Male patients with hypogonadism have an increased risk of developing rheumatic diseases. Most causes of hypogonadism related with rheumatic disease are karyotype abnormality such as Klinefelter’s syndrome or Turner’s syndrome and gonadal dysgenesis. A 24-year-old year male was admitted for pain of both hip joints that had worsened over 2 months. He had hip joint involvement from ankylosing spondylitis and did not show secondary sex characteristics. His sex hormones and gonadotropins levels indicated hypergonadotropic hypogonadism. The karyotype was 46 XY, and there was no obvious cause of hypogonadism. Here we report on clinical features of this first Korean case of primary hypogonadism accompanying ankylosing spondylitis.
응급실에 내원한 파상풍 환자의 임상 양상 및 예후 영향 인자 분석

Purpose: Tetanus is difficult for emergency physician to diagnosis at an early stage because of its low incidence and atypical initial clinical features. Therefore, we reviewed the clinical features of 54 tetanus cases and attempted to identify factors associated with prognosis in order to improve early diagnosis and treatment. Methods: We carried out a retrospective chart review of 54 adult tetanus patients who presented to the ED of Chonnam National University Hospital between January 1996 and December 2005. We collected information on demographics, SAPS II score, clinical feature, course of infection and prognosis. We divided the patients to two groups according to the survival and compared their data statistically. Results: Fifty four patients, 33 men and 21 women, were included in the study. The mean age was 50.97±15.95. The most common initial diagnosis in ED were electrolyte imbalance (n=15), neck and facial dystonia (n=9). The mean incubation period was 10.27±6.47 days. The initial SPAS II score 17.29±8.45 and the Wood score was 3.12 ±1.47. Most commonly symptom was dyspnea and dysarthria. Twenty one patients were supported by mechanical ventilation, and the duration of mechanical ventilation was 5.92±4.52 days. On multivariate logistic regression analysis, four variables were found to be associated with the prognosis: incubation period, Wood score, Time interval from initial symptom to mechanical ventilation, initial SAPS II. Conclusion: Four variables correlated well with the prognosis: Incubation period, Wood score, Time interval from initial symptom to mechanical ventilation, initial SAPS II score.

‘코로나19 대유행의 종식과 예방을 목표로 더 건강하고, 안전하고, 공정한 세상’을 만들자는 주제의, 제74차 세계보건총회(WHA)가 개최되었다. 본 고에서는 코로나19 백신 이행 사항과 평가 결과를 검토하였다. 이를 계기로 코로나19 백신 수급 불균형 문제 해결을 위한 공정한 분배 전략이 조속히 범세계적으로 확립되길 기대해 본다.

목적 : 국내 작업치료사의 필수 역량을 정의하여 작업치료사의 업무범위 및 필요로 하는 유무형의 역량을명확히 함과 동시에 작업치료 교육에 있어 구체적이고 측정 가능한 방향성을 제시하고자 하였다. 연구방법 : 국내외 작업치료사 역량관련 자료들을 종합분석 하였으며 도출된 결과를 가지고 한국형 작업치료사필수역량 초안을 도출하였다. 이를 내용 전문가 및 직무 전문가 집단을 섭외하여 워크샵 과정을 통해 수정· 보완하였다. 결과 : 최종 도출된 한국형 작업치료사 필수역량은 7개 단위(unit), 20개 요소(element), 71개 수행기준(performance criteria)으로 구성되었다. 내용적인 특성으로는 기존 작업치료사의 역량이 평가 및 중재에 한정되어 교육되고 국가시험을 통해 평가되었다면 본 작업치료사 핵심역량을 통하여 작업치료 전문가로서의 태도와 실천, 작업치료 과정관리, 전문성 개발 및 교육, 책임과 윤리 등 기존 작업치료 영역에서 다소 비중이미약했던 전문가로서의 태도 및 책임과 윤리, 전반적 과정 관리 측면이 대폭 강화되었다. 결론 : 작업치료 교육은 지식 전달 중심의 암기형 교육에서 벗어나 지식(knowledge), 기술(skill), 태도(attitude)가통합된 ‘역량 중심 교육’으로 전환되고 있는 것이 세계적 추세이다. 본 연구를 통하여 많은 교육기관들이 실무중심의 교육방법을 도입하여 학생들의 수행능력을 극대화하는 차별화된 교육과정을 수립하기를 제언하는 바이다. Objective : It is to suggest a define the essential competencies for an occupational therapist, as well as clearly defining required tangible and intangible competencies. Methods : A comprehensive analysis of relevent data (related to occupational therapy competencies), and a draft for Korean occupational therapy essential competencies was drawn up with the acquired and derived results. With an invited group of experts of the field and related professionals, through a workshop setting, modifications and alterations were made. Results : The final draft consisted of the essential competencies for occupational therapy in Korea, divided into 7 units, 20 elements, and 71 performance criteria. Using the characteristics in the information, if occupational therapy competency evaluation is limited, and only being evaluated through education and national licensing exam, then core occupational therapy competencies should be evaluated through occupational therapist professional attitude, practice, occupational process management, professional development and education, and responsibility and ethics, with the weaker components being attitude, responsibility and ethics, and the representative aspect being overall process management. Conclusion : A suggestion to establish differentiated curriculums to maximize student’s performance abilities is being made, using the research as a way to introduce educational programs to the shift toward practice centered curriculums.
