      • 高壓露出 白鼠의 體液 및 血液學的 變化에 關한 實驗的 硏究

        The deep sea is a fascinating part of our environment. However, no man can go down to the indefinite depth with safety because there are many problems in excursion to the deep sea. One of the problems in deep-sea diving is that of fluid changes in association with the increased ambient pressure. However, that problem is still poorly understood. On the contrary, various body fluid and hematologic responses as a result of the expcsure to high altitudes have been studied throughly, and a considerable amount of evidence, which indicates that there may be major shifts in body fluid compartments, has been accumulated. The present study was aimed to determine whether there might be any shifts of body fluids in different fluid compartments and if any, to examine closly the direction and magnitude of the shift in response to an increased ambient pressure throught the experiments on rats. To stimulate an ambient pressure equivalent to that of the depth of current deep-sea, a land-based hyperbaric chambers were used. The animals were divided into two groups each of eight animals, and they were exposed to simulated dapths of 0 fsw or sea level (1ATA) for control study and 33 fsw (2ATA) for experimental study, respectively. The exposure was continued for a duration of 24 hours, and the supply of water and cereal were maintained throughtout. Urinary flow rate rather decreased in the experimental group by its mean value, though this not being statistically significant. Urianry osmolality decreased significantly along with excertion of sodium. Osmolar clearance decreased as well, and free water reabsorption tended to decrease in the experimental group. The plasma volume and blood volume increased significantly, while the extracellular fluid volume tended to increase. Red blood cell cont, hemoglobin levels, and hematocrit valued decreased significantly. Despite the insignificant and small variation in plasma osmolarity osmolarity, plasma protein concentration fell significantly in the experimental group and it was suggested that the hyperbaric exposure caused hemodilution. Plasma sodium concentration showed no significant difference between the two groups, No notable alteration could be observed in the PRA, but it tended to decrease. Thus both aninapparent plasma sodium concentration difference and an apparent hemodilution along with decreased urinary sodium excretion suggest that the hyperbaria caused sodium retention and therefore fluid volume expansion. The antinatriuresis seems to be caused enhanced sympathetic neural activity which stimulates sodium reabsorption in the proximal tubules. And it also might be suggested that hypoosmotic fluid was mobilized into the intravascular space and possibly into the interstitial fluid spaces as well from the intracellular compartment. This is possibly caused by an increase in the osmotic pressure of the plasma due to the addition of physically dissolved inert gas(nitrogen) molecules during compression. Some of these pressure effects on body fluid and hematological parameters were compared with those in rats acutely exposed to high altitudes, and were found to be opposite qualitatively.

        직무스트레스가 정신신체적 긴장 및 소화기계 증상에 미치는 영향

        To examine the effects of job stress on psychosomatic strain, gastrointestinal symptom, and disease, a total of 227 male blue collar workers of 21 departments of the factory producing fabric and videotape in Gumi city were surveyed using Karasek's Job Content Questionnaire. Twelve items of JCQ(skill discretion, created skill, decision authority, decision latitude, psychological job demand, job insecurity, coworker support, supervisor support, social support, psychological exertion, toxic exposure and psychosomatic strain), gastrointestinal symptoms, and UGI series were assessed for the all subjects. It was found that psychosomatic strain is associated with skill discretion, created skill, psychological job demand, physical exertion, and social support and gastrointestinal symptom is associated with social support. Multiple regression analysis suggested that psychological job demand, physical exertion, decision latitude, and job insecurity are main factors for psychosomatic strain and physical exertion is main factor for gastrointestinal symptoms. The Job Demands-Control model was not supported in this study, suggesting the level of psychosomatic strain are not modified by decision latitude.

        This study was undertaken to investigate the occupant's satisfaction and actual condition of residential space in a apartment house and put a regional limit to Donghae and Samcheok city. which are classified into small-medium city located in southern part of Yongdong area. Apartment complex of over 250 households and individual apartment resident of a group from each city located in this area were chosen as the target for data analysis. Apartment chosen as a target for data analysis includes all floor plans within the apartment complex. The subject of study is limited to apartment that mas constructed since 1990 in order to suggest course of residential spaces in future apartment. The scope of ths study includes interior materials. finishing materials, electronics, illumination. interior design: standard of living, problem with internal structure and satisfaction in indoor space. This study also analyzed individual evaluation of internal structure and indoor space in apartment in which they live. The purpose of this study is to point out problems of apartment house and suggest improvement plan for it by preparing basic data for various plan for interior space at occupant's discretion and design concept through the materialization of occupant's sense of interior space which was acquired from above analysis as a multilateral approach to apartment house floor plan.

        리마틸 정(부시라민 100㎎)에 대한 부시린 정의 생물학적 동등성

        Bucillamine is a novel cysteine derivative with two free intramolecular sulfhydryl groups, and has a preventive and therapeutic effect on adjuvant arthritis, suggesting its antirheumatic action. With respect to the effect on the immune system, bucillamine-exerted such immunoregulating actions are to nomalize an excessive reduction or acceleration in immune reaction. It is useful not only in patients with early stage of rheumatoid arthritis (RA) but also in those with active RA retained for more than 10 years. The purpose of the present study was to evaluate the bioequivalence of two bucillamine tablets, Rimatil^TM (Chong Kun Dang Pharmaceutical Co., Ltd.) and Bucilin^TM (Kuhn Il Pharmaceutical Co., Ltd.), according to the guidelines of Korea Food and Drug Administration (KFDA). Eighteen normal male volunteers, 23.67±2.09 years in age and 65.03±6.73 kg in body weight, were divided into two groups and a randomized 2×2 cross-over study was employed. After three tablets containing 100 mg of bucillamine per tablet were orally administered, blood was taken at predetermined time intervals and the concentrations of bucillamine in serum were determined using GC/MS with mass selective detector. Pharmacokinetic parameters such as AUC_t, C_max, and T_max were calculated and ANOVA test was utilized for the statistical analysis of the parameters. The results showed that the differences in AUC_t, C_max and T_max between two tablets were -0.29%, -3.20% and 8.22%, respectively, when calculated against the Rimatil^TM tablet. The powers (1-β) for AUC_t and C_max were 84.31 % and 91.16%, respectively. Minimum detectable differences (Δ) at α=0.10 and 1-β=0.8 were less than 20% (e.g., 18.58% and 16.51% for AUC_t and C_max respectively). The 90% confidence intervals were within ±20% (e.g., -12.77∼12.20 for AUC_t and -14.30∼7.90 for C_max). Two parameters met the criteria of KFDA for bioequivalence, indicating that Bucilin^TM tablet is bioequivalent to Rimatil^TM tablet.

        Early Gonadal Differentiation of the Protogynous Red Spotted Grouper, Epinephelus akaara

        Red spotted grouper, Epinephelus akaara is a popular aquaculture species in many Asian countries. This species is a protogynous hermaphrodite that first differentiates into female and changes to male later. Due to this reproductive characteristic, stable supply of male and female gametes is a key to the success of seed production in this species. Thus, understanding early gonadal differentiation is required to develop effective sex control techniques. Red spotted grouper were reared in indoor tanks and sampled every 5 days from 40 days post-hatch (DPH) to 130 DPH. Changes of gonadal tissues were examined and analyzed by means of histology. A pair of gonadal primordium has already existed underneath the kidney in the posterior part of the body cavity at 38 DPH when this study began. Gonadal primordia of 38, 40 DPH consisted of germ cells surrounded by a few somatic cells. The blood vessel was observed in the gonadal primordium at 45 DPH. The number of somatic cells and size of gonadal primordium increased age-dependently up to 60 DPH. Formation of ovarian cavity was obvious by two protuberant aggregations of somatic cells at 65 DPH. Completed ovarian cavity and oogonia were first observed in the gonad of one fish sample at 105 DPH. Based on these histological observations, it can be suggested that induction of primary male differentiation could be more successfully applied at around 60 DPH in this species.

        Clinical significance of positive Pneumocystis jirovecii polymerase chain reaction in non-human immunodeficiency virus immunocompromised patients in a real practice

        Background/Aims: Pneumocystis jirovecii polymerase chain reaction (PCR) can be helpful in diagnosing Pneumocystis pneumonia (PCP); however it has limitations. We evaluated the prevalence of positive P. jirovecii PCR from non-human immunodeficiency virus (HIV) immunocompromised patients and tried to determine the risk of PCP development. Methods: Between May 2009 and September 2012, P. jirovecii PCR was performed in bronchoscopic specimens from 1,231 adult non-HIV immunocompromised patients suspected of respiratory infection. Only 169 patients (13.7%) who were tested positive for P. jirovecii PCR were enrolled. Retrospective chart review was performed. PCP was defined in patients with positive P. jirovecii PCR who were treated for PCP based on the clinical decision. Results: From 169 P. jirovecii PCR-positive patients, 90 patients were in the PCP group (53.3%) and 79 patients were in the non-PCP group (46.7%). In the PCP group, 38% of patients expired or aggravated after therapy, whereas the majority of patients (84%) in the non-PCP group recovered without treatment for PCP. Independent risk factors for PCP by binary logistic regression analysis were underlying conditions- hematological malignancies, solid tumors or solid organ transplantation, dyspnea, age < 60 years, and albumin < 2.9 g/dL. Conclusions: This study suggests that not all P. jirovecii PCR-positive patients need to be treated for PCP. Among P. jirovecii PCR-positive patients, those who are less than 60 years old, with hematological malignancies, solid tumors or solid organ transplantation, low albumin, and with symptoms of dyspnea, the possibility of PCP might be higher. Treatment should also be selected to these patients.

        Regulation properties of phospholipase C$\delta$ cloned from Misgurnus mizolepis

        Phosphoinositide-specific phospholipase Cδ (PLCδ) plays an important role in many cellular responses and is involved in the production of second messenger. The present study was conducted to characterize the catalytic and regulatory properties of the PLCδ of Misgurnus mizolepis (ML-PLCδ). The ML-PLCδ gene was cloned and expressed under according to the method of the previous report (Kim et al., 2004), and its recombinant protein was purified by successive chromatography using Ni2+-NTA affinity column. The recombinant ML-PLCδ showed a concentration-dependent PLC activity to phosphatidylinositol 4,5-bisphosphate (PIP2) or phosphatidylinositol (PI). Its activity was absolutely Ca2+-dependence, which was similar to mammalian PLCδ isozymes. The Ca2+ concentration yielding maximal activation of ML-PLCδ was 100 μM. However, the activity was decreased interestingly by a polyamine, such as spermine and spermidine. In vitro assay using cholate-micelle cell, ML-PLCδ activity was inhibited in dose-dependent manner by sphinogosine but increased by phosphocholine . In the lipid-binding assay, ML-PLCδ was strongly bound to LPA, PI(3)P, PI(4)P, PI(5)P, PI(3,5)P2, PI(4,5)P2, PI(3,4,5)P3 and PA, but it showed the low affinity to S1P, PI(3,4)P2 and PS. Taken together our results, it is suggested that the general catalytic and regulatory properties of ML-PLCδ are similar with those of mammalian PLCδ1 isozymes, but the N-terminal extended piscine phospholipase Cδ1 (ML-PLCδ) might reflect some distinctions in regulatory properties and inositol-lipid binding specificity between piscine ML-PLCδ and mammalian PLCδ isozymes.

        Background: Cytomegalovirus(CMV) pneumonia is uncommon in hematologic malignancy patients and clinical feature has been unclear. Methods: The clinical features of 16 patients with hematologic malignancy who were diagnosed as CMV pneumonia from August 2008 to July 2014 were retrospectively analyzed. Results: 43% (7/16) patients were female and average age was 42.5. All patients were performed bronchoscopy at the time of diagnosis. CMV PCR titer from bronchial washing fluid ranged from 6.71 x 103 to 1.5 × 108 copies/ml, and mean value was 9.74 × 106 copies/ml. CMV culture from bronchial washing fluid was positive in 43% (7/16) patients. Cytology from bronchial washing fluid was consistent with CMV virus in 1 case and lung biopsy was done in 4 cases to confirm CMV pneumonia. 75% (12/16) patients with CMV pneumonia had coinfection (5 cases of bacteria, 3 of virus, 2 of pneumocystosis, 4 of aspergillosis, 1 of tuberculosis, 1 of non-tuberculosis mycobacteria, 1 of mycoplasma). 62% (10/16) patients had undergone hematopoietic stem cell transplantation and only 1 patient was in nadir state. All patients were treated with antiviral agent in CMV pneumonia patients. Ganciclovir was used as first line agent in 87% (14/16) patients and 4 patients had changed medication to foscarnet due to treatment failure or complication of ganciclovir. 12% (2/16) patients were treated with foscarnet as first line therapy. 25% (4/16) patients received CMV immunoglobulin. 28-day mortality was 50%. Conclusions: CMV pneumonia is one of the most fatal conditions rarely accompanied in hematologic patients. We analyzed clinical feature in hematologic malignant patients with CMV pneumonia including bronchoscopic findings, coinfection, treatment and prognosis.
