Gastritis cystica profunda (GCP) is a rare disease characterized by gastric foveolae elongation along with hyperplasia and cystic dilatation of the gastric glands extending into the tissue beneath the submucosa. GCP mainly develops at the gastroenterostomy stoma but can arise in a stomach that has not undergone surgery. The proposed pathogenesis is related to chronic inflammation, ischemia and the presence of a foreign body. GCP may present as a submucosal tumor, Polyp or a giant gastric mucosal fold. Endoscopic ultrasonography (EUS) is an effective diagnostic method for differentiating GCP from protruding and elevated gastric lesions. We report a case of GCP along with the endoscopic and EUS findings that were indistinguishable from a malignant submucosal tumor.

Tumors of neurogenic origin are rare and constitute.about 496 of all tumors found i the stamach. Neurilemoma (schwannoma, neuroma) and neurofibroma are the most prevalent. Gastrointestinal symptoms of neurogenic tumors result from obstruction or bleeding. A 57-year-old female presented with epigastric pain for 9 months. UGI series and ahdominal CT showed a submucosal tumor at gastric fundus. Endoscopic exam revealed submucosal tumor with central ulceyation and bridging folds. Microscopic exam of endoscopic biopsy suggested neurilemoma. Local tumor excision was performed. The nerve origin of this tumor was confirmed by positive staining of histchemical study with S-100 protein and by demonstrating Schwann cells with long cytoplasmic processes. on electron microscopic study. The finaI histologic diagnosis was neurilemoma. So we reprot a case with a review of relevant literatures.
Non-Helicobacter pylori, Non-NSAIDs 소화성궤양

Non-Helicobacter pylori, non-NSAID peptic ulcer disease, termed idiopathic peptic ulcer disease (IPUD), is being increasingly recognized. Current data suggest that the relative proportion of patients with IPUD among those with peptic ulcers has been increasing for the past decade in both the West and East, while the prevalence of H. pylori ulcers has decreased. Potential causative or risk factors of IPUD include cigarette smoking, genetic predisposition, psychological factors, infections other than H. pylori, non-NSAID drugs or toxins, systemic inflammatory disease, and local mucosal defects. Diagnosis is made after confident exclusion of H. pylori infection by at least two different standard tests and accurate exclusion of unrecognized or surreptitious NSAID use by careful history taking and serologic assay. IPUD is characterized by higher complication and recurrence rates than other forms of peptic ulcer disease and has become a main cause of refractory ulcer disease. Maintenance therapy using proton pump inhibitors may be required at higher doses and for longer durations than in H. pylori- or NSAID-associated peptic ulcer diseases. Well designed nationwide epidemiologic studies are required to fully elucidate this emerging condition. (Korean J Med 2014;86: 678-685)

N/A The effect of short-term levamisole therapy on serum ferritin and E-rosette forming lymphocytes was studied in 29 patients with various malignant tumors. And 8 normal person. In patients with malignant tumors, active form of T-lymphocytes were more decreased than total form of T-lymphocytes and serum ferritin was significantly increased. Levamisole had a moderate enhancing effect on E-rosette formation in patients with malignant tumors. These effect was more prominent in total form of T-lymphocytes than active form of T-lymphocytes, which suggested that active form of T-lymphocytes were more seriously affected than total form of T-lymphocytes and former may be more important in cellmediated immunity of the malignant patients. Serum ferritin was decreased in average 22% after levamisole therapy, but none of cases reduced to normal range in patients with significantly elevated serum ferritin level. There was close correlation(r=0.51) between decrease in serum ferritin and enhancing effect on E-rosette formation in malignant tumors after short-term levamisole therapy. These results indicate that levamisole as a role of immunotherapy may be beneficial in patients with malignant tumors.
