        황색포도알균 패혈증 환자에서 발생한 스트렙토콕쿠스미티스에 의한 내인성 안내염 1예

        Purpose: To report a case of endogenous endophthalmitis due to Streptococcus mitis in a patient with Staphylococcus aureus sepsis. Case summary: A 77-year-old male complained of sudden visual loss and ocular pain in his right eye and was treated with intravenous antibiotics for Staphylococcus aureus sepsis. With a diagnosis of endogenous endophthalmitis, the patient received pars plana vitrectomy and intravitreal injection of antibiotics. Vitreous fluid was obtained before surgery, and the organism was identified as Streptococcus mitis. Twenty-one days after the surgery, intraocular inflammation was stabilized, and visual acuity was improved from light perception to 20/60. Conclusions: Appropriate sampling and culture of vitreous fluid are important for the diagnosis of endogenous endophthalmitis. The possibility that the causative organism of endogenous endophthalmitis may be different from the result of blood culture should be considered. J Korean Ophthalmol Soc 2012;53(8):1190-1193

        정상안압녹내장에서 당뇨 유무에 따라 분석한 안토시아노사이드, 은행잎추출물의 치료 효과

        Purpose: This study was performed to evaluate the effect of anthocyanoside and ginkgo biloba extract (GBE) in patients with normal tension glaucoma (NTG), according to the presence of diabetes mellitus (DM). Methods: A chart review of patients with normal tension glaucoma was retrospectively analyzed. All patients underwent a Humphrey visual field (HVF) test and logarithm of the minimal angle of resolution best-corrected visual acuity (log MAR BCVA) was measured over a 6 months period. Changes in mean deviation (MD), pattern standard deviation (PSD) of visual field and log MAR BCVA were compared among anthocyanoside, GBE and no medication (control) groups. Patients were divided according to the presence of DM. Results: A total of 406 NTG patients, including 151 DM patients, were included in the present study. MD was improved in the anthocyanoside and GBE groups, but not in the control group. PSD was not significantly different in all groups. BCVA was improved in the anthocyanoside group, but deteriorated in the control group. The results were similar in patients with or without DM. The generalized linear model demonstrated that systemic medication affected changes in visual indices. Conclusions: The results from the present study suggest that anthocyanoside and GBE may be helpful for improving visual function in some patients with NTG regardless of their DM status.         Alcaligenes eutrophus 균주의 성장과 Ploy-${\beta}$-hydroxybutyrate 생합성에 대한 속도론

        It is very important to have a good kinetic model which considers the effects of both ammonium and glucose for the control and optimization of the poly-${\beta}$-hydroxybutyrate (PHB) fermentation. A kinetic model for the growth of Alcaligenes eutrophus and the biosynthesis of PHB under both ammonium and glucose limitation was proposed. Growth rate of residual biomass was expressed as a function of concentrations of residual biomass, glucose and ammonium having glucose inhibition. PHB production rate was expressed as a function of concentrations of residual biomass, glucose, ammonium and PHB content having ammonium and product inhibitions. Novel approaches were made to estimate the parameters in the model equations which considered two limiting substrates. Model parameters were evaluated by graphical and simplex methods. The proposed kinetic model fitted the data very well.

        광합성색소 분석을 통한 광양만 갯벌 퇴적물 중 저서미세조류의 계절변화

        광양만 조간대 갯벌에 서식하는 저서미세조류와 해수 중 식물플랑크톤의 생체량과 군집조성의 계절변동 양상을 밝히기 위하여, 2002년 1월부터 11월까지 매월 high-performance liquid chromatograph(HPLC)를 이용한 광합성색소(photosynthetic pigments) 분석을 실시하였다. 광합성색소 분석결과 규조류(diatoms)의 주요 지표색소인 fucoxanthin, diadinoxanthin 그리고 diatoxanthin이 퇴적물과 상부 해수에서 연중 우점하여 나타났다. 이들을 제외한 다른 색소들은 상대적으로 낮은 농도를 보였다. 깊이 0.5 cm까지의 표층퇴적물에서 측정한 chlorophyll ${\alpha}$ 농도는 3.44${\sim}$169 mg $m^{-2}$의 범위로 연평균 농도는 상부, 중부 그리고 하부 갯벌 퇴적물에서 각각 $68.4{\pm}45.5,\;21.3{\pm}14.3,\;22.9{\pm}15.6mg\;m^{-2}$로 나타났다. 반면 퇴적물 상부 수주 전체 수심(2.6 m)에 대해 적산한 chlorophyll ${\alpha}$ 농도는 1.66(11월)${\sim}$11.7(7월) mg $m^{-2}$의 농도 범위로 평균 $6.96{\pm}3.04mg\;m^{-2}$를 나타내었다. 이로부터 갯벌 퇴적물 내에 존재하는 저서미세조류의 농도는 해수 중 존재하는 식물플랑크톤의 농도보다 3${\sim}$10배 정도 높은 것으로 추산되었다. 퇴적물과 상부 해수 중 미세조류의 농도에서 유사한 월별변화와 군집조성은 재부유된 저서미세조류가 광양만 해수 중 식물플랑크톤 생체량의 상당부분을 설명할 수 있음을 시사하였다. 따라서 이들 저서미세조류는 갯벌 생물뿐만 아니라 재부유에 의한 수송을 통하여 인근 연안 생태계의 생물군에 대한 중요한 영양원 역할을 할 수 있다는 점에서 이들의 거동에 대한 지속적인 관찰이 요구된다. Seasonal variations of microalgal biomass and community composition in both the sediment and the seawater were investigated by HPLC pigment analysis in an estuarine muddy sandflat of Gwangyang Bay from January to November 2002. Based on the photosynthetic pigments, fucoxanthin, diadinoxanthin, and diatoxanthin were the most dominant pigments all the year round, indicating that diatoms were the predominant algal groups of both the sediment and the seawater in Gwangyang Bay. The other algal pigments except the diatom-marker pigments showed relatively low concentrations. Microphytobenthic chlorophyll ${\alpha}$ concentrations in the upper layer (0.5 cm) of sediments ranged from 3.44 (March at the middle site of the tidal flat) to 169 (July at the upper site) mg $m^{-2}$, with the annual mean concentrations of $68.4{\pm}45.5,\;21.3{\pm}14.3,\;22.9{\pm}15.6mg\;m^{-2}$ at the upper, middle, and lower tidal sites, respectively. Depth-integrated chlorophyll ${\alpha}$ concentrations in the overlying water column ranged from 1.66 (November) to 11.7 (July) mg $m^{-2}$, with an annual mean of $6.96{\pm}3.04mg\;m^{-2}$. Microphytobenthic biomasses were about 3${\sim}$10 times higher than depth-integrated phytoplankton biomass in the overlying water column. The physical characteristics of this shallow estuarine tidal flat, similarity in taxonomic composition of the phytoplankton and microphytobenthos, and similar seasonal patterns in their biomasses suggest that resuspended microphytobenthos are an important component of phytoplankton biomass in Gwangyang Bay. Therefore, considering the importance of microphytobenthos as possible food source for the estuarine benthic and pelagic consumers, a consistent monitoring work on the behavior of microphytobenthos is needed in the tidal flat ecosystems.

        본논문은 시스템 공학연구소가 주관이 되어 수행하고 있는 원격공동연구 및 실험시스템 과제를 위하여 설계한 원거리간 공유실험실시스템을 기술한다. 원거리간 공유실험실시스템은 원격지에 분산되어 있는 각종 실험장비와 수퍼컴퓨터를 포함하는 컴퓨터시스템들을 공동으로 이용하게 하고 실험자료 및 결과를 자유롭게 공유할수 있게 하려는 것을 목표로하고 있다. 원격공동연구 및 실험시스템과제는 과학기술처와 정보통신부, 전산원의 지원을 받아서 수행되고 있으며 약 3억불의 연구비를 투자한 미국 PNNL(Pacifie Northwestem National Laboratory)의 EMSL(Environmental Molecular Science Laboratory)과제와 유사한 점이 많다. 원격지에서 각종 실험장비등을 자유롭게 사용하는 방법으로 Teleoperation과 Telepresence가 있다. 본 논문에서는 Teleoperation과 Telepresence를 포용하고 과학적가시화, 실험자료 및 결과를 자유롭게 처리하는 객체지향 밀티미디어 데이터베이스서비스, 가상기기실험실, 실감형원격컴퓨터이용환경등의 기능을 제공하는 원거리간공유실험 실시스템을 제시한다. 대전과 서물에 산재한 4개의 기관(SERI, KAIST, 서울대, 숭실대)을 중심으로 하여 국가시범초고속망으로 연결한후 제시된 설계내용의 일부를 시스템으로 구축하여 내년중에 시범서비스를 시작할 예정이다. 4년후에는 상당수의 과기처산하기관, 전국각시의 대학, 중소기업체, 국외연구기관이 이용기관으로 참가하고 본논문의 설계내용전부가 시범서비스될 예정이다.

        광선입술염과 편평세포암의 임상, 병리조직학적 관찰 및 p53 단백 발현양상

        Background: Actinic cheilitis (AC) is the main precancerous lesion of the lip. Knowledge of prognostic and diagnostic markers can have positive impacts, although the exact transition rate from AC to lip squamous cell carcinoma (LSCC) is unknown. Objective: We analyzed 59 patients to investigate the relationship between clinical and histopathological features and the expression of the p53 protein in AC and LSCC. Methods: We retrospectively reviewed 59 patients who received biopsy for AC and LSCC between January 2005 and December 2012 and compared clinical and histopathological features of AC and LSCC. Results: The ratios of males to females for AC and LSCC were 1.3:1 and 1.7:1. The mean ages of those with AC and LSCC were 66.4 and 70.1 years. All of the lesions involved the lower lip, and the most frequently affected site was the right side of the lip in LSCC. Persistent fissures with scales and exudative ulcers were noted 90.6% and 43.7% of the AC cases, respectively. The intensity of the inflammatory infiltrate was significantly associated withthe degree of epithelial dysplasia. Most LSCCs (96.2%) were immunoreactive to the p53 protein. The intensities of AC and LSCC p53 protein expression were 1.1 and 2.4. The degree of epithelial dysplasia was the only histopathological finding significantly associated with p53 protein expression. Conclusion: An intense inflammatory infiltrate in AC was predictive of a microinvasive SCC. Therefore, p53 protein immunoreactivity may be an important indicator in lip carcinogenesis and the degree of epithelial dysplasia. (Korean J Dermatol 2014;52(10):701∼710)

        중장거리 사이클 대회에 참여한 우수/준우수선수 경기기술의 운동학적 비교 분석

        The purpose of the study was to perform comparative analysis of the kinematical factors among skilled and less skilled cyclist and aims to contribute to performance enhancement by understanding the technical characteristics involved during a tournament. In order to achieve the purpose of the study, subject of the male middle and long distance 4 km standing start event were chosen and divided into two groups, i.e. top ranked 8 cyclists were termed as skilled group and bottom ranked 8 cyclists as a less skilled group. Four high speed cameras were installed on straight phase to analyze the right leg pedaling technique. The characteristics obtained from analyzing skilled cyclist are as follows: Firstly, skilled group’s horizontal velocity is significantly higher than less skilled group. Secondly, during the mid lap and final lab, skilled group’s ankle angular velocity is significantly lower than less skilled group. Also, the cyclist should significantly increase movement in the knee joint rather than ankle joint. In order to improve the cycling performance, it is recommended the mixed training including resistance and muscle endurance.

        점증적 최대운동부하와 5부간 독주 검사간의 대학교 장거리 사이클 선수의 유산소성 능력 비교

        본 연구는 장거리 사이클 선수의 유산소성 능력을 필드에서 측정 가능한 측정방법을 개발하고자 실험실의 전형적인 점증적 최대운동부하 방법과 5분간 독주 검사방법을 비교하였다. 8명의 지구성 훈련이 된 사이클 선수들이 본 연구에 참여하였다. 점증적 최대운동부하방법은 초기에 운동부하 150watts로 시작하여 1분마다 25watts씩 증가시키면서 80rpm을 유지하면서 탈진할 때가지 실시하였으며, 5분간 독주 검사는 선수들이 사용하는 자전거를 이용하여 선택한 속도로 5분간 지속적인 독주를 실시하였다. 두 가지 검사 방법의 비교결과 절대적 및 상대적 최대산소섭취량은 5분간 독주 검사가 유의하게 높았으며(p<0.05), 두 가지 검사 방법 간에 절대적(r=0.87, p<0.01) 및 상대적(r=0.76, p<0.01) 최대산소섭취량은 유의한 상관관계를 보였다. 결론적으로 선수 자신들의 자전거를 이용한 필드에서 유산소성 능력 평가를 위해서 5분간 독주 검사 방법 또한 유용하다고 여겨진다. The purpose of this study was to compare the aerobic capacity of endurance cyclist between the incremental maximal exercise(IME) and individual pursuit for constant duration(5min)(5M). Eight elite male endurance-trained cyclists with at least 5 years of competitive riding experience participated in the study. To analyze aerobic capacity between tests, subjects was performed the incremental maximal exercise test which start 150W of 80rpm following increment of 25W·min-1 until all-out using cycle ergometry and the individual pursuit for constant duration(5min) by self-cadence using cyclists` private racing bike. Relate and absolute VO2max were significantly higher in 5M compare with IME(p<0.05). And, there were a significant correlation for absolute(r=0.87, p<0.01) and relative VO2max(r=0.76, p<0.01) between IME and 5M test. In conclusion, the findings of the current study suggest that 5M test may be used to evaluate endurance cyclists` aerobic capacity using private racing bike.
