Emphysematous pyelonephritis is an unusual, severe gas-forming infection of renal parenchyma and its surrounding areas. It is a rare cause of septic pulmonary embolism. We report on a case of emphysematous pyelonephritis complicated with renal vein thrombosis and septic pulmonary embolism with review of the literature. A 51-year-old diabetic woman was admitted to our hospital with symptoms of fever, diffuse abdominal pain and nausea. Her initial laboratory findings showed pyuria and leukocytosis. She was diagnosed with acute pyelonephritis with abscess formation on contrast enhanced abdominal CT. She was treated with antibiotics and percutaneous abscess aspiration, but progressed to emphysematous pyelonephritis complicated with renal vein thrombosis and septic pulmonary embolism. Finally she underwent the left total nephrectomy.

This study was conducted to evaluate the effects of Glycyrrhizae Radix water extract, known as depigmenting agent, on melanin biosynthesis in the HM3KO human melanoma cells. The inhibitory effect of Glycyrrhizae Radix water extract on melanogenesis was identified by mushroom tyrosinase assay in vitro. To determine whether Glycyrrhizae Radix water extract suppress melanin synthesis in cellular level, HM3KO cells were cultured in the presence of different concentrations of Glycyrrhizae Radix water extract and the effects on cell proliferation, melanin contents and tyrosinase activity were examined after 3 days. Treatment with Glycyrrhizae Radix at various concentrations did not exhibit any change of cell viability, and increased the cell proliferation. And the water extract of Glycyrrhizae Radix inhibited melanin contents and tyrosinase activity in a dose-dependent manner, compared with untreated group. These results suggest that the inhibitory effect of Glycyrrhizae Radix water extract on melanogenesis is due to the suppression of tyrosinase in HM3KO cells.

Background: It is essential to evaluate hemodynamics of vascular disease in addition to anatomic details. It has been known that the plethysmography represents intravascular(arterial or venous) volume or pressure. Photoplethysmography(PPG) is convenient to use and has been widely used for measurement of venous refill time(VRT). However, it has not been used for evaluation of venous function. Objective: Our study aims at testing the feasibility of evaluation of venous function using PPG. Methods: The maneuver established for air PG was slightly modified and applied for the PPG. The test was performed with the object seated in a chair and the legs dependent. The PPG probe was attached to the limbs within 5 cm from medial malleolus. After standardized procedure of leg-raising, putting the leg in dependent position, 1 tip-toe exercise, and 10 tip-toe exercises, the measured parameters were initial emptying time(E1), amount of volume change(V1), refilling time by hydrostatic pressure(F1), refilling time after 1 tip-toe exercise(F2), and ejected volume(V2) and refilling time after 10tip-toe exercises(F3). Results: The mean(±S.D.) values of each parameters were; El(±S.D.)= 14.5±6.8 s(N=29), V2/Vl= 0.39±0.14(N=30), Fl=44.9±18.3s(N=30), F2=15.5±3.9s(N=30), and F3=23.2±5.9s(N=29). There was no correlation between El and V2/Vl(r = -0.09, P=0.63), but the parameters of venous refill(F3, F2, Fl) showed significant correlation(r = 0.424, P=0.02, F3 vs. F2, and r= 0.627, P = 0.0003, F3 vs. Fl). Conclusion: El and V2/Vl are independent parameters representing different functional status I.e. emptying time by hydrostatic pressure and relative emptying volume during exercise respectively, and Fl and F2 are parameters of venous refill related to F3. They could be useful for evaluation qf venous function in addition to conventional measurement of VRT.

<P>1. <?ri?>A novel diacylglyceride acyltransferase-1 (DGAT-1) inhibitor, 2-(4-(4-(5-(2-phenyl-5-(trifluoromethyl) oxazole-4-carboxamido)-1H-benzo[d]imidazol-2-yl)phenyl)cyclohexyl) acetic acid (KR-69232), was synthesized for a potential therapeutic use against several metabolic disorders, such as obesity, insulin resistance, and type II diabetes, characterized by excessive triglycerides (TGs) in the blood.</P><P>2. <?ri?>The half-lives against phase I metabolism were measured as 75.3 ± 20.9 min and over 120 min in rat and human liver microsomes, respectively. In Caco-2 cell monolayers, extremely low permeability (<0.13 × 10<SUP>−6 </SUP>cm/s) was seen in the absorptive direction, predicting limited intestinal absorption of KR-69232. This compound was highly bound to rat and human plasma proteins (>99.8%).</P><P>3. <?ri?>With the intravenous administration of KR-69232 in rats (1, 2, and 5 mg/kg), non-linear kinetics were observed at the highest dose, with significantly higher systemic clearance, higher volume of distribution, and lower dose-normalized AUC. Following oral administration, it exhibited low bioavailability (<10%) and was absorbed slowly (<I>T</I><SUB>max</SUB>, 3.8-5.2 h) over the dose range. We also confirmed that considerable KR-69232 remained in the intestine at <I>T</I><SUB>max</SUB>, demonstrating its limited absorption into the systemic circulation.</P>

<P>Exquisite surface wettability control of separation system surface is required to achieve separation of liquids with low surface tension difference. Here, we demonstrate a series of surface-energy-controlled homogeneous copolymer films to control the surface wettability of polyester fabric, utilizing a vapor-phase process, termed as initiated chemical vapor deposition (iCVD). The homogeneous copolymer films consist of a hydrophobic polymer, poly(2,4,6,8-tetramethyl-2,4,6,8-tetravinylcyclotetrasiloxane), pV4D4, and a hydrophilic polymer, poly(4-vinylpyridine), p4VP. Because the mixing of two or more components is always favorable in vapor phase, the iCVD process allows the formation of homogeneous copolymers from two immiscible, hydrophilic/hydrophobic monomer pairs, which is highly challenging to achieve in liquid phase. Simply by tuning the flow rate ratio of monomer pairs, a series of homogeneous copolymers with systematically controlled surface energy were formed successfully. The fabricated separation system could separate water (surface energy = 72.8 mJ/m<SUP>2</SUP>), glycerol (64 mJ/m<SUP>2</SUP>), ethylene glycol (48 mJ/m<SUP>2</SUP>), and olive oil (35.1 mJ/m<SUP>2</SUP>) sequentially with excellent selectivity, just by choosing a copolymer-coated polyester fabric with proper surface energy. Pure red cell aplasia(PRCA) is characterized by anemia, absence of reticulocytes in the peripherial blood and selective erythroid hypoplasia in the bone marrow. Acquired PRCA is often associated with thymoma but may also occur in many diverse conditions, such as chromic lymphocytic leukemia, systemic lupus erythematosus, autoimmune disorders, T gamma lymphocytosis, acquired hypogammaglobulinemia, acquired immunodeficiency syndrome. Recently we experienced a case of T cell chronic lymphocytic leukemia associated with pure red cell aplasia. A 65-year-old man was presented with severe anemia. absolute reticulocytopenia, hepatosplenomegaly and lymphocytosis(T cell marker' CD2, CD7 positive). Bone marrow findings showed a marked decrease in erythroid precursors and normal maturations of granulocytic and megakaryocytic series, which were consistent with pure red cell aplasia. So we report this case with a review of the literature.
Ultrasonographic findings of the normal diaphragm: thickness and contractility

Background: Neuromuscular ultrasound can be used to assess the diaphragm. Before it can be used clinically, the reference ranges of diaphragm thickness and contractility must be determined. Methods: We measured the thickness of the diaphragm and the diaphragmatic thickening fraction (DTF) in 80 healthy volunteers with ultrasound and collected their demographic information to determine if age, sex, and body mass index (BMI) influence these measures. Results: The thickness of the diaphragm at resting end expiration was $0.193{\pm}0.044cm$ on the right side and $0.187{\pm}0.039cm$ on the left. The DTF was $104.8{\pm}50.6%$ on the right side and $114.9{\pm}49.2%$ on the left. Sex, weight, height, and BMI significantly affected the thickness of the diaphragm, but had little effect on the DTF. Conclusions: Normal reference values for the diaphragm should be helpful when evaluating the diaphragm. The DTF appears more useful than resting diaphragm thickness because it is affected less by individual variation.
