We previously reported that the extract of Taraxacum platycarpum (AF-343) had several biological properties such as skin hydration and anti-inflammatory effects, thereby AF-343 be a promising anti-atopic dermatitis agent. However, few studies have been conducted to evaluate its effect on modulation of extracellular matrix proteins in human skin fibroblasts. The purpose of this study was to investigate the expressions of type I collagen, MMP-1, Smad2/3, and TIMP-1 proteins in AF-343-treated human skin fibroblasts. Human skin fibroblasts were treated by various concentrations of AF-343 (0~2 mg/ml). The expressions of type I collagen, matrix metalloproteinase-1 (MMP-1), Smad2/3, and TIMP-1 proteins were analyzed by Western blot analysis. In addition, level of type I collagen mRNA was analyzed by CAT assay. Expression of type I collagen protein was increased in AF-343-treated human skin fibroblasts by dose and time-dependent manners. Consistent with this result, the expressions of phospho-Smad2/3 in skin fibroblasts were increased and MMP-1 expression was decreased by AF-343 treatment. TIMP-1 expression was not significantly changed in AF-343 treated skin fibroblasts. Extract of Taraxacum platycarpum (AF-343)-induced up-regulation of type I collagen expression was through increased expression of phospho-Smad2/3. These results were occurred combined with down-regulation of MMP-1 in skin fibroblasts. Taken together, this study indicated that AF-343 has property of the modulation of ECM in tissue as well as skin hydration and anti-inflammation.

Hypoglycemia associated with the administration of drugs has been reported previously. Here, we report a case of hypoglycemia induced by Orthosiphon stamineus, which is a popular medicinal plant in Southeast Asia. A 27-year-old man was admitted to our hospital with tremor. He had ingested medication that included orlistat 360 mg and Orthosiphon stamineus extract 900 mg daily for more than 2 months to treat obesity. He had not experienced hypoglycemia prior to starting the above-mentioned medication and his symptoms disappeared rapidly with a carbohydrate-rich diet and discontinuation of the medication. Blood analysis after fasting provocation for 24 hours showed hypoglycemia (glucose level, 3.44 mmol/L) and simultaneous serum levels of insulin, C-peptide, and proinsulin were increased above normal to 17,835.45 pmol/L, 2.33 mmol/L, and 709 pmol/L, respectively. We report a case of hypoglycemia caused by Orthosiphon in a young non-diabetic man and propose that Orthosiphon stamineus extract, either alone or in combination with orlistat, might be beneficial for controlling glucose levels in diabetic patients. Statin이 혈중 근효소에 미치는 영향을 알아보기 위하여 이 연구를 시행하였으며, cerivastatin을 사용한 환자 군에서 다른 statin 사용 군보다 의미 있는 CK 및 AST치의 상승을 보였다(p<0.05). 또한 CK가 기준치보다 3배 이상 증가된 환자도 cerivastatin을 사용한 환자 군에서는 발생하였으나, 다른 statin 사용 군에서는 발생하지 않았다(p<0.05). Hypercholesterolemia is firmly established as a risk factor for the development of atherosclerosis. Cerivastatin is a third generation statin, a synthetic HMG-CoA reductase inhibitor which lowers plasma cholesterol level by inhibiting endogenous cholesterol synthesis. It has also been reported that cerivastatin is relatively well tolerated and has a low incidence of adverse effects. It has been also reported that less than 1% of the patients treated with cerivastatin experienced clinically significant elevation of muscle enzymes. The purpose of this study was to investigate the effect of cerivastatin on serum muscle enzymes. The patients with primary hypercholesterolemia were treated with statin at least 4 weeks in Keimyung University Dongsan Medical Center. We compared the effect of Cerivastatin treatment (group A, 33 patients) on changes of muscle enzymes including creatine kinase (CK), aspartate transaminase (AST) and lac-tate dehydrogenase (LDH) with other statin treatment group (group B, 46 patients). The group A produced statistically significant elevation of CK and AST (p<0.05) after treatment with cerivastatin when compared to the group B. More than three fold elevation of CK level was developed only in the group A (p=0.02). These results indicate that cerivastatin induces higher incidence of muscle enzymes elevation than other statins.

Infectious mononucleosis is characterized by fever, sore throat, lymphadenopathy, atypical lymphocytosis and heterophil antibody. Icteric hepatitis is rare in infectious mononucleosis. We report a patient with icteric infectious mononucleosis hepatitis. The diagnosis of infectious mononucleosis was confirmed by the demonstration of high serum titier of antibodies against Epstein-Barr virus antigens. Epstein-Barr virus infection causing hepatitis should be suspected when tests for other hepatotrophic viral infections were negative.
