior rhizotomy(SPR) has been known to reduce the spasticity as well as to improve the quality of life in patients with intractable spasticity. Twenty patients underwent SPR under intraoperative electrophysiological monitoring(IOM). Fifty-two percent of sacrolumbar rootlet were cut after electrical stimulation. Eighteen patients(90%) with spastic type of cerebral palsy showed marked reduction of spasticity and functional improvement without any complication. The patients who did not respond to SRP had mixed types of spasticity. IOM facilitate the selection of pathological rootlets regardless of anaesthetic level. intensity of electrical stimulation, and individual variability of responses. It is concluded that careful selection of surgical candidates and of pathological rootlets under IOM are important to reduce the spasticity. thereby to obtain a better outcome.

        This study is to examine the origin of museum and its spatial structure. The purpose is to understand architecture in the modern life as the origin of the modern architecture in the 20th century, and to reveal the authenticity of modern spaces through spaces of museums. Museums are one of the leading architectural themes from the nineteenth century to the present day. Having a vast influence on entire Europe, they are new modern institutional buildings emerging with new modern technologies and culture. This study analysis museums through three viewpoints in the modern institutional buildings which were 'time', 'boundary' and 'perception'. As a result, spatial structure in museums were formed by things, people and space which were related to the three viewpoints. The phenomenon and the process in the spatial structure of museum are related to the modern architecture, and also relations in the space of museum are extended to the city and the architecture in the present day.

      • DFF알고리즘을 이용한 깊이 정보 추출

        영상으로부터 물체의 깊이를 추출하는 방법은 structured light, texture, stereo, focusing 등의 방법이 사용되고 있는데 본 논문에서는 focusing을 이용하여 물체의 깊이정보를 획득하는 알고리즘을 논의하였다. 기존의 stereo 방식은 서로 다른 위치에서 얻어진 두 장의 영상 사이의 관계를 해석하여 3차원 정보를 얻는다. 그러나 이 방법은 두 영상간의 대응 관계를 정하는 알고리즘이 매우 복잡하며, 일치되는 대응점을 구하지 못하면 3차원 거리를 구할 수 없다는 단점이 있다. 본 논문에서는 이와 같은 대응점의 문제가 발생하지 않고 초점으로부터 비교적 정확한 깊이정보를 획득할 수 있는 DFF(Depth From Focus)에 대해 논하고 실험을 통하여 효과적으로 3차원 정보를 추출함을 보였다.

        응급실에서 시행된 기관내 삽관의 현황

        Background : The purpose of this study was to analyze the endotracheal intubation cases performed in the emergency department. Methods : We investigated retrospectively 326 cases of endotracheal intubation performed in the emergency department of a tertiary care center form April 1, 1998 to March 31, 1999. We focused on operators, medications used, its success rate and immediate complications, and the relationship between its success rate and medications. Results : Of 326 consecutive intubations, 193 patients(59.2%) were done by emergency medicine residents or attending physician. While 320 patients(98.2%) were successfully intubated, 6 patients could not be intubated and 2 patients underwent tracheostomy. Of 50 cases of intubations(15.3%) attempted with paralyzing agents, 48 cases were done with succinylcholine and 46 cases underwent by emergency physicians. Intubations with neuromuscular paralysis resulted in high success rates at the first attempt. Of 55 immediate adverse events were encountered in 47 patients(desaturation=17, bronchial intubation=15, hypotension=8, bradycardia=4, cardiac arrest=2, others=5). Conclusion : At this institution, paralyzing agents were used infrequently, but almost all of them were used by emergency physicians.

        Modular memory is an essential component of any digital 1C design library. The major application of high speed register file has been as the main memory of large-scale computers, super computer, microcomputer, workstation, so on. So, in this thesis, The designed high speed register file for RISC processor, which is possible to processing concurrent I/O by seperating read/write bitline and word access TR has become one of the key embedded elements for processor VLSI's, because it allows simultaneous access from multiple resource and the parallel operations yield a high system throughput. Benefits include reduced constraints on the client units and simpler register to register programming models. Stable output characteristics was obtained by testing at peripheral circuits of operation and was designed by using 1.2[μm] CMOS design rule.

        응급의료 헬리콥터를 이용한 환자이송에 대한 고찰

        Background: The first EMS helicopter was introduced to Korea in 1996 and from Dec. 1. 1997, it was used for transporting emergent patients. Authors, here upon, report the transporting experiences. Methods: From Dec. 1, 1996 to Dec. 31, 1997, Samsung Medical Center's EMS helicopter was used for air evacuation of critically ill patients. The patients data prospectively analyzed. Result: A total of 65 patients were transported. Male to female ratio was 1.95:1. The mean transport time was 64.1 min(10 - 160 min). Majority of the evacuated patients was surgical patients (General Surgery : 16, Orthopedic surgery : 10, Neurosurgery : 6, Internal medicine : 13, Pediatrics : 3, and others : 3). Twenty-one of the 65 patients transported were admitted to ICU and 31 did not require ICU care. During the air evacuation, one physician and one nurse trained for air evacuation attended the patients. No medical problems or deaths developed during the air evacuation period. Conclusion: The first EMS helicopter was introduced to Korea in 1996. From Dec. 1, 1996 to Dec. 31, 1997, Samsung Medical Center's EMS helicopter was used for air evacuation of critically ill patients. The patients data reported.

        METRx™ 기구를 이용한 후방 경추부 추궁추간공 제거술의 예비 결과

        Objectives: Posterior cervical microscopic laminoforaminotomy has been a preferred procedure for a posterolateral cervical disc or foraminal stenosis. However, neckpain from wide skin incision and muscle injury are some of disadvantages. The authors performed cervical microscopic laminoforaminotomy with METRx™ tubular dilator system and were compared the results with classical cervical microscopic foraminotomy. Methods: Six patients underwent posterior cervical microscopic laminoforaminotomy using the METRx™ during 6 months. We have selected 10 patients from our institution as a comparison group. Preoperative and postoperative pain score composed of the radicular and neck component was examined and disability score also recorded. Diameters and lengths of laminoforaminotomy on the postoperative CT also were compared with two methods Results: Postoperative pain scores to the radiculopathies were not different but postoperative pain scores to the neckpain on the follow up periods were significantly different. The average vertical and transverse diameters of the laminoforaminotomy defect on the postoperative CT scans were not different between two groups. Average hospital stay of the classical methods was 8.8±7.5 days comparison with 4.5±0.54days of new procedure. Conclusions: Posterior cervical microscopic laminoforaminotomy with METRx™ systems seems to provide similar decompression and clinical results as classical methods compared to classical methods and shows superior results for postoperative neckpain and hospital stays.

        The study on the implementation of signal processing ASIC is advanced m the parts of the DS/CDMA using the direct sequence method which is better than any other type in the encryption aspect. Therefore, in this thesis, Pseudo Noise Code Generator suitable for the encryption aspect. Therefore, in this thesis, Pseudo Noise Code Generator of Code Tracking Sysytem for Digital Mobile Communication suitable for the DS/CDMA method including the additional characteristics for the spread spectrum signal processing technique, encryption technique which is protect to privacy between users, the decrease effects of the self-interference and transmission power spectral density, is designed to get the improved effects for the conventional (exciting) channel noise by using the Verilog-HDL.
