        Hypomelanosis of Ito with Multiple Congenital Anomalies

        Hypomelanosis of Ito (HI) is a neurocutaneous disorder, also known as incontinentia pigmenti achromians. HI has been associated with chromosomal abnormalities, especially mosaicism. Herein, we report a case of HI with multiple congenital anomalies. A 2-month-old girl presented with multiple linear and whorling hypopigmentation on the face, trunk, and both extremities and patch alopecia on the scalp. Moreover, she had conical teeth, aniridia of the both eyes, and multiple musculoskeletal problems, including syndactyly and coccyx deviation. Cytogenetic analysis on peripheral blood was normal 46, XX, and no mutation was found in IKBKG gene test.

        Good Clinical Responders to Topical Timolol in Patients with Infantile Hemangiomas: A 7-Year Retrospective Study of 328 Korean Patients

        Background: Topical timolol is widely used for treatment of superficial infantile hemangioma(IH). However, little is known about factors that affect the response to topical timololtreatment. Objective: This study aimed to investigate the efficacy, safety, and predictive value for goodresponse to topical timolol for IH. Methods: A retrospective review of medical records and clinical photos of 328 patientswith IH treated with topical timolol 0.5% solution was conducted. Serial clinical photographswere compared with those at the initial visit using a 100-mm visual analogue scale(VAS). Treatment response was defined as an improvement of at least 75% from baselinein IH lesions within 12 months of treatment. Results: Overall, IH patients treated with topical timolol showed significant improvementfrom baseline, showing that the final VAS score within 12 months of treatment was69.7±20.4. The multivariable logistic regression analysis showed age at initiation of treatment(p=0.007), length of gestation and fetal growth (p=0.03), depth (p=0.01), and flexuralarea (p=0.007) were significantly associated with treatment response. Only four patients(1.1%) reported local irritation. Conclusion: This study demonstrated that topical timolol treatment was an effective andwell-tolerated treatment for IHs. Physicians are encouraged to consider several patient- orlesional factors that might affect treatment response to achieve better clinical outcomes.

        Thermal-Corrosion-Free Electrode-Integrated Cell Chip for Promotion of Electrically Stimulated Neurite Outgrowth

        In neural tissue engineering, the use of electrical stimulation has been proposed to cure patients with damaged nervous systems. Electrical stimulation can promote nerve cell differentiation and regeneration. Due to certain properties of electrode materials, including thermal problems and corrosion, the wide application of electrical stimulation has been hindered. Here, we fabricated an electro-spun, nano-porous, membrane-based, electrode-embedded cell chip capable of electrical stimulation of cells. To provide conductivity on an electro-spun nano-porous membrane, a 1.0 μm-thick layer of Au was deposited by electron beam evaporation. The electrode had a length and height of 4 and 0.025 cm, respectively, and was fixed to the inner walls of transparent polydimethylsiloxane (PDMS) cell chip channels. For 5 days, when 1 kHz of electrical stimulation was given at 50 and 100 mVpp, the promotion of neurite outgrowth of neuroblastoma SH-SY5Y cells was significantly exhibited, while no thermal problems or corrosions were detected in the electrodes. Furthermore, at 100 mVpp of electrical stimulation, not only neurite outgrowth but also secondary branch induction was observed. We believe that the newly proposed cell chip can be applied to the study of neuronal differentiation and regeneration with stable and controllable electrical stimulation.

        생물학 제제 치료 중인 한국인 건선 환자에서 Interferon-gamma Release Assay 추적검사결과에 대한 연구

        Background: Biologics are widely used for the treatment of moderate to severe psoriasis as their safety and efficacy have been determined. However, as biologics act as antagonists of Interleukin (IL) or Tumor necrosis factor-alpha (TNF-α), there are concerns regarding immunity to infectious diseases including tuberculosis (Tb). Objectives: We analyzed the serial interferon-gamma release assay (IGRA) for detecting latent tuberculosis infection in patients treated with biologics (ustekinumab, secukinumab, guselkumab, ixekizumab) for psoriasis. Methods: A retrospective analysis of IGRA results was performed during follow-up sessions of patients with psoriasis between January 2013 and March 2022. Results: Among the total 122 patients, the initial IGRA results of 78 patients (63.9%) were negative and 44 patients (36.1%) were positive for Tb. During the 9 years of follow up, 3 patients (3.8%) of the 78 who were initially IGRA negative showed seroconversion and tested IGRA positive; 10 patients (22.7%) of the 44 patients who were initially IGRA positive showed seroreversion and tested IGRA negative. Of the three patients with seroconversion, none had active tuberculosis infection. Conclusion: Seroconversion was observed in three patients treated by biologics in this study. Considering the epidemiologic characteristics of Korea having a high incidence rate of tuberculosis, annual follow-up examination along with preventive tuberculosis treatment is necessary.

        소아 전신농포건선에 대한 사이클로스포린 치험 2예

        Generalized pustular psoriasis (GPP) is a rare, life-threatening form of psoriasis characterized by widespread sterile pustules on erythematous skin and is even rarely found in children. Prompt diagnosis and treatment are necessary as GPP flare can be life-threatening. Due to its rarity, there are currently no treatment guidelines for childhood GPP in Korea. In this report, we present two cases of 8-year-old boys with childhood GPP who were successfully treated with cyclosporine. Both patients had extensive erythema with papulopustular lesions on the trunk and extremities. One patient had been treated with systemic corticosteroid in another clinic and showed resolution but eventually showed recurrence and flare. Another patient had intermittently tried low dose oral acitretin and topical steroid treatment in another clinic. We started treatment with cyclosporine for periods of 15 weeks and 10 weeks respectively, until their lesions showed clearance. Our cases suggest that cyclosporine is a safe and effective treatment option for acute GPP in children.

        A Case of Disseminated Coccidioidomycosis Confirmed by Skin Biopsy in Korea

        Disseminated coccidioidomycosis is an uncommon fungal infection primarily observed in immunocompromised individuals. Infection typically occurs when individuals inhale fungal spores in endemic areas such as the southwestern United States and parts of Central and South America. However, globalization has increased the number of reported cases outside endemic areas. Herein, we present an unusual case of coccidioidal meningitis, a rare but fatal complication of disseminated coccidioidomycosis, in an immunocompetent patient who previously resided in an endemic area.

        Analysis of the Microbiome of the Ear Canal in Normal Individuals and Patients with Chronic Otitis Externa

        Background: Recently, microbiome research has been actively conducted for various skinareas. However, no study has yet compared the microbiome of bacteria and fungi in the earcanal of healthy individuals and patients with chronic otitis externa in Korea. Objective: This study aimed to investigate the difference in the distribution of fungal andbacterial microbial communities in ear canal samples of healthy individuals and patientswith chronic otitis externa. Methods: In 24 patients with bilateral chronic otitis externa and 24 healthy controls, cottonswabs were used to obtain samples from the bilateral ear canal. To characterize the fungaland bacterial communities, we sequenced and analyzed the 16S rRNA V4–V5 and ITS1 re-gions using Quantitative Insights into Microbial Ecology 2, respectively. Results: The alpha diversity analysis for bacteria and fungi confirmed that both richnessand evenness decreased in the patient group. The beta diversity analysis for bacteria con-firmed that these parameters differed between the control and patient groups. The beta di-versity analysis for fungi showed no difference between the groups. Conclusion: We observed different skin microbiomes in the patients with chronic otitis ex-terna compared with those in the healthy individuals.

        Plaque Patterned Cutaneous Candidiasis on the Lower Legs: A Case Report

        Diabetes mellitus (DM) is a metabolic disorder that causes chronic hyperglycemia, leading to immune dysfunction and increased vulnerability to infections. As a result, patients with DM are more susceptible to fungal infections, including Candida species, due of their weakened immune systems. Cutaneous candidiasis is a fungal infection that appears as a red plaque with peripheral satellite papules and pustules around the periphery. Patients with chronic mucocutaneous candidiasis may have erythematous plaques and thickened skin thickening with an overlying scale, which can mimic plaque psoriasis. Therefore, fungal infections should be considered when assessing chronic refractory psoriasiform lesions that are difficult to treat, especially in patients with DM. This report describes a case of cutaneous candidiasis in both lower legs, which was initially misdiagnosed as psoriasis.

        콜라겐 자극 필러를 이용한 코입술주름 교정의 효과와 안전성: 체계적 문헌고찰 및 메타분석

        Background: Nasolabial folds (NLFs) constitute a pivotal aspect of facial aesthetics and are the primary focal points in facial rejuvenation procedures. Collagen-stimulating fillers have gained popularity due to their long-lasting effects and biocompatibility. Objective: This study aimed to comprehensively evaluate the therapeutic efficacy and safety profiles of collagen- stimulating fillers for NLF correction. Methods: A thorough search of the databases including PubMed, Embase, and Cochrane was performed. Randomized controlled trials published between 2000 and 2023 on the treatment outcomes of collagen-stimulating fillers for NLFs were included. A systematic review and meta-analysis were conducted using the Wrinkle Severity Rating Scale (WSRS), Global Aesthetic Improvement Scale (GAIS), and adverse events. Results: Among the 144 pertinent studies, nine met the inclusion criteria, including 299 participants for polycaprolactone (PCL), 291 for poly L-lactic acid (PLLA), and 382 for calcium hydroxylapatite (CaHA). PCL showed significantly higher improvement in GAIS (risk ratio [RR], 3.14; 95% confidence interval [CI], 2.29∼4.30; p<0.001) and WSRS (mean difference, 0.71; 95% CI, 0.36∼1.06; p<0.001) at a 12-month interval compared with hyaluronic acid (HA). CaHA showed marked 12-month GAIS improvement versus HA (RR, 1.51; 95% CI, 1.21∼1.88; p<0.001). PLLA exhibited superior wrinkle improvement compared with HA at week 24, especially in individuals under 52 years of age. No severe complications occurred. Conclusion: Collagen-stimulating fillers have the potential to be safe, effective, and long-lasting options for NLF correction. Nevertheless, due to the limited data and heterogeneity among the included studies, cautious interpretation is required. Further high-quality clinical trials are required to validate these findings.
