Hypermedia 학습자료를 활용한 수업이 초등학생의 영양과 건강 단원에 대한 선개념과 오개념 교정에 미치는 효과

The purpose of this study is to develop the instructional strategy used hypermedia(ISH) in health-related subject of 6th grade in elementary school science and to verify the difference this ISH to the typical instructional strategy used lecture in correcting the students' preconceptions and misconceptions. The experimental class is treated with ISH, while the control class is treated with the typical instructional strategy used lecture. The results shows that (1) ISH is significantly higher than the typical strategy used lecture to correct the students' preconceptions and misconceptions at 'nutrition and digestion', 'circulation and 'excretion' themes of health-related subject. (2) ISH is not significantly different from typical strategy used lecture to correct the students' preconception and misconception at 'respiration' theme of health-related subject. This results suggest that the instructional strategy used hypermedia has positive influence on changing the students' preconceptions and misconceptions.
Elevated BMPR2 expression amplifies osteoblast differentiation in ankylosing spondylitis

Objective: Bone morphogenetic protein receptor type 2 (BMPR2) has been associated with radiographic changes in ankylosing spondylitis (AS), but further characterization of the cellular signaling pathway in osteoprogenitor (OP) is not clearly understood. The aim of this study was to investigate the expression of BMPR2 and bone morphogenetic protein 2 (BMP2)-mediated responsibility in AS. Methods: We collected 10 healthy control (HC) and 14 AS-OPs derived from facet joints. Subsequently, we then conducted RNA sequencing with two samples per group and selected BMP-related genes. Facet joint tissues and derived primary OPs were evaluated by validation of selected RNA sequencing data, immunohistochemistry, and comparison of osteogenic differentiation potential. Results: Based on RNA-sequencing analysis, we found that BMPR2 expression is higher in AS-OPs compared to in HC-OPs. We also validated the increased BMPR2 expression in facet joint tissues with AS and its derived OPs in messenger RNA and protein levels. Additionally, primary AS-OPs showed much greater response to osteogenic differentiation induced by BMP2 and a higher capacity for smad1/5/8-induced RUNX2 expression compared to HCs. Conclusion: The expression of BMPR2 was found to be significantly increased in facet joint tissues of patients with AS. These findings suggest that BMPR2 may play a role in the BMP2-mediated progression of AS.

This study, which employed HBM, showed that perceived susceptibility and self-efficacy among other HBM variables were found to be the only predictive variables with direct effect on tuberculosis preventive behavior intention in Korea. However, the effect of perceived susceptibility is minimal, insufficient to explain tuberculosis preventive behavior intention. Consequently, self-efficacy was found to be the only significant variable that can predict tuberculosis preventive behavior intention in Korea, among perceived severity, perceived susceptibility, perceived benefit, perceived barrier and self-efficacy, which were known to be predictive variables for HBM. In contrast to the precedent studies both home and abroad, this study showed that perceived severity and perceived susceptibility-the major HBM variables - as well as perceived benefit and perceived barrier that continued to be supported as predictive variables did not have direct effect on health preventive behavior intention. Cues to action was also found to have no direct effect on tuberculosis preventive behavior intention. However, perceived severity, perceived susceptibility, perceived benefit, perceived barrier and cues to action had indirect effect, rather than direct, on tuberculosis preventive behavior intention. Perceived severity and perceived susceptibility affected tuberculosis preventive behavior intention by way of fear, while perceived benefit and perceived barrier had effect by way of self-efficacy. This study showed that perceived severity and perceived susceptibility as cognitive factors have indirect, rather than direct, effect on tuberculosis preventive behavior intention by way of fear. This indicates that predicting health preventive behavior intention with only cognitive variables of perceived severity and perceived susceptibility toward certain diseases, without fear, has its limitations. Thus it is necessary for the future health communication studies to actively consider fear and other emotional factors.
Pediatric Vascular Surgery Review with a 30-Year-Experience in a Tertiary Referral Center

Pediatric vascular disease is rare, and remains a big challenge to vascular surgeons. In contrast to adults, surgery for pediatric vascular disease is complicated by issues related to small size, future growth, and availability of suitable vascular conduit. During the last 30 years, 131 major vascular operations were performed in a tertiary referral center, Seoul National University Hospital, including aortoiliac aneurysm, acute or chronic arterial occlusion, renovascular hypertension, portal venous hypertension, trauma, tumor invasion to major abdominal vessels, and others. Herein we review on the important pediatric vascular diseases and share our clinical experiences on these rare diseases.
Femoral Endovenectomy with Iliac Stenting for Chronic Iliofemoral Venous Occlusion

A 62-year-old man with progressive swelling on his left thigh and leg was referred to the Division of Vascular Surgery. Anticoagulation therapy did not prevent or improve the symptoms of chronic deep vein thrombosis (DVT) from his left external iliac vein to posterior tibial vein. He had no trauma history nor any accidents. He underwent femoral endovenectomy, thrombectomy and stent insertion in left iliac vein. The patient had additional balloon angioplasty for stenosis in left common femoral vein. He had an uneventful postoperative recovery without complication. Leg swelling has been improving and follow-up continues under anticoagulation. We report a case of femoral endovenectomy with iliac stenting, which may be an efficacious treatment for chronic DVT.
하지정맥류의 3세대 치료법: 혈관 내 접착제 폐색술과 기계약물 폐색술

Background: The mainstream of incompetent saphenous veins treatment has dramatically changed from the first-generation conventional high ligation and stripping surgery to the second-generation endovenous thermal ablation as a minimally invasive technique using laser or radiofrequency. The third-generation treatment of non thermal non-tumescent techniques is already available, including cyanoacrylate adhesive closure (CAC) and mechanochemical ablation (MOCA). Current Concepts: The non-thermal non-tumescent techniques are developed to overcome the complications of thermal ablation, including nerve injury and vein perforation. The technique also reduces the need for painful tumescent anesthesia and postoperative compression. MOCA employs a dual injury using a single-catheter-based delivery system consisting of a mechanical abrasion with a rotating wire and chemical ablation with a sclerosant. CAC is executed by injecting a glue that produces a polymer with the blood and obliterates the lumen. CAC does not need tumescent anesthesia and postoperative compression. Therefore, patient recovery is fast and satisfaction is excellent. However, the new devices are expensive and not included in the medical insurance in Korea, and long term effects of the new treatments are unproven; therefore, the cost-effectiveness is unconfirmed. Discussion and Conclusion: MOCA and CAC are newly developed minimal invasive treatments for varicose veins. They are reported to be safe and effective techniques. However, further studies are needed to evaluate the long-term outcomes and cost-effectiveness.
