비장티푸스 살모넬라증에 의한 장열과 장천공은 매우 드물다 저자들은 방어기전의 손상이 없는 건강한 성인에서 비장티푸스성 D군 살모넬라 장열에 동반된 장천공이 발생한 예를 경험하였기에 문헌 고찰과 함께 보고하는 바이다. The case of enteric fever and bowel perforation caused by nontyphoidal Salmonella spp. is extremely rare. We report a case of a 28-year-old man who suffered from enteric fever with complicated a small bowel perforation, which is a known complication of S. typhi and S. paratyphi infections. The culprit later proved to be nontyphoidal group D Salmonella spp. in our case.

Purpose : To compare cephalometric measurement between measuring methods in digital and conventional lateral cephalometric radiograph. Materials and Methods : Twenty digital and conventional lateral cephalometric radiographs were selected. In digital group, cephalometric measurements were performed manually using hardcopies and automatically using VCeph^(TM) program on the monitor. In conventional group, the same measurements were performed manually on conventional films, and for automatic measurement conventional films were digitized by scanner. All measurements were performed twice by 4 observers, and 24 cephalometric variables were calculated and the time spent for each measurement was recorded. The differences in measurements data and the time spent for each measurement were compared within each group. Intra-observer and inter-observer comparisons were performed. Results : In both groups, no statistically significant difference between manual and automatic measurements was observed and most of the variables didn’t show statistically significant differences between methods. The observer with less experience tended to show statistically significant differences of measurements between methods, and differences from other observers. The differences of measurements between methods in digital group were lesser than those of conventional group with statistical significance in 8 variables out of 24. With automatic method and in digital group, the spent time was shorter. Conclusion : With direct digital radiograph, automatic method using manually idenitified landmarks can be preferable in cephalometric analysis.

<P><B>Abstract</B></P> <P>We report that the molecular orientation of a disk-shaped Pt(II) complex dopant in organic thin films is linearly proportional to the orientation of the host molecules. We ascribe this relationship to the parallel alignment of the Pt complex with the host molecules induced by a π-π interaction. This would be caused by their planar and conjugated structure, indicating that the intermolecular interaction and steric effect play an important role. 초고속 Marx Generator의 N₂, SF₆, N₂-SF₆ 혼합가스에 따른 출력 특성 연구

The application field of the pulse power is very wide. Recently, Pulse power technologies take a large place in several applications. Then, many civil and military applications proceed. Marx generator is widely used in high voltage applications. Marx generator is widely used in high voltage applications, such as eletromagnetic wave and power lasers. This paper, we described about the high voltage pulse generator. A compact size high voltage pulse generator with nanosecnd rise time has been fabricated and investigated experimentally. The marx generator has 2 stages. Each stage was constructed one charging capacitor, two electrodes and one charging resistor. A inductance structure is used in order to improve the switching performances fo the whole generator. The experiments of rise time in pure gas and mixtures of gases were described. We tested the Marx generator at different insulation gas. the results show that the dielectric strength of the N₂-SF? mixture was significantly increased compared with pure nitrogen gas. The experimental results show that the rise time characteristics of the Marx generator can be controlled through varying insulation gas.

N/A Mineralocorticoids influences on acid-base homeo-stasis by the regulation of urine acidification. But its mechanism of acion is not well known in human. This study compared the acid-base status and the indices of urine acidification before and after mineralocorticoid administration in human, and analyzed the effect of mineralocorticoids on human acid-base homeostasis. We administered 9a-fludrocortisone in 6 chronic renal failure patients and 6 normal controls 0.5mg daily for 7 days. The results were as following ' 1) After administration of 9a-fludrocortisone in patients group, serum aldosterone level changed from 120.2±71.0pg/mL to 44.8±32.2pg/mL(mean±SD, p< 0.05). Serum HCO- level was not changed. Urine ammonium excretion was incresed from 24.6±12.3 mmol/day to 43.7±19.0(p<0.05), but there were no change in urine pH and urine anion gap, Serum potassium level decreased from 5.5±0.7mBq/L to 4.1±0.5mEq/L(p<0.05), and TTKG increased from 3.9 to 8.9(p<0.05). 2) After administration of 9a-fludrocortisone in control group, serum aldosterone level changed from 99.7±44.5pg/mL to 25.1±3 mL(p<0.05). Serum HCO- level was not changed. Urine ammonium ex-cretion was incresed from 44.3±21.6mmoVday to 76.3±19.6(p<0.05), but there were no change in urine pH and urine anion gap. Serum potassium level decreased from 4.8±0.5mEq/L to 3.9±0.2mHq/L(p< 0.05), but there was no change in TTKG. 3) No patient or control showed any discomfort after 9-fludrocortisoneadministration, and there was no elevation in diastolic blood pressure, increase in body weight, electrolyte abnormality. In summary, after 9α-fludrocortisane administration, urinary ammonium excretion increased in both patients and control group, and this phenomenon occured with correction of hyperkalemia without urine pH change. This result implies urinary ammonium excretion increase by mineralocorticoid. In human increase in renal distal acidification by mineralocorticoid is due to increase in renal ammo- niagenesis rather than stimulation on proton excretion.

<P>We demonstrate an experimental in situ observation of the temperature-dependent evolution of doping- and stress-mediated structural phase transitions in an individual single-crystalline VO2 nanobeam on a Au-coated substrate under exposure to hydrogen gas using spatially resolved Raman spectroscopy. The nucleation temperature of the rutile R structural phase in the VO2 nanobeam upon heating under hydrogen gas was lower than that under air. The spatial structural phase evolution behavior along the length of the VO2 nanobeam under hydrogen gas upon heating was much more inhomogeneous than that along the length of the same nanobeam under air. The triclinic T phase of the VO2 nanobeam upon heating under hydrogen gas transformed to the R phase and this R phase was stabilized even at room temperature in air after sample cooling. In particular, after the VO2 nanobeam with the R phase was annealed at approximately 250 C in air, it exhibited the monoclinic M1 phase (not the T phase) at room temperature during heating and cooling cycles. These results were attributed to the interplay between hydrogen doping and stress associated with nanobeam-substrate interactions. Our study has important implications for engineering metal-insulator transition properties and developing functional devices based on VO2 nanostructures through doping and stress.</P>

<P>Using Raman spectroscopy, we demonstrated photothermally induced crystallographic phase transitions of vanadium dioxide (VO<SUB>2</SUB>) nanobeams clamped to and free-standing on a substrate. Compared to the temperature-dependent Raman measurements, the laser-power-dependent Raman characteristics provide substantial evidence for the photothermal origin of the phase transitions of the VO<SUB>2</SUB> nanobeams. The laser power necessary to cause phase transitions in the free-standing nanobeam was approximately eight times smaller than the laser power used in the substrate-clamped nanobeam. Our study will enhance the understanding of the complex phase transitions of strongly correlated oxides and thereby provide a foundation for engineering desirable properties in novel devices.</P>
신부전 만성도 평가에 있어 혈중 carbamylated hemoglobin 치의 유용성

Objective: Differentiation of acute from chronic renal failure is an important but difficult task in treating the azotemic patients. It is known that the degree of carbamylation reflects the duration and the severity of the uremia. To assess the usefulness in differentiating acute(ARF) from chronic reanl failure (CRF), we measured the carbamylated hemoglobin (CarHb). Methods: We measured CarHb(measured as ㎍ of carbamyl valine per g hemoglobin) prospectively in 64 patients with azotemia (24 ARF, 40 CRF) referred to the Seoul National University Hospital. High performance liquid chromatography was used for measuring CarHb. Thirteen normal controls were also studied to match 64 patients. Results: Patients with ARF had lower median (range) CarHb concentration than those with CRF [56.0(24.6-97.1) ㎍CV/gHb vs 120.4(31.7-286.5) p<0.01]. It is not different from normal control [56.0 (24.6-97.1) vs 40.9(19,9-62.9); p<0.05]. The ratio of CarHb to serum creatinine(mg/dl) was lower in ARF than in CRF [10.5(4.9-34A) vs 18.2(4.7-52.2) p<0.01]. The cut-off value for CarHb to be CRF is 80 ㎍CV/gHb(sensitivity 70%, specificity 71%, positive predictive value 80%). Conclusion: The carbamylated hemoglobin is useful in determining the chronicity of the renal failure and it could be of clinical value in deciding the further management.
