        특발성 자발성 신장 파열 1예

        Primary aldosteronism is a syndrome characterized by hypertension, hypokalemia, suppressed plasma renin activity, and elevated serum aldosterone levels. Conn first described it 1955 in association with adrenal cortical adenoma(Conn's syndrome). Today, it can be divided into at least six distinctive sub-groups. The diagnosis of primary aldosteronism is usually suspected when the patient presents with poorly controlled hypertension with hypokalemia. The main issues in the evaluation of primary aldosteronism are to differentiate an adenoma from hyperplasia and to localize the adenoma. Basic hormonal studies and computed tomographic(CT) scanning can be used effectively for that differentiation. An adenoma is one of a few potentially curable forms of hypertension, and it is best treated by removing the adrenal tumor. We experienced a case of a typical adrenal adenoma. The patient was a 37-year-old male who had experienced in both his legs over a period of several days a weakness due to hypokalemia. He was diagnosed using basic hormonal studies and adrenal CT scanning. After, he received laparoscopic adrenalectomy and was discharged with improved condition.

        Ni-Zn Ferrite는 1200℃이상의 온도에서는 소결성이 좋으며 Bi₂O₃를 첨가하여 소결한 것은 1250℃에서 최대투자율 2050을 얻었으며 보자력은 0.13 Oe로서 가장 낮은 값을 나타내었다. 그러나 주파수에 따른 초투자율은 높은 주파수 영역에서는 다른 조건에서 소결시킨 시료에 비하여 낮은 값을 가진 것으로 나타났다. 사용되는 주파수의 영역에 따라서 적절한 소결온도, 적절한 첨가물이 결정될 수 있을 것으로 예상된다. The magnetic properties of Ni-Zn ferrites with additive(Bi₂O₃;0.3wt%)and without additive have been measured. The maximum permeability is 2050, the initial permeability is 1538 at 1.55MHz and the coersive force is 0.130Oe.

        The K-edge absorption spectra of Fe and FeO are measured by X-ray absorption spectroscope Comparisons to theory are made for both the Fe and FeO data. We attempt to interpret origins of the spectral near-edge structure. For themetal Fe, We find agreement between the data and one-electron calculations. Peaks of near edge structure of FeO can not explain with one-electron transition model. For explanation of these peak, we find that the core-exciton and many-body effects must be taken into account. From the extended X-ray absorpticn fine structure (EXAFS), we determined distance to the nearest neighbour. The results agree with the value from the X-ray diffraction experiment.

        In this paper we describe a visualization method for Bus Information System(BIS) on Mobile Environment. A BIS is a key service for Intelligent Transport System(ITS) on public transportation area. We present a client-server based visualization technique for bus route information over Internet Protocol and user-oriented regional information service with a voice and animation. We also describe the design and implementation of this kind of schematic map generation system with icon and label.

        Skin entrance dose for digital and film radiography in Korean dental schools

        Purpose : This study was aimed to compare skin entrance dose of digital radiography with that of film radiography and to show the dose reduction achievement with digital systems at 11 dental schools in Korea. Materials and Methods : Forty six intraoral radiographic systems in 11 dental schools were included in this study. Digital sensors were used in 33 systems and film was used in 13 systems. Researchers and the volunteer visited 11 dental schools in Korea. Researchers asked the radiologic technician (s) at each school to set the exposure parameters and aiming the x-ray tube for the periapical view of the mandibular molar of the volunteer. The skin entrance doses were measured at the same exposure parameters and distance by the technician for each system with a dosimeter (Multi-O-Meter : Unfors instruments, Billdal, Sweden). Results : The median dose was 491.2 μGy for digital radiography and 1,205.0 μGy for film radiography. The skin entrance dose digital radiography was significantly lower than that of film radiography (p<0.05). Conclusion : Fifty-nine percent skin entrance dose reduction with digital periapical radiography was achieved over the film radiography in Korean dental schools.

        Clinical Outcomes of Intrascleral Fixation of Intraocular Lens Compared to Ciliary Sulcus Implantation and Transscleral Fixation

        Purpose: To compare the clinical outcomes of intrascleral fixation of the three-piece intraocular lenses (IOLs) 2.5 mm posteriorto the limbus with ciliary sulcus implantation and transscleral fixation 2.5 mm posterior to the limbus. Methods: Sixty-five eyes of 65 patients who underwent ciliary sulcus implantation or transscleral or intrascleral fixation ofthe AMO Sensar AR40e IOL were retrospectively reviewed. The postoperative refractive prediction error, back-calculatedeffective lens position (ELP), corrected distance visual acuity (CDVA), and postoperative residual cylinder were compared. Results: There were significant differences in the median (interquartile range) postoperative refractive prediction error (diopters[D]) among the three groups (p < 0.001): for ciliary sulcus implantation (33 eyes), −0.89 D (−1.21 to −0.56 D); for transscleralfixation (10 eyes), −0.40 D (−0.78 to −0.22 D); and for intrascleral fixation (22 eyes), 0.01 D (−0.28 to 0.34 D). Significantdifferences (p < 0.001) were observed in the median back-calculated ELP: for ciliary sulcus implantation, 4.35 mm (3.95 to4.55 mm); for transscleral fixation, 4.51 mm (4.34 to 4.76 mm); and for intrascleral fixation, 4.90 mm (4.56 to 5.35 mm). Therewere no differences in the median postoperative CDVA (0, 0.10, and 0 logarithm of the minimum angle of resolution, respectively;p = 0.083) and the residual cylinder (−0.75, −1.50, and −0.63 D, respectively; p = 0.074) among three groups. Conclusions: Intrascleral fixation showed no myopic shift and the most posterior lens position, while ciliary sulcus implantationinduced the greatest myopic shift and the most anterior lens position. However, there was no significant difference in thepostoperative CDVA or astigmatism among the eyes with different IOL insertion methods, demonstrating good IOL stabilityand vision outcomes.

        우수한 전기화학적 센싱 성능을 지닌 티타늄 와이어 기반의 휴대 및 일회용 pH 센서 개발

        본 연구에서는 하이드로늄 이온 농도를 모니터링하기 위한 티타늄(Ti) 와이어를 기반으로 하는 휴대용 일회용 pH 센서를 성공적으로 개발하였다. 센싱 전극은 Ti 와이어에 이리듐 산화물을 전기화학적으로 증착하여 제조하였고, 기준전극으로 Ti 와이어에 Ag/AgCl 잉크를 코팅하였다. pH 센서에 두 개의 전극을 결합한 후 pH 센서를 다양한 pH 용액에 담그면 개방 회로 전위 신호를 수집할 수 있다. 상기 제조된 pH 센서는 감도, 응답 시간, 반복성, 선택도 및 안정성 측면에서 우수한 전기화학적 감지 성능을 보여 주었다. 현장 검사 응용을 시연하기 위해서 pH 센서를 모바일 애플리케이션과 통신할 수 있는 무선 전자 모듈과 통합하였다. 이 휴대용 pH 센서는 실제 샘플의 pH 변화를 정확하게 측정했으며 결과는 상용 pH 측정기의 데이터와 일치하였다. A portable and disposable pH sensor based on Ti wire was successfully developed for monitoring hydronium ion concentrations. A sensing electrode was prepared by electrochemically depositing iridium oxide onto a Ti wire, while a reference electrode was fabricated by coating Ag/AgCl ink on a Ti wire. Combining the two electrodes in the pH sensor enabled the collection of open circuit potential signals when the sensor was immersed in solutions of various pH values. The pH sensor exhibited excellent electrochemical sensing performance in terms of sensitivity, response time, repeatability, selectivity, and stability. To demonstrate point-of-measurement applications, the pH sensor was integrated with a wireless electronic module that could communicate with a mobile application. The portable pH sensor accurately measured pH changes in real samples. The results obtained were consistent with those of using a commercial pH meter.

        The Effects of Subliminal Music with Balance Imagery Training on Balance and Concentration

        Purpose: The purpose of this study was to estimate the effects of subliminal music with balance imagery training on balance and concentration. Methods: The participants were 45 seniors in an undergraduate school in Korea. The subliminal music with balance imagery training intervention was carried out for 20 minutes. Other interventions were also carried out for 20 minutes. 12 seniors(Group A) listened to subliminal music with balance imagery training, 12 seniors(Group B) listened to subliminal music, 11 seniors(Group C) received balance imagery training, and 10 seniors(Group D) had no intervention(Control group). The grid test is related to measured levels of concentration intensity. Romberg one legged standing test was carried out for 30 seconds. The collected data was analyzed by one-paired t test and one way ANOVA using the SPSS Windows 12 ver. program. Results: The major findings of this study were as follows: Concentration levels of Group A and C improved, and balance levels of Group C and D improved. There was a statistically significant decrease in concentration between Group A and B, Group A and C after intervention. Conclusion: These findings suggest that listening to subliminal music with balance imagery training may be useful in managing concentration in seniors. So it provides basic information for further concentration on improving education on music with balance imagery training.

      • PDK4 Deficiency Suppresses Hepatic Glucagon Signaling by Decreasing cAMP Levels

        <P>In fasting or diabetes, gluconeogenic genes are transcriptionally activated by glucagon stimulation of the cAMP-protein kinase A (PKA)-CREB signaling pathway. Previous work showed pyruvate dehydrogenase kinase (PDK) inhibition in skeletal muscle increases pyruvate oxidation, which limits the availability of gluconeogenic substrates in the liver. However, this study found upregulation of hepatic PDK4 promoted glucagon-mediated expression of gluconeogenic genes, whereas knockdown or inhibition of hepatic PDK4 caused the opposite effect on gluconeogenic gene expression and decreased hepatic glucose production. Mechanistically, PDK4 deficiency decreased ATP levels, thus increasing phosphorylated AMPK (p-AMPK), which increased p-AMPK-sensitive phosphorylation of cyclic nucleotide phosphodiesterase 4B (p-PDE4B). This reduced cAMP levels and consequently p-CREB. Metabolic flux analysis showed that the reduction in ATP was a consequence of a diminished rate of fatty acid oxidation (FAO). However, overexpression of PDK4 increased FAO and increased ATP levels, which decreased p-AMPK and p-PDE4B and allowed greater accumulation of cAMP and p-CREB. The latter were abrogated by the FAO inhibitor etomoxir, suggesting a critical role for PDK4 in FAO stimulation and the regulation of cAMP levels. This finding strengthens the possibility of PDK4 as a target against diabetes.</P>
