Objective : The treatment of malignant posttraumatic brain swelling remains a frustrating endeavor for neurosurgeon. Mortality and morbidity rates remain high depite advances in medical treatment of increased intracranial pressure. If conventional therapy fails in patients suffering from intracranial hypertension, there is only small number of second-tier option left including decompressive craniectomy. The role of decompressive craniectomy in posttraumatic brain swelling remains controversial. We assessed the efficacy and indications of decompressive craniectomy. Methods : The authors performed decompressive bifrontotemporal craniectomy in 22 patients with malignant posttraumatic brain swelling. Epidural pressure monotoring was performed in all patients. The clnical data and surgical outcomes were reviewed retrospectively. Result : The favorable outcome(GOS score 4-5) was 59%(13 of 22 patients), whereas the mortality rate was 32% (7 of 22 patients). Two patients(9%) remained in severely disabled state. Increased rate of favorable outcome was seen in the patients who had 8 or more of GCS score at admission and exhibited B wave in ICP monitoring and who showed steady state or slow deterioration in clinical course. Conclusion : If conservative therapy fails, decompressive bifrontotemporal craniectomy should be considered in the management of malignant posttraumatic brain swelling before irreversible ischemic brain damage occur.

Objectives: Salivary gland tumors pose considerable difficulty in diagnostic and prognostic assessment based on the histopathologic features alone. We studied the expression of type IV collagen and fibronectin in salivary gland tumors with special emphasis on the differential diagnostic significance. Materials and Methods: We did immunohistochemical stain on paraffin embedded tissues of 33 benign and 24 malignant salivary gland tumors using monoclonal antibody for type IV collagen and polyclonal antibody for fibronectin. Results: 1) Well preserved linear basement membrane-like staining of type IV collagen was detected in duct-cell-derived benign salivary gland tumors. But pleomorphic adenoma exhibited a heterogeneous pattern as focal augmentation and interruption. 2) In malignant tumors, type IV collagen was distributed in an irregular, interrupted manner or completely absent. Adenoid cystic carcinomas displayed a marked staining of the basal membrane associated substances in the pseudocysts. 3) The staining pattern of fibronectin was similar to that of type IV collagen execpt more dense in the stroma. 4) Salivary gland tumors which have a prominent myoepithelial cell component revealed a particular deposition of basement membrane materials adjacent to the myoepithelial cells. Conclusion: The study of the basal membrane substances may be helpful for differential diagnosis of benign and malignant salivary gland tumors and identifying special features of salivary gland tumors such as pseudocystic pattern of adenoid cystic carcinoma. Also we think that the myoepithelial cells contribute to the formation of basement membrane materials.

To overcome solubility of poorly water soluble drugs, we prepared solid dispersions containing nanoparticles of water insoluble pranlukast using spray dryer. These solid dispersions formulated to improve the dissolution of pranlukast. PVP-K30 used as a water soluble carrier for the solid dispersion and poloxamer used as a surfactant. Characterization of pranlukast solid dispersion analyzed by scanning electron microscope(SEM), differential scanning calorimeter(DSC) and particle size analyzer. SEM and DSC were found that pranlukast is amorphous in solid dispersions. Particle size analyzer was used to investigate size of pranlukast in solid dispersions. The in vitro dissolution rate of pranlukast solid dispersion was significantly higher than the conventional drugs(Onon(R) and Prakanon (R)), as 11~33 folds. This studies illustrated the potential use of solid dispersion for the delivery of poorly water soluble drug, such as pranlukast by the oral route.

Since the discovery of superconducting oxides, enormous amount of research has been conducted on the preparation of high-T_c superconducting films. It is obvious that future microelectronic devices will require well-crystallized films of smooth surface and uniform structure prepared at substrate temperatures as low as possible. The high-T_c superconducting Y-Ba-Cu-O thin films in this work were prepared by thermal chemical vapor deposition method on MgO(100) substrate without postannealing using β-diketone metal chelates of Y(thd)₃, Ba(thd)₂, and Cu(thd)₂ as source materials and O₂ as a reactant gas. Superconductivity above liquid nitrogen temperature(T_(C, onset)=90.1 K and T_(C, 0)=87.3 K) was obtained for the film prepared at 650℃ and 0.0126 torr. The thin films consisted of Y-Ba-Cu-O with mostly c-axis perpendicular to the substrate plane and with small amounts of a-axis normal to film surface.
오피스빌딩 냉난방시스템의 수명주기비용 분석에 관한 연구 : 흡수식 냉온수 유닛의 설치 유형을 중심으로 한 사례연구

본 연구는 사례대상 건물의 냉난방설계용량에 대한 냉온수 유닛의 설치 유형을 단일설치, 분할설치(균등분할), 분할설치 (차등분할)의 유형으로 구분하여 각각의 사례를 선정하고 LCC기법을 이용한 경제성 평가를 실시하였다. 이를 위하여 냉온수 유닛의 LCC 구성항목을 조사하고, 구성항목별 비용 자료를 조사하였다. 위와 같은 과정을 통해 예측된 각 설치 유형별 LCC를 상호 비교 분석함으로써 경제성 평가를 실시하였다. 상기와 같은 목적과 방법에 따라 진행된 본 연구의 결과를 요약하면 다음과 같다: 1) 냉난방설계용량에 대한 단일설치 사례와 분할설치(균등분할), 분할설치(차등분할) 사례를 선정하고 LCC 구성항목에 대한 자료조사를 통하여 초기투자비(냉동기 설치비, 냉각탑 설치비, 펌프설치비), 유지관리비(교체 수선비, 가스에너지비, 전기에너지비), 해체비(노무비, 폐기비, 잔존가치)를 고려하여 단위면적당$(100m^2)$분석기간 15년 동안의 LCC를 분석 하였다. 2) 냉온수 유닛의 단일설치, 분할설치(균등분할), 분할설치(차등분할)의 설치 유형에 따른 경제성 평가 결과 설치 유형 중 단일설치 유형이 가장 경제적인 것으로 분석 되었다. The purpose of this study is to execute economical analysis for two types of office building HVAC system using LCC technique : single vs, separated HVAC(equal partition installation, unequal partition installation) system. The research method of this study includes the case analysis and questionnaire surveys. The results of this study are as follows: (1) LCCs of 3 types of HVAC system are estimated and the most economical one is investigated as single HVAC system, (2) In this case study, the single HVAC system was investigated as 34% lower in LCC during 15 years per $100m^2$ compared to equal partition installation, as 41% compared to unequal partition installation.

Periodontal ligament cells may have a role in the regulation of hard and soft periodontal tissues, but their specific function has not yet to be determined. To evaluate further their role in periodontal regeneration, they were examined for osteoblast-like behavior. Periodontal ligament cells and gingival fibroblasts were primarily cultured from extracted premolar with non-periodontal diseases. Cells were cultured with DMEM at $37^{\circ}C$, 5% $CO_2$, 100% humidity incubator, and as a measure of cell characterization, it was examined that the morphology, alkaline phosphatase activity, collagen synthesis, and immunocytochemistry for osteonectin, osteocalcin, and collagen type I. Healthy periodontal ligament cells has more osteoblastic-like cell property in alkaline phosphatase activity. and collagen synthesis than gingival fibroblast. Immunocytochemistry localization explained that calcitonin were expressed in periodontal ligament cells only, and osteonectin and type I collagen were produced in both cells simultaneously. This results indicate that the growth characteristics of periodontal ligament cells and gingival fibroblasts exhibit some differences in proliferative rates and biochemical synthesis. The differences may help to calrify the role such cells play in the regenearation of periodontal tissues.
