Elevated BMPR2 expression amplifies osteoblast differentiation in ankylosing spondylitis

Objective: Bone morphogenetic protein receptor type 2 (BMPR2) has been associated with radiographic changes in ankylosing spondylitis (AS), but further characterization of the cellular signaling pathway in osteoprogenitor (OP) is not clearly understood. The aim of this study was to investigate the expression of BMPR2 and bone morphogenetic protein 2 (BMP2)-mediated responsibility in AS. Methods: We collected 10 healthy control (HC) and 14 AS-OPs derived from facet joints. Subsequently, we then conducted RNA sequencing with two samples per group and selected BMP-related genes. Facet joint tissues and derived primary OPs were evaluated by validation of selected RNA sequencing data, immunohistochemistry, and comparison of osteogenic differentiation potential. Results: Based on RNA-sequencing analysis, we found that BMPR2 expression is higher in AS-OPs compared to in HC-OPs. We also validated the increased BMPR2 expression in facet joint tissues with AS and its derived OPs in messenger RNA and protein levels. Additionally, primary AS-OPs showed much greater response to osteogenic differentiation induced by BMP2 and a higher capacity for smad1/5/8-induced RUNX2 expression compared to HCs. Conclusion: The expression of BMPR2 was found to be significantly increased in facet joint tissues of patients with AS. These findings suggest that BMPR2 may play a role in the BMP2-mediated progression of AS.
Hypermedia 학습자료를 활용한 수업이 초등학생의 영양과 건강 단원에 대한 선개념과 오개념 교정에 미치는 효과

The purpose of this study is to develop the instructional strategy used hypermedia(ISH) in health-related subject of 6th grade in elementary school science and to verify the difference this ISH to the typical instructional strategy used lecture in correcting the students' preconceptions and misconceptions. The experimental class is treated with ISH, while the control class is treated with the typical instructional strategy used lecture. The results shows that (1) ISH is significantly higher than the typical strategy used lecture to correct the students' preconceptions and misconceptions at 'nutrition and digestion', 'circulation and 'excretion' themes of health-related subject. (2) ISH is not significantly different from typical strategy used lecture to correct the students' preconception and misconception at 'respiration' theme of health-related subject. This results suggest that the instructional strategy used hypermedia has positive influence on changing the students' preconceptions and misconceptions.

This study, which employed HBM, showed that perceived susceptibility and self-efficacy among other HBM variables were found to be the only predictive variables with direct effect on tuberculosis preventive behavior intention in Korea. However, the effect of perceived susceptibility is minimal, insufficient to explain tuberculosis preventive behavior intention. Consequently, self-efficacy was found to be the only significant variable that can predict tuberculosis preventive behavior intention in Korea, among perceived severity, perceived susceptibility, perceived benefit, perceived barrier and self-efficacy, which were known to be predictive variables for HBM. In contrast to the precedent studies both home and abroad, this study showed that perceived severity and perceived susceptibility-the major HBM variables - as well as perceived benefit and perceived barrier that continued to be supported as predictive variables did not have direct effect on health preventive behavior intention. Cues to action was also found to have no direct effect on tuberculosis preventive behavior intention. However, perceived severity, perceived susceptibility, perceived benefit, perceived barrier and cues to action had indirect effect, rather than direct, on tuberculosis preventive behavior intention. Perceived severity and perceived susceptibility affected tuberculosis preventive behavior intention by way of fear, while perceived benefit and perceived barrier had effect by way of self-efficacy. This study showed that perceived severity and perceived susceptibility as cognitive factors have indirect, rather than direct, effect on tuberculosis preventive behavior intention by way of fear. This indicates that predicting health preventive behavior intention with only cognitive variables of perceived severity and perceived susceptibility toward certain diseases, without fear, has its limitations. Thus it is necessary for the future health communication studies to actively consider fear and other emotional factors.

Purpose: Percutaneous transluminal angioplasty (PTA) shows an attractive outcome for treating infrainguinal arterial occlusive disease (IAOD). However, this procedure requires repeated revascularization and additional stenting. Directional atherectomy (DA) has been reported to be a modality that requires less frequent use of stenting. The aim of this study was to compare the duplex-based outcomes between PTA and DA for treating IAOD. Methods: A retrospective review of consecutive patients with IAOD treated with PTA and DA was completed. The demographics and procedural data were obtained. A provisional stent was placed in those cases with flow-limiting dissection and residual stenosis after PTA or DA. Technical success was defined as a residual stenosis < 30%. The primary patency rate was evaluated by performing duplex scanning. The clinical outcomes and frequency of stent placement for each procedure were evaluated. For statistical analysis, the data was analyzed using SPSS 22.0 software (IBM Corp, Chicago, Ill). All P values were considered significant if less than 0.05. Results: Seventy-one patients were included in the study. Their mean age was 71.5 ± 10.2 years. The male-to-female ratio was 49:22. Thirty-three patients underwent PTA. DA was performed in 38 patients (42 limbs). Technical success was achieved in all the patients. The duplex-based clinical outcome and limb salvage rate were similar in the two groups. Bailout stent placement was performed in 8 of 33 patients (24.2%) in the PTA group and in no patient in the DA group. Conclusion: The clinical outcomes were similar for both the DA and primary PTA groups. Atherectomy reduced the need for bailout stent placement as compared with bailout stent placement in primary PTA.

A 71-year-old woman presented with an enlarging mass in the right buttock, with pain and tingling sensation in sitting position. Five years ago, she was diagnosed with acute limb ischemia due to acute thrombosis of right persistent sciatic artery (PSA), and she underwent successful thromboembolectomy and femorotibioperoneal trunk bypass. Computed tomography angiography revealed a huge PSA aneurysm (PSAA). During the previous bypass, the distal popliteal artery was ligated just above the distal anastomosis to exclude the PSAA, whose proximal end was already thrombosed. However, PSAA has grown to cause compression symptoms, and the mechanism of aneurysm growth can be ascribed to type 1a or type 2 endoleak. In order to relieve the compression symptoms, aneurysm excision was performed without any injury to the sciatic nerve. A postoperative tingling sensation due to sciatic-nerve stimulation in the supine position resolved spontaneously one month after surgery.
Femoral Endovenectomy with Iliac Stenting for Chronic Iliofemoral Venous Occlusion

A 62-year-old man with progressive swelling on his left thigh and leg was referred to the Division of Vascular Surgery. Anticoagulation therapy did not prevent or improve the symptoms of chronic deep vein thrombosis (DVT) from his left external iliac vein to posterior tibial vein. He had no trauma history nor any accidents. He underwent femoral endovenectomy, thrombectomy and stent insertion in left iliac vein. The patient had additional balloon angioplasty for stenosis in left common femoral vein. He had an uneventful postoperative recovery without complication. Leg swelling has been improving and follow-up continues under anticoagulation. We report a case of femoral endovenectomy with iliac stenting, which may be an efficacious treatment for chronic DVT.
