Body condition score (BCS) is a useful management tool for distinguishing differences in nutritional needs of cows in the herd. Although it is not always possible to quantify the nutrient content of the feed supplied to the donor cow, the nutritional status can be determined by the BCS. The objective of this study was to evaluate in vivo embryo production, return to estrous of donor and pregnancy rate of recipients following BCS in Hanwoo superovulation. Sixty nine Hanwoo donor cows were flushed on day 7 of estrus cycle with same FSH and artificial insemination by the same technicians. Embryos were recovered on 7 days after the third insemination by flushing the uterus with Embryo Collection Medium. The results obtained were as follows: No differences were observed in the efficiency of superovulation rates regardless of BCS (≤2.0, 2.5 to 3.0, and ≥3.5). The mean number of total embryos was each 5.20±0.86, 11.56±1.04, and 6.23±1.07. The mean number of transferable embryo from ≤2.0, 2.5 to 3.0, and ≥3.5 of BCS was 2.60±0.87, 7.94±0.89, and 4.75±1.32, respectively (p<0.05). Return to estrous regardless of donor BCS was no difference. The pregnancy rates of recipient were BCS ≤2.0 11.76%, 2.5 to 3.0 40.79%, and ≥3.5 11.11%, following transfer of fresh embryos produced in vivo, respectively. These results indicate that if the Hanwoo with BCS 2.5 to 3.3 are used for donor and recipient, the embryo production and the conception rate will be greater.

The development of in vitro assays has been recommended to screening and test-ing the potential endocrine disruptors (EDs). These assay systems focus only on identifying thethe thyroid hormone (TH) disruptors. The aim of this study was to evaluate a test system to detectTH disruptors using rat pituitary tumor GH3 cells. The test system is based on the TH-dependentincrease in growth rate. As expected, L-3,5,3-triiodothyronine (T3) markedly induced a morphologicalchange in GH 3 cells from flattened fibroblastic types to rounded or spindle-shaped types. T3 stimu-lated GH3 cell growth in a dose-dependent manner with the maximum growth-stimulating effect9 M. In addition, T3 increased the release of growth hor-mone and prolactin into the medium of the GH3 cells culture. Using this assay system, the TH-dis-rupting activities of bisphenol A (BPA) and its related compounds were examined. BPA,dimethylbisphenol A (DMBPA), and TCI-EP significantly enhanced the growth of GH3 cells in therange of 1 × 10-5M to 1 × 10-6M concentrations. In conclusion, this in vitro assay system might bestandardization before it can be used as a broad-based screening tool.

<P><B>Abstract</B></P> <P><B>Background</B></P> <P>Hyponatremia is a well-known risk factor for worse outcomes in heart failure (HF) patients. The impact of hyponatremia according to the ejection fraction (EF) is unclear. We evaluated the prognostic value of hyponatremia according to HF type.</P> <P><B>Methods and results</B></P> <P>The Korea Acute Heart Failure (KorAHF) registry consecutively enrolled 5625 patients. Hyponatremia was defined as serum sodium level<135mmol/L at hospital admission. HF with preserved and reduced ejection fraction were defined as an LVEF ≥50% and LVEF ≤40%, respectively.</P> <P>Among 5103 patients with available EF, 2088 (60%) had HFrEF, and 1284 (25%) had HFpEF. There was no difference in serum sodium level between the groups (HFrEF: 137.4±4.7mmol/L vs. HFpEF: 137.5±5.0mmol/L, P=0.710).</P> <P>Hyponatremic patients had higher in-hospital mortality or urgent heart transplantation in all (11.3% vs. 4.5%, P<0.001), in HFrEF (13.1% vs. 4.9%, P<0.001), and in HFpEF (6.0% vs. 1.9%, P<0.001).</P> <P>After adjustment for significant covariates, hyponatremia was associated with 1.5 fold increased risk for 1-year post-discharge death in the HFrEF group (HR, 1.52; 95% CI, 1.24–1.86), but not in the HFpEF group (HR, 1.16; 95% CI, 0.84–1.61). During admission, the sodium status changed in 22% of the patients and the discharge sodium status had greater prognostic value.</P> <P><B>Conclusions</B></P> <P>Hyponatremia is independent of HF type. Hyponatremia is a significant risk factor for adverse in-hospital outcomes; however its long-term prognostic value is only limited to patients with HFrEF, but not for those with HFpEF.</P>

<P>Although the introduction of human epidermal growth factor receptor (HER)2‐directed therapy including trastuzumab, pertuzumab, lapatinib and trastuzumab emtansine (T‐DM1) in the treatment of HER2‐positive metastatic breast cancers (mBCs) favorably changed the natural history of this disease, most cases of HER2‐positive mBC will eventually progress. Poziotinib is an oral pan‐HER kinase inhibitor showing potent activity through irreversible inhibition of these kinases. This open‐label, multicenter phase II study was designed to evaluate the efficacy and safety of poziotinib monotherapy in patients with HER2‐positive mBC who had progressed from more than two HER2‐directed therapies. Patients received 12 mg poziotinib once daily on a 14‐day on/7‐day off schedule. Progression‐free survival (PFS) as the primary endpoint, the objective response rate (ORR), overall survival (OS) and safety were evaluated. From April 2015 to February 2016, 106 patients were enrolled in the trial from seven institutes in South Korea. They had a median age of 51 years (range 30–76) and had received a median of four prior therapies including two HER2‐directed therapies for advanced or metastatic cancers. The median follow‐up duration was 12 months. The median PFS was 4.04 months (95% confidence interval [CI], 2.94–4.40 months), and median overall survival has not been reached. The most common treatment‐related adverse events were (total/grade ≥3) diarrhea (96.23%/14.15%), stomatitis (92.45%/12.26%) and rashes (63.21%/3.77%). Poziotinib showed meaningful activity in these heavily treated HER2‐positive mBCs. Diarrhea and stomatitis were the major toxicities. Biomarker studies analyzed are warranted to support further evaluation of this treatment in such cases.</P>
Public Trust in Community Pharmacists in South Korea : A Survey Study

Background: Trust is a key component for the good relationship between patients and healthcare professionals but trust for community pharmacists has not been studied much. Objectives: This study aimed to measure public trust in community pharmacists and to investigate variables that affect trust level in South Korea. Methods: A total of 25 questions, including 13-items for three dimensions of trust (pharmacists’ behavior/attitude, technical competence, communication skills) and 1-item for overall trust were developed. The survey was conducted online and the data from 416 respondents were analyzed with a t-test, an ANOVA and a multiple linear regression analysis. Results: The average scores (mean ± standard deviation) for the three dimensions of trust in community pharmacists were 3.47±1.05 (out of 5 points) for pharmacists’ behavior/attitude, 3.67 ± 0.99 for technical competence, and 3.66±0.99 for communication skills. The average of the 13 items incorporating all parameters was 3.56±1.02 and the overall trust level was 7.16±1.62 (out of 10 points). The total sum of the 13 items differed significantly by age group (p=0.02) and frequency of pharmacy visits (p=0.04). Each dimension had an independent impact on the trust level, and pharmacists’ behavior/attitude had the greatest impact on trust levels. Conclusions: This study showed that pharmacists’ behavior/attitude had the most significant impact on the trust level. However, the level of trust in pharmacists’ behavior/attitude is not yet sufficiently satisfactory, and further improvements are required to increase trust in community pharmacists.

Aims: METAVIR staging classifies structural deformation caused by hepatic fibrosis semi-quantitatively. However, there could be disagreement of fibrosis staging by METAVIR among pathologists. Quantification of fibrosis using computer-assisted image analysis can offer relative objective information for liver fibrosis. We measured hepatic fibrosis quantitatively using collagen proportionate area (CPA) and assessed its impact on predicting the development of liver decompensation (which was defined as the presence of ascites, variceal bleeding and hepatic encephalopathy). Methods: During January 2010 to June 2018, we assessed 582 patients who got liver biopsy and computer assisted image analysis (ZEN 2.3 lite software by ZEISS) were available. Clinical and laboratory data were collected at baseline and at the time of the last follow-up or progression to liver decompensation (LD). Forty-two patients with acute hepatitis who had no underlying chronic liver disease were excluded. Results: The mean age was 45.3±13.7 years, and most common etiology of liver disease was chronic hepatitis B (28.6%) and followed by fatty liver disease (26.9%). Median follow-up duration was 37 months during which 28 out of 540 patients experienced LD. Mean analyzed dimension of collagen was 5653362±2423925 μm2 and included portal tract was 8.9±3.9. Mean CPA was 8.91±7.10%. A positive correlation between CPA and liver fibrosis stage was observed (r=0.553, P<0.001) (Figure 1). Albumin at baseline (HR: 0.257, 95% CI: 0.094- 0.701, P=0.008), CPA (HR: 1.107 per 1% increase, 95% CI: 1.059-1.157, P<0.001), presence of diabetes mellitus (HR: 4.315, 95% CI: 1.063-17.510, P=0.041), and presence of alcoholic hepatitis (reference : chronic hepatitis B) (HR : 5.811, CI : 1.351-24.987, P=0.018) were independent predictors of liver decompensation on multivariate Cox-regression analysis. The concordance indices of CPA and METAVIR stage for progression to LD were 0.803±0.044 and 0.758±0.041, respectively, without significant difference. When dividing patients with calculating cut-point with maximally selected rank difference, higher CPA (≥16.6%) predicts LD better than lower CPA (Logrank test: P<0.001) (Figure 2). Conclusions: The CPA correlates very well with the METAVIR stage of liver fibrosis and also is an independent predictor of clinical outcomes in liver disease. It is expected to be useful quantitative determination of liver fibrosis and prognosis.

<P><B>Abstract</B></P><P>Senescence marker protein 30 (SMP30), an important aging marker molecule that is highly expressed in the liver, has been known to protect hepatocytes from apoptosis by the synthesis of vitamin C. To explore the function of SMP30 in liver fibrosis, the effect of SMP30 deficiency on liver fibrosis was investigated in SMP30 knockout (KO) mice. Moreover, the <I>in vivo</I> results were further confirmed by way of hepatic stellate cell (HSC) isolation. We demonstrated that carbon tetrachloride (CCl<SUB>4</SUB>)‐induced liver fibrosis and the nuclear translocation of p‐Smad2/3, the immediate downstream of transforming growth factor beta (TGF‐β), were significantly inhibited in the liver of SMP30 KO mice compared with wildtype (WT) mice. We also confirmed that both WT and SMP30 KO HSCs did not express SMP30. Finally, we further confirmed that up‐regulation of peroxisome proliferator‐activated receptor‐gamma (PPAR‐γ) caused by a lack of vitamin C was the pivotal factor in the mechanisms for attenuated liver fibrosis of SMP30 KO mice, and feeding with vitamin C restored CCl<SUB>4</SUB>‐induced liver fibrosis in SMP30 KO mice. <I>Conclusion:</I> Vitamin C deficiency by SMP30 depletion attenuated liver fibrosis by way of up‐regulated PPAR‐γ expression in SMP30 KO mice. Our results provide, for the first time, the possible mechanisms underlying inhibition of HSC activation associated with vitamin C and PPAR‐γ up‐regulation in liver fibrosis of SMP30 KO mice. 국내 육성 품종인 ‘매향’ 딸기의 육묘 기간 중 광합성유효광량자속의 환경 조절을 통하여 러너 및 러너플랜트의 발생과 생산 효율을 증대시키고자 하였다. 딸기 육묘에 이용된 증식체의 엽수와 관부 직경은 각각 3.1±0.4와 7.0±1.1㎜였다. 광합성유효광량자속을 각각 140, 210 및 280μmolㆍm?²ㆍs?¹로 설정한 9기의 인공광 이용형 육묘 모듈에서 35일 동안 육묘하였다. 러너플랜트 생산 효율은 증식체당 일일 0.27주로 처리구 중 유의하게 가장 높았으며 관행의 딸기 육묘 방식의 러너플랜트 생산 효율과 비교하여도 크게 증진되었음을 확인하였다. 폐쇄형 육묘 시스템을 활용하여 육묘 기간 중의 PPF를 280μmolㆍm?²ㆍs?¹로 조절하면 국내 육성 품종인 ‘매향’의 러너 발생 및 러너플랜트 생산을 증진시킬 수 있음이 확인되었다. 따라서 최근 국내에서 육성된 신품종 딸기의 급속 보급을 위한 증식체계 구축에 본 연구 기술이 활용되면 육성된 품종의 조급 보급이 가능할 것으로 판단된다. The formation and elongation of runners, growth of runner plants, and transplant propagation rates of 'Maehyang' strawberry were investigated at various photosynthetic photon flux (PPF) levels. Strawberry plants having 3.1 ± 0.4 leaves and 7.0 ± 1.1 ㎜ of crown diameter were used as propagules and were cultured for 35 days in 9 transplant production modules using fluorescent lamps as artificial lighting sources. Applied PPF levels were 137.4 ± 2.1, 217.0 ± 1.0, and 274.7 ± 8.4 μmolㆍm?²ㆍs?¹ as measured on the surfaces of empty shelves. The numbers of runners and runner plants per propagule were the greatest at 280 μmolㆍm?²ㆍs?¹ PPF. The runner plant propagation rate was 0.27 plant/day/propagule at 280 μmolㆍm?²ㆍs?¹, which was significantly greater than that of conventional propagation methods. Results indicate that high PPF levels promotes the formation of runners and runner plants of strawberry and that the rapid propagation method with high PPF levels can be feasible for production of vigorous transplants in a closed transplant production system.
