말티즈견에서 발생한 승모판폐쇄부전 1례

A 10 years old, intact male, Maltese dog with clinical signs of labored respiration, exercise intolerance and increasing of coughing was referred to the Veterinary Medical Teaching Hospital of Chungnam National University. Physical examination revealed tachycardia, panting and mild cyanosis. This case was diagnosed as mitral valve insufficiency by clinical signs, ascultation, blood chemistry, radiography and electrocardiography. This dog was treated with ACE inhibitor (enalapril), diuretics (furosemide) and bronchodilator (aminophylline) for 14 days. After treatment, clnical signs were disappeared and also this dog was needed to be continuous monitoring and sodium restricted diet.

도널드 위니캇의 놀이와 치료

본고는 말더듬이 증상을 어떻게 치유할 것인가에 대한 질문의 답을 구하기 위해 2010년 개봉된 영화 <킹스 스피치>를 분석하며, 영국 국왕 조지 6세의 생애초기 제공된 부적절한 환경과 장애 그리고 치료과정을 다룬다. 어머니가 유아의 요구에 얼마나 잘 반응해주는가 하는 문제는 유아의 성장·발달과정에서 장애의 원인이자 해결책으로 제시되며, 놀이와 관련되어 있다. 따라서 언어치료사 라이오넬 로그는 놀이 규칙과 방식을 적용하여 조지 6세의 과거 기억과 무의식에 접근한다. 도널드 위니캇에 따르면, 어머니와 유아의 관계는 창조적인 삶과 순응적인 삶을 결정하는 주요인이다. 그는 어머니와 유아 사이의 공간을 놀이를 위한 공간으로 가정하였고, 그의 놀이 이론은 조지 6세가 생애 초기 왜곡된 보살핌으로 인해 상실한 놀이를 분석하는 도구로 사용된다. 그는 먼저, ‘일차적 모성몰입"과 ‘존재의 연속성’ 에 주목하였으며, 그 이론들은 ‘충분히 좋은 엄마" 그리고 ‘자아 관계성"으로 연결된다. 따라서 본 연구는 조지 6세의 자아탐색이 왜곡된 양육, 엄격한 아버지에 대한 두려움, 그리고 그의 사명감과의 동일시로 인해 좌절되었음을 보여준다. 필자는 위니캇의 이론을 나열하기보다는 소아과 의사로서 그가 어린 아이들을 면담한 사례들을 강조하고 그 과정에서 치료자가 지켜보는 가운데 놀이에 몰입하는 아이들이 장애로부터 가벼워지거나 극복되는 현상을 추적한다. 아버지의 엄격한 규칙과는 달리, 놀이와 치료를 위한 로그의 규칙은 강압적이지 않지만 단호한, 유연하지만 일관된 특징을 지닌다. 이제, 그의 규칙은 조지 6세로 하여금 자신만의 놀이에 적극적이도록 이끌고 자신의 목소리를 내는 참자아로의 그의 점진적인 변화를 가능하게 만든다. This paper analyzes a movie The Kings Speech(2010) to answer the question of how to cure symptoms of a stammer, and deals with George VI"s improper environment provided by his mother or nurturer during the early years of life, his disability derived from the environment, and therapies through playing. How well mothers respond to their young children is causes and solutions of children"s growth and development, and it is related to playing. Therefore, Lionel Logue, who is a language therapist, approaches closely to the memory and unconsciousness of young George VI. According to D.W. Winnicott, mother-child relationship is a crucial factor to decide creative and compliant living. He postulates the potential space between mother and baby for the place to playing, and the playing young George VI lost is traced by Winnicott"s object relations theory. First of all, his theories notice the ‘primary maternal preoccupation" and "continuity of being", and then they are linked to ‘good enough mother", and "ego-relatedness". Therefore, this study shows that George VI"s search for self is frustrated by distorted nurture, excessive fear to father, and assimilation with his sense of duty. This paper especially focuses on illustrative cases of young children rather than theory itself and has noticed a surprising phenomenon which overcomes or relieves their disability through the preoccupation to playing itself. Unlike father"s strict rules, Logue"s rules for playing and therapy have unoppressive but decisive, and flexible but coherent features. Hereby, they make it possible for George VI to fall into his own playing, and as a result, they"ve led his gradual changes to ‘true self", expressing his own thinking.

가부장적 흑인 가족관계의 극복과 스토리텔링의 중요성

본 논문은 토니 모리슨(Toni Morrison)의 작품 『솔로몬의 노래』에 나타난 밀크맨과 그의 대화 상대자들 간의 대화를 분석함으로써 가부장적 미국 흑인 가족관계를 분석하고 스토리텔링을 통한 가족관계의 회복을 추적하는 것을 목표로 한다. 진정한 스토리텔링이란 등장인물들이 경험한 외상적 기억을 극복하기 위해서 그들의 과거 기억을 얼마나 성공적으로 끌어내느냐 하는 것이다. 이 작품에 적용된 스토리텔링은 밀크맨이 종족, 조상, 가족, 그리고 진정한 자아와 관련된 새로운 이해를 발견할 때 더 의미 있게 다루어진다. 이 소설에서 가족관계의 단절은 주로 가부장적 아버지 메이컨의 냉담함과 제멋대로인 아들 밀크맨의 무지와 허영에서 비롯된다. 부자지간인 메이컨과 밀크맨은 자신의 가족들을 존중하지 않는다. 메이컨은 아내 루스를 미워하고 자신의 딸들, 레나와 코린시언스를 무시하며 자신의 여동생 파일럿과 의절하는데, 밀크맨 또한 가족을 대하는 아버지의 방식을 암묵적으로 받아들이면서 그에게 가족은 늘 뒷전이다. 고인이 된 아버지 닥터포스트의 권위를 고수하려고 고집하는 어머니로 인해 밀크맨은 왜곡된 모성적 돌봄을 경험하였고, 그것으로 인한 미움과 분노를 가지고 있다. 그의 삶에서 남성적 특권 행사를 가능하게 뒷받침해 준 누나들과의 관계 또한 단절되어 있다. 모리슨의 등장인물들이 보여주는 가족관계의 단절은 그들이 그들 자신의 과거, 현재, 미래에서 인간관계를 성취하지 못하는 원인으로 작용한다. 이 가족의 모습은 권위에 가까워지려는 것뿐 아니라 가부장 제도에 편승하려는 미국 흑인들의 필사적인 노력이 그들 자신에게 아무런 도움이 되지 않는다는 사실을 말해준다. 따라서 본 연구는 가부장적 권위가 지닌 폭력이 가족에게 얼마나 치명적일 수 있는가 그리고 세상을 움직이는 방식들로는 결코 설명될 수 없는 보이지 않는 힘들이 인간관계와 변화에 얼마나 중요한 영향을 미칠 수 있는가에 집중한다. 따라서 밀크맨의 변화는 파일럿의 주술과 두려움에 대한 그녀의 과거 경험에 대한 확신에서 시작되며, 그의 성장은 오랫동안 지속되어 온 인종차별과 권리박탈의 역사를 담고 있는 남부로의 여정에 의해 의미 있는 영향을 받는다. 남부로의 여정은 밀크맨에게 가부장적 아버지에 의해 무시되어온 소통의 단절에 대한 대안을 제공한다. 아버지와 함께 한 어린 시절과 대자연에 대한 사랑을 집착으로 바꾸어 놓았던 메이컨의 과거는 이제 밀크맨에게 다른 의미로 다가온다. 자신의 땅에 귀속해서 동생 파일럿을 돌보며 아버지의 인정과 아버지를 향한 그 지역 흑인들의 존경을 함께 받았던 메이컨의 과거가 밀크맨에게 재현되면서 그로 하여금 가족관계에 대한 즐거운 의무감에 대해 책임을 느끼도록 이끈다.

다중인격 장애와 대상관계이론

본고는 유년기 성학대 피해를 경험한 아동의 트라우마가 다중인격 장애와 대인관계 형성에 끼친 영향을 분석한다. 영화 〈23개의 인격〉(Split)과 소설 『빌리 밀리건』(The minds of Billy Milligan, 1982) 은 각각 어머니로부터의 학대 또는 계부로 부터의 성적 학대를 아동의 극단적인 트라우마로 제시한다. 다중인격 장애(multiple personality disorder) 또는 해리성정체감 장애(dissociative identity disorder)를 겪는 환자의 95~100%가 어린 시절 근친상간이나 심각한 신체적, 정신적 학대를 경험한다는 점에 비추어 볼 때, 유아기의 주관적인 경험은 자아형성과 관계형성에서 가장 중요한 요인이다. 또한 본고는 다중인격 장애와 관련하여 대상 항상성에 기초한 좋은 환경, 어머니(양육자)의 담아내기, 그리고 박탈을 논의한다. 과도한 트라우마를 어떻게 다룰 것인가 하는 문제는 다중인격 장애에 관한 연구에서 핵심적인 부분이며 그것은 투사적 동일시와 정동이론으로 분석된다. The purpose of this paper is to analyze the effects of trauma of children who experienced sexual abuse in childhood on multiple personality disorder and interpersonal relationships. The movie Split(2017) and the novel The minds of Billy Milligan(1982) each suggest the child abuse from mother or sexual abuse from the stepfather as the young child"s extreme trauma. Given that 95 to 100% of patients suffering from multiple personality disorder or dissociative identity disorder experience childhood incest or severe physical and mental abuse, the experiences of early childhood are the most important factors in self and relationship formation. This paper also discusses the good environment based on object constancy, the containing and deprivation of mother(caregiver) in relation to multiple personality disorder. The question of how to deal with excessive trauma is a key part of the study of multiple personality disorder, which is analyzed by projective identification and affect theory.

Purpose: Allergic rhinitis (AR) is one of the common chronic diseases. Although it is not a life-threatening disease, its persistent symptoms may cause fatigue, mood change, discomfort at work, and academic disability as well as the decrease of quality of life. The prevalence of AR has been increasing steadily due to the Westernized lifestyle and environmental change. In previous studies, it has been found that AR has a clear relationship with smoking. However, there is no relationship study between AR and electronic cigarettes smoking (ECS). Methods: The study was conducted on >19 -year-old adults who participated in the 2015 Korea National Health and Nutrition Examination Survey. Sex, age, residence status, tobacco smoking, alcohol drinking, stress level, economic status, and diagnosis of AR were analyzed by using univariate and multivariate logistic regression analyses. Results: AR tended to be associated with ECS in the Korean adult population in univariate analysis, but ECS was not statistically significant in multivariate analysis. By multivariate analysis, AR was significantly related with younger age, male sex, alcohol consumption, and stress. Moreover, the prevalence of AR was linearly decreased as age increased from 19 to 69 years. Conclusion: A diagnosis of AR was not significantly associated with ECS. Instead, AR showed an increased prevalence in adults at younger age, of male sex, and with alcohol consumption and high stress. To derive statistically significant results of relationship between AR and ECS, more well-designed studies focusing on the temporal causal are needed. (Allergy Asthma Respir Dis 2017;5:344-350)

고등학교 인터넷경매 중독성향수준별 집단 특성

The purpose of this study was to find the characteristics of internet auction user groups by the levels of addictive tendencies among high school students. The questionnaire was conducted from December 16 to 23, 2008, and 288 high school students who were living in Cheongju area were selected. The main results were as follows. First, 13.4% of respondents were visiting internet site almost every day, and about 20% of them stayed more than 2 hours for visiting the internet auction site. Second, the price of purchasing goods through internet auction site(shopping mall) was less than 50,000 won. Third, the level of addictive tendencies of respondents was under average(2.08 out of 5 point scale), and about 10% of them evaluated by themselves as an internet auction. Fourth, relatively higher addictive tendencies group showed the characteristics such as lower self-esteem, higher depression, and higher feeling of isolation, internet related(general internet use, internet shopping, and internet game) addictive tendencies, and more often visiting and staying longer in internet auction sites and other internet sites.
