        Cervical necrotizing fasciitis is a rare but well known life-threatening soft tissue infection primarily affecting the superficial and deep cervial fascial planes. It may rapidly spread into the mediastinum along fascial planes. So, if proper diagnosis and prompt surgical debridement is delayed, the infection may cause descending necrotizing mediastinitis with extensive necrosis of overlying skin, destroying fascia and muscle. Therefore, the key to the successful outcome is early recognition and rapid initiation of definitive radical debridement. The authors report, along with a literature review, a case of cervical necrotizing fasciitis with atypical spreading into the lateral chest wall.

        안면신경 봉합 후 지방조직으로 둘러싼 부목의 임상적 적용

        Facial deformity after nerve injury changes ones' social life. We experienced a few patients with healthy early recovery of muscle contraction after the operation with soft tissue wraparound splint. Under general anesthesia, exploration to find as many injured nerve stumps with ${\times}2.5$ loopes was undertaken at first. Interfascicular repair was done with minimal tension by 10-0 nylon under a microscope, and the suture site was sealed by approximating the surrounding fat flaps. This conjoined adipose tissue flap was a splint as a wraparound environment to reduce the tension in the coaptation site, and to increase the relative concentration of releasing neurotrophic factors by surrounding it. A 45-year-old man fell down in a drunken state and had deep laceration by broken flowerpot fragments with facial muscle weakness on the right cheek. His injured mandibular branches of the facial nerve were found. A 31-year-old female suffered from motionlessnesss of frontalis muscle after a traffic accident. She had four frontal branches injured. The man had his cheek with motion after seven days, and the woman two months after the operation. The nerve conduction test of the woman showed normalized values. Facial nerve repair surrounded by adipose tissue wraparound splint can make the recovery time relatively short.

        합리적 관개용수량 산정에 관한 연구

        In this study, the actual water management were investigated and analyzed on site for effective use of the paddy irrigation water. And this study is tried to analyze the difference between simulated irrigation water according to current design method and actual water supply in experimental reservoirs. The reservoirs in Idong, Yongdeok and Misan, which are managed by Korea Rural Community & Agriculture Corporation, were selected fur field investigation. The purpose of this study is to suggest an improved method for irrigation water calculation through quantitative and periodical analysis considering the difference between design and actual field condition.

        Tubularized Penile-Flap Urethroplasty Using a Fasciocutaneous Random Pedicled Flap for Recurrent Anterior Urethral Stricture

        This report describes the use of a tubularized random flap for the curative treatment of recurrent anterior urethral stricture. Under the condition of pendulous lithotomy and suprapubic cystostomy, the urethral stricture was removed via a midline ventral penile incision followed by elevation of the flap and insertion of an 18-Fr catheter. Subcutaneous buried interrupted sutures were used to reapproximate the waterproof tubularized neourethra and to coapt with the neourethra and each stump of the urethra, first proximally and then distally. The defect of the penile shaft was covered by advancement of the surrounding scrotal flap. The indwelling catheter was maintained for 21 days. A 9 month postoperative cystoscopy showed no flap necrosis, no mechanical stricture, and no hair growth on the lumen of the neourethra. The patient showed no voiding discomfort 6 months after the operation. The advantages of this procedure are the lack of need for microsurgery,shortening of admission, the use of only spinal anesthesia (no general anesthesia), and a relatively short operative time. The tubularized unilateral penile fasciocutaneous flap should be considered an option for initial flap urethroplasty as a curative technique.

        Palliative effect of <sup>131</sup>I-MIBG in relapsed neuroblastoma after autologous peripheral blood stem cell transplantation

        신경모세포종은 소아에서 발생하는 흔한 두개외 고형 종양 중 하나로서 진행된 경우 고용량 항암요법 및 자가 말초혈조혈모세포이식 후에도 재발이 잘 되어 예후가 매우 나쁘다. Iodine-131 metaiodobenzylguanidine ($^{131}I-MIBG$)는 치료에 잘 반응하지 않는 신경모세포종 4기 환자를 위한 대증적 치료 요법으로 제한적으로 이용되어 왔다. 저자들은 자가 말초혈조혈모세포이식 후 재발된 신경모세포종 3례에서 $^{131}I-MIBG$를 이용하여 통증을 경감시키고 생존 기간을 늘이는 고식적인 치료 효과를 얻어 이들에 대한 증례 보고를 하는 바이다. Neuroblastoma is one of the most common extracranial solid tumor of childhood, and treatment of refractory neuroblastoma remains a significant clinical problem. Iodine-131-metaiodobenzylguanidine ($^{131}I-MIBG$) therapy is an alternative approach to treat stage IV neuroblastoma. We report the palliative effect of $^{131}I-MIBG$ in three cases of relapsed neuroblastoma after autologous peripheral blood stem cell transplantation. $^{131}I-MIBG$ is an effective and relatively nontoxic palliative therapy resulting in reduction of pain and prolongation of survival.

      • 위암세포 사멸에 미치는 겨우살이 추출물과 항암제의 효과

        목적: 여러 암 치료의 보조 및 대체요법으로서 겨우살이 추출물은 주로 유럽지역에서 이십세기 초부터 널리 사용되었고 현재 국내에서도 항암 대체 치료제로 점차 사용하는 추세이나 그 항암 기전은 아직 명확하게 밝혀져 있지 않다. 본 연구는 겨우살이 추출물이 위암에 미치는 영향을 세포주 실험을 통해 규명하였다. 대상 및 방법: 위암 세포주는 SNU-719 세포주를 사용하였고 처리한 겨우살이 추출물은 (주)한국 아브노바사로부터 제공받은 ABNOBAviscum-Q와 ABNOBAviscum-F 두 가지를 사용하였다. 겨우살이 추출물과 함께 처리한 항암제는 5-FU와 Cisplatin을 사용하였다. CCK-8 assay kit를 사용하여 세포 생존율을 측정하였고 젖산탈수소효소(LDH) assay kit로써 세포 사멸률을 측정하였다. Caspase 3 assay kit를 이용하여 세포자멸사에 관여하는 caspase 3의 활성 변화를 알아보았고 Western blot analysis를 통해 세포자멸사에 관여하는 Bcl2와 p53, 그리고 항암 작용을 하는 PTEN의 단백질 발현량을 측정하였다. 결과: 겨우살이 추출물 Q와 F를 각각 단독 처리한 실험군 보다 항암제인 5-FU와 cisplatin을 병합처리 하였을 때 세포생존율은 더 낮아졌다. Caspase 3의 활성은 겨우살이 추출물 및 항암제 5-FU 처리를 함으로써 대조군에 비해 4~6배 까지 증가하였다. Bcl2의 단백질 발현량은 대조군보다 겨우살이 추출물과 항암제 처리를 통해 감소하였고 두 약물을 함께 처리하였을 경우 더 낮아졌다. p53의 발현은 겨우살이 추출물 처리에는 영향을 받지 않았고 항암제 처리를 통해서만 증가되었다. 또한 항암 작용을 하는 PTEN의 단백질 발현 정도는 겨우살이 추출물 처리를 통해서 의미 있는 변화가 없었다. 결론: 겨우살이 추출물과 항암제인 5-FU 및 cisplatin 등을 병합 처리 할 경우 위암 세포 사멸에 있어 상승효과를 보였다. 본 연구결과에서 겨우살이 추출물의 항암효과는 caspase 3의 활성화와 Bcl2 발현의 감소를 통해 세포자멸사를 유발하며 이 세포자멸사 유도 기전은 p53과는 상관없음(p53-independent)을 알 수 있었다. Purpose: Mistletoe extract was widely used for cancer treatment as complementary or alternative therapy in European area from early twenty century. It is currently used as alternative anti-cancer remedy by piecemeal in domestic medical group, however, the anti-cancer mechanism of mistletoe extract was not known precisely until now. In this study the effect of mistletoe extract on gastric cancer was studied vis cell line experiments. Materials and Methods: The SNU719 gastric cancer cell line was used, and ABNOBAviscum-Q and ABNOBAviscum-F were treated to cells as mistletoe extract, or 5-FU and cisplatin were used with mistletoe extract. The cell viability and cell death rate were estimated by CCK-8 assay kit and lactate dehydrogenase (LDH) assay kit in each. Caspase 3 assay kit was used to measure caspase 3 activity. The protein expression amounts of Bcl2, p53, and PTEN were estimated through Western blot analysis. Results: The co-treatments of mistletoe extract Q/F and 5-FU/cisplatin decreased lesser cell viability than only mistletoe treat. Caspase 3 activity was increased 4~6 times in co-treatment of mistletoe extracts and 5-FU than control. Bcl2 protein expression was reduced by mistletoe extracts or anti-cancer drugs, further more, the co-treatment of mistletoe extracts and 5-FU/cisplatin diminished more the expression than only mistletoe treatment. Mistletoe extracts did not affect the protein expressions of p53 and PTEN. Conclusion: It was concluded that the anti-cancer mechanism of mistletoe extracts was made by caspase 3 activation and lowered Bcl2 expression, and this apoptosis inducing mechanism was independent to p53.

        The Transmembrane Domain of AtTco64 and Its C-terminal Lysine-rich Flanking Region Are Targeting Signals to the Chloroplast Outer Envelope Membrane

        The targeting mechanism of chloroplast outer enve-lope membrane proteins remains largely unknown. We investigated the targeting of AtToc64. In protoplasts, the transmembrane domain (TMD) and its C-terminal lysine-rich flanking region (LFR) were both necessary and sufficient for targeting to the outer envelope membrane. The lysine residues of the flanking region were critical; without the LFR, the TMD was targeted to the ER or the plasma membrane. In addition, the types of amino acid residues of the TMD, but not the amino acid sequence per se, is a signal for targeting to the chloroplast envelope membrane. TMDs containing phenylalanines were not targeted to the chloroplast in vivo. Based on these results, we propose that the chloroplast targeting signal of AtToc64 comprises two different components: 1) the LFR, which is a signal for evading SRP-mediated co-translational translocation and 2) the hydrophobic amino acid side chains of the TMD, whose size functions as a signal for a cytosolic factor that mediates transport to the chloroplast

        현장 측정을 통한 관개용수로의 손실량 추정

        Water losses in irrigation canals are mainly estimated as the sum of conveyance and delivery water loss. The losses occur via the evaporation, infiltration, gate operation and water distribution processing. Recently, the study regarding these water losses are not satisfactory enough, also delivery water loss has not been mainly considered on field design. The objective of this study is to investigate and analyze the volume of water loss in irrigation canals considering condition of actual farm land. A field measurement was performed at four research sites, which are managed by Korea Rural Community & Agriculture Corporation, to evaluate conveyance and delivery water loss for 2 years. The measurement was performed by canal type, size and designed flow using the inflow-outflow method at a major points such as start and end of each canal, derivation point of canal and inlet of paddy fields. Results of this study showed that water loss ratio in lateral canals was bigger than that of main canal unlike current design standard and the loss decrease as flow increase. The total of water loss ratio including conveyance and delivery water loss in several irrigation canals ranged between 33.25 and 45.0%.

        갯장어(Muraenesox cinereus) 장으로부터 호기적 조건에서 분리된 미생물의 다양성 및 세포외 효소 생산능 분석에 관한 연구

        국내 삼천포 및 서천에서 채집한 갯장어(Muraenesox cinereus)의 장내기관으로부터 분리된 미생물들의 다양성 및 특성에 관하여 조사하였다. 장내 미생물의 순수 분리를 위하여 marine agar 배지를 사용하였으며 37℃에서 호기적으로 배양하였다. 순수 분리 후, 49 균주를 분리하였으며 16S rRNA 염기서열 분석 결과를 바탕으로 계통학적 분석을 실시한 결과, 3문, 13과, 14속, 34종으로 구성되어 있는 것을 확인하였다. 특히, Proteobacteria 문은 83.7%의 분포를 나타내었으며 8과, 8속, 26종으로 Aeromonadaceae, Pseudoalteromonadaceae, Shewanellaceae, Enterobacteriaceae, Morganellaceae, Moraxellaceae, Pseudomonadaceae와 Vibrionaceae로 분포하는 것을 확인하였다. 그리고 분리한 균주들이 amylase, lipase, protease와 같은 산업적으로 유용한 효소를 생산하는지 확인하기 위하여 효소 활성 평가를 실시하였으며, 39 균주가 최소 한 종류 이상의 효소 활성을 가지고 있는 것을 확인하였다. 특히 Aeromonas 속의 균주들은 테스트한 모든 효소의 활성을 나타내는 것을 확인하였다. 이러한 결과는 본 연구를 통하여 분리한 미생물들의 산업적 활용 가능성을 나타내었다. 그러므로 이번 연구는 국내 유전자원 확보 및 갯장어의 장내마이크로비옴의 과학적 지식 확장에 도움이 될 것으로 생각된다. This research confirmed the diversity and characterization of gut microorganisms isolated from the intestinal organs of Muraenesox cinereus, collected on the Samcheonpo Coast and Seocheon Coast in South Korea. To isolate strains, Marine agar medium was basically used and cultivated at 37℃ and pH7 for several days aerobically. After single colony isolation, totally 49 pure single-colonies were isolated and phylogenetic analysis was carried out based on the result of 16S rRNA gene DNA sequencing, indicating that isolated strains were divided into 3 phyla, 13 families, 15 genera, 34 species and 49 strains. Proteobacteria phylum, the main phyletic group, comprised 83.7% with 8 families, 8 genera and 26 species of Aeromonadaceae, Pseudoalteromonadaceae, Shewanellaceae, Enterobacteriaceae, Morganellaceae, Moraxellaceae, Pseudomonadaceae, and Vibrionaceae. To confirm whether isolated strain can produce industrially useful enzyme or not, amylase, lipase, and protease enzyme assays were performed individually, showing that 39 strains possessed at least one enzyme activity. Especially the Aeromonas sp. strains showed all enzyme activity tested. This result indicated that isolated strains have shown the possibility of the industrial application. Therefore, this study has contributed for securing domestic genetic resources and the expansion of scientific knowledge of the gut microbial community in Muraenesox cinereus of South Korea.

      • 조혈모세포이식 후 치료관련사망의 원인

        Bactground:The etiologies of treatment related mortality (TRM) after hematopoietic stem cell transplantation (HSCT) have been variable according to the disease status or the centers. We evlauated the etiologies of TRM for the pediatric patients at Yeungnam University Hospital (YNUH). Materials and Methods:The records of 66 patients, 19 years of age or younger, who had HSCT at YNUH from September 1995 to August 2007 were reviewed. 1) Results:Among 66 patients, allogeneic bone marrow transplantation (Allo-BMT) was done in 21 (19 related, 2 unrelated), allogeneic peripheral blood stem cell transplantation (Allo- PBSCT) in 1, cord blood transplantation (CBT) in 12 (1 related, 11 unrelated), autologous peripheral blood stem cell transplantation (Auto-PBSCT) in 32 patients. The TRM rates of Allo-BMT, CBT, and Auto-PBSCT were 19%, 33.3%, and 12.5%, respectively. Among four patients who had TRM after Allo-BMT, two were related transplantation and the others were unrelated. All four patients developed severe acute GVHD of at least grade Ⅲ. Sepsis developed in three patients, acute renal failure (ARF) in two, veno-occlusive disease (VOD) and thrombotic microangiopathy (TMA) in one patient each. All four patients who had TRM after CBT had two mismatches in HLA-A, B, DR, and engraftment syndrome developed in three. Sepsis developed in all four patients, VOD in two, encephalopathy in two, TMA and ARF in one patient each. All four patients who had TRM after Auto-PBSCT developed sepsis and ARF in two, VOD and TMA in one patient each. Conclusion:Although the number of cases were not large enough for firm conclusion, sepsis was the most common TRM after HSCT. Therefore, prevention and control of sepsis are very important in reducing TRM after HSCT. Outcomes of severe acute GVHD after Allo-BMT and engraftment syndrome after CBT are very poor and contribute for TRM. Continuous effort to reduce the incidence of GVHD and engraftment syndrome are needed.
